Purpose: The purpose of the study was to compare the effects of Tai Chi exercise, Tai Chi self help program, and self help management program on arthritic pain, stiffness, fatigue and difficulties in performing daily activities in patients with osteoarthritis. Method: The pre-post experimental design was utilized with convenient sampling at Public Health Centers. Total of 38 subjects agreed to participate in the study with the inclusion criteria of being diagnosed as osteoarthritis and no previous regular exercise. The 12 subjects were assigned to Tai Chi exercise group, 13 subjects to Tai Chi exercise combined with self help management program, and the other 13 to self help management program. Tai Chi exercise group performed exercise twice a week for one hour per each session. Tai Chi self help management group came to the class twice a week, one hour for Tai Chi exercise, and another hour for self help management program. The third group came to the class once a week for one hour to participate self help management program. At the completion of 6 week programs, 10 subjects from Tai Chi exercise, 9 from Tai Chi self help, another 10 from self help management groups completed the post-test measures. The data were analyzed by SPSSWIN 11.0. The homogeneity tests for three experimental groups were conducted by $X^2-test$, Fisher's exact test, Kruskal-Wallis test. The final analysis of the post-test measures was conducted by ANCOVA with fatigue, pain, stiffness, and difficulties in daily activities of the pretest measures as covariates. Results: The results were as follows. There were no significant differences in fatigue, pain, and difficulties of daily activities among three groups for the pretest measures. The score of arthritic stiffness was significantly different among groups after controlled for covariates, but the post-hoc tests showed no group differences. Conclusion: The nursing interventions applied for osteoarthritis patients have been previously tested to show the cost effectiveness of symptoms management of this population.
Purpose: The study was conducted to identify factors influence suicidal ideation among older adults with osteoarthritis. Methods: This study used a cross-sectional design with secondary analysis of the Korean National Health and Nutrition Examination Survey. Data for 174 men and 939 women who aged${\geq}65$, and were diagnosed osteoarthritis from the Korea National Health and Nutrition Examination Survey 2010~2012 were included. Suicidal idea, age, sex, education, marital status, living alone, income, body mass index, smoking and alcohol use, exercise, perceived health status, activity limitation, joint pain and stiffness, depressive mood, and perceived stress were measured. Influencing factors and odds ratios were analyzed using complex sample multivariate logistic regression. Results: In the results, 29.9% of the elderly with osteoporosis had suicidal idea. The adjusted odds ratios (ORs) of suicidal ideation were significantly higher among those over 80 years older (OR=2.88, 95% CI=1.49~5.58), those with depression (OR=4.01, 95% CI=2.66~6.04), and those with high levels of stress (OR=2.76, 95% CI=1.79~4.24). Conclusion: Therefore, older adults with osteoarthritis should be screened for depression and stress to decrease suicidal ideation and prevent suicide attempt.

Cephalic tetanus is a rare subtype of tetanus in which trismus is a charateristic symptom. The paralysis of one or more cranial nerves can occur. The 7th cranial nerve is most frequently involved. It account for 1 to 3% of the tetanus and has a mortality of 15 to 30%. The incubation period is 1 to 14 days, and approximately two thirds of tetanus cases progress to generalized tetanus. Generally, the symptoms of cephalic tetanus can include : facial pain, trismus, dysphagia, muscle twitching spasms of the face and jaw (risus sardonicus), neck stiffness and malaise. We present a case of cephalic tetanus who 54-year male patient had trismus and dysphagia. There was no history of trauma. As there was a delay in diagnosis of cephalic tetanus, respiratory disorder and intermittent general spasm occurred. The patient was treated by injection of antibiotics, muscle relaxant, and human anti-tetanus immunoglobulin. His symptoms were disappeared, and he was discharged ambulatory.

Stiffness of the posterior deltoid is as a causative factor in the limited range of glenohumeral horizontal adduction and various other shoulder pathologies including shoulder impingement syndrome, frozen shoulder, and humerus anterior glide syndrome. The purpose of this study was to compare the effects of two techniques (soft tissue massage and cross-body stretch) on increasing the range of horizontal adduction. Thirty-two subjects with a $10^{\circ}$ or greater difference between the right and left sides in horizontal adduction were selected. Sixteen subjects from each group were allocated randomly. Interventions were applied on six occasions for 2 weeks, and the range of horizontal adduction was measured using an inclinometer at pre-and post-intervention. A $2{\times}2$ analysis of variance (intervention${\times}$time) was used to compare the effects of the two techniques. In the soft tissue massage group, the angle of horizontal adduction significantly increased compared with the cross-body stretch group. These findings indicate that the soft tissue massage of the posterior deltoid muscle is a more effective method to increase the flexibility of the glenohumeral horizontal adduction.
The objectives of this work here focus on the differences in responses to multiple cyclic tests of different sections along the length of the same tendon. Tendon specimens were obtained from the hindlimbs of canines and frozen to -70$^{\circ}C$. After thawing, specimens were mounted in the immersion bath at room temperature (22$^{\circ}C$) , preloaded to 0.13 N and then subjected to 3% or 4% of the initial length at a strain rate of 5%/sec. It was found that different sections of the same long tendons had different resistances to deformation. In general, the bone end sections were stiffer and carried greater loads for a given strain than the muscle end sections, and the mid-portions were the least stiff and carried the smallest loads for a given strain. The results of this study offer new information about the mechanical responses of collagenous tissues. We know more about their responses to multiple cyclic extensions and how their responses are different from the positions along the length of the tendon specimen. The nature and causes of these differences in the stiffness are not fully known. However, it is clear that differences in the mechanical response of tendons and other connective tissues are significant to musculoskeletal performance.

The stop consonants in Korean are classified into three types according to the manner of articulation as unaspirated (UA), slightly aspirated (SA) and heavily aspirated (HA) stops. Both the UA and the HA types are always voiceless in any environment. Generally, the voice onset time (VOT) could be measured spectrographically from release of consonant burst to onset of following vowel. The VOT of the UA type is within 20 msec of the burst, and about 40-50 msec in the SA and 50-70 msec in the HA. There have been many efforts to clarify properties that differentiate these manner categories. Umeda, et $al^{1)}$ studied that the fundamental frequency at voice onset after both the UA and HA consonants was higher than that for the SA consonants, and the voice onset times were longest in the HA followed by the SA and UA. Han, et $al^{2)}$ reported in their speech synthesis and perception studies that the SA and UA stops differed primarily in terms of a gradual versus a relatively rapid intensity build-up of the following vowel after the stop release. Lee, et $al^{3)}$ measured both the intraoral and subglottal air pressure that the subglottal pressure was higher for the HA stop than for the other two stops. They also compared the dynamic pattern of the subglottal pressure slope for the three categories and found that the HA stop showed the most rapid increase in subglottal pressure in the time period immediately before the stop release. $Kagaya^{4)}$ reported fiberscopic and acoustic studies of the Korean stops. He mentioned that the UA type may be characterized by a completely adducted state of the vocal folds, stiffened vocal folds and the abrupt decreasing of the stiffness near the voice onset, while the HA type may be characterized by an extensively abducted state of the vocal folds and a heightened subglottal pressure. On the other hand, none of these positive gestures are observed for the SA type. Hong, et $al^{5)}$ studied electromyographic activity of the thyroarytenoid and posterior cricoarytenoid (PCA) muscles during stop production. He reported a marked and early activation of the PCA muscle associated with a steep reactivation of the thyroarytenoid muscle before voice onset in the production of the HA consonants. For the production of the UA consonants, little or no activation of the PCA muscle and earliest and most marked reactivation of the thyroarytenoid muscle were characteristic. For the SA consonants, he reported a more moderate activation of the PCA muscle than for the UA consonant, and the least and the latest reactivation of the thyroarytenoid muscle. Hong, et $al^{6)}$ studied the observation of the vibratory movements of vocal fold edges in terms of laryngeal gestures according to the different types of stop consonants. The movements of vocal fold edges were evaluated using high speed digital images. EGG signals and acoustic waveforms were also evaluated and related to the vibratory movements of vocal fold edges during stop production.
Tetanus is fatal neurological disease caused by Clostridium tetani on contaminated wound that is characterised by muscle spasm, muscle pain, and autonomic dysfuction. C. tetani exist on contaminated wound frequently that developed clinical tetanus under low oxygen condition. Tetanus have four symptomatic form: generalized, localized, cephalic, and neonatal. The incubation period is about 7 days and mortality is high. The commonest presenting symptom is trismus and other is stiffness of neck and back(opisthotonos), muscle spasm, dysphagia, facial pain, risus sardonicus. Trismus is primary presenting symptom in 50~75% of the cases and this have high possibility of initial visit to dental office. This case report of a patient who visit in our department with trismus as chief complaint.

Tetanus is rare in Korea due to the introduction of vaccination programs and the advancement in public health. Its common signs are trismus, voice disturbance, neck stiffness, and difficulty in swallowing, etc. A 56 years old man was injured by grasping a high voltage electric cable. After the accident, he fell down on a steel plate and had a head trauma. When he visited Emergency Department, there was multiple electric burn wound on left arm and left facial area. He was hospitalized on a department of neurosurgery, because intra cranial hemorrhage was presumed. 12 days later, he was referred to department of OMFS with developed painful masseter spasms and trismus. That night he violently bit his tongue with his denture. Because masseter muscle and temporal muscle constriction was involuntary, tongue was lacerated and denture was distorted. At first we supposed that the symptom was related with neurologic disturbance following head trauma or electric shock. But it was revealed that trismus was caused by tetanus on an electrophysiological test. By using mechanical ventilation and administration of tetanus immunoglobulin, muscle-relaxant, and sedatives at ICU, symptoms had subsided (4-weeks). Because tetanus is rare disease, we rarely suppose tetanus infection to be a cause of a trismus. Especially it is more difficult to diagnose in patient who has head trauma, burn and neurologic problem as in this case.

Purpose: This study compared the effects of Static stretching and Mulligan's Two-leg rotation about the Hamstring flexibility, Hip range of motion, and pain. Methods: The subjects were allocated randomly into two groups: Static stretching group (n=13) and Mulligan's two leg rotation group (n=14). The study was designed with stretching protocols for four minutes and thirty seconds per day five times a week for three weeks in total. Measurements, including the Sit and reach test, Active/Passive Straight leg raise, and Visual analogue scale, were conducted before and after the intervention. The data were analyzed using a paired t-test and independent t-test. Results: The flexibility of the two leg rotation group was higher than the static stretching group after the intervention. The post-sit and reach test value of the two leg rotation group was significantly higher than the pre-sit and reach test value of the static stretching group. In addition, the variance of the sit and reach test of the two leg rotation group was significantly higher than that of the static stretching group. Conclusion: These results showed that two leg rotation techniques have a positive effect on the changes in the sit and reach test and active straight leg raise test. Two leg rotation techniques can be recommended as a self-stretching and easier way to stabilize the lumbopelvic rhythm, reduce the stiffness of the muscle and relieve pain. This is effective in preventing muscle damage, enabling muscle relaxation and reducing the risk of injury to the spine during daily lives and in sports activities.

Purpose : Low back pain (LBP) is reported as a risk of experiencing musculoskeletal disorders due to muscle stiffness and hypokinetics. The lumbar spine in an unstable state causes imbalance and lumbar instability. Therefore, This study examined the effects of lumbar stabilization exercise and self-complex exercise program on pain, function, psychosocial level, static balance ability, and transverse abdominal muscle (TrA) thickness and contraction ratio in patients with lumbar instability. Methods : The design of this is a randomized controlled trial (RCT). Twenty-six LBP patients participated in this study. Screening tests were performed and assigned to the experimental group (n=13) and control group (n=13) using a random allocation program. Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the self-complex exercise program. All interventions were applied three times per week for four weeks. The quadruple visual analog (QVAS), the Korean version of the Oswestry disability index (K-ODI), Korean version of fear-avoidance belief questionnaire (FABQ), static balance ability, TrA thickness, and contraction ratio were compared to evaluate the effect on intervention. Statistical significance was set at 𝛼=.05. Results : Both groups showed significant differences before and after the intervention in QVAS, K-ODI, FABQ, static balance ability, and TrA thickness in contraction (p<.05). In addition, significant differences in K-ODI and FABQ were observed between the experimental group and control group (p<.05). Conclusion : A lumbar stabilization exercise and self-complex exercise program resulted in reduced dysfunctions, psychosocial stability in patients with lumbar instability. Therefore, Lumbar stabilization exercise and self-complex exercise program for patients with lumbar instability are effective method with clinical significance in improving the function and psychosocial stability.
