Treatment Experience of Pulsed Radiofrequency Under Ultrasound Guided to the Trapezius Muscle at Myofascial Pain Syndrome -A Case Report-

  • Trigger point injection treatment is an effective and widely applied treatment for myofascial pain syndrome. The trapezius muscle frequently causes myofascial pain in neck area. We herein report a case in which direct pulsed radiofrequency (RF) treatment was applied to the trapezius muscle. We observed that the RF treatment produced continuous pain relief when the effective duration of trigger point injection was temporary in myofascial pain.

측두하악장애와 경부근육 압통 간의 상관성 (Association Between Temporomandibular Disorders and Cervical Muscle Pressure Pain)

  • 목적: 측두하악장애 환자들에서 압통 검사를 통하여 경부근육 통증의 정도와 위치를 파악하고, 측두하악장애와 경부근육 통증과의 관련성을 규명하고자 하였다. 방법: 측두하악장애 환자(n=129, 여자 65.9% 평균=28.8세)에 대해서 두통, 목의 통증, 정서적 스트레스, 수면 장애, 이상 기능 습관 및 통증 강도에 대한 설문을 실시하였다. 저작계에 대하여 하악 운동범위, 악관절음, 악관절 촉진, 저작근 촉진 검사를 시행하였다. 빗목근 상부(sternocleidomastoid upper), 빗목근 중간(sternocleidomastoid middle), 등세모근 상부(upper trapezius), 머리널판근(splenius capitis), 머리반가시근(semispinalis capitis), 중간 목갈비근(scalene medius), 어깨올림근(levator scapulae)의 7부위의 경부 근육에 대하여 촉진에 의한 압통 검사를 하였다. 압통의 정도를 무통(0), 경도(1), 중등도(2), 심도(3)로 구분하여 판정하였다. 압통점수로부터 여러 통증점수 합계를 계산한 후 이후의 통계분석에 사용하였다. 결과: 80명(62.0%)의 환자가 설문에서 목의 통증을 경험한다고 답하였다. 측두하악장애 통증 점수와 경부 근육통 점수 간에는 유의한 상관관계가 있었다(r=0.538, P < 0.001). 경부 근육 중에서 중등도 이상의 압통이 40% 이상 발생하는 근육은 빗목근과 등세모근 상부였고 저작근 중에서는 깨물근(masseter) 중간에서 36%의 중등도 통증이 나타났다. 129명에 대한 경부근육통점수합과 측두하악장애통증점수합 사이에는 상당한 관련성이 있었으며($\rho$=0.502, P < 0.001), 측두하악장애통증점수합은 경부근육통점수합이 증가함에 따라 함께 증가하는 경향을 보였다(Y = 0.395 ${\cdot}$ X, $R^2$ = 0.659, P < 0.001). 저작근장애 환자에서 빗목근등세모근상부통증점수합과 관자근교근통증점수합은 중등도의 관련성($\rho$ = 0.375, P < 0.001)을 보였으며, 두 변수는 비례 관계에 있었다(Y = 0.359 ${\cdot}$ X, $R^2$ = 0.538, P < 0.001). 편측통증점수의 편상관관계분석에서 우측경부근육통증점수합과 좌측경부근육통증점수합은 가장 높은 상관성(r=0.802, P < 0.001)을 보였다. 우측측두하악장애통증점수합과 좌측측두하악장애통증점수합은 중등도의 상관성(r=0.481, P < 0.001)이 있었다. 편측성 측두하악장애 통증이 있는 20명의 환자에 대한 편상관관계분석에서 우측과 좌측의 편측경부근육통점수간의 상관성이 가장 높았고(r = 0.597, P = 0.009), 측두하악장애측통증점수합과 동측경부근육통증점수합 사이의 상관성(r = 0.564, P = 0.015)이 그 다음이었다. 결론: 측두하악장애 통증은 촉진에 의한 압통반응의 측면에서 경부근육 통증과 관련성을 보인다. 경부근육 중에서 빗목근과 등세모근상부가 중등도 이상의 통증을 흔하게 나타내며 저작근통증과 밀접한 관련이 있다. 경부근육에서는 통증의 대칭적인 이환 특성이 두드러지지만, 측두하악장애가 경부근육통의 수준에 영향을 주어 대칭적인 특성을 변화시킬 수 있다. 두부와 경부에 복잡한 통증 질환의 증상과 징후를 보이는 환자에서 경부 근육 통증의 진단과 치료에 관심을 가져야 한다.

  • Objective: A single-patient case study on the use of OCNT for cervical spinal stenosis. Methods: A50-year-old Korean male with frequent leg muscle cramps and severe muscle weakness was treated with OCNT. Results: After OCNT, pain and cramps disappeared, and muscle strength improved to the point of no longer hindering daily activities. Conclusion: OCNT can be beneficial in alleviating symptoms of pain and muscle weakness in patients with cervical spinal stenosis.

라인댄스가 폐경 이후 여성의 근력, 균형감, 유연성, 통증 및 기분상태에 미치는 효과 (The Effects of Line Dance Program on Muscle Strength, Balance, Flexibility, Pain, and Mood State in Post-menopausal Women)

  • Purpose: The purpose of this study was to examine the effects of line dance program on muscle strength, balance, flexibility, pain, and mood state in post-menopausal women. Methods: A quasi-experimental research design (one group pretest-posttest) was employed. A total of 13 post-menopausal women completed 12-week line dance program(60min/day, 2days/wk) in S city. Descriptive statistics, Wilcoxon signed rank test were used in the data analysis by SPSS 18.0. Results: Muscle strength and balance were higher, and pain level were lower than before the line dance program. However, flexibility and mood state were not significantly changed after the line dance program. Conclusion: Findings of this study indicated that the 12-week line dance program were effective on muscle strength, balance, and pain relief in the post-menopausal women. Future study is needed to explore the effect of the line dance program on flexibility and mood state with the post-menopausal women.

Effects of Rehabilitation Exercise Combined with Electrical Muscle Stimulation on Pain, Muscle Strength, and Function in Soldiers Undergoing Knee Meniscectomy

  • Objective: Meniscal injuries are a common and high-risk condition among military personnel, leading to difficulties in performing missions.The objective of this study is to investigate the effects of combining electrical muscle stimulation therapy with exercise therapy during rehabilitation on pain, muscle strength, and function in patients after meniscectomy. Design: A two-group pretest-posttest design Methods: A total of 30 subjects were included in this study. They were randomly assigned to either the experimental group (n=15), which received knee extensor strengthening exercise along with electrical muscle stimulation, or the control group (n=15), which received only knee extensor strengthening exercise. Pre-test was conducted prior to the intervention, which consisted of 30 minutes of treatment five times a week for a total of 20 sessions. Post-test was performed after a 4-week period. Pain, strength, and function were assessed before and after the intervention. Results: The results of the study showed that there was a significant difference in pain reduction and muscle strength improvement in the experimental group, and a significant difference was also found between the experimental group and the control group in terms of functional evaluation. Conclusions: The combination of exercise therapy and electrical muscle stimulation therapy resulted in greater improvements in pain, strength, and function assessment, contributing to improved overall function.

대퇴신경통에 관한 연구 (Studies on Femoral Neuralgia)

  • Most patients who have suffered from pain and muscle weakness on femoral nerve distributing area show no definite pathologic finding on X-ray or laboratory examinations. Therefore proper diagnosis is difficult to determine for the proper treatment of the symptoms. Based on my clinical experiences and anatomical studies, I have found most of these symptoms are a result of femoral nerve compression on trigger point of psoas major muscle. Accordingly, releasing the compression of femoral nerve by Laser stimulation and local anesthetic injection to the identified trigger point of psoas major muscle was found to be an effective treatment for femoral neuralgic pain.

지연성근육통 예방을 위한 준비운동과 정리운동이 통증과 근활성도에 미치는 효과 (Effects of Warm-up and Cool-down Exercises for Preventing Delayed Onset Muscle Soreness on Pain and Muscle Activation)

  • The purpose of this study was to determine the effects of warm-up and cool-down exercises on pain and muscle activation of delayed onset muscle soreness after intense exercise. Delayed onset muscle soreness was caused by the eccentric exercise in the elbow flexor muscle of the non-dominant upper limb. Forty-four subjects volunteered to participate in this study and were randomly assigned to one of the following groups: warm-up and cool-down group, only warm-up group, only cool-down group, or control group with no intervention. The level of perceived pain using the visual analogue scale and electromyographic activation change in maximal voluntary isometric contraction were measured 4 times at the following times: 10 min, 24 hr, 48 hr, and 72 hr after the exercise. The results revealed the main effect between the groups and interaction effect between the group and measurement session (p<.05). The warm-up and cool-down group showed most favorable results with respect to reduced perceived pain level and increased muscle strength in most measurement sessions, and the only warm-up group showed significantly more decreased pain level than the control group at 24 hr and 48 hr and more increased muscle activation than the cool-down group at 48 hr (p<.05). However, there were no significant differences in pain level and muscle activation between the only cool-down group and control group at all measurement sessions (p>.05). The findings suggest that the warm-up exercise performed before an intense exercise had beneficial effects on the symptoms of delayed onset muscle soreness, whereas cool-down exercise performed after the intense exercise did not.

스트레칭 기법에 따른 뒤넙다리근의 유연성, 근력, 압통 역치, 근 긴장도에 미치는 효과 비교 (A Comparison of The Effect of Stretching Technique on Hamstring Muscle for Flexibility, Strength, Pressure Pain Threshold Value and Muscle Tone)

  • PURPOSE: The effectiveness of the stretching to increase the range of motion of the joint, reduce the risk of injury, enhance athletic performance or decrease post exercise muscle soreness. This study was to compare the effect of the stretching method applied on the shortened hamstring muscle for flexibility, strength, pressure pain threshold value and muscle tone. METHODS: This study is a two-group pretest- posttest design. Fifty-four healthy young adults were randomly assigned to a vibration-assisted stretching group (VASG, n=27) or a static stretching group (SSG, n=27). Participants performed each stretch in 4 sets of 30 seconds each. A 30-second break time was provided between the sets. The range of motion, strength, pressure pain threshold values and muscle tone of the hamstring muscle were measured to compare the effects of the stretching methods. RESULTS: Both the VASG and the SSG participants showed significant improvement in the range of motion and strength (p<.05); however, the increase in the VASG was significantly higher than that in the SSG (p<.05). The pressure pain threshold values and muscle tone were significantly decreased only in the VASG (p<.05). CONCLUSION: These findings indicate that vibration-supported stretching is an effective intervention for people with hamstring shortening, with high pain level and muscle tone decrease.

씹기근육 수축을 동반한 머리목굽힘운동이 목통증 환자의 깊은목뼈굽힘근의 수축두께와 목장애지수에 미치는 영향 (Effect of Craniocervical Flexion Exercise with Masticatory Muscle Contraction on Deep Cervical Muscle Thickness and Neck Disability Index in Patient with Neck Pain)

  • PURPOSE: The purpose of this study was to determine the effect of craniocervical flexion exercise with masticatory muscle contraction in patients with neck pain. Patients with neck pain also experience muscle weakness and limitation of activities of daily living. Craniocervical flexion exercise with masticatory muscle contraction may increase the thickness of the deep cervical flexor muscles, thereby reducing pain and limitations on activities of daily living. METHODS: Twenty-six volunteers participated in this study. The subjects were randomly divided into an experimental group (n=13) and a control group (n=13). Both groups performed craniocervical flexion exercise using a pressure biofeedback unit; the experimental group performed craniocervical flexion exercise with masticatory muscle contraction for 20 minutes, 3 times a week for 4 weeks. Assessment tools included ultrasonography for measurement of muscle thickness and the Neck Disability Index for the level of pain and function. RESULTS: The rate of change in muscle thickness in both groups significantly increased (p<.05), with a significantly greater increase in the experimental group than in the control group (p<.05). The Neck Disability Index score significantly improved (p<.05) in both groups, with significantly greater improvement in the experimental group compared with the control group (p<.05). CONCLUSION: These results suggest that craniocervical flexion exercise with masticatory muscle contraction can be effective in increasing muscle thickness and improving the Neck Disability Index score in patients with neck pain.

경근 이완 기법과 그라스톤 기법이 어깨 관절 근막 통증 증후군 환자의 통증 및 기능 향상에 미치는 효과 (Effect of Meridian Muscle Release and the Graston Technique on Pain and Functional Movement in Patients with Myofascial Pain Syndrome of the Shoulder Joint)

  • PURPOSE: This study examined the effects of the meridian muscle release technique on the pain and functional movement of patients with myofascial pain syndrome (MPS) of the shoulder joint. METHODS: The subjects of this study included 45 patients with MPS of the shoulder joint. The subjects were divided randomly into the following groups; the meridian muscle release technique group (n=15), the Graston technique group (n=15), and the control group (n=15). Both the meridian muscle release group and the Graston technique group received conventional therapy for 35 minutes initially and were then treated using the meridian muscle releases technique and Graston technique for 10 minutes, respectively. The control group received only conventional therapy for 35 minutes. All three groups underwent treatments three times a week for four weeks. Each subject was evaluated randomly using the VAS, PPT, SPADI and ROM both before and after treatment. RESULTS: The Graston technique group showed a significantly more substantial increase in functional movement (p<.05) than the meridian muscle release technique and control groups. The meridian muscle release technique group had significantly less pain (p<.05) compared to the Graston technique and control groups. CONCLUSION: These findings suggest that the meridian muscle release technique can be useful for decreasing pain and increasing the functional movement of patients with MPS of the shoulder joint.
