Autosomal dominant polycystic kidney disease (ADPKD) is the most frequent hereditary renal disease and causes terminal chronic renal failure. ADPKD is characterized by bilateral multiple renal cysts, which are produced by mutations of the PKD1 and PKD2 genes. PKD1 is located on chromosome 16 and encodes a protein that is involved in cell cycle regulation and intracellular calcium transport in epithelial cells and is responsible for 85% of ADPKD cases. Although nine cases of unilateral ADPKD with contralateral kidney agenesis have been reported, there have been no reports of early childhood ADPKD. Here, we report the only case of unilateral ADPKD with contralateral kidney dysplasia in the world in a four year-old girl who was intrauterinely diagnosed since she was 20 weeks old and followed for four years until present.
Purpose: The purpose of this study was to describe the status of the functional health literacy of the elderly living in a rural community and to identify the influencing factors on the functional health literacy. Methods: Subjects(n=238) aged over 60 years were selected for data collection. Functional health literacy was measured by Korean Health Literacy, which consisted of 15 items including eight numeracy and seven comprehension items. Descriptive statistics, t-test, ANOVA with Scheffe test, and stepwise multiple regression were used to analyze the data. Results: The mean score of total functional health literacy was $6.66{\pm}3.31$, numeracy was $3.52{\pm}1.76$, and comprehension was $3.41{\pm}2.10$. Of the subjects, 39.1% showed a low score below the median split. In the results of stepwise multiple regression, the factors affecting functional health literacy were basic literacy, age, number of chronic diseases, self-reported health status, and gender. These factors accounted for 33.0% of functional health literacy. Conclusion: These results suggest that interventions for improving functional health literacy are necessary to enhance elders' ability of self-care management. In addition, it is needed to take into account the individual characteristics of the subjects depending on their gender, age, and health status.
Objectives : This study was performed to apply thermography as an method in diagnosis of hwabyung patients. We studied 11 Hwabyung patients who visited to chronic diseases center and circulatory oriental internal medicine of Kangnam oriental medicine hospital and 11 patients control group. Methods : Diagnosis of Hwabyung was based on the dignostic criteria of Hwabyung. The temperature was measured on Chonjung(CV17) Shimsu(B15), Kansu(B18), Kyonjong(G21) in each group. The ${\Delta}T$ was measured between Chonjung(CV17) and Chungjong(CV16), left and right Chungjong(CV16), Shimsu(B15), Kansu(B18), Kyonjong(G21) in each group. We compared the ${\Delta}T$ and DITI types between patients and control group. Results : The ${\Delta}T$ between left and right Chungjong(CV16), Shimsu(B15), Kansu(B18), Kyonjong(G21) were not statistically significant. But the ${\Delta}T$ between Chonjung(CV17) and Chungjong(CV16) was statistically significant(P<0.05) in each group. In control group, DITI type was straight 36%, diamond 27%, multiple small spot 18%, others 18%. In Hwabyung patients group, DITI type was inverse triangle 64%, multiple small spot 9.1%, round 9.1%. Conclusions : The ${\Delta}T$ between Chonjung(CV17) and Chungjong(CV16) and DITI type is considered useful diagnostic methods on Hwabyung patients.
Vocal polyp is one of the representative chronic diseases of vocal folds, and it can be cured by voice therapy and/or laryngeal microsurgery. However, the existing therapeutic methods about vocal polyp are in great demand. The purpose of this study was to evaluate the effect of vocal improvement between laryngeal microsurgery and $SKMVTT^{(R)}$ (Seong-Tae Kim's Multiple Voice Therapy Technique), which was designed by the author. We identified 37 patients, who were diagnosed with unilateral vocal polyp, aged from 21 to 62 years(mean age: 46 years). 21 patients were treated by the $SKMVTT^{(R)}$ and the other 16 patients were only treated by the laryngeal microsurgery. All patients who were treated by the $SKMVTT^{(R)}$, received 12 sessions of treatment, and were evaluated before therapy and after finishing the 12th session. The patients who were treated by laryngeal microsurgery, were evaluated prior to and at least 8 weeks after surgery. The results showed that the $SKMVTT^{(R)}$ produced better results compared to the laryngeal microsurgery alone. The $SKMVTT^{(R)}$ produced better results, especially, at the initial stage of voice therapy compared with those of laryngeal microsurgery. In this study, we can suggest that $SKMVTT^{(R)}$ may be useful in improving the voice qualities of vocal polyp patients. However, more data should be collected and evaluated to be widely used in other clinics.
Purpose: The purpose of this study was to investigate the factors that increase of the risk for falls in low-income elders in urban areas. Methods: The participants were elderly people registered in one of public health centers in one city. Data were collected by interviewing the elders, assessing their environmental risk factors, and surveying relevant secondary data from the public health center records. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 14. Results: Stroke, diabetes, visual deficits, frequency of dizziness, use of assistive devices and moderate depression were statistically significant risk factors. The comorbidity of chronic diseases with other factors including depression, visual deficit, dizziness, and use of assistive devices significantly increased the risk of falls. From multiple logistic regression analysis, statistically significant predictors of falls were found to be stroke, total environmental risk scores, comorbiditiy of diabetes with visual deficits, and with depression. Conclusion: Fall prevention interventions should be multifactorial, especially for the elders with stroke or diabetes, who were identified in this study as the high risk group for falls. A fall risk assessment tool for low-income elders should include both the intrinsic factors like depression, dizziness, and use of assistive devices, and the extrinsic factors.
Vocal nodule is one of the representative chronic diseases of vocal folds, and it can be cured by voice therapy. However, the existing therapeutic methods about vocal nodule are in great demand. The purpose of this study was to evaluate the effect of therapeutic methods between Accent method and SK-MVTT (Seong-Tae Kim's multiple voice therapy technique), which was designed by the author. We identified 40 females, who diagnosed having vocal nodules, aged from 21 to 52 years (mean age: 40 years). Twenty females were treated by the SK-MVTT and the other 20 females the Accent method. All subjects received 12 sessions of treatment, and were evaluated after finishing the 6th and the 12th session. The results showed that the SK-MVTT produced a better results compared to the Accent method. The SK-MVTT was better especially at the initial stage of voice therapy compared to the Accent method. In this study, we can suggest that SK-MVTT may be useful in improving the voice qualities of vocal nodule patients. However, more data should be collected and evaluated before it can widely be used in other clinics.
Objectives: This study examined demographic factors hampering access to healthcare at hospitals and suggests policy approaches to improve healthcare management in Thailand. Methods: The data for the study were drawn from a health and welfare survey conducted by the National Statistical Office of Thailand in 2017. The population-based health and welfare survey was systematically carried out by skilled interviewers, who polled 21 519 384 individuals. The independent variables related to demographic data (age, sex, religion, marital status, education, occupation, and area of residence), chronic diseases, and health insurance coverage. The dependent variable was the degree of access to healthcare. Multiple logistic regression analysis was subsequently performed on the variables found to be significant in the univariate analysis. Results: Only 2.5% of the population did not visit a hospital when necessary for outpatient-department treatment, hospitalization, or the provision of oral care. The primary reasons people gave for not availing themselves of the services offered by government hospitals when they were ill were-in descending order of frequency-insufficient time to seek care, long hospital queues, travel inconvenience, a lack of hospital beds, unavailability of a dentist, not having someone to accompany them, and being unable to pay for the transportation costs. Multiple logistic regression analysis showed that failure to access the health services provided at hospitals was associated with demographic, educational, occupational, health welfare, and geographic factors. Conclusions: Accessibility depends not only on health and welfare benefit coverage, but also on socioeconomic factors and the degree of convenience associated with visiting a hospital.
Pulmonary siderosis is a pneumoconiosis caused by chronic iron inhalation. A diagnosis of pulmonary siderosis is based on a patient history of iron inhalation, on chest radiographic findings, and on accumulation of iron oxide in macrophages within the lung. A typical radiographic finding of pulmonary siderosis includes ill-defined micronodules that are diffusely distributed in the lung. We experienced a 52-year-woman with a $1.3{\times}1.5$-cm mass in the left upper lobe with multiple nodules in both lungs. Because the radiographic findings were atypical, we conducted a video-assisted thorascopic lung biopsy procedure to exclude the diagnosis of metastatic lung cancer. After confirming iron deposition in the lung tissue and knowing the patient's occupational history of welding iron, we concluded that this was a case of pulmonary siderosis.
