임상검체에서 분리된 Methicillin Resistant Staphylococcus aureus의 다제내서양상과 내성 관련 유전자의 검색 (Detection of multidrug resistant patterns and associated-genes of Methicillin Rdsistant Staphylococcus aureus(MRSA) isolated from clinical specimens)

  • Methicillin Resistant Staphylococcus aureus (MRSA) was obtained from the clinical specimens at Pusan national university Hospital, Pusan, Korea. The sensitivities against various antibiotics were examined by using disc diffusion test and associated genes such as mecA, mecR1, mecI and femA were detected by polymerase chain reaction. Among Seventy-nine strains of MRSA, 38 strains(48.1%)were sensitive to streptomycin and 32 strains(40.5%) to cefoperazone, while one strain(1.3%) were resistant to vancomycin. In considering the result of this study, 7 strains showed resistance to 9 kinds of different antibiotics, 12 strains were to 8 kinds, 24 strains were to 7,25 strains were to 6, 9 strains were to 5, and 2 strains were to 4 antibiotics. Among 79 strains of MRSA, 67 strains were coagulase positive and 12 were coagulase negative. In the detection of MRSA associated genes by PCR method, mecA, mecR1, mecI, and femA genes were detected in 30 strains(44.8%), 28 strains(41.8%), 23 strains(34.3%) and 15 strains(22.4%), respectively. MecA type that is without femA were found in 21 strains(31.3%), femA type that is without regulator genes were shown in 4 strains(6.0%), while mecA-mecR1-mecI type with regulator genes were shown more to be 17 strains(25.4%). There was little statistical significance between multidrug resistance and MRSA associated genes. Considering these result, it is necessary to include moecular biological studies of related genes to the study drug resistance.

Whole genome sequencing analysis on antibiotic-resistant Escherichia coli isolated from pig farms in Banten Province, Indonesia

  • Importance: The emergence and rapid increase in the incidence of multidrug-resistant (MDR) bacteria in pig farms has become a serious concern and reduced the choice of effective antibiotics. Objective: This study analyzed the phylogenetics and diversity of antibiotic resistance genes (ARGs) and molecularly identified the source of ARGs in antibiotic-resistant Escherichia coli isolated from pig farms in Banten Province, Indonesia. Methods: Forty-four antibiotic-resistant E. coli isolates from fecal samples from 44 pig farms in Banten Province, Indonesia, were used as samples. The samples were categorized into 14 clusters. Sequencing was performed using the Oxford Nanopore Technologies MinION platform, with barcoding before sequencing with Nanopore Rapid sequencing gDNA-barcoding (SQK-RBK110.96) according to manufacturing procedures. ARG detection was conducted using ResFinder, and the plasmid replicon was determined using PlasmidFinder. Results: Three phylogenetic leaves of E. coli were identified in the pig farming cluster in Banten Province. The E. coli isolates exhibited potential resistance to nine classes of antibiotics. Fifty-one ARGs were identified across all isolates, with each cluster carrying a minimum of 10 ARGs. The ant(3'')-Ia and qnrS1 genes were present in all isolates. ARGs in the E. coli pig farming cluster originated mainly from plasmids, accounting for an average of 89.4%. Conclusions and Relevance: The elevated potential for MDR events, coupled with the dominance of ARGs originating from plasmids, increases the risk of ARG spread among bacterial populations in animals, humans, and the environment.

Effects of Vinorelbine on Cisplatin Resistance Reversal in Human Lung Cancer A549/DDP Cells

  • Multi-drug resistance (MDR) is an essential aspect of human lung cancer chemotherapy failure. Recent studies have shown that vinorelbine is involved in underlying processes in human tumors, reversing the MDR inseveral types of cancer cells. However, the roles and potential mechanism are not fully clear. In this study, we explored effects of vinorelbine in multi-drug resistance reversal of human lung cancer A549/DDP cells. We found that vinorelbine increased drug sensitivity to cisplatin and intracellular accumulation of rhodamine-123, while decreasing expression of P-glycoprotein (P-gp), multi-drug resistance-associated protein (MRP1) and glutathione-S-transferase ${\pi}$ (GST-${\pi}$) in A549/DDP cells. At the same time, we also established downregulation of p-Akt and decreased transcriptional activation of NF-${\kappa}B$ and twist after vinorelbine treatment. The results indicated that vinorelbine might be used as a potential therapeutic strategy in human lung cancer.

Multidrug-resistant Staphylococci Found on Book Surfaces in East London Libraries

  • There is an increase in the presence of drug-resistant staphylococci outside of the nosocomial and healthcare setting. Although the presence of staphylococci has been studied in several public spaces, nothing is known on the presence of staphylococci in public libraries. Book surfaces from public libraries in the East London area, United Kingdom were swabbed and cultured and identity of the isolates determined by matrix-assisted laser desorption/ionization time of flight (MALDI-TOF) mass spectrometry (MS). Seven different staphylococcal species were identified by MALDI-TOF-MS analysis. This short study provides evidence of the presence of multidrug-resistant staphylococci in public libraries in the East London area.

Gliotoxin is Antibacterial to Drug-resistant Piscine Pathogens

  • By activity-guided fractionation, gliotoxin was isolated as an antibacterial metabolite of the fungus Penicillium decumbens which was derived from the jellyfish Nemopilema nomurai. Gliotoxin was further evaluated for antibacterial activity against several piscine and human MDR (multidrug resistance) pathogens. Gliotoxin showed significant antibacterial activity against Gram-positive piscine pathogens such as Streptococcus iniae FP5228, Streptococcus iniae FP3187, Streptococcus parauberis FP3287, Streptococcus parauberis SPOF3K, S. parauberis KSP28, and Lactococcus garvieae FP5245. Gliotoxin showed strong activity especially against S. parauberis SPOF3K and S. iniae FP5228, which are resistant to oxytetracycline. It is noteworthy that gliotoxin effectively suppressed streptococci which are the major pathogens for piscine infection and mortality in aquaculture industry. Gliotoxin also showed strong antibacterial activity against multidrug- resistant human pathogens (MDR) including Enterococcus faecium 5270 and MRSA (methicillin-resistant Staphylococcus aureus) 3089.

Structure of the Tripartite Multidrug Efflux Pump AcrAB-TolC Suggests an Alternative Assembly Mode

  • Escherichia coli AcrAB-TolC is a multidrug efflux pump that expels a wide range of toxic substrates. The dynamic nature of the binding or low affinity between the components has impeded elucidation of how the three components assemble in the functional state. Here, we created fusion proteins composed of AcrB, a transmembrane linker, and two copies of AcrA. The fusion protein exhibited acridine pumping activity, suggesting that the protein reflects the functional structure in vivo. To discern the assembling mode with TolC, the AcrBA fusion protein was incubated with TolC or a chimeric protein containing the TolC aperture tip region. Three-dimensional structures of the complex proteins were determined through transmission electron microscopy. The overall structure exemplifies the adaptor bridging model, wherein the funnel-like AcrA hexamer forms an intermeshing cogwheel interaction with the ${\alpha}$-barrel tip region of TolC, and a direct interaction between AcrB and TolC is not allowed. These observations provide a structural blueprint for understanding multidrug resistance in pathogenic Gram-negative bacteria.

1981년부터 2004년까지 보건소 재치료 결핵 환자의 항결핵제 내성률 추이 (A Trend in Acquired Drug Resistances of Tuberculosis Patients Registered in Health Centers from 1981 to 2004)

  • 연구배경 : 치료 경력이 있는 환자의 약제 내성은 진료의사의 처방의 적절성 및 환자의 복용 순응도를 복합적으로 나타내어 주는 지표가 될 수 있으므로, 우리나라에서 재치료 대상 결핵 환자에서 장기간의 약제내성률 추이를 파악하고자 하였다. 방 법 : 연도별 보건소 등록환자의 약제감수성 검사 결과 및 결핵연구원의 전산 입력된 자료를 활용하여 1981년부터 2004년 사이에 주요 약제에 대한 내성률의 변화를 분석하였다. 결 과 : Isoniazid 내성은 90%에서 20%로 감소하였다. Ethambutol 내성률도 45%에서 6%로 감소하였다. Rifampin 내성률은 13%에서 28%까지 증가하다가 13%까지 감소하였다. 다제내성률은 rifampin 내성률보다 약 2-3% 낮았다. Pyrazinamide 내성률은 5% 미만에서 10% 정도까지 증가하였다가 다시 5%까지 감소하였다. 2차약제에 대한 내성률은 1-2% 정도였다. 내성 빈도는 남녀 간에 차이가 없었으며, 대도시 지역이 중소도시/농촌지역보다 낮은 약제 내성률을 보였다. 결 론 : 우리나라보건소에 등록되는 재치료 대상 결핵 환자의 주요 항결핵 약제에 대한 내성률은 2004년을 기준으로 isoniazid 20%, rifampin 13%, 다제내성 11%, ethambutol 6%, pyrazinamide 5% 기타 2차 항결핵제에 1-3%로 나타났으며 1981년 이후 모든 항결핵 약제에서 그 내성률이 유의하게 감소하고 있었다.

소아 화상환자에서 배양된 Acinetobacter baumannii에 대한 항생제 감수성 양상의 변화 (The Changing Patterns of Antibiotic Susceptibility for Acinetobacter baumannii in Pediatric Burn Patients)

  • 목 적 : 원내 감염 균주의 하나인 A. baumannii는 최근 다약제 내성을 가지며 성인에 대해서는 보고되고 있으나 소아 환자에 대한 체계적인 연구는 미흡한 실정이다. 이에 저자들은 소아 화상환자에서 분리된 A. baumannii에 대해 항생제 감수성의 변화 및 저항 균주의 출현에 대해 알아보고자 본 연구를 시행하였다. 방 법 : 1999년 1월부터 2005년 12월까지 만 7년간 화상을 주소로 한림대학교의료원 한강성심병원에 입원한 15세 미만의 환아들 가운데 A. baumannii가 분리된 환아 56명, 105주를 대상으로 후향적으로 자료를 분석하였다. 결 과 : 균이 분리된 환아의 남녀 성비는 1.15:1 이었으며 중앙연령은 48.3개월이었다. 균의 배양부위는 상처 부위 69례(65%), 객담 21례(20%), 혈액 6례(6%), 카테터 관 5례(5%), 기관 삽관용 튜브 2례(2%), 소변 2(2%)순이었으며 주로 3월과 11월, 12월에 분리되었으나 연중 분포하는 양상을 보였다. 항생제의 내성률은 2000년 이후 대부분 70% 이상을 보였으며, 98례(93.33%)에서 다약제 내성을 보였다. 한편 carbapenem과 ciprofloxacin에 대한 내성균주의 출현 여부에 대해 3례(25%)에서 내성균주가 출현하였고 항생제 사용 기간은 평균 10일로 매우 짧았으며 혈액에서 균주가 분리된 환아 6례는 모두 다약제 내성 균주였으며 이중 1례는 사망하였다. 결 론 : 다약제 내성 균주인 A. baumannii에 대해 원내 감염의 심각성을 인식하고 동시에 단순 격리보다는 철저한 치료 계획이 필요하다. 즉, 감수성 결과에 입각한 항생제 사용 및 새로운 항생제 조합, 의료인을 포함한 숙주가 될 수 있는 환경에 대한 철저한 관리 등이 필요하겠다.

Identification of a Novel Cassette Array in Integron-bearing Helicobacter Pylori Strains Isolated from Iranian Patients

  • Helicobacter pylori as the second most common cause of gastric cancer in the world infects approximately half of the developed countries population and 80% of the population living in developing countries. Integrons as genetic reservoirs play major roles in dissemination of antimicrobial resistance genes. To the best of our knowledge, this is the first study to report carriage of class 1 and 2 integrons and associated gene cassettes in H. pylori isolates from Iran. This cross-sectional study was conducted in Tehran among 110 patients with H. pylori infection. Antimicrobial susceptibility testing (AST) for H. pylori strains were assessed by the micro broth dilution method. Class 1 and 2 integrons were detected using PCR. In order to determine gene cassettes, amplified fragments were subjected to DNA sequencing of both amplicon strands. The prevalence of resistance to clarithromycin, metronidazole, clarithromycin, tetracycline, amoxicillin, rifampin, and levofloxacin were 68.2% (n=75), 25.5% (n=28), 24.5% (n=27), 19.1% (n=21), 18.2% (n=20) and 16.4% (n=18), respectively. Frequency of multidrug resistance among H. pylori isolates was 12.7%. Class 2 integron was detected in 50 (45.5%) and class 1 integron in 10 (9.1%) H. pylori isolates. The most predominant gene cassette arrays in class 2 integron-bearing H. pylori were included sat-era-aadA1, dfrA1-sat2-aadA1, blaoxa2 and, aadB whereas common gene cassette arrays in class 1 integron were aadB-aadA1-cmlA6, aacA4, blaoxa2, and catB3. The high frequency of class 2 integron and multidrug resistance in the present study should be considered as a warning for clinicians that continuous surveillance is necessary to prevent the further spread of resistant isolates.

Effect of Hypoxia on the Doxorubicin Sensitivity of Human MCF-7 Breast Cancer Cells

  • Intrinsic or acquired resistance to chemotherapeutic drugs is one of the major obstacles to effective cancer treatment. Hypoxia is widespread in solid tumors as a consequence of decreased blood flow in the tumor-derived neovasculature. The recent finding of a link between hypoxia and chemoresistance prompted us to investigate whether hypoxia induces doxorubicin resistance in human MCF-7 breast cancer cells. Low oxygen concentration decreased the doxorubicin sensitivity in MCF-7 cells. The expression of p-glycoprotein, a major MDR-related transporter, and those of apoptosis-related proteins (anti-apoptotic Bcl-2, Bcl-XL and pro-apoptotic Bax) were not altered by hypoxia in MCF-7 cells. Intracellular uptake of doxorubicin was significantly decreased under hypoxic conditions. Decreased cellular uptake of doxorubicin under hypoxia may contribute to causing doxorubicin resistance in these cells. The use of agents that can modulate the doxorubicin uptake for adjuvant therapy may contribute to improving the therapeutic efficacy of doxorubicin in breast cancer patients.
