수정된 강제-유도운동치료와 양측성 상지훈련이 만성 뇌졸중 환자의 상지 수행 능력에 미치는 영향 (The Effects of Modified Constraint-Induced Movement Therapy and Bilateral Arm Training on the Upper Extremity Performance of Individuals with Chronic Hemiparetic Stroke)

  • Purpose: The intention of this study was to investigate the effects of modified constraint-induced movement therapy (mCIMT) with bilateral arm training (BAT) on the motor performance and daily activity performance of individuals with chronic hemiparetic stroke. Methods: Sixteen subjects one year after stroke participated in this study with a control group; the pretest-posttest method was used. The subjects were randomly allocated into two groups: combination of bilateral arm training and modified constraint-induced movement therapy (n=8), and modified constraint-induced movement therapy (n=8). The mCIMT group received therapy for 90 minutes in 3 sessions per week over a period of 4 weeks. The patients receiving a combination of mCIMT and BAT were treated for the same period and frequency. The results were evaluated using the Fugl-Meyer Assessment, Action Research Arm Test (ARAT), and Motor Activity Log-Amount of Use, and Quality of Movement (MAL-AOU, QOM) assessment tools. Results: The Fugl-Meyer Assessment showed that hand and wrist performance improved significantly more in the mCIMT group than in the Combination group (p<0.05). Result from the ARAT assessment showed greater scores for gross movement in the combined group than in the mCIMT group (p<0.05). The MAL-AOU showed that there was greater improvement in the combined group than in the mCIMT group (p<0.05). Conclusion: The forced use of the more affected side can be important for the enhancement of upper extremity performance for chronic hemiparetic stroke patients during their daily activities.

구두 굽 높이의 차이가 균형 수행능력에 미치는 영향 (Effects of the Differences in Shoe Heel-height on Balanced Performances)

  • Purpose : The purpose of this study is to research the effects of the differences in shoe heel height on balanced performances with a balance master. Method : 40 healthy female adults were divided into 2 groups at random. All the female participants were tested by a balance master system. Differences were analyzed according to high heel shoes and low heel shoes. The ability of static postural balance control was measured by modified CTSIB and unilateral stance test, while that of dynamic postural balance control was measured by LOS (limit of stability) and rhythmic weight shift, among the programs of a balance master. Result : 1. There is no significant correlation between the ability of static and dynamic postural balance control before main test without the shoes. 2. Movement velocity and directional control ability were significant correlation compared result of in high heel shoes and low heel shoes group. Conclusion : In conclusion, the test results of the high heeled shoe group and the low heeled shoe group through a balance master system showed that the former one is inferior to the latter one.

PNF를 이용한 발목 강화 운동이 급성파종성 뇌척수염 환자의 균형 및 보행 향상에 미치는 영향 -단일사례연구- (Effect of an Ankle Strengthening Exercise that Uses PNF on the Balance and Walking Ability of Patients with Acute Disseminated Encephalomyelitis -A Single Case Study-)

  • Purpose: Peripheral neuropathy accompanied by sensory disturbance, such as limb paralysis and hemiplegia, is mainly caused by acute disseminated encephalomyelitis (ADEM). This case study aimed to determine the effect of ankle strengthening exercises that use proprioceptive neuromuscular facilitation (PNF) on the gait, balance, ankle-control ability, and sit-to-stand ability on a patient with ADEM. Methods: A 10-year-old male with quadriplegia and ankle-control impairment participated in this 4-week training intervention. The patient, diagnosed with ADEM, was treated with ankle strengthening exercises that used PNF. Results: The patient demonstrated improvements in balance, ankle-control ability, sit-to-stand ability, and gait performance. Outcome measures (manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5 time sit-to-stand test, and 10 m walk test) were taken before and after the training program. Conclusion: The results of this case suggest that an ankle strengthening exercise that uses PNF can improve the gait, balance, ankle-control ability, and sit-to-stand ability in patients with ADEM. In ADEM, the initial treatment is important, and the use of ankle strengthening exercises with PNF could lead to meaningful results. However, there is limited research due to an insufficient number of cases. In the future, more patients will need to be studied.

편마비 환자의 환측 기능회복을 위한 연구 (The study of paralytic side functional recovery on hemiplegia)

  • This study was conducted to assess the changes in the functional levels of affected upper extremities after treating hemiplegic patients by applying constraint-induced movement therapy(CIMT). The subjects were selected from 20 hemiplegic patients with scores of 25 or more in Mini Mental State Examination(MMSE-k), transferred to the departments of physical therapy in two university hospitals in Busan from December, 2001 to march, 2002, and were divided into two groups. Eleven subjects with fixing unaffected arms by CIMT were assigned to the experimental patient group and the other 9 patients to control group without fixing unaffected arms. The function of upper arms for both groups were evaluated by using Actual Amount of Use Test(AAUT) and Motor Active Log(MAL) before and after physical therapy. The malts were as follows: The recovery rates of upper extremity by AOU(Activity of Use) and QOM(Quality of Movement) were 23.9% and 27.3% for CIMT treated group, and by 8.3% and 4.6% for the control group on the average, respectively, in AAUT after physical therapy, showing statistically significant differences between two groups. And in MAL, the average recovery rates were 27.3% by AOU and 22.6% by QOM for CIMT treated group while 3.1% by both AOU and QOM for the control group, and were significantly different between twogroups.

복식호흡을 병행한 척추 안정화운동이 요통환자의 균형능력과 오스웨스트리 장애지수에 미치는 영향 (The Effects of Stabilization Exercise with Abdominal Breath on Balance and Oswestry Disability Index for Low Back Pain Patients)

  • PURPOSE: This study was to investigate the effect of stabilization exercise with abdominal breath on Balance and Oswestry Disability Index for Low Back Pain Patients. METHODS: The subjects were 18 low back pain patients in their twenties. The subjects were randomly assigned into experimental group and control group. Control group and experimental group both participated in 30 minutes of stabilization exercise for 8 weeks(5 times/week), and experimental group participated in stabilization exercise with abdominal breath. The subjects were tested balance and pain using BioRescue and Oswestry Diability Index respectively, before and after intervention. RESULTS: The experimental group showed a statistical significance in movement areas of the body's center(p<.05), but there was no significant differences between groups (p>.05). The experimental group showed a statistical significance in movement distances of the body's center with eyes open(p<.05), and there was significant differences between groups(p<.05). Both groups showed a statistical significance in ODI between pre and post test(p<.05) but there was no significant differences between groups(p>.05). CONCLUSION: As a result, the group participating in stabilization exercise with abdominal breath had a more effective improvement than the control group. Therefore, the stabilization exercise with abdominal breath may be used improving balance and pain in low back pain patients.

진동운동이 노인의 균형 조절 능력과 보행 능력 및 우울에 미치는 효과 (Effects of Vibration Exercise on the Balance Control Ability, Gait Ability, and Depression in the Elderly)

  • Purpose: The aim of this study was to examine the effects of vibration exercise on balance control ability, gait ability, and depression in the elderly. Methods: A total of 24 elderly subjects were recruited and randomized into an experimental (n = 12) or a control (n = 12) group. The experimental group performed both a vibration and a stabilization exercise, and the control group performed a stabilization exercise alone. Balance ability was measured using the functional reach test (FRT), gait ability was measured using the timed up and go test (TUGT), and depression was measured using the Center for Epidemiologic Studies Depression Scale (CES-D). Results: The experimental and control groups showed significant differences for all pre- and post-experiment FRT, TUGT, and CES-D measurements (p <0.05). In a comparison between the two groups, the experimental group in which vibration exercise was applied showed more significant difference in FRT, TUGT, and CES-D than the control group (p <0.05). Conclusion: This study showed vibration exercise to be effective in balance control ability, gait ability, and depression in the elderly.

비포장노면 차량 거동 분석을 위한 타이어 테스트베드 개발 (Development of Tire Test Bed for Dynamic Behavior Analysis of Vehicles on Off-roads)

  • When a vehicle is driven off a road surface, the deformations of the road surface and tire are combined. Consequently, the dynamic behavior of wheel movement becomes difficult to predict and control. Herein, we propose a tire test bed to capture the dynamic behavior of tires moving on sand and soil. Based on this study, it is discovered that the slip rate can be controlled, and the vertical force can be measured using a load cell. The test results show that this test bed can be useful for capturing the dynamic behavior of the tire and validating dynamic simulations. In fact, the tire test bed developed in this study can be used to verify the results of computer simulations. In addition, it can be used for basic experiments pertaining to the speed control of unmanned autonomous vehicles.

  • A robot crane truck is being developed by the Bridge Inspection Robot Development Interface(BRIDI) for an automated and/or teleoperated bridge inspection. At the end of the telescoping boom allows the operator to scan the bridge structure under the deck trough the camera. Boom vibration induced by wind and deck movement can cause serious problems in this scanning system. This paper presents a control system to mitigate such vibration of the robot boom In the proposed control system an actuator is installed at the end of the working boom. This control system is studied using a mathematical model analysis with LQ control algorithm and a scaled model test in the laboratory. The study indicates that the proposed system is efficient for the vibration control of the robot booms, thereby demonstrating its immediate applicability in the field.

율동적 운동 프로그램이 여성노인의 생리적 지수, 생활만족, Calcium, Phosphorous, Osteocalcin, Deoxypyridinoline에 미치는 영향 (The Effects of Rhythmic Exercise Program on Physiologic Variables, Life satisfaction, Calcium, Phosphorous, Osteocalcin, Deoxypyridinoline in the Elderly Women)

  • Recently, the number of the elderly has increased according to the improvement of socioeconomic status and the efficient medical care system. In spite of the development of medicine, the elderly suffers from the various health problems caused by weakness of musculoskeletal system, cardiopulmonary function and immunologic dysfunction. Regular rhythmic exercise program is known to be one of the effective tools to enhance the health condition in the elderly. However, there has been few studies to evaluate the comprehensive effects of rhythmic movement program on the elderly. This study was focused to evaluate the indices of cardiopulmonary function, life satisfaction, calcium, phosphorous, osteocalcin and deoxypyridinoline which are the essential factors of health problems in the elderly women. Twenty six subjects, aged between 68 and 72, who can do the ordinary activities and do not have cardiovascular dysfunction and mental disorder, participated in this study. They were divided into two groups: 13 in the experimental group and 13 in the control group. The experimental group participated in the rhythmic movement program at the welfare center located in G-city. The program were consisted of three sessions a week during 10 weeks. Each session had three parts: warming up(10 minutes), main exercise(40 minutes), finishing(10 minutes). Heart rate, blood pressure and peripheral arterial oxygen saturation were measured for the evaluation of cardiopulmonary function. Serum calcium, phosphorous, osteocalcin and urine deoxypyridinoline were measured as the indices of bony metabolism. Data were analyzed with mean, standard deviation, $x^2$-test, t-test, paired t-test using SPSS PC+ program. The results of this study were as follows. 1) Heart rate of the experimental group showed significant decrease following the rhythmic movement program. Peripheral arterial oxygen saturation of the experimental group showed significant increase following the program. 2) The degree of life satisfaction of the experimental group showed significant increase following the program. 3) Calcium showed significant decrease following the program but remained within normal range. There was no significant difference of phosphorous between two groups. 4) Osteocalcin, the index of bone formation, showed no significant change following the program, but significant increase in the experimental group comparing with the control group. 5) Deoxypyridinoline, the index of bone resorption, in urine of the experimental group showed significant decrease following the program. In conclusion, the rhythmic exercise program in the elderly showed the improvement of physiologic function and favorable effects on life satisfaction and bony metabolism. According to the above results, the regular rhythmic movement program can be strongly recommended for the improvement of health in the elderly women.

  • Nuclear regulation requires a periodic visual test for inside structures of reactor to guarantee safe operation of nuclear power plant. However, existing visual test, which is proceeded manually, needs lots of time and labor. Even more, test workers should be exposed in radioactive environment during the test. An underwater robot system has being studied for more efficient and safer test. The position and pose estimation are important issue for the movement control of the robot. An algorithm was presented in this paper, which estimate the location and pose of the underwater robot clearly using vision system.
