마우스 형태에 따른 상지의 근활성도의 변화 (A Study of Muscle Activation in Upper Extremity According Mouse Shape)

  • Background : The purpose of the study was: to investigate muscle activation of upper arm according mouse shape. Methods : Twenty person(mean age : 23. 7) who have healthy condition was participated this study, we collected data of muscle activation using by EMG from upper trapezius(Tr), deltoid middle fiber(De), extensor digitorum(Ed), first dorsal interosseous(Di) during participants was performed click and drag according various mouse. Mouse shape was divided 4 level as follow shape 1 was very small, 2 was small, 3 was moderate, 4 was large. Data was analyzed ANOVA, independent t-test using by SPSS ver18.0. Results : There was significantly difference of muscle activation among each muscle according mouse shape in drag and click. In shape 1, 4, there was significantly difference of muscle activation of Tr, De, Ed between drag and click except Di. In shape 2, 4, there was significantly difference of muscle activation of all muscle between drag and click. Conclusion : We knew that extensor digitroum showed more higher muscle activation than other muscle in drag, first dorsal interoseous showed more higher muscle activation that other muscle in click. We suggest that mouse shape was very important factor in order to prevent skeletal muscular disorder for computer user, and mouse shape can reduce muscle fatigue during computer work.

작업대 높이가 승모근의 근긴장도에 미치는 영향 (The Effect on the Tension Trapezius Muscle of the Height Keyboard Computer)

  • Purpose: Many kinds of musculoskeletal disease and symptom are caused by the longtime computer works. However, trapezius muscle tonus has not been established in regarding to keyboard height during typing. Therefore, this study is to evaluate the relationship between trapezius muscle tonus and the height of keyboard while typing, controling for the postures of neck, Lumbar, cervical vertebra. Methods: The experimental height of keyboard was set at elbow height, 3cm higher, 6cm higher, 9cm higher, than elbow. We studied trapezius tonus with the mean value for 2 minutes by EMG in 15 males and 15 females worker of hospital in seoul, who did not have a history of muscle disease, neurological signs, nerve damage. Results: In this experimental, as the height of the keyboard went up, the trapezius tonus significantly increased with shoulder abduction of brachium. Second, right and left trapezius tonus appeared similar while typing. Third, the best height that release the trapezius tonus the was as high as elbow and 3cm higher than elbow. Conclusion: With these above results, we suggest that the appropriate height of keyboard during typing to release the trapezius tonus most is the height of the elbow and 3cm higher than elbow. The study has important implications for focusing on the height of VDT worktable and complaining of a pain by oneself which are useful to establish a method of prevention of musculoskeletal disorder in work in the future.

두개하악장애 환자에 대한 초음파와 극초단파 심부투열치료 효과의 임상 및 근전도학적 연구 (Clinical and Electromyographic Study of the Effects of Ultrasonic Wave and Microwave Diathermy Treatment on the Craniomandibular Disorder Patients)

  • This study was performed to observe the effect of micro-wave diathermy and ultrasonic-wave diathermy on the craniomandibular disorder patients. 19 patients were classified into 12 acute and 7 chronic groups according to the duration of 6 months. They were treated with micro-wave diathermy and ultrasonic-wave diathermy for 2 weeks and pain, maximum comfortable opening, active range of motion were checked before and after therapy. Electromyographic activities of temporal and masseter muscles were also measured at physiologic rest position, clenching and mastication before and after therapy. The obtained results were as follows : 1. After treatment, pain were reduced and active range of motion and maximum comfortable opening were increased. 2. Temporoal and masseter muscle activities of post-treatment in rest position, clenching and mastication were lower than those of pre-treatment. 3. In rest position, temporal and masseter muscle activities of pre-treatment on affected sides were higher than those on unaffected sides, but there were no differences in muscle activities between affected ad unaffected sides on clenching and mastication in pre and post-treatment respectively. 4. There were no significant differences in active range of motion, pain and maximum comfortable opening between acute and chronic groups in pre and post-treatment but there were significant differences between pre-treatment and post-treatment in acute and chronic groups respectively. 5. Muscle activities of masseter and temporal muscles in acute and chronic patients were reduced in rest position after treatment.

길입성 진전의 병태생리: 다증 증례 연구 (Pathophysiology of orthostatic tremor: a multiple case study)

  • Introduction : Orthostatic tremor develops in the legs while standing up with no weakness, pain or imbalance in the leg and the tremor is characteristically not observed when walking. However there have been some confusions about orthostatic tremor in several aspects. For the past ten years, we have observed 4 patients with orthostatic tremor. In each case tests were performed to investigate the following three important areas of inquiry about orthostatic tremor. Firstly, whether this disorder is an independent diagnostic entity or a variant of essential tremor. Secondly, whether the progress of this disorder is specifically related with standing posture. Lastly, the nature of the pathophysiologic mechanism behind the appearance of the tremor when standing after the lapse of a certain latent period and its disappearance upon the commencement of walking. Methods : Our 4 cases of orthostatic tremor were studied clinically, electrophysiologically, and pharmacologically. Electrophysiological tests included tremor spectrum test and electromyography. Results : We observed the presence of this tremor in several other tonic postures, as well as its absence, in a vertically lifted position from all our cases. Our cases registered a variable tremor frequency between 5 and 12 Hz according to the tremor spectrum test and EMG. Furthermore all our 4 cases demonstrated patterns of both synchronous EMG activity and alternating EMG activity at various times in homologous muscles of both legs. Orthostatic tremor was improved significantly with propranolol as well as clonazepam. Conclusions : From the results of our study we drew the following conclusions. It is probable that orthostatic tremor is simply a variant of essential tremor rather than being an independent diagnostic entity and that in most cases its development is specifically related with muscle contraction rather than merely with the act of standing. Furthermore we discovered a clue in the previously described neural control mechanism that the nuclear bag fibers in the muscle spindle have lag time of several seconds in their response to muscle strength and that their baseline does not reset fully in rapidly moving muscle. This neural control mechanism could offer sufficient explanation for the phenomena of tremor appearance when standing and disappearance when walking in orthostatic tremor.

  • The SRP program(stress reduction program) is reported has a good effect to patient to escape from anxiety. To reduce several symptoms due to anxiety, we performed some SRP programs to our patient who had suffered anxiety disorder. The measurement tools of patient's improvement we used are Heart Rate Variability and State-Trait Anxiety Inventory. Through this programs, symptoms and feeling of anxiety were decreased than first time and Autonomic nerve system balance was much improved. this is caused by effort of self-control meditation, mindfullness, breathing instruction and muscle relaxation program.

The Flowers of Carthamus tinctorius : Potential Agent for Postmenopausal Disorder

  • In this study, 75% ethanol extract from the flowers of Carthamus tinctorius was prepared and biological activities were examined. The extract showed the inhibitory activity of vascular smooth muscle contraction and antithrombotic activity judged by bleeding time measurement. It also showed anti-inflammatory and potent analgesic activities in vivo. By oral administration of the extract, no acute toxicity was observed up to 5 g/kg in mice and rats. All these results strongly suggest that this extract may be beneficial for postmenopausal disorder by enhancing blood circulation.

A New Therapeutic Approach to Dystonia

  • Dystonia is a neurological disorder characterized by involuntary and uncontrollable muscle tonus abnormalities. It is a huge burden not only to the patients and their families, but also to the field of medicine, in that there has hardly been any substantial change in the concept of and approach to this intractable disorder and therefore no breakthrough to its diagnosis, evaluation and treatment. As an effort to solve the current impasse, this review briefly summarizes the current concept, etiology, diagnosis, treatment and management, and then suggests a rather new therapeutic approach to this disorder, based on the concept of neurological balance and TMJ integrative approach. These new approaches will provide a platform for the clinicians and researchers to have a leap in the concept, diagnosis and therapeutics.

  • The author studied the changes of subjective, objective symptoms and muscle activities with EM2 (myo-tronics Co., Seattle, USA) before and after MRA therapy. The 11 patients were treated with MRA and active range of motion, pain and mouth opening limitation were checked at each visit for 6-8 weeks. Electromyographic activities were measured in both anterior lobe of temporalis and middle fibers of masseter at the position in rest, clenching and mastication. The obtained results were as follows : 1. There were significant decrease in pain and mouth opening limitation and significant increase in active range of motion after MRA therapy. 2. The muscle activities tended to decrease in general, especially in the temporal and masseter muscles on clenching and in the masseter on mastication after MRA therapy. 3. There were no significant differences in muscle activities between affected and unaffected side, but there was significant differences in temporal muscle on clenching side after therapy. 4. There were no significant differences in active range of motion, pain and mouth opening limitation between acute and chronic groups. 5. There was more significant decrease in muscle activities of the affected side in acute group than those in chronic group.

Magnetic Resonance Imaging as a Biomarker for Duchenne Muscular Dystrophy

  • Muscular dystrophy is a hereditary musculoskeletal disorder caused by a mutation in the dystrophin gene. Duchenne muscular dystrophy (DMD) is one of the most common, and progresses relatively faster than other muscular dystrophies. It is characterized by progressive myofiber degeneration, muscle weakness and ultimately ambulatory loss. Since it is an X-linked recessive inheritance, DMD is mostly expressed in males and rarely expressed or less severe in females. The most effective measurement tool for DMD is magnetic resonance imaging (MRI), which allows non-invasive examination of longitudinal measurement. It can detect progressive decline of skeletal muscle size by measuring a maximal cross-sectional area of skeletal muscle. Additionally, other techniques in MRI, like $T_2$-weighted imaging, assess muscle damage, including inflammation, by detecting changes in $T_2$ relaxation time. Current MRI techniques even allow quantification of metabolic differences between affected and non-affected muscles in DMD. There is no current cure, but physical therapist can improve their quality of life by maintaining muscle strength and function, especially if treatment (and other forms of medical intervention) begins in the early stages of the disease.

하지혈류제한과 병행한 계단운동이 무릎 폄근에 미치는 영향 (The Effect of Stair Exercise with Restriction Blood Flow on Knee Extensor Muscle)

  • PURPOSE: Low-intensity exercise with restriction of blood flow has been proposed as an alternative exercise to secure the disadvantage of a high-intensity resistance exercise. However, studies of how affects the muscle using functional exercise are lacking. Thus, the purpose of this study was to investigate knee extensor muscle strength during stair exercise of functional exercise with leg blood flow restriction. METHODS: Twenty two healthy young adults with no history of musculoskeletal or neurogical disorder were participated in this study. participant were randomized into either non-restriction group(11 subject) or restriction group (11 subject). The restriction blood cuff attached to the proximal end of the leg. Measurement of knee extensor strength was used by cybex dynamometer. Data analyzed in independent t-test and paired t-test. RESULTS: Knee extensor muscle strength was significantly different between groups. Also, there were significant differences in the strength of knee extensor within the group. CONCLUSION: This study found that stair exercise with restriction of blood flow did influence to knee extensor muscle strength. These results will also be able to promote the effect of increasing the muscle power applied to functional exercise. Henceforth, studies will be made in the intervention method that can be applied to health vulnerable person.
