Carved Merocel®을 이용한 비골골절의 고정 (Fixation of Nasal Bone Fracture with Carved Merocel®)

  • Purpose: In most cases of nasal bone fracture, closed reduction with internal or external splint fixation approach is selected. However, because of indiscriminate insertion of the internal splint without considering of anatomical difference or deformity, insufficient fixation happens frequently that need additional fixation. Therefore, we suggest a new method for providing adequate support in reduced nasal bone by carving $Merocel^{(R)}$ that is fixed for the anatomical structure. Method: Closed reduction and internal fixation with carved $Merocel^{(R)}$ was performed in 15 nasal bone fracture patients from March, 2010 to July, 2010. Each patient was evaluated by physical examination, facial photographic check, simple X-ray, and computerized tomography. On the first day post-operation, location of packing and amount of reduction were checked by follow up X-ray and computerized tomography. In addition, patients' symptoms were evaluated. During the 3-month post-op follow up at out-patient clinic, operator, 2 doctors in training and one assistant performed the objective evaluations by physical examination on nasal dorsal hump, nasal deviation, nasal depression, nasal breath difficulty, and nasal airway obstruction. A survey of subjective patients' satisfaction in 4-stages was also performed. Results: The results of follow-up computerized tomography of the 15 patients revealed that 11 patients had good reduced state. Three patients with combined maxillary frontal process fracture had over reductions. A survey performed on the first day post-operation showed that 14 of 15 patients answered that their current symptoms were more than tolerable. At the 3-month follow-up physical exam, one case had a dorsal hump. However, there were no nasal deviations, nasal depressions, nasal breath difficulties, or nasal airway obstructions. Twelve of the 15 patients answered more than moderate on the 3-month survey. Conclusion: Intranasal packing after carving the $Merocel^{(R)}$ considering anatomical structure is a new effective method to promote proper-reduction, maintain stability, and minimize patients' symptoms by addition of a simple procedure.

Comparison of three midsagittal planes for three-dimensional cone beam computed tomography head reorientation

  • Objective: This study compared three prominent midsagittal planes (MSPs) to identify the MSP that best approximates the true symmetrical MSP. Methods: Forty-three patients (mean age, 23.0 ± 8.20 years) were grouped as follows: group 1 consisted of 10 patients with skeletal Class I and a menton (Me) deviation of < 2 mm; group 2, 11 patients with skeletal Class III and a Me deviation < 2 mm; group 3, nine patients with skeletal Class III and a Me deviation of 2 to less than 4 mm; and group 4, 13 patients with skeletal Class III and an Me deviation ≥ 4 mm. The candidate MSPs were established by three-dimensional (3D) cone beam computed tomography (CBCT) reorientation methods (RMs): (1) the MSP perpendicular to the Frankfort horizontal (FH) plane while passing through the crista galli and basion; (2) the MSP including the nasion, incisive foramen, and basion; (3) the MSP including the nasion, anterior nasal spine, and posterior nasal spine. The mean absolute distances (MADs) to the MSPs were calculated from the coordinates of 1,548 points on 129 CBCT images. The differences in the values of the 3D coordinates among RMs were compared. Results: The MADs of the three RMs showed significant differences (p < 0.05). Most of the differences in values of the coordinates were not significant among RMs. Conclusions: Although the differences in distance among the three MSPs were minor, the MSP perpendicular to the FH plane while passing through the crista galli and basion best approximated the true symmetrical MSP.

Correction of the deviated tip and columella in crooked nose

  • The primary procedural components of deviated nose correction are as follows: osteotomy to correct bony deviation, septal deviation correction, manipulation of the dorsal septum to correct upper lateral cartilage deviation, and correction of functional problems (manipulation for correction of internal valve collapse and hypertrophy of the inferior turbinate). The correction of tip and nostril asymmetry cannot be overemphasized, because if tip and nostril asymmetry is not corrected, patients are unlikely to provide favorable evaluations from an aesthetic standpoint. Tip asymmetry, deviated columella, and resulting nostril asymmetry are primarily caused by lower lateral cartilage problems, which include deviation of the medial crura, discrepancy in the height of the medial crura, and asymmetry or deformity of the lateral crura. However, caudal and dorsal septal deviation, which is a more important etiology, should also be corrected. A columellar strut graft, correction of any discrepancy in the height of the medial crura, or lateral crural correction is needed to correct lower lateral cartilage deformation depending on the type. In order to correct caudal septal deviation, caudal septal shortening, repositioning, or the cut-and-suture technique are used. Surgery to correct dorsal septal deviation is performed by combining a scoring and splinting graft, a spreader graft, and/or the clocking suture technique. Moreover, when correcting a deviated nose, correction of asymmetry of the alar rim and alar base should not be overlooked to achieve tip and nostril symmetry.

비골골절 시 골절정복과 동시에 시행된 융비술 (Simultaneous Augmentation Rhinoplasty with Bony Reduction in Nasal Bone Fracture)

  • Purpose: The nasal bones are the most common fracture sites of the facial bones, and a careful reduction may still result in secondary deformities, such as saddle nose, deviated nose, hump nose etc, requiring secondary cosmetic rhinoplasty. Therefore, this study examined the clinical characteristics of nasal bone fractures to propose guidelines for patient selection and surgical procedures to achieve more satisfactory results and to prevent secondary deformities with simultaneous augmentation rhinoplasty and bony reduction. Methods: The study was based on 26 out of 149 nasal bone fracture patients who underwent simultaneous augmentation rhinoplasty with bony reduction between May 2008 and April 2009. Retrospective analysis was performed according to the clinical data, surgical techniques and postoperative results. Results: Of the 26 patients, there were 15 males and 11 females. The incidence according to the Stranc's classification revealed that 62% of patients were injured by a frontal impact and 38% by a lateral impact. Frontal impact plane I (50%) was the most frequent type. At the follow up, 18 (81.2%) out of 22 patients were satisfied with their postoperative outcome, and the remaining 4 patients were fair. No one was dissatisfied. However, 5 cases in 3 patients (23%) had some complications; minimal implant deviation in 2 cases, minor irregularity on the nasal dorsum in 2 cases and palpable implant movement under palpation in 1 case. None of these cases required surgical correction. Conclusion: With the proper guidance, simultaneous augmentation rhinoplasty with bony reduction can prevent secondary deformities and satisfy the cosmetic outcomes.

비골 골절 환자에서 골절 정복과 동시에 시행한 코성형술 (Simultaneous Rhinoplasty with Fracture Reduction in Nasal Bone Fracture)

  • Purpose: The nasal bone fracture is most common fracture in facial bone injuries. Regardless of the severity or type of fracture, closed reduction has traditionally been the common method of treatment. However, through detailed pre-operative evaluation, we found out that many patients consider rhinoplasty prior to trauma due to aesthetic desire or nasal deformity with or without septal deviation. In treatment of nasal bone fracture, we focused not only on the fracture management but also on the patients' desire prior to trauma, and we made additional operation according to patients' desire with fracture reduction and gained rewarding outcomes. Methods: From March 2005 to June 2007, total 263 patients were treated for nasal bone fracture. Among these patients, 57 patients (21%) had the additional operation with nasal fracture reduction. The additional operations were categorized in three types: augmentation rhinoplasty with tip plasty (40%), septoplasty only (16%), corrective rhinoplasty (44%). The mean follow-up period was 5.6 months and results were evaluated by scoring. Results: Forty four of 57 patients (77%) were highly satisfied regardless of any additional operation kinds. The complications were one septal perforation, two displacement of implant and four remnant nasal deformities. For the septal perforation, no further management was performed because we lost the contact with the patient. Then 4 of the other complicated patients were revised. Conclusion: In general, many physicians tend to consider nasal fracture as a simple trauma. However through the strict history taking, physical examination and professional counseling, we could catch the patient's cosmetic desire and get the eyes on new concept: the nasal fracture is not only a trauma but a cosmetic and functional field. In the treatment of nasal bone fracture, if additional rhinoplasty is performed, patients will be more satisfied and we also can expect higher profits.

Agreement between cone-beam computed tomography and functional endoscopic sinus surgery for detection of pathologies and anatomical variations of the paranasal sinuses in chronic rhinosinusitis patients: A prospective study

  • Purpose: The diagnosis of chronic rhinosinusitis requires a comprehensive knowledge of the signs and symptoms of the disease and an accurate radiographic assessment. Computed tomography (CT) is the superior imaging modality for diagnosis of chronic rhinosinusitis. However, considering the lower dose and higher resolution of cone-beam computed tomography (CBCT) compared to CT, this study aimed to assess the agreement between the findings of CBCT and functional endoscopic sinus surgery (FESS). Materials and Methods: This descriptive prospective study evaluated 49 patients with treatment-resistant chronic rhinosinusitis who were candidates for FESS. Preoperative CBCT scans were obtained before patients underwent FESS. The agreement between the CBCT findings and those of FESS was determined using the kappa correlation coefficient. The frequency of anatomical variations of the paranasal sinuses was also evaluated on CBCT scans. Results: Significant agreement existed between pathological findings on CBCT scans and those of FESS, such that the kappa correlation coefficient was 1 for mucosal thickening, 0.644 for nasal deviation, 0.750 for concha bullosa, 0.918 for nasal polyp, 0.935 for ostiomeatal complex (OMC) obstruction, and 0.552 for infundibulum thickening. Furthermore, 95.9% of patients had 1 or more and 79.6% had 2 or more anatomical variations, of which nasal deviation was the most common (67.3%). Conclusion: Considering the significant agreement between the findings of CBCT and FESS for the detection of pathological changes in the paranasal sinuses, CBCT can be used prior to FESS to detect chronic rhinosinusitis and to assess anatomical variations of the OMC.

Correction of bony deviation in rhinoplasty

  • In rhinoplasty, osteotomy is becoming more and more frequent as a way to achieve aesthetically pleasing and functional results, as well as patient satisfaction. In procedures to correct a deviated nose, osteotomy to correct the bone plays an essential role in addition to correction of the septum and cartilage, and osteotomy can reduce the wide nose bridge and give a slightly higher appearance in Asian rhinoplasty. However, osteotomy is relatively invasive, and the nasal bones of Asians are often low and thick, so bleeding or swelling during surgery can be somewhat more severe, and a stuffy nose can occur after surgery if osteotomy is performed incorrectly. Since side effects are possible, it is necessary to have a precise understanding of the relevant anatomy and technique. Several articles have described nasal bone osteotomy in rhinoplasty, and this review article introduces the methods presented in various articles, describes indications and limitations, and reviews the relevant anatomical structures and techniques in an accurate manner. We introduce a method that can increase patients' satisfaction and the completeness of surgery through accurate osteotomy, as well as reducing the risk of side effects.

  • It was the purpose of this study to obtain nasalance values for Chinese and Korean based on Vowels. The NasalView was used to measure the amount of nasal acoustic energy in the speech of 96 normal adults from China and Korea. Means and standard deviations for the nasalance and deviation scores are presented for each of three different vowels. The Chinese speakers were found to have significantly higher nasalance scores on vowel /a/ and /u/.

비강수술로 호전된 폐쇄성 수면무호흡증후군 1 례 (One Case of Nasal Surgery in Obstructive Sleep Apnea Syndrome)

  • 폐쇄성 수면무호흡증은 이비인후과영역에서 흔하게 관찰되는 질환 중 하나로 수면 중에 상기도의 폐쇄로 인한 호흡정지가 주요한 병인이며 상기도를 구성하는 비강, 구강, 인두, 후두 중 어느 한 부분에서 폐쇄가 일어나도 질환의 원인이 될 수 있다. 비폐색의 가능한 원인들로는 주로 비중격 만곡, 비용, 기포성 갑개, 후비공 폐쇄, 종양, 이물, 술 후 또는 외상 후 발생한 유착, 여러 가지 비염, 기타 질환 등이 있다. 폐쇄성 수면무호흡증의 수술적 치료에는 비강수술, 구개인 두수술, 편도 및 아데노이드 절제술, 설부 축소수술, 기관절개술 등 여러 가지가 있는데 문헌들을 고찰한 결과 지금까지 성인에서 수면무호흡증 치료로 시행한 비강수술의 효과에 대해서는 아직 논란의 여지가 있는 상태이다. 최근 저자들은 비폐색과 동반된 코골이 및 수면무호흡증 환자에서 비폐색과 수면무호흡증의 원인으로 생각되는 비중격 만곡증 및 비후성비염을 교정하기 위해 비중격 교정술 및 하비갑개 절제술을 시행한 결과 수술 전, 후의 증상 및 수면 다원검사 비교에서 뚜렷한 호전을 경험하였기에 보고하는 바이다.

코리안 쇼트헤어 고양이에서 발생한 크립토코쿠스 감염증 의심 1증례 (A Case of Nasal Cryptococcosis in a Domestic Shorthair Cat)

  • 8세 중성화 암컷 고양이가 만성 비강 삼출과 호흡곤란으로 내원하였다. 신체검사에서 오른쪽 비강의 출혈농성 삼출이 관찰되었고, 흡기성 호흡곤란과 코골기 증상, 종대된 오른쪽 하악 림프절이 확인되었다. 전혈검사와 혈청화학 검사에서 미약하게 증가한 헤마토크리트 값과 고글로불린혈증이 나타났으며, 혈청학적 및 PCR 기법을 이용한FeLV, FIV, Chlamydophila felis, Feline Calicivirus, Herpesvirus, Bordetella, Mycoplasma felis, H1N1 influenza 검사에서는 모두 음성이었다. 방사선 검사에서는 오른쪽 비강의 연조직 밀도 상승이 관찰되었고, CT촬영에서는 비중격의 위축과 골 용해가 확인되었다. 추가 검사로 실시한 하악 림프절 세포학 검사에서는 다양한 두께의 염색이 안 되는 협막을 갖는 곰팡이가 관찰되었으며, narrow based budding을 보이는 곰팡이도 관찰되어 크립토코쿠스 감염증으로 잠정진단하였다. 혈청학적 검사에서 크립토코쿠스 항원가는 1 : 32,768로 매우 높게 나왔다. 검사결과에 기초해서 곰팡이 감염 치료를 위해 fluconazole, clindamycin, tocopherol투여를 실시했으며 약물 투여 후 3일 이내에 환자의 증상은 극적으로 개선되었다. 장기적인 관찰과 추가 항원역가 검사를 실시하고자 하였으나 환자는 증상 개선 후 퇴원하여 재 내원하지 않았다.
