운동치료가 급성요통환자의 통증감소와 근력에 미치는 영향 (Effects of Exercise Therapy on Pain Control and Muscle Strength in Acute Low Back Pain Patient)

  • Objectives : In these days Low back pain is most common, and the weakness of muscle strength is important factor. The aim of this study is to prove the influence of the exercise therapy on pain control and muscle strength in acute low back pain patient. Methods : Subjects of this research were 30 inpatients with acute low back pain from 2008. 3. 20 to 2008. 6. 20. 15 patients in each, 2 groups; Non exercise therapy, exercise therapy were divided and the conclusions for treatment are as follows. Results : 1. Regarding alleviation of pain by VAS, both non exercise therapy Group I and exercise therapy Group II showed significant decrease of VAS. 2. Muscle strengthening by extension and flexion peak torque, the exercise therapy Group II showed higher rate of improvement than the non exercise therapy Group I in extension peak torque. 3. Muscle strengthening by extension and flexion peak torque ratio, the exercise therapy Group II showed higher rate of improvement and distribution approach to normal ratio than the non exercise therapy Group I. Conclusions : Exercise therapy was effective in muscle strengthening of acute low back pain.

지연성 근육통 유발 후 유지-이완 기법이 통증과 관절가동범위에 미치는 영향 (The Effects of Hold-relax Techniques after Inducing Delayed Muscle Pain on the Pain and Range of Motion of Joints)

  • Purpose: The purpose of the present study was to compare the effects of both general hold-relax techniques and hold-relax techniques using pumping on pain due to delayed muscle pain and on the range of motion (ROM) of joints. Methods: Thirty-nine young adult males and females were randomly assigned either to a hold-relax technique application group of 20 subjects or to a group of 19 subjects with hold-relax techniques applied using a pumping application. Tenderness thresholds, the ROM of joints, and pain intensities were measured on the biceps of the nondominant arm of the subjects in both groups before exercises were performed to induce delayed muscle pain. Tenderness thresholds were again measured 24hours, 48hours, and 72 hours after inducing delayed muscle pain. The relevant intervention methods were applied to the two groups after conducting the measurement at 48hours. As a statistical analysis method, repeated measure ANOVAs were conducted to examine the tenderness thresholds, ROMs of elbow joints, and pain intensities in the individual groups at the time points. Results: At 48 hours and 72 hours after application of the interventions, the general hold-relax technique application group showed greater changes in the tenderness thresholds, the ROMs of elbow joints, and the pain intensities than did the group applying hold-relax techniques using a pumping application (p>0.05). Conclusion: Based on the results of the present study, the application of hold-relax techniques is thought to have beneficial effects on pain resulting from delayed muscle pain and on limited ROMs of joints.

만성요통환자의 한발서기 시 체간 안정화 근육의 활성화 변화 (Trunk Stabilization Muscle Activity in Chronic Low Back Pain Patients during One Leg Stance)

  • Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).

족삼양경근(足三陽經筋)의 근육학적(筋肉學的) 고찰(考察) (A Study on Muscular System of Foot Three Yang Meridian-Muscle)

  • Objectives : This study was performed to understand the interrelation between 'Foot three yang meridian-muscle' and 'muscular system'. Methods : We have researched some of the literatures on Meridian-muscle theory, anatomical muscular system, myofascial pain syndrome and anatomy trains. And especially we have compared myofascial pain syndrome to anatomy trains and researched what kind of relationship is exist between them. Results : It is considered that Foot taeyang meridian-muscle includes Abductor digiti minimi m., Gastrocnemius m., Biceps femoris m., Longissimus m., Omohyoid m., Occipital m., Frontal m., Orbicularis oculi m., Trapezius m., Sternocleidomastoid m., Sternohyoid m., Zygomaticus m. Foot soyang meridian-muscle includes Dorsal interosseus m., Tendon of extensor digitorum longus m., Extensor digitorum longus m., Iliotibial band, Vastus lateralis m., Piriformis m., Tensor fasciae latae m., Internal abdominal oblique m., External abdominal oblique m,, Internal intercostal m., External intercostal m., Pectoralis major m., Sternocleidomastoid m., Posterior auricular m., Temporal m., Masseter m., Orbicularis oculi m. Foot yangmyung meridian-muscle includes Extensor digitorum longus m., Vastus lateralis m., Iliotibial band, Iliopsoas m., Anterior tibial m., Rectus femoris m., Sartorius m., Rectus abdominis m., Pectoralis major m., Internal intercostal m., External intercostal m., Sternocleidomastoid m., Masseter m., Levator labii superioris m., Zygomatic major m., Zygomatic minor m., Orbicularis oculi m., Buccinator m. and the symptoms of Foot three yang meridian-muscle are similar to the myofascial pain syndrome. Superficial back line in anatomy trains is similar to the pathway of Foot taeyang meridian-muscle. Lateral Line in anatomy trains is similar to the pathway of Foot soyang meridian-muscle. Superficial Front Arm Line in anatomy trains is similar to the pathway of Foot yangmyung meridian-muscle. Conclusions : There is some difference between myofascial pain syndrome and meridian-muscle theory in that the former explains each muscle individually, while the latter classifies muscular system in the view of integrated organism. More studies are needed in anatomy and physiology to support the integration of muscular system of Foot three yang meridian-muscle in aspect of anatomy trains.

목빗근에 대한 허혈성 압박법이 위등세모근의 만성 통증에 미치는 영향 (Effect of Ischemic Compression on Sternomastoidcleido Muscle on Chronic pain in Upper Trapezius Muscle)

  • 최근 스마트폰 사용 등과 같은 이유로 잘못된 자세가 발생하고 있다. 이로 인하여 근육들의 불균형 단축이 발생하고 있는데 특히 목빗근이 단축되면 더부신경이 압박되어 위등세모근의 허혈성 통증을 일으킬 수 있다. 그리하여 본 연구는 목빗근 1/3지점의 더부신경 포착점에 허혈성 압박법을 중재하여 위등세모근의 통증에 미치는 영향을 알아보았다. 본 연구는 여성 8명을 대상으로 4주간 주 2회 중재하였고, 중재 전, 후 그리고 중재 종료 3주 후 평가하였다. 대상자들의 통증을 알아보기 위해 시각적 사상 척도와 맥길 통증 설문지 그리고 압력 통각계를 사용하였다. 본 연구 결과 치료 후 통증이 유의하게 감소한 것을 확인하였으며, 중재 종료 3주 후 역시 통증 감소에 효과가 있는 것을 확인하여 목빗근의 더부신경 압박점의 중재가 위등세모근의 통증 감소와 지속성이 있음을 확인하였다. 차후 연구에서 대상자를 늘려 위등세모근의 통증 감소뿐만 아니라 목의 기능개선도 확인할 필요성이 있을 것이다.

요통환자군과 정상대조군 사이의 표면근전도값 비교에 관한 연구문헌 고찰 (A Review of Studies Comparing of Surface Electromyography Values between the Low-back Pain Group and Healthy Controls)

  • Objectives To present reviews of studies comparing surface-electromyography (SEMG) values between low back pain group and control group. Methods We searched 8 databases including KoreaMed, Google, KISS (Korean studies Information Service System), RISS (Research Information Sharing Service), OASIS (Oriental medicine Advanced Searching Integrated System), Pubmed, Ovid-MEDLINE and EMBASE. After searching, we conducted study selection by using inclusion and exclusion criteria and quality-assessment. We reviewed the selected studies concerning about the subject's measuring position, findings, sensitivities and specificities. Results 27 Studies were searched and reviewed. In static surface electromyography, more muscle activities observed in low back pain subjects than in controls. In dynamic surface electromyography, the low back pain subjects showed more muscle activites during flexion, while the control group showed more muscle activities during extension. Faster muscle fatigue observed in isometric muscle analysis. Conclusions Surface electromyography values will be able to be objective marker for evaluating low back pain. Further research is needed to determine additional unified protocol such as the type of SEMG and its directions.

근통증이 유발된 흰쥐에 있어 TENS와 냉적용이 이차성 열 통각과민에 미치는 영향 (The Effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain)

  • The aim of this study was to investigate the effects of TENS and cold application on secondary thermal hyperalgesia in rats induced by muscle pain. Muscle pain was induced in male Sprague-Dowley rats by intra-muscular injection of gastrocnemius with $3\%$ carrageenan. The paw withdrawal latency(PWL) and tail flick test(TFT) to heat were used to detect secodary thermal hyperalgesia induced by the muscle pain. PWL and TFT were quantified before and 4, 10, and 24 h after induction of muscle pain and after application of TENS(100Hz, $100{\mu}s$, sensory intensity) and cold($4^{\circ}C$). TENS and cold significantly reduced the PWL and TFT to heat stimuli when compared with controls receiving no TENS and cold(p<.05). These results suggested that application of TENS and cold attributed to decrease secodary thermal hyperalgesia in rat induced by muscle pain.

일반적인 자세가 요통에 미치는 영향에 대한 고찰 (Posture and Low Back Pain)

  • Main cause of low back pain is a poor posture. Most low back pains are due to a poor posture. The poor posture induces muscle tension and finaliy low back pain. The poor posture arehabitually trained from the childhood by the environmetal factors. In general, maintaining good posture during working and sleeping hours are the first line of defence against back pam. (1) Supine posture is the easiest posture that relaxes and fixes muscles. Supine posture is thus a starting position for on exercise. Lying down releases the weight pressures of head and shoulder and thus body can be relaxed and extended which are helpful for treating back pain. However, supine posture can increase the pressure in ribcage posture aspect and disphragram due to visceral oragans. (2) Sitting in one position for a long time results in fatigue and relaxation of spinal muscies. Finally, body strength is weakened and sitting posture will become poor. If this poor posture continues for a longer time, pain will be accompanied due to overelongation of muscle ligaments. The habitual poor posture could induce intervertebral disc distortion. If the intervertebral disc is damaged, sitting in one position or movement causes pain. (3) Abnormal lumbar curve induces the tention of abdominal muscle and paravertebral muscle groups as well as tention of lower limb muscle group connected to pelvis. For a person with weak body strength, muscle relaxation increases curvature in lumbar, chest and cervical regions. This will induce a pelvic anterior tilting of the imaginary line between A. S. I. S. and P. S. I. S. Hip joint extensor muscle acts on releasing the pelvic anterior tilting. Contrections of hamstring muscle and femoral muscle recover the imaginary line between A.S.LS. and P.S.I.S. from pelvic anterior tilting. thus, contraction of rectus abdominis muscle are required to maintain the normal lumbar curve.

승모근과 전거근 강화운동이 기능적 어깨충돌증후군을 가진 척수손상 환자의 통증과 근 활성도에 미치는 영향 (The Effects of Trapezius and Serratus Anterior Strengthening Exercise on Pain and Muscle Activation in Spinal Cord Injury Patients with Functional Shoulder Impingement Syndrome)

  • Purpose: The aim of this study was to investigate the effects of trapezius and serratus anterior strengthening exercise on the shoulder pain and muscle activation of patients with spinal cord injury and functional shoulder impingement syndrome. Methods: The study consisted of 10 patients with spinal cord injury who were hospitalized in Rehabilitation Hospital U, Uijeongbu, South Korea. The exercise was implemented three times a week for 10 weeks. In each session, the subjects performed one of a total of five types of exercise at mid-level intensity. The shoulder pain and disability index (SPADI) was used to evaluate the patients before and after the intervention. The muscle activation of the upper trapezius, middle trapezius, lower trapezius, and anterior serratus muscle was assessed by surface electromyography (EMG) at the beginning of the experiment and 10 weeks later. Wilcoxon's singed-rank test was conducted to determine differences in the pain index and muscle activation before and after the exercise. The level of statistical significance was set at ${\alpha}=0.05$. Results: SPADI scores significantly decreased after the exercise (p<0.05). In comparisons of muscle activation, there was a significant improvement in the upper trapezius at $60^{\circ}$ shoulder joint flexion (p<0.05). There was no significant improvement at $90^{\circ}$ shoulder joint flexion. The middle trapezius showed a significant improvement at $120^{\circ}$ shoulder joint flexion (p<0.05). Conclusion: Trapezius and serratus anterior strengthening exercise reduced pain in spinal cord injury patients with functional shoulder impingement syndrome. The decreased muscle activation of upper trapezius and increased muscle activation of the anterior serratus muscle at $60^{\circ}$ shoulder joint flexion point to positive effects of the exercise on supraduction of the scapula.

수태양소장경근(手太陽小腸經筋)에 대한 근육학적(筋肉學的) 고찰(考察) (The study of muscular system about small intestine channel of hand taiyang muscle)

  • We have conclusions after the study of muscular system about small intestine channel of hand taiyang muscle. Judging from many studies of interrelation between Meridian muscle and muscle, it is considered that Meridian muscle theory has some similarities with modern anatomical muscular system. It is considered that Small intestine channel of hand taiyang muscle contains Flexor digitorum profundus muscle, Extensor digiti minimi muscle, Abductor digiti minimi muscle, Extensor carpi ulnaris muscle, Flexor carpi ulnaris muscle, Triceps brachii muscle, Infraspinatus muscle, Levator scapulae muscle, Sternocleidomastoid muscle, Masseter muscle, Temporalis muscle. The symptoms of small intestine channel of hand taiyang muscle is similar to referred pain of modern Myofascial Pain Syndrome, and the medical treatment of "I Tong Wi Su(以痛爲輸)" is also similar to that of Myofascial Pain Syndrome. Small intestine channel of hand taiyang muscle is one of the three yang channels of hand muscle, and it has unity in extension of upper limb and trunk in the movement. And it is thought that weakness of small intestine channel of hand taiyang muscle is related with muscular system causing Round Shoulder and Head Forward Position.
