본 연구는 어깨뼈 설정 운동과 몸통 교정 테이핑 융합이 뇌졸중 환자의 몸통 근활성도, 몸통 균형 및 팔 기능개선에 효과적인지 알아보고자 하였다. 뇌졸중환자 20명을 연구군(테이핑+어깨뼈 설정) 10명과 대조군(어깨뼈 설정) 10명으로 나눈 뒤 4주간 주당 30분의 중재를 실시하였다. 연구군은 몸통 교정 테이핑을 적용받았고 동시에 어깨뼈 설정운동을 받았다. 측정은 몸통 근활성도, 몸통손상척도(Korean version Trunk Impairment Scale, K-TIS), 팔 기능검사 (Manual Function Test of upper extremity, MFT)를 실시하였다. 중재 전 후 연구군에서 몸통 근활성도, K-TIS, MFT 점수에 유의한 증가를 보였고(P<.05), 대조군 보다 K-TIS와 MFT 점수에 유의한 개선을 보였다(P<.05). 본 연구를 통해 어깨뼈 설정 단일 운동 보다 몸통 교정테이핑 융합 적용이 뇌졸중 환자의 몸통 근활성도, 몸통 균형 및 팔 기능개선에 더욱 효과적인 것을 알 수 있었다. 향후 연구에서 개별 근수축 개시시간(onset time)에 관한 변화를 확인한다면 더욱 임상적 의의가 있겠다.
본 연구는 초등학교 지적장애아동의 자세조절 특성을 파악하기 위해 시각, 안뜰감각 그리고 몸감각을 조합하여, 실험 조건에 따라 자세유지를 위하여 어떤 감각을 활용하고 있는지, 부적절한 감각자극에 대해서 통합시스템에서 어떻게 반응하는지, 외부 동요에 대해 신경근육의 자세조절 전략 시 동원되는 근육의 수축 개시시간과 개시순서를 각각 측정하여, 지적장애 아동의 자세조절 특성을 살펴보고, 나아가 초등학교 지적장애아동과 일반아동 간에 어떠한 차이가 있는지를 비교분석하여 자세조절능력 향상 프로그램에 적용 시 방향을 제시해주는데 있다. 이를 위하여 성별과 연령을 고려하여 초등학교 지적장애아동 26명과 일반아동 26명의 아동으로부터 자세조절능력을 비교하였다. 그 결과 자세조절과 감각계와의 관계를 파악한 평형점수와 감각활용 분석, 엉덩관절 전략에 의지하는 자세조절 전략, 근수축 개시지연과 근수축 개시순서의 변화 등을 종합해 볼 때 초등학교 지적장애아동의 자세조절의 문제는 운동시스템, 감각시스템, 통합시스템의 자세조절 시스템 전반에 걸친 총체적인 문제로 나타났다. 따라서 초등학교 지적장애아동의 자세조절 문제 해결을 위해서는 운동시스템, 감각시스템, 통합시스템을 포함하는 포괄적 자세조절 훈련 프로그램의 접근이 필요하다는 것을 알 수 있다. 자세조절 프로그램을 구성할 때, 다양한 감각을 제공하여 주고, 제공된 감각을 효율적으로 활용할 수 있는 감각훈련과 함께 고위중추에서 입력된 감각들을 정상적인 자세조절 발달에 활용할 수 있도록 훈련 시켜주어야 한다. 적응 반응을 계획하고 조직화하여 의미 있는 활동을 통해 중추신경계의 처리능력을 향상시키는 치료프로그램이 제공되어야 한다. 또한 자세조절을 안정적으로 할 수 있는 안정성 한계를 넓힐 수 있는 신경발달학적 치료와 함께, 근골격계에서 빠른 근수축 반응과 효율적인 근수축 개시순서를 유발 할 수 있도록 고유수용성 운동조절 프로그램, 자세반응 촉진 프로그램과 같은 훈련이 필요하다고 할 수 있다.
Objective: The purpose of this study was to investigate the relationship between anticipatory postural adjustment (APA), single task, dual tasks and physical performances. The trunk muscles of APA consist of bilateral erector spinae (ES) and bilateral internal oblique (IO) adnominal muscles, during rapid stepping with the affected or unaffected leg in a sitting posture. Design: Cross-sectional study. Methods: In patients with chronic stroke, electrodes of surface electromyography (EMG) were attached on the bilateral erector spinae (ES), bilateral internal oblique adnominal (IO), and bilateral rectus femoris (RF) muscles. RF acts as the prime mover. The stroke patients performed hip flexion until $20^{\circ}$ as fast as possible at each leg in a sitting posture according to a visual cue. The visual cue unexpectedly appeared on monitor in front of the stroke patient. The single task was the Timed Up and Go (TUG) test. The dual tasks were the TUGconitive, which increased cognitive capacity, and the TUGmanual task, which had an external focus. Results: All EMG data showed earlier onset latency before the prime mover. In affected leg raising, the onset time of unaffected ES muscle of the stroke patients was correlated with the single and dual tasks (p<0.05). In unaffected leg raising, the onset time of the affected IO muscle was related to all the tasks (p<0.05). Gait speed showed a relationship with the unaffected ES muscle only. Conclusions: The trunk muscles of the bilateral ES and bilateral IO play an important role in APA. The single and dual tasks using TUG test were correlated with the APA s of ES and IO muscles. Dual task by the TUG test is a good measuring tool for reflecting the real life in patients with chronic stroke.
Despite research to treat delayed onset muscle soreness (DOMS), no effective treatment has been reported. The purpose of this study is to investigate the effect of a hold-relax technique and cryotherapy on DOMS. Thirty-three subjects were randomly assigned to one of three treatment groups: control, hold-relax technique, or hold-relax technique and cryotherapy. DOMS was induced in the non-dominant biceps muscle through repeated eccentric contractions. Resting elbow joint position, flexion and extension (universal goniometer), pain (Visual Analogue Scale; VAS), and WBC count (blood analysis) were measured one hour before DOMS was induced and 24, 48, and 72 hours after DOMS was induced. The data were analyzed by repeated measure of two-way ANOVA. The results of this study were summarized as follows: 1) While analysis showed no significant differences between groups in relation to a resting elbow joint position, there were significant differences over time, especially at 24, 48 hours after DOMS was induced compared with resting elbow joint position before DOMS was induced. 2) While analysis showed no significant differences between groups in relation to range of flexion, there were significant differences between range of flexion before DOMS was induced and range of flexion 72 hours after DOMS was induced. 3) There were no significant differences between groups or over time in relation to range of extension. 4) While analysis showed no significant differences between groups in relation to VAS, there were significant differences over time in different hours. 5) There were no significant differences between groups or over time in relation to WBC count. 6) There were no interactions between groups or over time in all variables. This results suggested that hold-relax technique and cryotherapy were not effective to reduce DOMS.
This study examined the effects of hold-relax with agonist contraction (HR-AC) on the symptoms of delayed onset muscle soreness (DOMS) induced by intensive eccentric exercise of the non-dominant biceps brachii. Ten men (mean age=26.7 yrs, mean height=172.1 cm, mean weight=66.2 kg) and ten women (mean age=27.4 yrs, mean height=165.9 cm, mean weight=60.7 kg) who had not participated in a regular exercise program for the upper extremities in the previous six months were randomly assigned to one of two experimental groups: the HR-AC group, or the control group. We measured joint range of motion (ROM), maximal voluntary isometric contraction (MVIC), and muscle soreness before eccentric exercise, and 24, 48, and 72 hours after eccentric exercise. The subjects in the HR-AC group received the HR-AC technique in the non-dominant biceps brachii. The HR-AC technique was applied 24 and 48 hours after eccentric exercise. There was no significant difference between the HR-AC and the control group. However, the HR-AC group, compared to the control group, had a significant difference between the time points of the various parameters. Increased ROM (p<.05), decreased muscle soreness (p<.05), and reduced MVIC (p<.05) were found in the HR-AC group after 72 hours. Decreased ROM (p<.05) and MVIC (p<.05), and increased muscle soreness (p<.05) were observed in the control group. These findings suggest that the HR-AC technique effectively reduces muscle soreness and increases ROM 72 hours after eccentric exercise.
최근 코로나19 사태 이후에 건강관리에 신경을 쓰는 사람이 늘어나고 있으며, 많은 사람들이 취미활동으로 스포츠나 운동 후 근육통의 발생으로 생활에 불편함을 느끼고 운동을 쉬는 경우가 많다. 오랜만에 운동하거나, 무리해서 운동하고 나면 운동 후 24~48시간 이내에 온몸에 알이 배긴 것 같은 통증이 나타난다. 이러한 통증을 '지연성 근육통(DOMS(Delayed onset muscle soreness))'이라고 한다. 본 연구에서는 심부열을 발생시키는 고주파 통증치료기를 사용하여서 지연성 근육통의 통증을 완화에 효과가 있는지 유효성 검증을 진행하였다. 평상시, 통증치료 전, 통증치료 후의 근전도를 측정하고 분석을 통해 RMS 값을 구하고 SPSS 프로그램을 이용해서 통계분석을 진행하였고, 통계적으로 유의성이 있는 것으로 판단하였다. 그리고 평상시와 통증치료 후의 RMS 값의 통계분석을 진행하여서 유의성이 없는 것을 확인하여서 통증치료 후 얼마만큼 평상시로 회복했는지 확인하였다. 또한, 통증은 같은 통증의 크기라도 사람마다 느끼는 기준이 다르기 때문에 통증의 크기에 대한 NRS 설문지를 진행하여서 피험자들이 느끼는 통증의 크기를 SPSS 프로그램을 이용해서 통계분석을 진행하였고, 통계적으로 유의성이 있는 것으로 판단하였다. 그래서 본 연구 결과 고주파 통증치료기가 통증부위의 심부열을 발생시켜 온도를 상승시키고, 동맥 및 모세혈관을 확장시켜 혈류량을 증가시켜서 혈액순환 및 신진대사가 증가되면서 지연성 근육통의 통증이 완화되는 효과가 있는 것으로 판단된다.
This study aimed to identify the asymmetry observed in the electromyography (EMG) activity patterns of selected trunk and thigh muscles between the affected and unaffected sides during the sit-to-stand movement in ambulatory patients with post-stroke hemiparesis. This study included 20 patients with post-stroke hemiparesis. The differences between stroke fast walkers (${\geq}8m/s$, 11 subjects) and stroke slow walkers (<8 m/s, 9 subjects) were compared. The activation magnitude and onset time of the multifidus, lumbar erector spinae, hamstrings, and quadriceps during the sit-to-stand movement were recorded through surface EMG. Moreover, the EMG activation magnitude and onset time ratios of each bilateral corresponding muscle from the trunk and leg were measured by dividing the relevant values of the unaffected side by those of the affected side. In all the subjects, the activation magnitudes of the multifidus, hamstring, and quadriceps on the affected side significantly decreased compared to those on the unaffected side (p<.05). The onset time of muscle activity in the affected side was markedly delayed for the multifidus and quadriceps during the task (p<.05). The activation magnitude ratios of the quadriceps were markedly decreased in the stroke slow walkers as compared to those in the stroke fast walkers. These findings indicate that the asymmetry in the multifidus, hamstring, and quadriceps muscle activation patterns in patients with post-stroke hemiparesis may be due to the excessive muscle activation in the unaffected side to compensate for the weakened muscle activity in the affected side. Our findings may provide researchers and clinicians with information that can be useful in rehabilitation therapy.
Background: Stroke patients experience multiple dysfunctions that include motor and sensory impairments. Therefore, new intervention methods require a gradational approach depending on functional levels of a stroke patient's activity and should include cognition treatment to allow for a patient's active participation in rehabilitation. Objects: This study investigates the effect of integrated revision of electrical sensory stimulation, which stimulates somatosensory and action observation training, which is synchronized cognition intervention method on stroke patients' functions. Methods: Twenty-one stroke patients were randomized into two groups. The two groups underwent twenty minutes of intervention five times a week for three weeks. This study used an electromyogram to evaluate symmetric muscle activation of lower extremities and muscle onset time when performing sit to stand before and after intervention. A weight-bearing ratio was used to evaluate the weight-bearing of the affected side in a sit to standing. To evaluate sit to stand performance ability, this study performed five timed sit to stand tests. Results: The two groups both showed statistically significant improvement in muscle onset time of lower extremity, static balance ability in a standing position, and sit to stand performance after the intervention (p < 0.05). In addition, the action observation and synchronized electrical sensory stimulation group showed significant improvement in symmetric muscle activation of lower extremities and weight-bearing ratio of the affected side (p < 0.05). Conclusion: action observation and synchronized electrical sensory stimulation (AOT with ESS) can have positive effects on a stroke patient's sit to stand performance, and the intervention method that provides integrated AOT with ESS can be used as new nervous system intervention program.
Purpose: The purpose of this study was to investigate how different knee alignments (genu varum and genu valgum) affected activations of quadriceps muscles with measurements of onset-time differences between vastus medialis and vastus lateralis during isometric contractions at both 30 and 60 degree knee flexion. Methods: Fifty-two adults (20 genu varum, 12 genu valgum, and 20 control) were enrolled in this study. Subjects with over 4cm distance between knee medial epicondyles were assigned to Genu varum, while subjects with over 4cm distance between ankles medial malleolus were considered as genu valgum group. Surface EMG was used to measure onset time of both vastus medialis and vastus lateralis during isometric contraction at 30 and 60 degree knee flexion. Results: The onset time of vastus lateralis was delayed in genu varum group, and that of vastus medialis was delayed in genu valgum group at both 30 and 60 degree knee flexions. Moreover, onset time difference at $30^{\circ}$ knee flexion between muscles was larger in genu valgum group than genu varum group. Conclusion: Subjects with genu varum or valgum activated quadriceps muscles with different orders pending on flexion degrees. Therefore, when quadriceps training program were planned to prevent pain or deformities, the findings that quadriceps were activated with different orders affected by knee alignments and joint degree at which trainings were performed, must be considered. If the selective training programs of quadriceps femoris are planned to prevent pain or deformities due to poor knee alignments, these should consider the subject's knee alignment condition.
The information obtained from the analysis of respiratory muscle elecromyographic (EMG) activities provides a mean for studying muscular activity in relation to the ventilatory process. Thus, in order to comprehend the airflow pattern and its brain control, signal processing is required to characterize respiratory muscle activity. This paper presents a computerized method for the analysis of the electrical activity of the respiratory muscles of premature lambs, and focuses upon the automatic determination of respiratory timing points such as onset and cessation points of the burst activity. Based on experimental results, reliable timing points can be obtained using the proposed methodology.
