침술치료 후 발생한 지연성 척추 경막하 혈종 (Late-Onset Spinal Subdural Hematoma after Acupuncture)

  • Acupuncture is generally regarded as a safe procedure and as a popular treatment for patients with musculoskeletal disorders. We report a case of a 47-year-old male patient with late-onset tetraplegia, developed after acupuncture. He had no trauma, medical, and social history relevant to tetraplegia. Right after the acupuncture, he felt discomfort in his right arm. After 6 days, all 4 extremity weakness developed. Whole-spine magnetic resonance imaging revealed the presence of spinal subdural hematoma extending from the C5 vertebra to the coccyx level. Hand coordination dysfunction, neurogenic bladder, and neuropathic pain were other symptoms. After the management, he recovered muscle strength, but incomplete bladder control and neuralgia were sustained. It is important to be aware of the possibilities of severe complications after acupuncture.

  • Objectives : The aim of this study was to report the effects of acupuncture on the patients with severe acute musculoskeletal pain transferred from emergency department(ED). Methods : Three patients were transferred from ED of western medicine, and treated with Korean medicine treatments composed of acupuncture mainly and cupping, herbal steaming, and herbal decoction additionally. Results : All patients showed pain-reduction and relaxation both physically and mentally immediately after the first acupuncture treatment. In cases 1 and 2, the pain numerical rating scale(NRS) decreased after the day of admission from 8 to 4 and 10 to 2, respectively. In case 3, pain NRS also gradually decreased from 8 to 4 after three days. Conclusion : Acupuncture was effective for immediate pain reduction in patients with severe acute musculoskeletal pain transferred form ED. Further studies are warranted to confirm the immediate pain-reduction effect of acupuncture on acute musculoskeletal patients visiting or transferred from ED.

체간의 나선방향운동이 운동능력에 미치는 효과 (The Effect Spiral Way Movement of a Trunk Exerts on the Movement Ability)

  • The purpose of this study was to examine spiral way movement of a trunk exerts on the movement ability. The details established to achieve for this article. This examination confirmed the weight, weight/height2 index, ratio of lumbar to pelvic, musculoskeletal quantity, push up for 2 minute, pitch a ball and voluntary isometric contraction with flexion and extension of knee joint of the subjects with spiral direct movement. Healthy eighteen subjects who understand fully the significance of procedure, consented to a plan, without neuromuscular disease were participated in two groups of experiment. The group were a spiral movement(9), rectilinear movement(9). Trunk movement tested 2 sessions of a spiral movement and rectilinear movement with a push up for 2 minute, 5days per a week, for the 4 weeks. This experiment tested 3 times with a sufficient rest for fatigue limitation. An analysis of the results used a paired samples t-test for difference from before and after experiment. The following results were obtained; At an internal change of the body, the musculoskeletal quantity was increased significantly to spiral movement group, but the weight was increased significantly, the musculoskeletal quantity was not significant to rectilinear movement. The movement ability evaluation for a external change was increased significantly in a push up for 2 minute, pitch a ball, isometric contraction with extension of knee joint of a spiral movement group, but a push up for 2 minute was increased significantly in a push up for 2 minute on the abdominal muscle training of a rectilinear movement group. As compared with a rectilinear movement, a spiral movement was more effect by cooperation with nerve and musculoskeletal system and an increase in movement ability was caused by learning acknowledgment, muscular reeducation. These results lead us to the conclusion that a spiral movement of trunk was more effect than a rectilinear movement, the coordination of nerve and musculoskeletal system was of importance of Multi-direction movement. Therefore, A further studies concerning the therapeutic exercise intervention and active-dynamic analysis could enhance the development of the most effect on the trunk.

무작위대조군연구(RCT)를 중심으로 한 근.골격계 질환의 약침 치료에 관한 고찰 (Review of Randomized Controlled Trials on Pharmacopuneture Treatment for Musculoskeletal Diseases)

  • Objectives : The purpose of this study is to review RCTs on pharmacopuncture treatment for musculoskeletal diseases and to establish standards of pharmacopuncture treatment model. Methods : We searched articles up to date of March 2009 via computerized databases of Pubmed, The Journal of Korean Acupuncture & Moxibustion, Journal of Korean institute of Herbal Acupuncture, Journal of Oriental Rehabilitation and Journal of Korean Oriental Medicine. Only Randomized Controlled Trials (RCT) concerning the effects of pharmacopuncture on musculoskeletal diseases. The pharmacopuncture treatment methods were assessed based on STRICTA and items considering the feature of pharmacopuncture. And the methodological quality of the trials was assessed by FEAS and modified Jadad score. Results : Eighteen trials of pharmacopuncture on musculoskeletal diseases were analyzed. Except for 4 trials comparing the effect of SBV and BV, positive outcome was reported in ten trials. Among eighteen trials; most of the trials were about Bee Venom acupuncture, and most of the trials used about five acupuncture points, mainly local acupuncture points. But, the amount of injection to each point and total injection were various. And most of trials were lack in the information about method of stimulation. The adjusted FEAS score ranged from 0 to 12, and modified Jadad scoreranged from 1 to 5. Conclusions: To standardize pharmacopuncture treatment, we need more well-designed, high quality clinical trials. And methodological assessment tools designed for pharmacopuncture treatment are also needed.

Post-COVID-19 pain syndrome: a descriptive study in Turkish population

  • Background: The new type of corona virus has a wide range of symptoms. Some people who have COVID-19 can experience long-term effects from their infection, known as post-COVID conditions. The authors aimed to investigate prolonged musculoskeletal pain as a symptom of the post-COVID-19 condition. Methods: This is a descriptive study on the patients who were diagnosed with COVID-19 in a university hospital, between March 2020 and March 2021. Patient records and an extensive questionnaire were used to obtain relevant demographic and clinical characteristics, including hospitalization history, comorbidities, smoking history, duration of the pain, the area of pain, and the presence of accompanying neuropathic symptoms. Results: Of the diagnosed patients, 501 agreed to participate in the study. Among the participants, 318 had musculoskeletal pain during COVID-19 infection, and 69 of them reported prolonged pain symptoms as part of their a post-COVID condition which could not be attributed to any other cause. The mean duration of pain was 4.38 ± 1.73 months, and the mean pain level was 7.2 ± 4.3. Neuropathic pain symptoms such as burning sensation (n = 16, 23.2%), numbness (n = 15, 21.7%), tingling (n = 10, 14.5%), stinging (n = 4, 5.8%), freezing (n = 1, 1.4%) were accompanied in patients with prolonged musculoskeletal pain. Conclusions: Patients with COVID-19 may develop prolonged musculoskeletal pain. In some patients, neuropathic pain accompanies it. Awareness of prolonged post-COVID-19 pain is crucial for its early detection and management.

근골격계 통증환자의 통증유형과 체형진단을 통한 신체지표 관련성 연구 (Research of Body Parameters Characteristics from Posture Analysis of Musculoskeletal Problem Patient)

  • Objectives : The purpose of this study is body parameters characteristics through posture analysis system of musculoskeletal problem patient Methods : Posture analysis system were performed for 164 patients to measure body parameters such as Q-angle, body inclination, neck inclination, PCMT(posterior cervical muscle tension), Knee flexion and posture balance. Statistical analysis using statistical analysis techniques and Pearson correlation coefficients was performed to assess the body parameters obtained by posture analysis system. Results : More than half of people out of 164 reported low back pain, 34.8% of the total was found to have neck pain. There was not a significant difference between genders from the characteristics of gender based body parameters expect for the statistical difference in Q angle, PCMT. There was a significant correlation between low back pain and multiple response status. There was a significant correlations between knee pain and Q angle. Also There was a significant correlations between pelvic pain and posture balance of ankle. Conclusions : Posture analysis system can be used to perform the analysis in place of X-ray measuring body posture and clinical parameters. The results of this study are expected to be the basis for further research on the clinical application of posture analysis system.

Effects of pain neuroscience education on kinesiophobia in patients with chronic pain: a systematic review and meta-analysis

  • Objective: One of the treatment strategies for controlling chronic pain and preventing disability is patient education. Pain neuroscience education (PNE) has been proven to be effective in explaining the biological and physiological processes associated with pain experiences to patients. The purpose of this review is to investigate the effectiveness of PNE for kinesiophobia such as avoidance response in patients with chronic pain. Design: A systematic review and meta-analysis. Methods: MEDLINE, EMBASE, CINAHL, PEDro, and the Cochrane Central Register of controlled trials databases were searched through November 2020 and included a randomized controlled trials evaluating kinesiophobia in musculoskeletal patients with chronic pain. In 8 randomized controlled trial studies, 'Cochrane's risk of bias (RoB) tool was used for qualitative analysis, and results of post-intervention were analyzed through RevMan 5.4 for quantitative analysis. Results: For this review, 8 randomized controlled trials of 369 patients with chronic pain were selected for PNE. A systematic review and meta-analysis also included 8 randomized controlled trials. The effect on kinesiophobia was more effective than the control group (-0.86; 95% confidence interval [CI], -1.22 to -0.51; heterogeneity [χ2=21.18, df=7, I2=67%]; overall effect [Z=4.80]). In addition, the effect on pain was more effective than the control group (-0.53; 95% CI, -1.05 to -0.01; heterogeneity [χ2=47.42, df=7, I2=85%]; overall effect [Z=2.01]). Conclusions: The results of this review suggest that PNE and combined PNE have a positive effect on the improvement of pain and kinesiophobia in patients with chronic pain.

Development of Evaluation Items for Physical Therapy Clinical Practice

  • Objective: The purpose of this study is to present practice guidelines on what physical therapy students must learn during clinical training and how to evaluate them. Design: Design & development research Methods: A Delphi survey was conducted with 16 experts. Experts who participated in the study were asked to respond to 15 musculoskeletal system, 14 nervous system, and 12 physical factor therapy items through an online survey with a 5-point Likert score for each item regarding suitability and importance. Results: As a result of defining the core basic physical therapy skills that students in the Department of Physical Therapy must learn through clinical practice and in the process of evaluating them, there was a strong opinion that among the specific fields of physical therapy, evaluation items related to the musculoskeletal system, nervous system, and cardiorespiratory system were important. On the contrary, physical factor therapy showed poor results. Conclusions: In a previous study, opinions on the specialty of physical therapists were collected from current physical therapists, and the field of physical factor therapy was evaluated low. I think this reflects the perception that treatment using electricity has been commonly used in the field of physical therapy for a long time and does not require deep expertise among clinicians. Physical therapy clinical practice guidelines and efficient operation plans for clinical practice were presented. Through this, it can be usefully applied in hospitals where physical therapy students undergo clinical training in the future.

한방병원 종사자의 근골격계 증상과 유해요인평가 (A Survey for Ergonomic Work Risk Factors of Musculoskeletal Disorders in a Oriental Medicine Hospital)

  • Objectives: The purpose of this study was to evaluate work-related risk factors of musculoskeletal disorders (MSDs) in various departments and tasks at an oriental medicine hospital. Methods: 269 hospital workers were selected for the study. The questionnaire survey included general characteristics, work-related characteristics and MSDs. Cross-tabulation analysis, multiple logistic regression, adjusted for age, work time, job demand and workload, were used to evaluate the effects of work-related factors on MSDs. Results: There were differences in MSDs depending on gender, age, occupation, and work-related factors. Prevalence rates of MSDs were 23.79% shoulder, 14.13% back, 10.41% neck. Risk factors for MSDs was packaging of drugs, transfer, and exercise therapy of physical therapist. Shoulder pain was significantly influenced by using power tools, over-reaching, and job demand while back pain was significantly influenced by weight-lifting operations and keyboard typing. Conclusions: The results of this study suggest that consideration of MSDs characteristics of workers in oriental medicine hospital is necessary to change unsuitable work environments and educate on preventing MSDs.

작업성 근골격계질환의 VDT 증후군에 관한 고찰 (A Review on VDT Syndrome of Work-Related Musculoskeletal Disorders)

  • The studys purpose were the effects of work posture, treatment method and prevention for Video Display Terminal(VDT) workers in Work-Related Musculoskeletal Disorders(WMSD) The results were as follows: 1. The prevalence rate of VDT workers in WMSD was $20{\sim}40%$. The complaint was mostly shoulder, neck, and hack area pain 2. VDT worker used to forward flexed posture and then affect of increase of muscle fatigue and pain 3. When exercise therapy PT and ADL training, were used workers decrease in pain, muscle strength, balance training, endurance strength and relief of psychiatricIn conclusion, VDT worker need good health and posture to rest and exercise with time space and treatment. It is best to prevent WMSD. VDT workers need health management by itself and systemic rehabilitation program by speciality therapist.
