병원 간 전원 상황에서 이동통신망을 이용한 음성화상정보통신 애플리케이션의 유용성에 관한 연구 (A feasibility study of audio-video communication application using mobile telecommunication in inter-hospital transfer situations)

  • Purpose: The aim of this study was to evaluate the usefulness of video communication with medical staff located at a remote location, through a communication application connected to a long term evolution (LTE) mobile communication network in a moving ambulance. Methods: In this study, we recruited patients who were transferred by mobile intensive care unit ambulance from one hospital to another. In the moving ambulance, the information of the patient was transferred to a physician using the application in real time. Recorded video files were evaluated by emergency physicians with experience in video direct medical control. Results: A total of 18 patients were evaluated, and the average score was 5.9 out of a possible 9. It was expected that applying the use of the technology to actual clinical sites would enable the sites to provide assistance. Conclusion: In this study, we confirmed the possibility of benefiting from the clinical field when using the video-audio communication application which is connected to the remote location in real time through the current LTE mobile communication network in the ambulance.

이동 중 구급차에서 1급 응급구조사의 기관내 삽관 (Endotracheal Intubation of Paramedics in a Moving Ambulance)

  • 본 연구는 이동 중 구급차에서 응급구조사의 삽관자세 변화에 따른 시야의 높이 변화가 기관내 삽관의 신속성에 영향을 미치는지 비교하고 환자의 삽관자세 변화가 기관내 삽관의 신속성에 영향을 미치는지 분석하여 심정지 및 외상성 응급 환자에게 효과적인 전문기도관리를 시행하는데 목적이 있다. 연구대상은 C도 13개 소방서에 근무하고 있는 1급 응급구조사 60(대조군30명, 실험군30명)명을 편의추출하여 시행하였으며, 자료 분석은 SPSS WIN 14.0 Version을 사용하여 하였다. 연구결과 전문기도관리의 신속성 향상을 위해서는 환자의 냄새맡기 자세와 주들것과 응급구조사 시트 사이 공간에서의 기관내 삽관이 효과적일 것으로 사료되고 실험 후의 삽관 자신감이 유의하게 상승 하였으므로 전문기도관리의 효율성 향상을 위해서는 구급차 내에서의 삽관자세와 방법에 대한 지속적인 교육(훈련)이 필요할 것으로 사료된다.

119 구급 서비스의 미이송 시간 분석 (Analysis of response time for patient non-transport by 119 ambulance services)

  • Purpose: The purpose of the study was to investigate the causes of non-transport in 119 ambulance services and to describe the difference in response time according to the reason for non-transport. Methods: This study analyzed 42,415 non-transport cases out of 123,158 cases using prehospital care reports in a metropolitan city. The Kruskal-Wallis test and the Wilcoxon-Mann-Whitney test were used to 6,857 cases for which time was recorded. Results: Non-transportation in 20${\bigcirc}{\bigcirc}$ comprised 33.1% of all ambulance services. The reason for non-transport were other reasons (25.5%), cancellation (23.8%), and moving to other vehicles (21.7%). There were differences in ambulance service times according to the reasons for non-transport. The activation interval was the longest (2.68 minutes) in the absence of the patient, and the response interval was the shortest (4.96 minutes) among the cancelled case. The total interval was the shortest time (21.97 minutes) in the cancellation cases and the longest time among the death cases (32.23 minutes). Conclusion: It is important to suggest the direction of development of emergency services by identifying the reasons for non-transport by ambulance services and describing the response time according to the reason for non-transport.

1급 응급구조사의 비디오후두경 기관삽관과 직접후두경 기관삽관의 신속성 및 정확도 비교 (Comparison of Video Laryngoscope and Direct Laryngoscope on Rapidity and Accuracy in Tracheal Intubation by Paramedic)

  • Objective : This study compares Video laryngoscope and Direct laryngoscope in tracheal Intubation on rapidity and accuracy by paramedic and aims to improve efficiency of airway management and survival rate in pre-hospital treatment for the patients with severe trauma, cardiac arrest or dyspnea caused by acute diseases. Methods : 60 paramedics were recruited from 13 fire stations located in C province. With the consent of the paramedics, likelihood ratio test was carried out and they were divided into two different groups; DL group (30) and GVL group (30). Regarding intubation conditions, difficult airway grade I, grade II and grade III as well as sniffing position and neutral position were examined. This study also compared between ambulance in motion and in stand still. Frequency, average and standard deviation were analyzed with statistics program, SPSS WIN 17.0 and repeated measure design was introduced to examine inter-relations between position, grade and groups. Results : Intubation was performed more rapidly in neutral position and GVL than in sniffing position and DL(F = 15.260, p = .000). Rapidity value was better with grade I and grade II than grade III and better with GVL than DL(F = 32.629, p = .000). Accuracy value was higher with neutral position and GVL than sniffing position and DL(F = 5.008, p = .011). grade III was less accurate than grade I, grade II and GVL was more accurate than DL(F = 10.966, p = .000). Ambulance motion status did not show any statistically significant differences in accuracy and rapidity. Conclusion : Given this study results, neutral position is better for the patient with severe trauma. For a better survival, GVL intubation can be considered since GVL can enhance accuracy as well as rapidity regarding difficult airway. Since there is no significant differences in ambulance motion factors, intubation can be recommended even in moving ambulance for shortening traveling time to a hospital.

응급상황시 이송중인 구급차에서 신뢰할 수 있는 혈압계 검증 (Verification of Reliable Blood Pressure Monitor in a Moving Ambulance during an Emergency)

  • 이 연구의 목적은 이동하는 구급차에서 혈압계의 신뢰성을 검증하기 위해 다양한 도로 상황에서 수동 및 자동혈압계를 이용하여 혈압 및 시간 측정값을 분석하는 것이다. 첫째, 비포장 도로에서 수동 혈압계 촉진 결과 수축기 혈압 편차가 5mmHg로 나타났다. 그러나 자동 혈압계는 2개의 측정 실패, 1개의 판독 실패를 보여주었으며 측정된 수축기 혈압 편차는 35mmHg였다. 측정 시간은 자동 혈압계보다 평균 102초 빨랐다. 둘째, 과속방지턱을 넘을 때 수동 혈압계의 촉진은 130mmHg로 일정하게 유지되었다. 그러나 자동 혈압계의 수축기 혈압 편차는 52mmHg였다. 측정 시간은 자동 혈압계보다 평균 61초 빨랐다. 마지막으로 급커브 도로에서 수동 혈압계 촉진 결과 수축기 혈압 편차는 5mmHg로 나타났다. 자동 혈압계는 판독 실패가 1회 있었고 측정된 수축기 혈압 편차는 21mmHg였다. 측정 시간은 수동 혈압계가 자동 혈압계보다 101초 빠른 것으로 나타났다. 그 결과 응급상황시 이송중인 구급차에서 수동 혈압계는 혈압 측정이 일정하고 측정 시간이 짧아 높은 신뢰도를 보였다.

이송 중 구조자용 벨트(EMT-belt) 착용에 대한 가슴압박 성공률 비교 (Comparison of success rates of chest compressions performed with and without a rescuer's belt during transfer)

  • Purpose: The aim of this study was to provide basic data to improve the survival rate of pre-hospital cardiac arrest patients. This study suggests a more effective method of performing effective chest compressions for a cardiac arrest patient in a moving ambulance. Methods: To compare the differences between gender and license (qualification), SPSS 18.0 (Windows) was used. Independent and paired t-tests were used for differences between before and after wearing a rescuer's belt. Results: The success rate of chest compressions according to gender was higher in males ($68{\pm}21.91%$) than in females ($25.04{\pm}16.88%$). There was no difference according to license ($44.70{\pm}26.63$ for paramedic, $45.05{\pm}19.25$ for nurse). However, the depth (mm) and the success rate (%) were improved during the evaluation of chest compressions when wearing the rescuer's belt (depth: $46.95{\pm}6.49$ vs. $49.55{\pm}6.05$, success rate: $44.80{\pm}24.66$ vs. $57.39{\pm}26.823$). Conclusion: Wearing a rescuer's belt in an ambulance during patient transport can result in deeper and more accurate chest compressions; therefore, it is expected to be effective in recovering the circulation of patient with cardiac arrest.

  • Tele-medicine and emergency medical system are necessary for moving from an accidental point or far distance to a hospital and emergency treatment or home treatment before a hospital. Emergency treatment is extremely important in the case of death before arriving a hospital and deformed or disabled by medical treatment delay. A necessary element for this medical system is the emergency communication system. This system is on preparing for an ability of furnishing patient status to a corresponding health service by monitoring the patient at an ambulance of the accident place. This is the transportation of basic biological information of a patient to a medical center by wireless communication system and the corresponding hospital or medical center examine the patient by monitoring, then they can send emergency medical order to the patient for emergency treatment. The TRS is most efficient way of emergency medical communication system, which is currently used with popularity. In this paper studied simultaneously a way of detecting and transporting bio-logical signals, and monitoring of transporting data with communication of voice in the accident place or ambulance.

구급차량 내 전자파 방출에 관한 조사 (Research on the emission of electromagnetic waves in ambulance)

  • Purpose: The study aims to provide basic data to enhance the health of paramedics responsible for patient transport and treatment by analyzing the exposure level of paramedics to electromagnetic waves generated by electric devices used in ambulances. Methods: The study measured electromagnetic waves in ambulances in N region from July to December 2018. ME3030B produced by German Gigahertz Solutions was used to measure these waves and the maximum value was selected by moving it slowly in various directions. Each measurement part was selected and the mean value was calculated by repeatedly measuring at 10-minute intervals three times in total: $1^{st}$, $2^{nd}$, and $3^{rd}$ phase. Results: Among the electrical devices installed in the patient room of the ambulance measured at distances of 1 cm or 30 cm, results showed a high level of electric waves at the inverter ($26.25{\pm}39V/m$) and high level of electromagnetic waves ($564.00{\pm}31.75nT$) at the ozone sterilizer. According to measurements toward the front near the driver's seat, results indicated high levels of electric waves ($3.67{\pm}1.15V/m$) and electromagnetic waves ($450.00{\pm}19.52nT$) at the black box hard drive. Conclusion: Electromagnetic waves within the ambulance were stable and not beyond the range that might impact human health. However, in the case of the black box hard drive ($3.67{\pm}1.15V/m$, $450.00{\pm}19.52nT$) located under the passenger seat, it may have a direct effect on the human body and, thus it is necessary to move it to a storage area further away from the paramedics to minimized the impact.

원격응급시스템의 방사선 영상장치 구현 (An Implementation of Radiologic Imaging Device of Remote Emergency Medical System)

  • 구급차에 장착하거나 사고 현장에서 사용할 수 있는 방사선 영상장치를 구현하였다. DC 12[V] 입력 전원에서 X-ray 튜브를 통해서 X-ray를 발생하는 장치를 구성한 후 오실로스코프를 활용하여 특성 실험을 하였다. 그리고 발생된 X-ray에 대해 디지털 디텍터를 이용하여 파일 형태로 저장하는 실험을 하였다. X-ray를 발생시킨 후 디지털 디텍터로 검출한 결과, 파일 크기가 1.67[MB]이고, jpg 형태로 이루어진 방사선정보를 저장할 수 있었다. 구현된 방사선 영상장치는 환자가 의사로부터 공간적으로 멀리 떨어져 있어 시간적으로 진단과 치료가 동시에 이루어지기 어려운 상황에 효율적으로 대처할 수 있는 장점이 있다. 따라서 구현된 방사선 영상장치는 긴박한 상황이 발생한 사고 현장이나 이동 중인 구급차 내에서 X-ray 촬영을 함으로써, 환자의 X-선 정보를 응급의료 센터의 의사에게 전달하고 의사의 처방을 받을 수 있는 의료 선진화를 구현하는 데 기여할 수 있을 것으로 기대된다.

병원 전 환경의 외상성 응급환자를 위한 전문기도관리 (Advanced airway management for the prehospital traumatic patient)

  • 본 연구의 목적은 주행 중인 구급차에서 기관내 삽관과 후두마스크(LMA) 삽입의 신속성과 성공률을 비교하여 구급대원의 전문기도관리 능력을 향상시키는 데 있다. 연구방법은 C 지역 구급대원 중 1급 응급구조사 60명(대조군 30명, 실험군 30명)을 편의추출 하여 마네킹을 대상으로 측정을 하였다. 자료 분석은 SPSS WIN 14.0 Version을 사용하였다. 연구결과 기관내 삽관이 냄새맡기 자세에서 신속성이 좋은 것으로 나타났고(t=-4.038, p<.001), 중립자세에서 그룹 간 신속성 차이는 없었으나 기관내 삽관 시 16명(53.3%)에게 치아파절이 발생했다. 또한, 그룹 간 성공률 차이는 없었고 두 그룹 모두 실험 후 자신감이 향상된 것으로 나타났다. 결론적으로 냄새맡기 자세가 불가능한 외상성 환자는 후두마스크(LMA)의 사용이 효과적이고 구급대원의 기도관리 능력을 향상시키기 위하여 지속적인 교육이 필요할 것으로 사료된다.
