노인 구강건조증에 대한 음허 진단의 유용성 및 주관적 구강건조감의 영향요인 : 전향적 단면 조사 연구 (Availability of Diagnosis of Yin-deficiency in Elderly People with Xerostomia and Factors Influencing Subjective Oral Dryness: A Prospective Cross-sectional Study)

  • Objectives: The aims of this study were to investigate the availability of diagnosis of Yin-deficiency in the elderly with xerostomia and factors influencing subjective oral dryness. Methods: We surveyed 50 patients recruited by the clinical trial, 'Efficacy of Yukmijihwang-tang on Xerostomia in the Elderly: A Randomized, Double-blind, Placebo-controlled, Two-center Trial'. The subjects were assessed on their subjective oral dryness using the Dry Mouth Symptom Questionnaire (DMSQ). Their salivary functions were measured by Unstimulated Salivary Flow Rate (USFR) measurements. In addition, the subjects were evaluated on their Qi-stagnation and Yin-deficiency conditions using the Qi-stagnation questionnaire and Yin-deficiency questionnaire. Results: There were statistically significant correlations between three variables (USFR, DMSQ score and Qi-stagnation score) and Yin-deficiency score. In the multiple regression analysis, the regression model was statistically significant (F = 10.273, p < .001). The factor most strongly influencing the subjective oral dryness was USFR (${\beta}$ = -0.386). Yin-deficiency had the next strongest impact on the subjective oral dryness (${\beta}$ = 0.371). Qi-stagnation affected the subjective oral dryness weakly (${\beta}$ = 0.075). In the simple regression analysis, Yin-deficiency had a statistically significant effect on each of six subscales of DMSQ (p < .01). Among the six subscales, DMSQ-1 ('Oral dryness at night or on awakening') was the most strongly influenced by Yin-deficiency. Conclusions: The results of this study show that the diagnosis of Yin-deficiency in the elderly with xerostomia was available and Yin-deficiency was an important factor influencing the subjective oral dryness. Therefore, the consideration of Yin-deficiency is significant for diagnosis and treatment in the elderly with xerostomia.

  • As the morbidity of chronic diseases such as diabetes and high blood pressure has increased, the frequency of oral dryness upon taking drugs for a long time or several drugs at the same time has greatly increased. This study was conducted to investigate the status of oral dryness and utilize the data to develop treatment products. Surveys were assembled from October to December 2018, and a statistical analysis of the 144 responses was conducted. The surveyed consisted of 60 males and 84 females, with 31.3% of those in their 70s and older. Among them, 100 people took drugs for chronic diseases, while 27 people took drugs for more than 10 years and less than 20 years. Exactly 23 respondents who took drugs suffered from more than two types of chronic diseases, of which high blood pressure was the most cited, followed by hyperlipidemia and diabetes. Regarding complications, oral dryness (33.8%) was the most frequent, and there were significant differences in the degree of discomfort when taking medicine, especially for oral dryness depending on the age (P<0.05). Groups that took medicine showed severe symptoms of dry mouth, especially when waking up at night or in the morning (P<0.001) and eating food (P<0.01), and had difficulty in swallowing food (P<0.01) and speaking (P<0.01). Exactly 89.6% of the respondents didn't take lozenges (candy) for improvement of oral dryness, and the type of the product that they wanted to purchase the most was candy (45.1%). Respondents worried about rising blood sugar, dental caries, and calories when eating candy for relieving dryness (P<0.01).

Comparison of Objective and Subjective Diagnostic Tests for Assessing Oral Dryness in Healthy Participants

  • Purpose: Xerostomia is subjective feeling of dry mouth. It is complicated and multifactorial, which burdens clinicians in diagnosis and treatment of the problem. The goal of this study was to discuss the clinical importance of salivary flow rate, pH and subjective symptoms for evaluating oral dryness among young healthy male subjects. Methods: Thirty male participants were recruited in this study (mean age±standard deviation of 25.70±1.84). All participants completed 'Xerostomia Inventory' to measure subjective oral dryness scores. Unstimulated saliva and stimulated saliva were collected from each participant twice a day at 12:00 pm and 5:00 pm, using spitting method. Salivary flow rates and pH were measured immediately after collection. Relationship between objective and subjective measurements were analyzed. Results: There were excellent intra-examiner reliability for salivary flow rate and pH and good internal consistency for Xerostomia Inventory. Objective measurements and subjective symptoms did not exhibit positive association. Salivary flow rate in unstimulated and stimulated condition showed positive association and also for salivary pH. Stimulated salivary flow rate also presented positive correlation with stimulated salivary pH. Conclusions: Comprehensive assessment of objective measurements and subjective symptoms may be complimentary for assessing oral dryness, which would assist in implementing early interventions to improve patient's quality of life.

한의 논문들의 고찰을 통한 구강건조증과 구강작열감증후군의 비교 연구 (A Comparative Study of Xerostomia and Burning Mouth Syndrome through Review of Korean Medicine Articles)

  • Objectives : By analyzing articles on xerostomia and burning mouth syndrome, two representative diseases that cause various symptoms in oral cavity. Methods : We analyzed articles by dividing them into review articles, original articles, case reports. The subjects were 11 articles on xerostomia and 13 articles on burning mouth syndrome published in Korean medical journals. Results : 1. The subjects were 11 articles on xerostomia and 13 articles on burning mouth syndrome published in Korean medical journals. 2. The first article was published in 2007 and the most published articles were three each in 2013, 2018 and 2022. 3. Classification by type of article was as follows: 2 review articles(8.3%), 16 original articles(66.7%) and 6 case reports(25.0%). 4. The journal with the most published articles was journal of internal Korean medicine and the topics were xerostomia(72.7%), burning mouth syndrome(76.9%). 5. For both diseases, the number of female patients was higher than that of male patients. 6. The main symptom of xerostomia is dryness in the mouth and the main symptom of burning mouth syndrome is pain in the oral cavity. 7. Questionnaires, diagnosis based on dialectics and diagnostic devices were used to diagnose xerostomia and burning mouth syndrome. Conclusion : Diagnosis of xerostomia and burning mouth syndrome require overall consideration the entire hum an body along with the oral cavity. Since there are many different diagnostic methods, appropriate methods must be carefully selected and used.

농촌 노인의 스트레스와 주관적 구강건조감의 관련성: 예비조사 (Relationship between stress and subjective oral dryness in the elderly in a rural region: a pilot study)

  • Objectives: Stress is a psychological reaction to stimuli such as anxiety or threat felt by a person either physically or mentally when placed in a difficult situation. Although a relationship between stress and dry mouth has been reported, it remains understudied. The purpose of this study was to investigate the association between stress and subjective dry mouth among the elderly living in a rural region. Methods: A total of 214 people aged ${\geq}60years$ living in a rural area were recruited with an informed consent for a cross-sectional analysis. Information about stress and subjective dry mouth was obtained by an interview survey with a constructed questionnaire. Data on other potential confounding factors (including oral factors) such as socio-demographic data, health-related behaviors, the number of remaining teeth, and subjective chewing ability were also gathered at the same time. Logistic regression models were used to analyze the relationship of stress and dry mouth using SPSS. Results: The Crude odds ratio (OR) of stress awareness affecting subjective dry mouth was 2.59 (95% confidence interval: 1.43-4.68). After adjusting for sex, education, income, smoking, and alcohol intake, the adjusted OR was 2.52 (95% confidence interval: 1.30-4.87) which was statistically significant. Conclusions: Elderly people who were stressed had an approximately 2-fold increase in experiencing subjective dry mouth when compared to their stress-free counterparts.

  • Allergic rhinitis is a specific IgE mediated inflammatory disease of the nasal mucosa, characterized by symptoms such as rhinorrhea, nasal congestion, nasal obstruction, nasal and eye itching, and sneezing. The prevalence of allergic rhinitis varies according to country, age, and surveying methods, but it seems to increase worldwide, also in Korea. Prolonged mouth breathing caused by allergic rhinitis can produce muscular and postural alterations, causing alterations on the morphology, position, growth direction of the jaws, and malocclusion. Also, mouth breathing leads to dryness of the mouth, causing various oral diseases; gingivitis, halitosis, inflammation of tonsil, increased risk of dental caries and dental erosion. In dental clinic, using rapid maxillary expansion to persistent allergic rhinitis patients with narrow maxilla can enlarge maxillary dental arch and nasal cavity anatomy, improving nasal breathing and reducing nasal cavity resistance. However, it is desirable to use along with otolaryngologic treatment. Dentists should be aware of the characteristics of allergic rhinitis and its effects on patients, and consider when planning dental treatment.

구강 자가관리 프로그램이 노인의 구강건조증과 구강건강 관련 삶의 질에 미치는 효과 (Effects of an Oral Self-care Program on the Elderly's Xerostomia and Oral Health-related Quality of Life)

  • Purpose: The purpose of this study is to investigate effects of an oral self-care program on oral dryness and oral health related quality of life in the elderly. Methods: The current research design isa nonequivalent control group protest-posttest design with two groups of elders from two different senior centers in D city. Tools for the research include paper of salivary absorption (WF41-1850, Whatman, Germany), 6 items for subjective oral dryness and OHIP-14 for oral health related to quality of life. The data were analyzed to test three hypotheses by using the SPSS 22.0 program. Results: The three hypotheses were significantly accepted. The subjective oral dryness score was lower in the experimental group than in the control group (t=-2.45, p=.022). Salivary absorption was higher in the experimental group than in the control group (t=3.83, p<.001).Oral health related quality of life was better in the experimental group than in the control group (t=-2.11, p=.044). Conclusion: The oral self-care program has appeared to be effective on oral dryness and an oral health related quality of life in the elderly. Therefore, it can be suggested that the oral self-care program is applicable as an intervention program motivating the elderly to maintain oral health.

전남 일부지역 노인의 구강건강관련 삶의 질 요인분석 (Quality-of-life factors related to the oral health of the elderly in Jeollanam-do)

  • Background: This study was aimed at obtaining basic data necessary to develop alternatives to improve the oral health-related quality of life of the elderly by investigating dry mouth and oral health-related quality of life of patients visiting the N Dental Clinic in Jeollanam-do. Methods: From November 2021 to September 2022, data were collected using a self-report survey of adults aged 50 years or older. Of the 300 questionnaires collected, 280 had insufficient data because of missing entries. Results: Analyzing the satisfaction level of oral health-related quality of life according to the participants' medical use and subjective oral health recognition, "what do you think your oral health is like" showed significant differences (p< 0.001). Dry mouth and satisfaction were positively correlated with the oral health-related quality of life (β=0.42, p= 0.00). Factors affecting the quality of life related to oral health were "total score of oral dryness" (β= 0.395) and "what do you think your oral health is like" (β=0.224). Conclusion: The results of this study suggested that systematic intervention, such as oral health programs, suitable for age may improve the oral health-related quality of life.

중풍환자(中風患者)의 상소증(上消證) 치험(治驗) 1례(例) (The clinical study on 1 case of stroke patient with symptoms of Sang-So(上消))

  • So-gal(消渴) is a oriental medical pathologic state that is characterized by 3 major symptoms - thirst, polyphasia, polyuria - of which especially thirst features Sang-so(上消). Diabetes mellitus and So-gal(消渴) have many points in common. But there are considerable differencies between them, because they have obviously different diagnostic bases from each other. That is, oriental medicine places more weight on symptoms than clinopathologic laboratory reports. This case of stroke patient complained of severe thirst and additional dryness in the mouth, choking sensation in the chest, hidrosis, insomnia, dark colored urine, constipation. The patient had no clinical history of diabetes mellitus. Based on oriental medical standards, we diagnosed the case as Sang-so(上消). Several herbal medicines were dispensed for 48 days. And considerably effective result were obtained. Object : 1 case of stroke patient who complained severe thirst. Medication : . Seonghyangjeonggisan for 5 days (2000. 6. 9 - 6. 13.) . Paljeongsan for 21 days (2000. 6. 14. -7. 5) . Ikwonsan for 8 days (2000. 7. 6 - 7. 13) . Okcheonsan for 14 days (2000. 7. 14 - 7. 27) Result : 1. After administration of Paljeongsan, dark colored urine turned normal. 2. After administration of Ikwonsan, dryness in the mouth improved. 3. After administration of Okcheonsan, thirst significantly improved. Constipation, choking sensation, hidrosis, insomnia also improved.

중풍환자의 음허변증 진단지표에 관한 연구 (Study of The Diagnostic Indicators of Deficiency of Eum Pattern Identification In Stroke Patients)

  • The purpose of this study was to investigate that which symptoms are adequate indicator of the deficiency of Eum pattern in the stroke patients. In the time period Dec. 2006 to Aug. 2007, 479 patients with a first-ever stroke admitted in the department of Internal Medicine of 12 Oriental Medical Hospitals were included. Patients were hospitalized within 1 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Eum-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Deficiency of Eum group included 65 case, Non Eum group 414 case out of 479 patients. dryness of mouth, short and rapid purse, white face and reddish zygoma, mirror-like tongue were higher among Eum group. Eum and Non Eum patients do not significantly differ in reddened tongue, dryness in tongue, night sweat, palpitation, afternoon tidal heat, palmar heat, sores of the mouth or tongue. This study was insufficiency because sample size is very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the Stroke.
