The purpose of this study is to obtain the normal range of mandibular movement for 13 year old boys and girls, and to compare it to the range of mandibular movement of class III patient and normal adult for diagnosis and prognosis of class III malocclusion and TMJ dysfunction. The subjects were 20 normal boys 20 normal girls 22 class III boys and 18 class III girls. The author measured the ranges and shapes of movement of the mandible in the frontal, sagittal and horizontal trajectories using Saphon visitrainer C II (Tokyo Shizaisha Inc.) for the subjects. The results obtained are as follows: 1) The mean for maximum right laterotrusion in the frontal trajectory were 11.96mm in N.B., 11.10mm in N.G., 11.32mm in III. B., 11.24mm in III G. The mean for maximum left laterotrusion were 11.48mm, 11.0mm, 10.91mm, 10.44mm respectively, Area of border movement were 7.16cm in N.B., 6.59cm in N.G., 7.29cm in III. B., 7.50cm in III. G. 2) The mean for maximum protrusion in the sagittal trajectory were 11.7mm in N.B., 11.4mm in N.G.,11.87mm in III B., and 11.02mm in III. G. 3) The mean for maximum protrusion in the horizontal trajectory were 10.20mm in N.B.,10.00 mm in N.G., 9.12mm for III. B. and 9.36mm in III. G. 4) The mean for maximum protrusion of Class III subjects were shorter than those of normal subjects. 5) There was no sexual difference in the range of mandibular movement for 13 year old subjects. Objective : Essential tremor(ET) is the most common movement disorder, however, there has been little agreement in the neurologic literature regarding diagnostic criteria for ET. Familial ET is an autosomal dominant disorder presenting as an isolated postural tremor. The main feature of ET is postural tremor of the arms with later involvement of the head, voice, or legs. In previous studies, it was reported that ET susceptibility was inherited in an autosomal dominant inheritance. As previous results, it would suggest that ET might be associated with defect of mitochondrial or nuclear DNA. Recent studies are focusing on molecular genetic detection of movement disorders, such as essential tremor and restless legs syndrome. Moreover, authors have analysed mitochondrial DNA(mtDNA) from the blood cell of positive control(PC) and ET patients via long and accurate polymerase chain reaction(LA PCR). Materials & Methods : Blood samples were collected from PC and 9 ET patients. Total DNA was extracted twice with phenol followed by chloroform : isoamylalcohol. For the analysis of mtDNA, LA PCR was performed by mitochondrial specific primers. Results : With this technique, deletions of large quantities were detected within several regions of mtDNA in ET patients except for D-loop and CO I regions. Conclusion : The authors believe that ET is a genentic disorder with deficiency of mitochondrial DNA multicomplexes and mitochondiral dysfunction could be one of major causative factors of ET. Mitochondrial dysfunction may play an important role in the pathogenesis and possibility of disease progression among familial group with ET patients.
Amomum cadamomum Linne (ACL) has long been utilized against the inhibited qi movement related diseases such as dyspepsia, acute gastroenteritis, vomiting and diarrhea in Korean medicine. We speculated that ACL could improve the metabolic disorders such as obesity and type 2 diabetes through removing the phlegm-dampness and promoting the qi movement or stagnation. This study was designed to investigate effects and molecular mechanisms of ACL extract on the improvement of adipocyte dysfunction induced by TNF-α in 3T3-L1 adipocytes. Potential roles of ACL extract in the lipogenesis, inhibition of inflammatory cytokines and insulin resistance, were investigated in this study. Also, we examined the adipose genes and signaling molecules related to insulin resistance and glucose uptake to elucidate its mechanism. Our data demonstrated that TNF-α significantly incresed the release of lipid droplets and the production of MCP-1 and IL-6 from adipocytes. In gene expression, TNF-α reduced the expression of aP2, PPARγ, C/EBPα, GLUT4, and IRS-1 related to lipogenesis and insulin sesitivity, while TNF-α increased the expression of MCP-1 related to inflammation. In addition, TNF-α down-regulated the PPARγ and IRS-1 protein and up-regulated the IRS-1 Ser307 phosphorylation. These alterations induced by TNF-α were prevented by the treatment of ACL extract. Thus, our results indicate that ACL extract can be used to prevent from the TNF-α-induced adipocyte dysfunction through insulin and PPARγ pathways.
Recently, the number of the elderly has increased according to the improvement of socioeconomic status and the efficient medical care system. In spite of the development of medicine, the elderly suffers from the various health problems caused by weakness of musculoskeletal system, cardiopulmonary function and immunologic dysfunction. Regular rhythmic exercise program is known to be one of the effective tools to enhance the health condition in the elderly. However, there has been few studies to evaluate the comprehensive effects of rhythmic movement program on the elderly. This study was focused to evaluate the indices of cardiopulmonary function, life satisfaction, calcium, phosphorous, osteocalcin and deoxypyridinoline which are the essential factors of health problems in the elderly women. Twenty six subjects, aged between 68 and 72, who can do the ordinary activities and do not have cardiovascular dysfunction and mental disorder, participated in this study. They were divided into two groups: 13 in the experimental group and 13 in the control group. The experimental group participated in the rhythmic movement program at the welfare center located in G-city. The program were consisted of three sessions a week during 10 weeks. Each session had three parts: warming up(10 minutes), main exercise(40 minutes), finishing(10 minutes). Heart rate, blood pressure and peripheral arterial oxygen saturation were measured for the evaluation of cardiopulmonary function. Serum calcium, phosphorous, osteocalcin and urine deoxypyridinoline were measured as the indices of bony metabolism. Data were analyzed with mean, standard deviation, $x^2$-test, t-test, paired t-test using SPSS PC+ program. The results of this study were as follows. 1) Heart rate of the experimental group showed significant decrease following the rhythmic movement program. Peripheral arterial oxygen saturation of the experimental group showed significant increase following the program. 2) The degree of life satisfaction of the experimental group showed significant increase following the program. 3) Calcium showed significant decrease following the program but remained within normal range. There was no significant difference of phosphorous between two groups. 4) Osteocalcin, the index of bone formation, showed no significant change following the program, but significant increase in the experimental group comparing with the control group. 5) Deoxypyridinoline, the index of bone resorption, in urine of the experimental group showed significant decrease following the program. In conclusion, the rhythmic exercise program in the elderly showed the improvement of physiologic function and favorable effects on life satisfaction and bony metabolism. According to the above results, the regular rhythmic movement program can be strongly recommended for the improvement of health in the elderly women.
지난 여러 해 동안 치매에서 나타나는 안구운동기능부전에 대해 수많은 논문이 발표되었다. 안구운동검사는 뇌의 기능을 평가하는 비침습적인 검사이며, 치매와 연관된 뇌의 이상을 밝혀내는데 유용한 방법이다. 저자는 치매와 연관된 안구운동이상에 대한 지금까지의 여러 연구들을 고찰해 보았다. 알쯔하이머병에서는 신속안구운동의 잠재기가 증가하고 정확도가 감소하며 항신속안구운동의 오류가 증가 한다. 전측두엽 치매 환자에서는 반사성 신속안구운동 억제의 장애와 항신속안구운동 잠재기와 오류 증가 소견이 나타난다. 그리고 헌팅톤병에서는 수의적인 신속안구운동의 시작 지연, 느린 신속안구운동, 항신속안구운동의 오류와 잠재기 증가 소견이 나타난다. 파킨슨병 치매와 루이체 치매 환자에서는 반사성 신속안구운동과 복합성 신속안구운동 실행 장애가 보고되었다. 크로이츠펠트-야콥 병에 관한 연구는 많지 않으며, 안구운동장애는 치매 증상이 명확해진 이후인 질병의 말기에 나타나고, 이차적으로 소뇌와 전정기관을 침범하게 된다고 한다. 진행성 핵상마비에서는 느린 신속안구운동과 측정저하 신속안구운동이 수직방향주시 장애가 오기 이전에 나타나는 경우가 많다. 수의적인 눈꺼풀 운동의 기능부전도 진행성 핵상마비의 특징적인 증상이다. 결론적으로 치매 환자들은 다양한 비정상적인 안구운동장애를 나타내며 이는 피질, 피질하 기능부전과 연관되어 있다. 치매 환자의 안구운동장애에 대한 다음 단계 연구는 치매에서 나타나는 임상적인 증상이 뇌의 어떤 부위 이상 때문에 발생하는지를 좀 더 명확하게 밝히는 것이 될 것이다.
Background: The functioning of the serratus anterior (SA) muscle is essential to normal scapulohumeral rhythm during forward flexion (FF) of the shoulder. Also, SA weakness and overuse of the upper trapezius (UT) is observed in patients with shoulder dysfunction and trapezius myalgia. We designed a combination exercise involving FF and scapular protraction with resistance (CFFSP) to activate the SA muscle and to deactivate the UT muscle. Objects: The purpose of this study was to determine whether or not CFFSP would be more effective in activating the SA muscle than FF alone and FF with scapular protraction (FFP). Methods: Nineteen subjects (12 men and 7 women) participated in this study and performed FF, FFP, and CFFSP at $120^{\circ}$. Surface electromyography was applied to the SA, UT, and pectoralis major (PM) muscles, as was one-way analysis of variance (ANOVA) with repeated measures. Statistical significance was set at .05. Bonferroni adjustment was used to counteract the problem of multiple comparisons, with a statistical level of significance of .017 (.05/3). Results: A statistically significant difference was found in relation to the three positions for the SA muscle (p<.001) and the SA/UT ratio (p=.005) using ANOVA. Significantly different results, depending on the position, were also demonstrated using the Bonferroni post-hoc test for the SA muscle ($FF=28.27{\pm}16.20$, $FFP=45.66{\pm}15.81$, and $CFFSP=62.4{\pm}27.21$) and for the SA/UT ratio ($FF=3.04{\pm}2.14$, $FFP=3.61{\pm}2.38$, and $CFFSP=5.95{\pm}3.01$). Significant differences between the three positions was not found regarding the average amplitude of SA/PM muscle ratio (SA/PM: p=.060). Conclusion: We recommend the use of CFFSP to strengthen the SA muscle at $120^{\circ}$.
The hub of weight bearing in the human body for both static and dynamic activities is the lumbopelvic region. It is a key region of extraordinary stability, since the trunk and ground forces converage in this region. The two sacroiliac joints form an integral part of this lumbopelvic unit. Considerable effort has been expended to study and quantify the normal range of movement of the sacroiliac joints Mitchell suggests that the ilium rotates in a posterior direction at heel strike and progresses in an anterior direction as the individual passes through the stance phase. The overall key appears to lie in determining the weight-bearing pattern of the sacroiliac (lumbopelvic) region from above and below that results in the familiar pain of sacroiliac dysfunction, assessing the status of the injured tissues, and intervening with the proper treatment protocols that maximize the body's healing processes. The purpose of this chapter is to provide a comprehensive overview of the sacroiliac joint's tissues and biomechanics, as well as concepts of evaluation and treatment. This overview is aimed at assisting the clinician in identifying the forces that are potentially destructive to the lumbopelvic tissues.
The author performed on epidemiological study of the TMD in 189 elderly people and 195 young people with Helkimo index. The clinical dysfunction index was based on data from clinical examination and the anamestic dysfunction index was based on data from the interview with the investigated person. The results were as follows : 1. In the elderly population, 27.5% reported that they had subjectively symptom of TMD but 43.4% had sign and symptoms of TMD in clinical examination. 2. The mean values for maximal opening differed significantly between elderly and young populations, 47.22mm and 51.44mm (p<0.001) and less than a 40mm opening was observed 7.9% in elderly population, 0.5% in young population (p<0.001). 3. The mean values for lateral movement to the right and left did not differ with age, which were 8.56mm and 8.47mm in elderly population, 8.90mm and 8.81mm in young population, but the mean value for protrusion differed significantly between elderly and young populations, 6.89mm and 7.64mm (p<0.01). 4. A higher incidence of TMJ noise was recorded in the elderly and young populations than young population, especially crepitus, but a higher incidence of clicking was recorded in young population (p<0.05).

The purpose of this review is to ascertain the integrated biomechanics among the lumbo-pelvic-hip complex by consideration of literature and to give sufficient information to be able to render accurate assessment and treatment for the syndromes described.
Purpose: The purpose of this study was to classify patients with chronic back pain according to the degree of their back pain, and to compare the pain dysfunction index with the qualitative changes in abdominal muscles. Therefore, we aimed to provide a basis for the treatment intervention method for patients with back pain. Methods: Twenty patients with chronic back pain were purposive sample to a group of 10 patients with a back pain index of 60 % or more and a group with less than 60 % of back pain, and the subjects who voluntarily participated in the study After receiving the letter, I conducted the research the dysfunction of back pain was measured by the Korean version of the Oswestry Disability Index (KODI), and the ultrasonic wave (Ultrasound MyLabOne, ESAOTE, Italy) And the white area index, and the abdominal muscle movement was used as the exercise instrument POWER breathe K5 (Hab direct, UK), which strengthens the respiratory muscles through threshold-muscle traction. Result: In this study, patients with chronic back pain were subjected to breathing exercises, which led to the decrease in back pain dysfunction. The ultrasonographic analysis of abdominal muscles revealed that both the white area index and muscle image density in the skeletal muscle and in the outer muscle of the abdomen gradually decreased over time. Conclusion: It is thought that introducing back pain patients to abdominal muscle reinforcement training is effective in improving the functions of the patients' muscles, thus increasing their quality of life.
