6 Clinical Reports of Temporary Severe Amnesia Patients -focusing on amnesia, hysteric convulsion, dissociative disorder (단기 기억상실을 주증(主症)으로 하는 6례(例)의 임상보고 -중기(中氣), 건망(健忘), 해리성 기억장애 중심으로)

  • Dissociative disorder is a psychiatric disorder characterized by a sudden loss of memory, but which has no organic disease or explanation. It usually occurs after heavy psychosocial stress or traumatic experience. A transient cerebral ischemic attack (TIA) is an acute episode of temporary and focal loss of cerebral function of vascular origin. TIAs are rapid in onset; symptoms reach their maximal manifestation in fewer than 5 minutes. Manifestations are of variable duration and typically last 2-15 minutes(rarely as long as 24 h). Most TIA durations are less than 1 hour. Of concern is the careful detection of changes in behavior, speech, gait, memory, movement, and vision. TIAs are uncommon in persons younger than 60 years. I treat 6 cases of Sudden Temporary Amnesia Patients with oriental medicine and they are improved. All of them had amnesia for $6{\sim}10\;hours$. During that time, they show behavioral changes and they are not on the state of unconsciousness. After recovery, they also forget what happen at the time. they have some emotional reason too. In conclusion, 4 cases of them belong to dissociative disorder and 2 other cases, TIA.

  • Objective : This single-subject study explored the effect of short-term intensive sensory integration therapy program which was provided in a 2006 sensory integration treatment course on adaptive response of a four-year-girl with sensory modulation disorder(mixed pattern). Method : Based on results of the assessment to the child, her therapist offers an organized sensory integration program during four days. Results : Improvement of child's movement quality and variety in physical environment in therapy rooms and interaction with her therapist were observed. Conclusions : This report of the case will be helpful for occupational therapists who plan to intervent with a short-term intensive sensory integration therapy program. In addition, the need for consideration of consultation with the child's parents is discussed.

  • Tic disorder show purposeless, repeated, unexpected, involuntary behavior and voice, can be divided into motor, vocal tic. this is about a patient who suffered from motor tic, tachycardia and other symptoms. We treated him with Sa-am acupuncture and other oriental medicine from the viewpoint of weak of kidney power. involuntary movement was estimated by doctor with Yale Global Tic Severity Scale(YGTSS). The patient's motor tic, tachycardia and other symptoms were improved and YGTSS also decresed.

  • A 3D camera-based on-site work-related musculoskeletal disorder risk assessment(WMDs) tool has been developed. The device consists of Kinect a 3D camera manufactured by Microsoft, a servo-motor, and a mobile robot. To complement inherent narrow field of view(FOV) of Kinect, Kinect is rotated according to PID servo-control algorithm by a servo-motor attached underneath, to track movement of a subject, producing skeleton-based motion data. With servo-control, full 360 degrees tracking of a test subject is possible by single Kinect. It was found from experimental tests that the proposed device can be successfully employed for on-site WMDs risk assessing tool.

  • Alcohol has been used as sedatives historically. The effect of alcohol on sleep is different according to its dose, timing of ingestion, and drinking frequency. Sleep problems may play a role in the development and course of alcohol-related disorders. Insomnia in alcohol-dependent patients is common and early treatment of insomnia may reduce the rate of relapse. Sleep apnea, restless legs syndrome, periodic limbs movement disorder, and altered circadian rhythm may be more frequent in this patients. Management of sleep and alcohol problems is important in treating alcohol-related disorder and sleep disorders, respectively.

  • Sleep changes substantially with age. There is a phase advance in the circadian sleep cycle and increased waking after sleep onset. The elderly people wake more frequently during the night and experience fragmented sleep and excessive daytime sleepiness. The prevalence of sleep disorders increases with age, and the composition of sleep disorders in the elderly differs from that in the young. The most frequently encountered sleep disorders are psychophysiologic insomnia, sleep disturbance due to dementia, sleeprelated respiratory disorder, restless legs syndrome and periodic limb movement disorder, and REM sleep behavior disorder. To treat the elderly sleep problem appropriately, it is important to know how sleep pattern changes as we age and to understand the cause of sleep-related symptoms. This article will review the sleep physiology and common sleep disorders in the elderly.

  • A Case Report of Communication disorder associated with Post-stroke Objectives : The Objective Is the Report of Improvement in Communication Disorder Associated with Post-Stroke. Methods : Acupuncture And Medicinal Treatments Were Performed from 17th March 2006 until 8th April 2006. The Medicines Bangpungtongsung-san and Chungpesagan-tang Were Used to Improve Patient Bowel Movement. The Sa-Am Acupuncture Method Was Used to Improve Patient Communication Disorders. Every 5 Days Articulation Accuracy, Vowel Accuracy, Alternation and Speed of Reading Sentences Were Evaluated. Results : After 23 Days of Treatment There was Improvement of Articulation Accuracy, Alternation and Speed of Reading Sentences.

Development of Diagnostic and Assessable Questionnaires for Cervical Movement System Impairment Syndromes (경추 부위 동태손상증후군의 진단·평가를 위한 설문 문항 개발)

  • Objectives The aim of this study is to develop diagnostic and assessable questionnaires for cervical movement system impairment syndromes. Methods We reviewed the previous study and literature, and organized various checkable items for differential diagnosis of four different cervical movement system impairment syndromes. Next, we selected items which can be developed as questionnaire items. Finally, we conducted a face validity study with twelve Korean medical doctors and carried out survey research to evaluate the importance score of the items with three experts. Results We developed a diagnostic and assessable questionnaire as follows: 9 items for cervical extension syndrome; 5 items for cervical flexion syndrome; 9 items for cervical rotation syndrome. By conducting 2 rounds of survey research, we were able to bridge the differences in the importance score of each item. Conclusions A questionnaire for the diagnosis and assessment of movement system impairment syndromes was developed. This questionnaire holds promising applications for objective diagnosis and assessment of cervical movement system impairment syndromes. This tool may also be used for detecting the sub-health status of musculoskeletal systems.

Difference in Sleep Characteristics between Restless Leg Syndrome With and Without Periodic Limb Movement during Wakefulness (각성 중 주기성사지운동 여부에 따른 하지불안증후군 환자의 수면 특성 차이에 대한 비교연구)

  • Objectives: Restless leg syndrome (RLS) is a common sensorimotor disorder and is frequently associated with periodic limb movement in sleep (PLMS). Also about one third of patient with RLS have periodic limb movement during wakefulness (PLMW). However there is little research on the correlation between PLMW and RLS. We aimed to evaluate difference in sleep characteristics between patients with RLS with PLMW and those without PLMW. Methods: Our study included twenty eight RLS patients. Subjects underwent suggested immobilization test (SIT) prior to one full-night polysomnography study. Patients were classified into two groups according to the presence of PLMW based on SIT-PLMW index. Polysomnographic findings, subjective sleep quality, and hematologic results were analyzed and compared between the two groups. Results: Mean age of patient with frequent PLMW (SIT-PLMW ${\geq}40/hr$) was significantly higher. RLS patients with frequent PLMW were also significantly related to insomnia severity. The PLMS index was higher in patients with PLMW and showed a significant correlation with the PLMW index. Conclusions: PLMW influence sleep quality such as insomnia and is correlated with movement during sleep.

  • In this article, we review the efficacy of early interventions after traumatic incidents and during acute stress disorder (ASD). There are some evidences that psychopharmacological medications such as propronolol, morphine, and hydrocortisone are effective in the prevention of posttraumatic stress disorder (PTSD). Considering the role of selective serotonin reuptake inhibitors in hippocampal neurogenesis and an animal model of PTSD, early administration of selective serotonin reuptake inhibitors is also fairly promising. Other pharmacological treatments including benzodiazepines did not treat ASD nor prevent PTSD. There are good evidences that cognitive behavioral therapy including cognitive therapy and prolonged exposure is a valuable intervention for ASD and the most effective prevention for PTSD. No contolled researches on eye movement desensitization&reprocessing, psychodynamic psychotherapy and hypnotherapy have performed. Recent randomized controlled studies using psychological debriefing did not prove as a useful intervention for the prevention of PTSD until now, although the efficacy of debriefing has been at the centre of controversy.
