STUDIES ON THE EFFECT OF WEEKLY SELF APPLICATION WITH SODIUM FLUORIDE SOLUTION (불소자가도포의 치아우식증 예방효과에 관한 연구)

  • The use of mouth-rinses containing sodium fluoride after tooth brushing has been proposed as a potentially useful measure for the prevention of dental caries in the practice of public health. About 700 middle school girls rinsed their mouths in the school once a week with sodium fluoride sodution after tooth-brushing. Tooth brushing was practiced everyday after meals and the rinses were carried out under the supervision of the teachers. The procedure consisted of gargling 10cc of fluoride solution for one minute and immediately followed by a second rinse with tap water. The rinsing was started in May 1976 and continued for one year. The results were as follows: 1) The inhibition of the 'D' Person rate was 76.04%, 'M' person rate was 80.79%, 'F' person rate was 4.06% and, 'D.M.F.' person rate was 76.86%. 2) the reduction of the 'D.T.' Index was 81.94%, 'M.T.' Index was 85.71%, 'F.T.' Index was 7.69% and, 'D.M.F.T.' Index was 76.83%. 3) Mouth rinsing with 0.3% NaF solution and tooth brushing are of value in the mass control of dental caries.

The Effect of Commercially Available Mouth Rinsing Solution and Alkaline Ionized Water on the Oral Bacteria (시판되는 구강 양치액과 알칼리 이온수가 구강 내 세균에 미치는 효과)

  • The mouth rinsing solution currently on the market is used for oral hygiene and reduction of halitosis, and alkaline ionized water is used for drinking purposes. The aim of this germiculture and observation through a microscope is to investigate the effect they have on the oral bacteria, and the results are as follows. 1. The change in the number of cultivated bacteria before and after the use of gargle was most dramatic for Hexamedin. It showed the highest rate of decrease in bacteria by 71.29%, followed by Listerine with 62.25%, Caregargle with 42.26%, Garglin with 33.52%, saline solution with 21.62%, alkaline ionized water with 16.08% and distilled water with 7.67%. 2. After careful analysis of statistics, Hexamedin was found to result in a significant decrease in the number of bacteria after gargling (p<0.05). 3. From observing through an light microscope after gram staining, it was evident that both a number of cocci and a few of filamentous were present before the use of gargle, whereas mostly cocci was present after the use of gargle. 4. The difference in the pH of oral rinses and alkaline ionized water had no significant influence on the number of oral bacteria. In conclusion, Hexamedin and Listerine, with the rate of decrease of bacteria of co71.29% and 62.25% respectively, have proven to be highly effective when applied before dental treatment. Other oral rinses and alkaline ionized water are relatively less efficient in decreasing the number of oral bacteria. Also, the decrease of bacteria is more affected by the antibacterial component of oral rinses than by the change in pH.

The clinical effects of modified full-mouth disinfection in the treatment of moderate to severe chronic periodontitis patients

  • Purpose: Full-mouth disinfection enables to reduce the probability of cross contamination from untreated pockets to treated ones, for completing the entire SRP under local anesthesia with chlorhexidine as a mouth wash in two visits within 24 hours. This study aimed to compare the clinical effects of modified full-mouth disinfection (Fdis) after 6 months with those of conventional SRP (cSRP). Methods: Thirty non-smoking chronic periodontitis subjects were randomly allocated two groups. The Fdis group underwent the entire SRP under local anesthesia in two visits within 24 hours, a week after receiving supragingival scaling. A chlorhexidine (0.1%) solution was used for rinsing and subgingival irrigation for Fdis. The cSRP group received SRP per quadrant under local anesthesia at one-week intervals, one week after they had received scaling. Clinical parameters were recorded at baseline, after 1, 3 and 6 months. Results: There are significant (P<0.05) decreases in the sulcus bleeding index, and plaque index, and the increases in gingival recession were significantly smaller with Fdis after six months compared with cSRP. There was significant improvement in the probing depth and clinical attachment level for initially medium-deep pockets (4-6mm) after Fdis compared with cSRP. Multi-rooted teeth showed significantly larger attachment gain up to six months after Fdis. Single-rooted teeth showed significantly more attachment gain, 1 and 6 months after Fdis. Conclusions: Fdis has more beneficial effects on reducing gingival inflammation, plaque level, probing depth, gingival recession and improving clinical attachment level over cSRP.

The effect of the oral health behaviors on the periodontal status in teenagers (청소년의 구강보건행태가 치주조직 상태에 미치는 영향)

  • Objectives : This study aims to improve health and the quality of life of teenagers by identifying an association between the periodontal status and oral health behaviors of Korean teenagers using 2010 Korean National Oral Health Survey data. Methods : Subjects were teenagers aged from 12 to 15 in Korean National Oral Health Survey (KNOHS). The questionnaire consisted of periodontal status, general characteristics, periodontal related factors, and oral health related behaviors. Results : Gingival bleeding was found in 56.9% of teenagers because unhealthy periodontal status and tartars were found. Those who were 15 years old had unhealthier periodontal status than those who aged 12 years old. Those who lived in rural areas tended to have worse periodontal status than those who lived in the big cities. Access to dental floss, mouth rinsing solution and regular dental checkup tended to make the healthier periodontal status. Smokers had the worse periodontal status than the nonsmokers. Conclusions : The oral health care in the teenagers aged from 12 to 15 is very important because the permanent dentition is completed in this stage. It is necessary to lay emphasis on the oral health improvement and dental caries prevention program to the teenagers.

Comparative Study on Surgical and Conservative Management of Bisphosphonate-related Osteonecrosis of the Jaw (BRONJ) in Disease Stage 2 (비스포스포네이트 관련 악골괴사(BRONJ)의 병기 2기에서의 외과적, 보존적 치료에 대한 비교 연구)

  • Purpose: This study evaluated the prognosis of conservative and surgical treatment according to the staging of bisphosphonate-related osteonecrosis of jaw (BRONJ) by American Association of Oral and Maxillofacial Surgeons and American Society for Bone and Mineral Research. Methods: We evaluated 53 patients of BRONJ who visited Department of Dentistry, Ajou University School of Medicine from May 2007 to February 2013. Twenty eight patients in stage 2, were divided into surgical and conservative groups with cessation of bisphosphonate therapy. Fifteen patients belonged to the conservative treatment group, in which mouth rinsing and antibiotics medication were done. Thirteen patients were treated with debridement or sequestrectomy, in the surgical treatment group. Each study list was analyzed by SPSS ver. 14.0 (SPSS Inc., USA) software and the favorable rate was verified by the Fisher exact test. P-values less than 0.05% were deemed significant. Results: Clinical outcome was evaluated on the basis of both clinical and radiographic findings. Of all the 28 patients of stage 2, 15 patients underwent conservative treatment and 13 patients received surgical treatment. In the surgical group, 9 of 13 (69.2%) showed good prognosis, 4 of 13 (30.7%) showed recurrence. In the conservative group, 13 of 15 (86.6%) showed no change duting the follow-up period. Two of 15 patients even showed a bad prognosis, such as pain and pus discharge, which are criteria for stage 3. P-value was 0.067 (>0.05). Conclusion: The results of the present study suggests that surgical intervention is good choice against the conservative treatment, after proper drug holidays period, while further investigation is needed for a definite solution to BRONJ.
