Purpose : Several studies suggest that smoking and vitamin D level is a risk factor of metabolic syndrome. The objective of this study is to evaluate the association between smoking status, vitamin D levels and the Korean adult male and female metabolic syndrome. Methods : We assessed 3796 participants aged 19 years and older from the Korean National Health and Nutritional Examination Survey 2013, 2014. Smoking statuses were collected from self-reported questionnaires. Subjects were divided into three categories: non-smokers, former smokers, and current smokers. Vitamin D was tested by Radioimmunoassay method and the value of serum 25-hydroxyvitamin D, which is an index of vitamin D status in the body, was used. The diagnosis of metabolic syndrome was made using criteria modified NCEP-ATP III. Logistic regression analysis was used to calculate odds ratios between smoking status, vitamin D levels, and metabolic syndrome. Results : The overall prevalence of metabolic syndrome was 23.0 % in men and 15.4 % in women. After adjusting for smoking, the odds ratio for men's metabolic syndrome in current smokers was 1.77 (95 % CI, 1.30~2.41), while for former smokers OR was 1.63 (95 % CI, 1.15~2.31) compared with nonsmokers. After adjustment vitamin D, the odds ratio for women's metabolic syndrome in vitamin D deficiency was 1.44 (95 % CI, 1.11~1.87) compared with normal. Conclusion : Smoking status was associated with an increased risk of metabolic syndrome in Korean adult males and decreased vitamin D level was associated with an increased risk of metabolic syndrome in Korean adult females.
There is a great public concern that higher consumption of eggs may increase the risk of chronic disease due to high levels of cholesterol in eggs. In contrast to this concern, limited studies have examined this relationship, and the study results have been inconsistent. This study was to evaluate the cross-sectional associations between egg consumption, blood markers and metabolic syndrome in Korean adults. We analyzed data from the combined 2007-2008 Korean National Health and Nutrition Examination Survey (KNHANES). Egg intakes were assessed using a food frequency questionnaire. Metabolic syndrome was ascertained using criteria of the NCEP ATP III (2001) and abdominal obesity criteria of the WHO. In 5,548 subjects, 937 subjects were defined as having metabolic syndrome. In linear regression models, subjects reporting one or more egg consumption per day had significantly lower levels of triglyceride, fasting blood glucose and blood pressure, and a higher level of HDL cholesterol, compared to those who rarely consumed eggs (P-trends < 0.05). After adjustments for potential confounding factors, the associations were no longer significant except for total cholesterol, showing higher levels with frequent egg consumption. More frequent intakes of egg (1 egg or more/day) were associated with lower risk of metabolic syndrome in a logistic regression model (OR: 0.69, 95% CI: 0.50-0.96), but this association also became insignificant after adjustments for multiple confounding factors (OR: 1.05, 95% CI: 0.71-1.57). In healthy Korean adults, frequent intakes of eggs did not adversely affect the risk of metabolic syndrome. Further studies are needed to confirm this finding.
The risks of the metabolic syndrome (MS) is known to be related to the dietary behavior. The objective of this study is to evaluate the association between the relative risks of MS and the dietary habit and to provide the ideal dietary habits for prevention of chronic disease of the middle-aged. Healthy subjects aged 40-64 years (male n=122, female n=173) were recruited throughout Seoul area. MS was defined according to NCEP-ATP III criteria except central obesity, and Asia-Pacific Area criteria for central obesity (2000) was adapted. Subjects were stratified into 3 groups according to the number of total risk factors:'MS group' was defined as the subjects who have three or more risk factors, 'Risk group' was defined as ones to have one or two risk factors, and 'Healthy group' defined as ones with no risk factor. In this study, 'Health group' comprised of $40.7\%$, the percentage of 'Risk group' was $49.0\%$, and the 'MS group' was $9.5\%$ of the total subjects. The MS incidence was associated with low education (p<0.001), low economic status (p<0.05), and low self-assessed health recognition (p<0.05). The risk of MS increased with adverse life styles such as cigarette smoking (p<0.05), irregular meal time (p<0.05), skipping lunch (p<0.05), low interests in balanced diet (p<0.05), and higher salt intake (p<0.01). Healthy group self-evaluated nutritional knowledge more highly (p<0.05) and scored higher nutritional knowledge (p<0.001). However, there was no difference in overall nutritional behavior among the three groups, which implies that nutritional education method should be developed for the subjects to practice their teaming efficiently.
Purpose: The aim of this study was to examine the effects of obesity on the physiological levels of adiponectin, leptin and components of metabolic syndrome (MS) in male workers, aged 30-40 years. Methods: Body mass index (BMI) was measured with Anthropometric equipment. Blood pressure and serum parameters were measured with an automatic digital sphygmomanometer and autochemical analyzer, respectively. Adiponectin and leptin were analysed by ELISA kits and MS was defined based on the NCEP-ATP III. Results: Body fat mass of waist and hip, systolic and diastolic blood pressure were significantly higher, as expected, in the BMI>25kg/$m^2$ in comparison with the $BMI{\leq}25kg/m^2$. While fasting glucose, insulin, HOMA-IR and leptin in the BMI>25kg/$m^2$ were also significantly higher compared with $BMI{\leq}25kg/m^2$, HDL-cholesterol and adiponectin were significantly higher in $BMI{\leq}25kg/m^2$. On multiple logistic regression analysis for the components of MS, exercise, adiponectin and leptin were an only independent factor for MS in non-obese male workers($BMI{\leq}25kg/m^2$) after adjustment for age, cigarette smoking and drinking habits. Conclusion: These results suggested that the obesity in men was associated with physiological levels of adiponectin and leptin contributing to feedback control of MS and that dysfunction and/or declination in feedback control system associated with changes in physiological levels of neurptrophics: adiponectin and leptin might ultimately induce MS.

The purpose of this study was to investigate the relationship between Kimchi intake and the prevalence of metabolic syndrome in Korean adults (over 19 years old), using data from the Korean National Health & Nutrition Examination Survey (KNHANES), 2007~2012. A total of 30,630 participants were analyzed in this study. Metabolic syndrome was diagnosed using the NCEP ATP III criteria standards. Out of the 30,630 participants, 8,230 (23.45%) had meatabolic syndrome. When the subjects were grouped by their Kimchi intake, those who consumed a sufficient amount (36.0~82.125 g) showed a lower prevalence of metabolic syndrome (P<0.001). However, multiple logistic regression analysis results after adjusting for sex and age, exercise status, smoking status, drinking status, energy intake, income level, occupation, area, frequency of eating-out, showed that the Kimchi intake did not significantly affect the odds ratio of metabolic syndrome. High sodium contents of Kimchi was the major cause of various cardiovascular diseases, and was associated with the hypertension risk factors of the metabolic syndrome. However, in this study, the Kimchi intake, even after adjusting sodium intake, did not have negative influence on the development of metabolic syndrome.
Objectives The purpose of this study is to propose a method to more specifically identify Sasang constitutional risk factors of metabolic syndromes by adjusting the cut-off value of Korea Sasang Constitutional Diagnostic Questionnaire (KS-15). Methods Data of 1997 participants in Korean medicine Daejeon Citizen Cohort study (KDCC) were analyzed. Metabolic syndrome was defined according to the NCEP-ATP III, lifestyle information, and hematologic information including KS-15 and demographic characteristics were used as covariates. Results The 179 subjects with metabolic syndrome accounted for 9.0% of the total. As a result of determining the Sasang constitution for the KS-15 response based on the cut-off values (approximate 0.33), 0.5, and 0.6 of the constitutional score, when performed at the 0.6 cut-off model, the odds ratio of TE was 2.46 which showed a statistically significantly higher risk than the borderline group. For the accuracy of the model and the Area under the curve (AUC), the model accuracy based on the original cut-off of the KS-15 was 0.902 and AUC was 0.737. The accuracy of the model with cut-off of 0.5 and with of 0.6 were 0.904 and 0.902, respectively, and the AUCs were 0.687 and 0.741, respectively. Conclusion In this study, we confirmed that it is effective to increase the cut-off value of KS-15 to 0.6 in the metabolic syndrome risk model. It is expected that this could increase the accuracy of identifying high-risk groups for metabolic syndrome.
The aim of the present study was undertaken to investigate the association between diagnostic indices of metabolic syndrome(MetS) with carotid intima-media thickness using ultrasonography. The participants in the study were 315 male employees without carotid atherosclerosis and other cardiovascular disease. This study was approved by the Institutional Review Board of Occupational Safety and Health Research Institute. Written informed consent for the participants in this study was obtained from all individuals. Anthropometric parameters and biochemical characteristics were done using each specific equipments and the NCEP-ATP III criteria were used to define MetS. They were examined by B-mode ultrasound to measure the carotid intima-media thickness(carotid IMT) at the near and far walls of common carotid and bifurcation(bulb). The mean carotid IMT was $0.739{\pm}0.137\;mm$ and it's thickness significantly increased with the increase in age. Also, amounts of systolic and diastolic blood pressure, triglyceride and fasting glucose were significantly increased with the increase in age. Carotid IMT were significantly correlated with BMI(r=0.170, p=0.004), systolic(r=0.148, p=0.011) and diastolic blood pressure(r=0.123, p=0.036) and HDL-cholesterol(r=-0.164, p=0.005). On multiple logistic regression analysis for the diagnostic indices of MetS, carotid IMT were significantly associated with blood pressure(OR=4.220, p<0.01) and MetS(OR=1.301, p<0.05). The results indicate that blood pressure and MetS are important risk factors for carotid atherosclerosis.
Objectives : To investigate changes in, and predictors of, metabolic syndrome(MetS) status over a 5-year period in chronic schizophrenic patients and to identify factors associated with the prevention of or recovery from MetS. Methods : In total, 107 patients, all of whom provided written informed consent, were followed from 2011 to 2016 at Naju National Hospital for this study. MetS was defined according to the revised National Cholesterol Education Program-Adult Treatment Panel III guidelines. Results : During follow-up period, 22(20.5%) patients were newly diagnosed to MetS, 14(13.1%) were disappeared, 77(66.4%) were not changed[MetS : 34(31.8%), No MetS 37(34.6%)]. Common significant factors in the two changed groups were triglyceride and waist circumference, not dose and type of antipsychotic medication. Multiple logistic regression analysis revealed that female gender(odds ratio[OR]=2.846, 95% confidence interval[CI] : 1.020-7.942), attending two or more outpatient visits per month(OR=3.155, 95% CI : 1.188-8.379) and taking antidepressant medication(OR=3.991, 95% CI : 1.048-15.205) were significantly associated with MetS after controlling for other confounding variables. Type and dose of antipsychotic medication were not significantly associated with MetS. Conclusions : Triglyceride and waist circumference were important manageable indicator of MetS. Adoption of a healthy lifestyle is more important than adjusting the dose or type of antipsychotic medication in the treatment of chronic schizophrenia patients with MetS.
Objectives : This study was performed to investigate characteristic indice of heart rate variability (HRV) according to presence of metabolic syndrome (MS), pattern identification. Methods : We analyzed 167 subjects who participated in a stroke check-up. We classified the subjects into two groups as MS (n=48) and non-MS (n=119) based on NCEP ATP III. First, we investigated pattern identification of all subjects based on Korean Standard Pattern Identifications for Stroke-III. Second, we surveyed significance of HRV indices between MS and non-MS groups. Third, we investigated significance of HRV indices between classified DP of MS and classified DP of non-MS. Fourth, we investigated significance of HRV indices between classified DP of MS and non-MS. Fifth, we investigated significance between components of MS and HRV indices. Results : 1. SDNN, RMS-SD, HF, Ln(HF), and HF (NORM) were significantly lower in the MS than non-MS group. LN (TP), LF/HF were significantly higher in the MS than non-MS. 2. Ln (HF), Ln (TP), and LF/HF were statistically significant in the classified DP of MS group. 3. RMS-SD, and Ln (HF) were significantly lower in the abdominal obesity group than in the non-abdominal obesity group. SDNN, RMS-SD, TP, Ln (TP), VLF, Ln (VLF), and Ln (LF) were significantly lower in the hypertension group than in the non-hypertension group. RMS-SD was significantly lower in the diabetes group than in the non-diabetes group. Mean HRT,LF/HF were significantly higher in the hypertriglyceridemia than non-hypertriglyceridemia group, but Ln (HF), RMS-SD, and HF (NORM) were significantly lower in the hypertriglyceridemia than non-hypertriglyceridemia group. Conclusions : We found a few significant relationships between classified DP of MS and HRV indices. The resluts demonstrate that MS has potentially negative effects on the cardiovascular system, and these effects could be detected by HRV.
Objectives This study was performed to investigate the cut-off values of abdominal circumference and Body Mass Index(BMI) according to Sasang Constitution. Methods A total of 1,773 persons, namely 440 male people and 1,333 female people, in ages from 40 to 69 years old, participated in this research. They are all included in the community based Genomic cohort in Wonju in years from 2006 to 2013. The diagnostic criteria of metabolic syndrome was used followed by the National Cholesterol Education Program in Adult Treatment Panel III (NCEP ATP III) and Asian pacific guideline for only abdominal obesity. Results The prevalence of metabolic syndrome was 34.8% for men and 37.6% for women in this research. Taeeumin was the highly significant risk type for the metabolic syndrome in both sexes. Cut-off values of abdominal circumference were 88.3 cm in men, 80.3 cm in women, and cut-off values of BMI were $24.2kg/m^2$ in men and $25.3kg/m^2$ in women. For men, cut-off values of abdominal circumference were 79.5cm in Soyangin, 88.8 cm in Taeeumin and 79.5 cm in Soeumin, and cut-off values of BMI were $23.2kg/m^2$ in Soyangin, $25.6kg/m^2$ in Taeeumin and $20.6kg/m^2$ in Soeumin. For women, cut-off values of abdominal circumference were 76.3 cm in Soyangin, 80.3 cm in Taeeumin and 76.8 cm in Soeumin, and cut-off values of BMI were $22.4kg/m^2$ in Soyangin, $24.9kg/m^2$ in Taeeumin and $21.3kg/m^2$ in Soeumin. Conclusions Although 90 cm in men, 80 cm in women as an Asian pacific guideline were regarded as quite approximate to the mean value of abdominal circumference and $25kg/m^2$ to BMI. But if the results could be allocated in terms of the Sasang constitutional method, the cut-off values for Soyangin and Soeumin should be lowered than now.
