Group Classification on Management Behavior of Diabetic Mellitus (당뇨 환자의 관리행태에 대한 군집 분류)

  • The purpose of this study is to provide informative statistics which can be used for effective Diabetes Management Programs. We collected and analyzed the data of 666 diabetic people who had participated in Korean National Health and Nutrition Examination Survey in 2007 and 2008. Group classification on management behavior of Diabetic Mellitus is based on the K-means clustering method. The Decision Tree method and Multiple Regression Analysis were used to study factors of the management behavior of Diabetic Mellitus. Diabetic people were largely classified into three categories: Health Behavior Program Group, Focused Management Program Group, and Complication Test Program Group. First, Health Behavior Program Group means that even though drug therapy and complication test are being well performed, people should still need to improve their health behavior such as exercising regularly and avoid drinking and smoking. Second, Focused Management Program Group means that they show an uncooperative attitude about treatment and complication test and also take a passive action to improve their health behavior. Third, Complication Test Program Group means that they take a positive attitude about treatment and improving their health behavior but they pay no attention to complication test to detect acute and chronic disease early. The main factor for group classification was to prove whether they have hyperlipidemia or not. This varied widely with an individual's gender, income, age, occupation, and self rated health. To improve the rate of diabetic management, specialized diabetic management programs should be applied depending on each group's character.

On the Evaluation of Physical Distribution Service in Ports (항만물류서비스의 평가에 관하여)

  • It is required to consider pricing and non-pricing factors and external economy in order to achieve the objects of physical distribution system in a port. Recently, among the three factors, much attention has been paid to non-pricing factor in the system. Although physical distribution service in a port(PDSP)has been frequently mentioned in documents and literature related to port and shipping studies, few study on it has not been systematically and scientifically made due to the following problems; $\circ$ there are not proper criteria to evaluate level and quality of PDSP and as a result it is difficult to set up a unified standard for doing so. $\circ$ algorithms to evaluate problems with complex and ambiguous attributes and multiple levels in PDSP are not available. This thesis aims to establish a paradigm to evaluate PDSP and to abvance existing decision making methods to deal with complex and ambiguous problems in PDSP. To tackle the first purpose, extensive and thorough literature survey was carried out on general physical distribution service, which is a corner stone to handle PDSp. In addition, through interviews and questionnaire to the expert, it have extracted 82 factors of physical distribution service in a port. They have been classified into 6 groups by KJ method and each group defined by the expert's advice as follows; a. Potentiality b. Exactness c. safety d. Speediness e. Convenience f. Linkage Prior to the service evaluation, many kinds of its attributes must be identified on the basis of rational decision owing to complexity and ambiguity inherent in PDSP. An analytical hierarchy process (AHP) is a method to evaluate them but it is not applicable to PDSP that have property of non-additivity and overlapped attributes. Therefore, probablility measure can not be used to evaluate PDSP but fuzzy measure is required. Hierarchical fuzzy integral method, which is merged AHP with fuzzy measure, is also not effective method to evaluate attributes because it has vary complicated way to calculate fuzzy measure identification coefficient of attributes. A new evaluation algorithm has been introduced to solve problems with multi-attribute and multi-level hierarchy, which is called hierarchy fuzzy process(HFP).Analysis on ambiguous aspects of PDSP under study which is not easy to be defined is prerequisite to evaluate it. HFP is different from algorithm existed in that it clarified the relationship between fuzzy measure and probability measure adopted in AHP and that it directly calculates the family of fuzzy measure from overlapping coefficient and probability measure to treat and evaluate ambiguous and complex aspects of PDSP. A new evaluation algorithm HFP was applied to evaluate level of physical distribution service in the biggest twenty container port in the world. The ranks of the ports are as follows; 1. The Incidence and Risk Factors of Metabolic Syndrome in Rural Area (농촌지역 주민의 대사증후군 발생률과 위험요인)

  • This study was conducted to investigate the incidence rate of metabolic syndrome, and the related factors were examined. 620 persons who had participated in both initial and second survey were selected. Initial survey was performed at the year of 2006 and second survey was performed at the year of 2010. Among them, 460 persons who didn't initially have a metabolic syndrome were selected as the final study subjects. They were classified into 2 categories; stationary normal group (352, 76.5%), and metabolic syndrome incidence group (108, 23.5%). The incidence rate of metabolic syndrome for the subjects was 25.2 per 1,000 person years. Sex, obesity, and smoking had significant effect on the incidence of metabolic syndrome. In multiple logistic regression analysis,after controlling variables, obesity index was found to be major factor in the incidence of metabolic syndrome. The risk of metabolic syndrome was increased by overweight or obesity. The strategy to control body weight should be emphasized for prevention of metabolic syndrome.

Impacts of Land Use and Urban Design Characteristics on Transit Ridership in the Seoul Rail Station Areas (서울시 역세권에서의 토지이용 및 도시설계특성이 대중교통이용증대에 미치는 영향 분석)

  • One of the efforts both to prevent urban sprawling development patterns and to promote use of public transportation is known as Transit-Oriented Development (TOD), including such planning elements as the density and diversity of land use and pedestrian-friendly urban design around a transit center. The aim of this study is thus to conduct impact analyses of TOD planning elements on transit ridership in the Seoul rail station areas. First, the authors investigate and draw out various actual elements of TOD planning by using GIS-based data and Smart Card data. Then the authors analyze impacts of TOD planning elements on transit ridership for the Seoul rail station areas. After condensing 34 variables presumably influencing transit ridership into seven factors by using factor analyses, the study utilizes multiple regression modeling methods to identify their impacts on transit ridership. The analysis results demonstrate that transit ridership tends to increase more in rail station areas where there is a non-residential high density, mixed use of land and narrow and small-size road network patterns. The implementation of TODs should be a useful method in inducing a Transit-Oriented City through redevelopment and new development.

Quality of Life of Some Obese Patients Wanted to Receive Korean Traditional Medicine (한방 비만클리닉에 내원한 일부 비만 여성의 삶의 질)

  • Obesity is chronic disease which influenced on health severly. The causes of obesity have been known as life change, lack of excercise, genetic factor, mental and social economic factors. Especially the obesity of women increased the risk of the diseases such as DM, osteoarthritis, cardiovascular disease, breast cancer and infertility. The limitations of the widely used negative definition of health as the absence of disease and WHO's 1946 definition of health as total social, psychological and physical well-being have long been recognized (WHO 1958). The Quality of Life (QoL) includes functional ability, the degree and quality of social and community interaction, psychological well-being as somatic sensation and life satisfaction. I investigated to compare the differences between obese women (n=63), non-obese women (n=37) in clinic and general women (n=43, control) on baseline characteristics and WHO QoL-BREF. The purpose of this study is to assist the diagnosis and treatment of obesity. WHO QoL-BREF is self administered type which consisted of 26 questions. The prospective question is calculated with 5 scores by Likert's method. The results are as follows : The means of physical, psychological, social, overall and total scores of QoL were significant among BMI group (P<0.05). The score of control group (BMI < 25) was higher than other groups significantly (P<0.05). In multiple regression analysis, the variable of high school/below middle school was significant in environmental and overall domain of QoL scores (P<0.05). The variable of college/below middle school was significant in environmental, overall domain and total score of QoL scores (P<0.05). The variable of above university/below middle school was significant in physical health, environmental, overall domain and total score in QoL scores (P<0.05). The variable of Health perception (moderate/bad) was positively significant in physical health, environmental, overall domain and total score of QoL scores (P<0.05). The variable of Health perception (good/bed) was positively significant in physical health, environmental, social, overall domain and total score of QoL scores (P<0.05). The variable as BMI non-=obese women/control was negatively significant in social domain of QoL scores (P<0.05). Above the results, It suggests that the variable as BMI did't affect on the QoL in patients and control, but the variables as education and health perception affected on the QoL scores. Further study is required to conduct QoL differences between before and after treatment of obese patients.

Current Status of Hospital-based Health Promotion Programs in Korea and the Factors Influencing Their Introduction (우리 나라 병원들의 건강증진 서비스 도입 현황과 이에 영향을 미치는 요인)

  • Objectives : To investigate the current status of hospital-based health promotion programs in Korea and to elucidate the factors which affect to the process of implementation. Methods : We conducted a mail survey of all 875 hospitals in Korea from March to May 2001. In reference to 12 specific kinds of health promotion programs, hospital CEOs were asked whether their hospital have such programs, whether they are fully staffed and whether the program is paid for by the patients. Contextual factors(location, hospital type, number of beds, length of operation, public/private status, economic level of the community, the level of competition) and organizational factors (the extent of market, compatibility with vision, formalization), strategic types of the CEOs (defender/analyser/prospector) were also surveyed. The relationships between each variable and the implementation of health promotion services, activation of services, and the target groups(patient/community resident) were analyzed by univariate analysis and the independent effects of these variables were examined with multiple logistic regression. Results : 100 of 125 hospitals responding (84.8%) had mere than one health promotion program. However, they showed fluctuations in the adoption rate of each programs, meaning that comprehensive health promotion services were not provided. Many programs were not fully staffed and few hospitals had paid programs. In factors affecting health promotion service implementation, private hospitals showed a higher rate in implementation than public hospitals. In contrast, when the competition among nearby hospitals was intense, the level of implementation of service lowered. In the strategic type of the CEOs, the prospectors were shown to have instituted more health promotion programs in their hospitals and the analysers had a greater tendency to have programs for community residents than the defenders. Conclusion . Considering the above results, contextual factors may contribute greatly to the introduction of health promotion services in Korean hospital, although the CEO's personal preference and organizational factors play a larger role in the activation of services. Additionally, the CEO's personal preference may be the major influencing factor in the introduction of programs for community residents.

The Association Between Apolipoprotein E Genotype and Lipid Profiles in Healthy Woman Workers (건강한 여성 근로자에서 Apolipoprotein E와 혈중 지질 농도와의 관련성)

  • Objectives: Plasma lipid profiles and Apolipoprotein E (ApoE) are established risk factors for cardiovascular disease (CVD). The knowledge of lipid profile may estimate the potential victims of cardiovascular disease before its initiation and progression and offers the opportunity for primary prevention. The most common ApoE polymorphism has been found to influence plasma lipid concentrations and its correlation with CVD has been extensively investigated in the last decade. Methods: The ApoE polymorphism and its influence on plasma lipid were investigated in healthy woman workers. The information on confounding factors was obtained through a self-administered questionnaire and ApoE polymorphism was investigated using PCR. Results: The relative frequencies of alleles E2, E3 and E4 for the study population (n = 305) were 0.127, 0.750 and 0.121, respectively. ApoE polymorphism was associated with variations in plasma HDL-cholesterol lipid profile. In order to estimate the independent effects of alleles E2 and E4, as compared with E3, on lipid profile, multiple regression was performed after adjustment for confounding variables such as age, BMI, blood pressure, education status, insulin, fasting glucose, HOMA-IR, menopause. ApoE2 had a negative association with HDL cholesterol and ApoE4 had a positive association with LDL cholesterol. Conclusions: This study identified that the ApoE and CVD risk factors contribute to the lipid profiles, similar to other studies. The analysis including dietary intake and other gene in further studies may help to identify clear effects on lipid profiles as risk factor for CVD.

Relationship Between Health Behaviors of Local Residents and Stomach Cancer Incidence (일부지역 주민들의 건강행태와 위암 발생과의 관련성 연구)

  • This study was performed to evaluate relationship between health behaviors of local residents and stomach cancer incidence. Case-control study was carried out on 91 cases matched for age and sex with 91 controls in a general hospital in Seoul. It was surveyed using questionnaires and interview from December 20, 2008 to January 30, 2009. Major findings are as follows. First, significant negative association was observed in recreational activity ; OR for active group was 0.23, 0.41 relative to the inactive group. Secondly alcohol consumption was a significant risk factor; relative to non-drinker for drinker of more than 30 years(OR=5.17 95% Cl=1.78-19.95). Thirdly, Eating broiled food showed a positive significant association with risk for stomach cancer(OR=3.79 95% Cl=1.61-9.84) for consumption of three times more per week. As a result of multiple logistic regression analysis, income, BMI 10 years ago, recreational activity, alcohol consumption, eating broiled and salt food were associated with stomach cancer. In conclusion, the correlation of stomach cancer incidence can be found in recreational activity, alcohol consumption, eating broiled and salt food, income, and BMI 10 years ago.

The Association of Self-esteem and Social support with Depressive symptoms in College Students (일부 대학생들의 자아존중감 및 사회적 지지와 우울과의 관련성)

  • The purpose of this study was to estimate self-esteem, social support and depressive symptoms among college students, as well as to reveal its related factors. The self-administered questionnaire was filled out by 471 students from university located in Daejon city and Chungnam during two weeks from April 23, 2013. As a results, lower the self-esteem is, the depression level comes out to be higher. Also weaker social support is the factor to give rise to depression. Self-esteem and social support showed negative correlation with depression. Social support showed positive correlation with self-esteem. According to hierarchical multiple regression analysis, following factors influence depression level with explanatory powers of 23.8%: sex, age, stress for body shape, perceived economical status, studying time, self-esteem, and social support. In the conclusion, this result imply that depression level of college students is closely related to self-esteem and social support. Especially, social support showed higher correlation with depression.

Characterization of Asphalt Pavement Distress Using Korean Pavement Research Program (한국형포장설계법을 이용한 아스팔트포장의 파손특성)

  • The main purpose of this study is to evaluate the main parameters involved in the asphalt pavement distresses, including IRI (International Rough Index), fatigue, and permanent deformation. The main parameters are the region (Seoul and Busan), traffic level, asphalt binder, maximum aggregate of surface course, thickness of the surface course and base. A total of 64 case studies were carried out under the auspices of the KPRP (Korea Pavement Research Program). From the analysis of the KPRP test results, the key factors for the asphalt pavement distress were determined. Considering the effect of one variable in the basic condition, asphalt binder was the major factor having an effect on the distresses for an AADT (Annual Average Daily Traffic) of 5000 in the Seoul area. Among the remaining factors, the results were found to be in the order of the base layer thickness (A), surface layer thickness (B), and aggregate particle size thickness (D). The same results were obtained for an AADT of 10000. In the case of Busan with an AADT of 5000, the same result was obtained as for Seoul. Among the remaining factors, the results were in the order of the base layer thickness (A), aggregate particle thickness (D), and surface layer thickness (B). Even though there was a slight difference in the effect of the traffic level and region, asphalt binder was the parameter having the greatest effect on the asphalt pavement distress. In the case where the effect of multiple parameters was analyzed, the combination of the asphalt binder and base thickness showed a relatively strong effect.
