Approach to Frontal Sinus Outflow Tract Injury

  • Frontal sinus outflow tract (FSOT) injury may occur in cases of frontal sinus fractures and nasoethmoid orbital fractures. Since the FSOT is lined with mucosa that is responsible for the path from the frontal sinus to the nasal cavity, an untreated injury may lead to complications such as mucocele formation or chronic frontal sinusitis. Therefore, evaluation of FSOT is of clinical significance, with FSOT being diagnosed mostly by computed tomography or intraoperative dye. Several options are available to surgeons when treating FSOT injury, and they need to be familiar with these options to take the proper treatment measures in order to follow the treatment principle for FSOT, which is a safe sinus, and to reduce complications. This paper aimed to examine the surrounding anatomy, diagnosis, and treatment of FSOT.

Primary extracranial meningioma presenting as a forehead mass

  • Meningioma originates from arachnoid cap cells and is the second most common intracranial tumor; however, it can also be found in an extracranial location. A very rare primary extracranial meningioma without the presence of an intracranial component has also been reported. Primary extracranial meningiomas have been found in the skin, scalp, middle ear, and nasal cavity. A computerized tomography or magnetic resonance imaging scan is necessary to determine the presence or absence of an intracranial meningioma, and a biopsy is essential for diagnosis. We report a case of primary extracranial meningioma located in the forehead skin of a 51-year-old male.

Endoscopic Endonasal Approach for Suprasellar Lesions in Children : Complications and Prevention

  • The endoscopic endonasal approach (EEA) has been popularized in adults and has been applied to an expanding range of surgical modules and indications in this population. However, its clinical application in pediatric neurosurgery has been impeded by the differences in anatomical features and the relatively low incidence of diseases to which it is applicable. In this review article, we mainly discuss the surgical indications, feasibility, and complications of EEA for suprasellar lesions in children based on a review of the literature, focusing especially on the age-related anatomical features of the nasal cavity, various pathologic entities, and the impact of EEA on long-term craniofacial growth.

결체증식성 법랑모세포종 (DESMOPLASTIC VARIANT OF AMELOBLASTOMA)

  • Desmoplastic variant of ameloblastoma is a new and unusual variant of ameloblastoma with extensive stromal desmoplastic proliferation. The authors experienced a case of desmoplastic variant of ameloblastoma with moderate-defined radiolucency on the right maxillary anterior area in 62-year-old female. As a result of careful analysis of clinical, radiological, histopathological examinations, we diagnosed it as desmoplastic variant of ameloblastoma, and the following results were obtained : 1. Main clinical symptoms were nontender bony swelling with normal intact overlying mucosa on the right maxillary anterior area. 2. Radiographically, moderate-defined, multilocular radiolucency on the right maxillary anterior area were shown, and severe cortical bony thinning and expansion to labial and palatal sides were also observed. And this lesion was shown to be extended to the right nasal cavity. 3. Histopathologically, follicle-like epithelial islands with densely abundant collagenous stroma were morphologically compressed.

소아 알레르기성 비염에 대한 통규탕(通竅湯) 증류추출액을 이용한 치료의 임상적 연구 (A clinical study on the effect of Tongkyu-tang distillate on pediatric allergic rhinitis)

  • Objective : Prevalance of allergic rhinitis in pediatric population appear to be rising. About acupuncture and herbal medicine treatment on allergic rhinitis is occasionally reported, but few treatment of using herbal medicine distillate was reported. The aim of this study was to investigate effect of Tongkyu-tang distillate on pediatric allergic rhinitis. Methods : Twenty patients treated in our hospital between January 2002 and December 2002, treated with Tongkyu-tang distillate were studied. Result : We treated pediatrc allergic rhinitis with Tongkyu-tang distillate spray on nasal cavity. and so the symptoms of allergic rhinitis were improved effectively Conclusion : We had effective results of Tongkyu-tang distillate on pediatric allergic rhinitis comparatively. And we wish that much more new treatment methods of allergic rhinitis are investigated.

부비동 유두종에서의 FDG 섭취 증가를 보인 예 (A Case of Paranasal Sinus Papilloma with Increased FDG Uptake)

  • The false-positive FDG uptakes on head and neck areas are common due to benign lesion, iatrogenic and physiologic changes. The Schneiderian papilloma is uncommon benign tumor arising from the mucosa of the paranasal sinuses and nasal cavity. The findings of paranasal papilloma on conventional modality such as CT and MRI are non-specific and they could be confused with inflammatory polyp or retention cyst. Despite of benign tumor, the papilloma usually shows locally aggressive growth with malignant potential, therefore the FDG can be actively accumulated in this lesion. We describe the case of 18F-FDG PET/CT finding in a 77-year-old woman who demonstrates oncocytic papilloma in maxillary sinus.

갑상선에 발생한 악성 림프종 1례 (A Case of Malignant Lymphoma of the Thyroid Gland)

  • Malignant lymphoma is the tumor of immune system which is known as the most common nonepitheloid tumor of head and neck region. It is mainly found in the cervical, axillary and inguinal lymph node, but gastrointestinal tract, palatine tonsil, nasopharynx, oropharynx, nasal cavity and salivary glands may be involved. Primary lymphoma of the thyroid gland is an uncommon condition, comprising approximately 2% of all malignant lymphoma and 5% of all thyroid malignant neoplasms. Recently, we experienced a case of malignant lymphoma of the thyroid gland, which was histopathologically proven. So we report our findings in this patient with review of literatures.

협부에 발생한 소타액선 다형성 선종 치험 1예 (A Case of Minor Salivary Gland Pleomorphic Adenoma on Cheek)

  • Pleomorphic adenoma is the most common benign mixed salivary gland neoplasm that accounts for 65% of all benign salivary glands tumors. While the majority arises from the parotid, only a 10% arises from the minor salivary glands. Pleomorphic adenoma of minor salivary gland in the palate is a common entity. However, they occur in the lip, cheek, tongue oropharynx and nasal cavity etc., rarely. We experienced a case of a 80 years old female presented a 10 years history of a right cheek mass which was proved to the pleomorphic adenoma on the histopathologic examination. We resected the mass and obtained good postoperative results. We report this case with a review of literature.

  • Non-Hodgkin's lymphomas are a group of highly diverse malignancies and have a strong tendency to affect organs and tissues that do not ordinarily contain lymphoid cells. Primary extra nodal lymphoma of the hard palate is rare. Here, we present a case of diffuse large B cell lymphoma in a 60-year-old male patient that manifested as slightly painful ulcerated growth on the edentulous right maxillary alveolar ridge extending onto the palate, closely resembling carcinoma of the alveolar ridge. Computed tomography images showed the involvement of the maxillary sinus and right nasal cavity, along with destruction of hard palate, superiorly extending into the orbit. This case report highlights the importance of imaging to evaluate the exact extent of such large malignant lesions, which is essential for treatment planning.
