CNS Relapsed T-cell Lymphoma in a Young Cat (어린 고양이에서 발생한 중추신경계로 재발한 T세포 림프종)

  • An 8-month-old domestic shorthair cat presented with decreased activity and anorexia. Diagnostic imaging revealed cranial mediastinal mass and enlarged mesenteric lymph nodes. Fine needle aspirates showed a marked increase in malignant lymphocytes. Multicentric lymphoma (stage V-b) was diagnosed. The cat treated with COP protocol chemotherapy, and complete remission was induced. CNS relapse developed 314 days after the initiation of chemotherapy. Treatment with rescue protocol greatly reduced the clinical signs for a short period. The cat was in partial remission for 33 days and overall survival time was 383 days. Multicentric T-cell lymphoma with brain involvement was confirmed after necropsy by histopathology and immunohistochemistry.

Multicentric T cell lymphosarcoma in a Jeju native boar

  • A 7-year-old Jeju native boar was requested to the Veterinary Pathology Laboratory of Cheju National University with a clinical signs of anorexia, melena, lethargy and sudden death. At necropsy, four coalescing firm masses were occupied in the abdominal cavity between ventral surface of stomach and pancreas. Individual mass was a yellowish white in color and up to 6 cm in diameter. These masses were not encapsulated and bulged from the cut surface. Liver and spleen were enlarged and pale nodules were presented in these tissues. Some yellowish white nodules up to 5 mm in diameter were scattered in kidneys. Histopathologically, lymphoblastic tumor cells were occupied in the abdominal masses, multifocal areas of liver, kidneys, and spleen. Morphologically lymphoblastic tumor cells were round to oval in shape, and medium to large in size. They had round to oval nuclei, moderate amount of eosinophilic cytoplasm, and many mitotic figures. Immunohistochemistry revealed that tumor cells were CD3-positive and $CD79{\alpha}$-negative, consistent with T-cell lineage. Based on gross, microscopic findings and immunohistochemistry, this case was diagnosed as porcine multi-centric T cell lymphosarcoma. In animals, as in human, the T-cell lymphomas are generally more aggressive than B cell types and respond less well to therapy. In our best knowledge, this is the first report for porcine T cell lymphosarcoma in Korea.

  • Castleman's disease is uncommon lymphoproliferative disorder as giant lymph node hyperplasia and angiofollicular lymph node hyperplasia. Multicentric variant of Castleman's disease, plasma cell type has been described that has more generalized lymph node involvement as well as involvement of other organ systems than localized type. Multicentric plasma cell type is frequently accompanied by systemic manifestations, such as weight loss, lowgrade fever and weakness. But the reported cases of pulmonary parenchymal involvement are rare and have almost consisted of hyalinized granuloma adjacent to a bronchus. We report a patient with Castleman's disease of the lung, pathologically proven interstitial pulmonary involvement.

Effects of glenohumeral corticosteroid injection on stiffness following arthroscopic rotator cuff repair: a prospective, multicentric, case-control study with 18-month follow-up

  • Background: This study aimed to analyze the efficacy of single-dose corticosteroid injection (CSI) administered at 6 weeks postoperative to treat stiffness following arthroscopic rotator cuff repair (ARCR). Methods: In this prospective, multicentric, case-control study, post-ARCR stiffness at 6 weeks was treated with either a single dose of intra-articular CSI (CSI group) or physical therapy with oral analgesics (non-CSI group). Pain intensity according to visual analog scale (VAS), functional outcome using the Constant Murley Shoulder Score, time to return to activities of daily living (ADLs), and retear rate were recorded at 6 weeks, 9 weeks, 12 weeks, 6 months, 12 months, and 18 months postoperatively in both groups. Results: A total of 149 patients (54.5%) in the CSI group and 124 patients (45.5%) in the non-CSI group were included in this study. Pain and function were significantly better in the CSI group at 9-week, 12-week, and 6-month (P<0.001) follow-up, whereas they were not significantly different when the groups were compared at 12- and 18-month follow-up. The mean duration to return to ADLs was significantly shorter (P<0.001) in the CSI group. The incidence of retears was not significantly different (P=0.36) between groups at the end of 18 months of follow-up. Conclusions: Single-dose intra-articular CSI administered at 6 weeks postoperative to treat post-ARCR stiffness significantly improved pain, function, and duration of return to ADLs without increasing the risk of retears compared to patients who did not receive intra-articular CSI.   • Endosalpingiosis is a condition that causes the non-neoplastic proliferation of ectopic tubal epithelium. Florid cystic endosalpingiosis is an atypical subtype that is very rarely reported. It presents as a mass-like feature and therefore needs to be differentiated from tumorous conditions. Here, we report the imaging findings of a case of multicentric florid cystic endosalpingiosis in the extraperitoneal pelvic cavity and the retroperitoneal spaces.

Giant Lymph Node Hyperplasia of Castleman - Report of 2 cases - (Castleman 거대림프절 증식증 2례 보)

  • Castleman`s disease, giant lymph node hyperplasia, is a rare benign disease. The lesion usually consists of a single node, unassociated with any abnormality of the adjacent lymph nodes or other organs. In the first accounts of giant lymph node hyperplasia of Castleman, the lesion was described as solitary and localized to the mediastinum, which is still the most frequent site of involvement. The disease occurs in all age groups and there is no particular sex preference. It is symptomless and is usually detected on chest films as an incidental finding. On a single involvement, it does not recur after excision, whether total or partial, and the main indication of operation is to rule out more serious tumors. Recently multicentric form appears to be a variant of classic giant lymph node hyperplasia and is associated with significant morbidity and mortality. Histologically, two distinct types have been reported; hyaline-vascular and plasma cell. The hyaline-vascular type of lesion is much more common than the plasma cell type. We report two cases of the hyaline-vascular type of Castleman`s disease.

Approach in the Surgical Management of Cardiac Myxoma - Clinical Experience and Long-term Result - (심장 점액종의 외과적 고찰임상 경험 및 장기 성적)

  • Between 1977 and 1987, 8 patients underwent excision of cardiac myxomas at the Hanyang University Hospital. All had a left atrial myxoma. There were 4 female and 4 male patients ranging from 15 to 62 years of age. Preoperative findings consist of symptoms and signs of congestive heart failure except one. Diagnosis was confirmed by echocardiography[8 cases] and angiography[2 cases], preoperatively. A biatrial operative approach was utilized in all but 2, who were small sized. Complete excision of the tumor with a cuff of normal tissue[1 was atrial wall and 7 were atrial septum] was performed. all heart chambers were carefully explored for presence of multicentric myxomas or tumor debris. There were no operative deaths or intraoperative embolization. Follow-up has been 1/3 to 10 years. There has been 1 late death, due to recurrence and 1 patient had reoperation for mitral regurgitation due to dilatation of the annulus by a huge tumor mass. Surgical excision of the myxoma can be performed with low morbidity, and it provides excellent and sustained symptomatic relief. The recurrence rate is low, but long-term follow-up and serial echocardiography are advisable.

Owners' assessments of their dog's quality of during palliative chemotherapy for lymphoma (화학요법으로 lymphoma 치료를 받은 애견의 삶의 질에 대한 보호자들의 평가)

  • Multicentric lymphoma에 걸쳐 화학요법치료를 받은 25case의 보호자들에게 그들 애견의 삶의 질의 평가를 조사했다. 이에 보호자들은 자기 애견의 삶의 질에 대한 68$\%$의 보호자가 lymphoma가 발생하기 전과 같이 좋았다고 이야기했고 나머지 32$\%$의 보호자들은 발병하기 전보다 더 좋진 않지만 만족한다는 답을 얻었다. 합병증은 52$\%$의 애견에서 발생되었다고 한다. 92$\%$의 보호자들은 그들 애견의 화학요법 치료선택에 대해서 후회가 없다고 했다. 이 결과는 화학요법 치료가 합병증을 유발함에도 불구하고 많은 보호자들이 그들 애견이 화학요법 치료를 받음으로써 그들 애견의 삶의 질이 좋았다고 생각한다는 것이다.

  • There have been numerous methods to obtain a stable arthrodesis of the tibiotalar joint. However, a controvercy has arisen over the most successful and reproducible method to obtain an arthrodesis. The objective of this report is to show that an ankle arthrodesis can be performed successfully and advantageously by using cannulated screws under arthroscopic control and that our csaes will be a part of the multicentric study of arthroscopic ankle arthrodesis in korea. Between May 1996 and May 1997, we perfomed 3 ankle arthrodesis under arthroscopy using cannulated screws. Two patients had post-traumatic osteoarthritis and one had rheumatoid arthritis. Clinical ankylosis in one case and radiological union in two cases were obtained in 12 weeks postoperatively. We propose that arthroscopic ankle arthrodesis is effective procedure in patient who has minimal deformity and bone loss, advantages like small incision, minimal morbidity, low infection rate and rapid return to normal daily activity, but more cases and comparision with open tecnique will be needed.

Chordomas Involving Multiple Neuraxial Bones

  • We present a patient with multifocal symptomatic osseous chordomas having unusual growth patterns with review of the pertinent literature. The patient was 62-year-old male and had multiple osseous chordomas located in sacral, thoracic, and paraclival jugular foramen areas. There was no metastasis in other organs. All affected sites were osseous. The multicentric chordomas are extremely rare. This case could be considered as a chordoma involving multiple neuraxial bones. But, the possibility of multicentricity could also be thought. In such cases radical resection should be performed for each lesion at the initial diagnosis. If complete surgical resections are infeasible or impossible, preoperative or postoperative radiation therapy should be planned for the highest possibility of successful treatment.
