피부근염에 이환되었던 환아의 Henoch-Schonlein 자반증 증례 1례 (A Clinical Case of Dermatomyositis Complicated by Henoch-Schonlein Purpura)

  • Dermatomyositis is an inflammatory cutaneo-muscular disease of unknown etiology, characterized by symmetrical proximal muscle weakness and typical dermatologic manifestation. Henoch-Schonlein purpura is most common in children. In Korea, dermatomyositis or Henoch-Schonlein purpura associated with herbal medicine has not been reported. The subject patient is a seven year-old girl diagnosed with dermatomyositis in August 1999 and Henoch-Schonlein purpura in January, July 2001. There were symptoms of dermatomyositis such as headache, arthralgia, eruption, itching and abdominal pain. I think the herb-prescription composed or Radix astragali (Huangqi), Herba agrimoniae (Xianhecao), Fructus jujubae (Dazao) had an ameliorating effect on Henoch-Schonlein purpura by decreasing these symptoms.

  • Sarcoglycanopathies are a rare group of autosomal recessive limb-girdle muscular dystrophies (LGMDs) caused by genetic variants in α-, β-, γ-, or δ-sarcoglycan that maintain membrane integrity and contribute to molecular signal processing. High-throughput nucleotide sequencing was performed in patients with slowly progressive proximal muscle weakness from early childhood with respiratory involvement, which detected a novel homozygous nonsense variant (c.601C>T;p.Gln201Ter) in SGCB. This report informs about the clinical characteristics of LGMD2E (type-2E LGMD) in Korea and provides genetic confirmation of the disease.

Acquired myasthenia gravis in a domestic shorthair cat with cranial mediastinal mass

  • A 7-year-old castrated male domestic short-hair cat presented with anorexia, constipation, depression, and voice alteration. Physical and neurological examinations revealed hyperthermia ($40.5^{\circ}C$), ventroflexion of the neck, reduced responses to external stimuli, generalized muscle weakness, and exercise intolerance. Thoracic radiographs revealed the presence of a cranial mediastinal mass. The history, clinical signs, and other examination results were compatible with acquired myasthenia gravis (MG). Acetylcholine receptor (AChR) antibody titers were determined to confirm MG and the serum AChR antibody concentration was 1.24 nmol/L (reference interval, < 0.3 nmol/L). This is the first diagnosis of acquired MG in a cat in Korea.

일부 요통환자의 인자분석 (Factor Analysis of Low Back Pain Patients)

  • Objectives: The objective of this study was to investigate factor analysis of low back pain patients. Methods: The data were collected from hospital located in Daegu. Observation was made on 40 low back pain patients and factor analysis study was also performed in terms of their abnormal physical findings. Results: The low back pain patients occupied 20% of all clinical patients in male group and 20% in female group. The occurrence of acute low back pain in the first factor tended to be higher among in male group than among in female group. While it was significantly higher muscle weakness of the back in female group. The most prevailing abnormal finding among low back pain patients in the first factor were tender point on the back, While it was significantly lower Decreased tendon reflex in both sexes Conclusions: The factor pattern of abnormal physical findings in low back pain patients was much different between male and female patients, suggesting the different pattern of etiology. Although low back pain is one of the most common symptoms causing limitation activity, as yet there is no known specific effective treatment.

Future Opportunities for life Science Programs in Space

  • Most space-related life science programs are expensive and time-consuming, requiring international cooperation and resources with trans-disciplinary expertise. A comprehensive future program in "life sciences in space" needs, therefore, well-defined research goals and strategies as well as a sound ground-based program. The first half of this review will describe four key aspects such as the environment in space, previous accomplishments in space (primarily focusing on amphibian embryogenesis), available resources, and recent advances in bioinformatics and biotechnology, whose clear understanding is imperative for defining future directions. The second half of this review will focus on a broad range of interdisciplinary research opportunities currently supported by the National Aeronautics and Space Administration (NASA), National Institute of Health (NIH), and National Science Foundation (NSF). By listing numerous research topics such as alterations in a diffusion-limited metabolic process, bone loss and skeletal) muscle weakness of astronauts, behavioral and cognitive ability in space, life in extreme environment, etc., we will attempt to suggest future opportunities.

중환자 다발신경병증 및 근육병 1예 (A Case of Critical Illness Polyneuropathy and Myopathy)

  • Critical illness polyneuropathy (CIP) and critical illness myopathy (CIM) occur commonly in the patients who have been on mechanical ventilation for more than 1 week. Even in some patients diagnosed with CIP, an underlying myopathy may be the primary cause of the muscle weakness. The cormorbid status of CIP and CIM is called as critical illness polyneuropathy and critical illness myopathy (CIPNM). We describe a 56-year-old man with acute quadriparesis and areflexia after systemic inflammatory response syndrome. The diagnosis of CIPNM is important to avoid unnecessary investigations and unreasonably pessimistic prognosis. Electrophysiologic studies are essential for the diagnosis and for planning further clinical management.

  • ntermediate syndrome (IMS) typically occurs at 24-96 hours following organophosphate (OP) poisoning, after an acute cholinergic crisis, but before OP-induced delayed polyneuropathy. It is characterized by proximal muscle weakness and respiratory insufficiency, which is a major contributing factor of OP-related morbidity and mortality. We report an atypical IMS case showing rapid-onset ascending paralysis and respiratory disturbance with an acute cholinergic crisis occurring 4-5 days after skin exposure to OP.

  • Postpolio syndrome (PPS) refers to a constellation of neuromuscular and orthopedic symptoms and signs that have been noted to occur in patients with remote antecedent poliomyelitis. It has been increasingly recognized that individuals recovering from acute poliomyelitis develop new symptoms, most commonly weakness, fatigue, and pain that develops decades after initial disease in the region previously affected. Associated symptoms may include dysphagia, respiratory insufficiency, new muscular atrophy, dysarthria, muscle cramps, fasciculations, sleep abnormalities, and cold intolerance. Although the concepts of PPS was first described in the late 1800s, it was not until nearly 100 years later that the concept of PPS was more widely recognized and defined. This was due largely to the polio epidemic of the 1940s and 1950s that left many survivors in the world. The virtual epidemic of PPS that occurred among these polio survivors in the 1980s and 1990s has served as a catalyst to attract medical attention to this syndrome.

Median Nerve Injuries Caused by Carpal Tunnel Injections

  • Local steroid injections are widely used for diagnostic and therapeutic purposes in the management of carpal tunnel syndrome. The median nerve injury is the most serious complication in association with carpal tunnel injections although the incidence is low. A median nerve injury will be presented with shooting pain at the injection time along with other sensory distortion, motor weakness and muscle atrophy. The management includes a conservative treatment and a surgical exploration. Carpal tunnel injections should be used at a minimum only. If such steroid injection is required, an appropriate needle positioning is vital for the nerve injury prevention. The patient should not be heavily sedated and should be encouraged to inform experiences of numbness/paresthesia during the procedure immediately.

  • The purpose of this study was to investigate the chest physical therapy in the hospital of seoul, kyungido and to activate the chest physical therapy. Dept. of physical therapy in 15 hospitals, 65 physical therapists disabled and analysis of interview. The results are as follows: 1) $86.7\%$ of respondents were not chest physical therapist in the hospital. $79.4\%$ of respondents were a lack of 5 patients per month. 2) The chief diagnosis of chest physical therapy was respiretory muscles weakness by reason of spinal cord injury and muscle diseases. Many physical therapists was obtain the chest physical therapy information from university or collage. 3) Management of chest Physical therapy Patient was to be alike physical therapist and doctor A problem of chest physical therapy was a lack of chest physical therapy information, cognition of a physician. The results of this study suggested that chest physical therapy need to organization of the treatment and magnification of the treatment area.
