우반신 무도증을 호소하는 Chorea Hyperglycemia Basal Ganglia Syndrome 환자에 대한 증례보고 : 양한방 복합치료적 접근으로 (A Case Report of a Right-sided Hemichorea Patient Diagnosed with Chorea Hyperglycemia Basal Ganglia Syndrome : A Combined Western-Korean Medicine Approach)

  • Objectives : The purpose of this report is to introduce a rare case of a patient with Chorea Hyperglycemia Basal Ganglia syndrome (C-H-BG) whose initial medical interventions were started 2 weeks after the onset, and to suggest the possibility of treatment using a combined Western-Korean medicine approach. Methods : A 75-year-old female C-H-BG patient complaining of persistent right-sided hemichorea was treated with a therapy that combined Korean and Western medicine from April 4, 2015 to April 29, 2015. Improvements of symptoms were measured by a motor assessment of Unified Huntington's Disease Rating Scale (UHDRS), Visual Analog Scale (VAS) and the number of involuntary movements. Results : Motor assessment of UHDRS, VAS and the number of involuntary movements all showed a gradually improving tendency during 26 days of admission treatment. However, the patient's hemichorea persisted. Conclusion : Rapid blood sugar control is the most important treatment for C-H-BG, because pathologic changes of basal ganglia seem to become irreversible as time goes by. A combined Western-Korean medicine approach to treating C-H-BG seems effective not only in reducing hemichorea, but also in the management of accompanying symptoms such as muscle pain and general weakness.

  • 좌측 제 11번 늑간 신경통과 요통을 주소로 입원한 49세 남자 환자에게 흉부 신경근 열 응고술을 시행한 후 3일간에 걸쳐 척수 압박 증상이 급속히 진행되었다. 흉 요추의 단순 X-선과 자기 공명 영상 촬영으로 밝힌 그 원인은 제 11 및 12 흉추의 척추 종양이었다.

Implications of Circadian Rhythm in Dopamine and Mood Regulation

  • Mammalian physiology and behavior are regulated by an internal time-keeping system, referred to as circadian rhythm. The circadian timing system has a hierarchical organization composed of the master clock in the suprachiasmatic nucleus (SCN) and local clocks in extra-SCN brain regions and peripheral organs. The circadian clock molecular mechanism involves a network of transcription-translation feedback loops. In addition to the clinical association between circadian rhythm disruption and mood disorders, recent studies have suggested a molecular link between mood regulation and circadian rhythm. Specifically, genetic deletion of the circadian nuclear receptor Rev-$erb{\alpha}$ induces mania-like behavior caused by increased midbrain dopaminergic (DAergic) tone at dusk. The association between circadian rhythm and emotion-related behaviors can be applied to pathological conditions, including neurodegenerative diseases. In Parkinson's disease (PD), DAergic neurons in the substantia nigra pars compacta progressively degenerate leading to motor dysfunction. Patients with PD also exhibit non-motor symptoms, including sleep disorder and neuropsychiatric disorders. Thus, it is important to understand the mechanisms that link the molecular circadian clock and brain machinery in the regulation of emotional behaviors and related midbrain DAergic neuronal circuits in healthy and pathological states. This review summarizes the current literature regarding the association between circadian rhythm and mood regulation from a chronobiological perspective, and may provide insight into therapeutic approaches to target psychiatric symptoms in neurodegenerative diseases involving circadian rhythm dysfunction.

환축추 회전 아탈구 환자에 대한 도수치료 효과 (Effect of Manual Therapy on a Patient With Atlantoaxial Rotatory Subluxation)

  • Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

경피적 전기 신경 자극의 적용 부위에 따른 자율신경계 활성도 변화 비교 및 평가 (Comparison and Evaluation of Autonomic Nervous System (ANS) Activity According to the Stimulus Position of Transcutaneous Electrical Nerve Stimulation (TENS))

  • Purpose: The aim of this study is to find the location of acupoints that are effective for increasing parasympathetic nervous system(PNS) activity among acupoints used to relieve symptoms such as dizziness and nausea. Materials and Methods: Twenty healthy adult men participated in this experiment. Transcutaneous electrical nerve stimulation(TENS) was applied to P6(Neiguan), ST36(Zusanli) and BL20(Pishu) for 30 minutes. For 5 minutes before and after stimulation, Electrocardiogram(ECG), Electrodermal activity(EDA), Respiration, and Skin temperature(SKT) were measured using biosignalsplux Professional(biosignalsplux; Portugal). Paired t-test was performed for the results before and after stimulation, and a one-way ANOVA test between stimulations was performed for the rate of change before and after stimulation. Results: When each acupoint was stimulated with TENS, it was found that the PNS was generally activated. In addition, when considering the change in biosignals after stimulation, the participants stimulated with P6 showed the most consistent results. Conclusion: As a result of stimulating each acupoint, it was found that stimulating P6 can effectively increase PNS activity. These results indicate that methods of stimulating P6 may be most effective in alleviating symptoms of motion sickness.

호르몬 대체요법에 따른 갱년기여성의 갱년증상과 삶의 질 (The Climacteric Symptoms and Quality of Life in Climacteric Women according to Hormone Replacement Therapy)

  • The purpose of this study is to find the difference of the climacteric symptoms and quality of life according to hormone replacement therapy (HRT) in climacteric women. The research design was a descriptive survey with questionaries. A convenience sample of 181 climacteric women aged 45-65 who had climacteric symptoms were selected in Kwangju city, Korea. Sixty-six climacteric women among those subjects were receiving HRT at K university hospital in Kwangju city. Data were collected by the interview with questionaries from Feb 3. 1999 to March 25, 1999. Climacteric symptoms were measured using scores developed by Neugarten(1965) and modified by Park(1989), and the quality of life scale related to climacteric symptoms developed by Hildtich(1996) and modified Kim(1998). Data were analyzed by $x^2$-test, t-test, ANOVA and Pearson's correlation procedures, using SAS/win PC+. The results were as follows ; 1. No statistically significant differences were found between the receiving HRT group and not receiving HRT group in demographic, obstetrical and health related characteristics. 2. There were no significant difference on the climacteric symptoms between the group of women receiving HRT (score 2.4) and the group of women who were not (score 2.6) except psychological area (t= 2.407, p= 0.017). 3. The quality of life of the group of women receiving HRT was significantly higher than that the group of women who were not (t=2.151, p=.032). 4. The quality of life of the group of women receiving HRT were high scores There were significantly high of the quality of life in the vaso-motor change area (t=2.634, p= 0.009), psycho-social change area (t=3.239, p=0.001), and physical change area (t=2.031, p=0,043) in the group of receiving HRT compared to not receiving HRT group. 5. The variables showed significantly differences on the degree of climacteric symptoms of the group of women not receiving HRT were the subjective health state (t=15.81, p=0.000), age (t=7.50, p=0.007), feeling of menopause (t=30.88, p=0,000) and climacteric periods (t=8.66, p=0.003), and receiving HRT were number of para (t=3.95, p=0.050) and feeling of menopause (t=3.94, p=0.050). 6. The variables showed significantly difference on the quality of life of the group of women not receiving HRT were the subjective health state (t=4.14, p=0.044) and feeling of menopause (t=10.86, p=0.001). 7. There were significantly positive correlations between the climacteric symptoms and the quality of life in climacteric women (r=0.512, p=0.000).

불안, 기분장애로 치료 중 보행장애 외에 서동과 강직을 동반한 정상뇌압수두증 증례 (Bradykinesia, Rigidity and Gait Disturbance Due to "Possible" Normal Pressure Hydrocephalus in a Patient with Anxiety and Bipolar Disorder : A Case Report)

  • 정상뇌압수두증은 보행장애, 요실금 및 치매 등의 전형적인 세 가지 증상 외에도 추체외로 증상과 다양한 신경심리 증상이 동반될 수 있다. 이 사례는 불안과 정동증상으로 치료 중 보행장애와 요실금 증상을 보였던 46세 여자 환자에서 항파킨슨 약물의 사용과 기존 정신과 치료 약물의 중단에도 불구하고 증상의 호전이 없었으나, 뇌 컴퓨터 단층 촬영상 뇌실의 확장 소견이 보였고 시험적 뇌척수액 배액에 의해서 수일 내에 증상들이 극적으로 호전되어 정상뇌압수두증을 감별해야 했던 경우이다. 뚜렷한 대뇌 실질의 위축 소견이 없으나 뇌실이 확장되어 있을 경우 추체외로 증상과 신경심리 증상이 있을 경우 정상뇌압수두증의 전형적 세 가지 증상을 보이지 않는다 하더라도 정상뇌압수두증의 가능성을 고려해서 시험적 뇌척수액 배액 등의 시술이 진단과 치료에 도움이 될 수 있음을 보여주는 사례라고 생각된다.

Stereotactic Hematoma Removal of Spontaneous Intracerebral Hemorrhage through Parietal Approach

  • Objective : To determine the advantages of parietal approach compared to Kocher's point approach for spontaneous, oval-shaped intracerebral hemorrhage (ICH) with expansion to the parietal region. Methods : We divided patients into two groups : group A had burr holes in the parietal bone and group B had burr holes at Kocher's point. The hematoma volume, Glasgow coma scale (GCS) score, and modified Barthel Index (mBI) score were calculated. At discharge, we evaluated the patients' Glasgow outcome scale (GOS) score, modified Rankin Scale (mRS) score, motor grade, and hospitalization duration. We evaluated the patients' mBI scores and motor grades at 6 months after surgery. Results : The hematoma volume in group A was significantly less than that in group B on postoperative days 1, 3, 5, 7, 14, and 21. Group A had significantly higher GCS scores than did group B on postoperative days 1 and 3. Group A had higher mBI scores postoperatively than did group B, but the scores were not significantly different. No differences were observed for the GOS score, mRS score, motor grade at discharge, or duration of hospitalization. The mBI score of group A at 6 months after surgery was significantly higher, and more patients in group A showed muscle strength improvement. Conclusion : In oval-shaped ICH with expansion to the parietal region, the parietal approach is considered to improve the clinical symptoms at the acute phase by removing the hematoma more effectively in the early stages. The parietal approach might help promote the long-term recovery of motor power.

대상포진 질환에 대한 이해와 물리치료적 접근 (The Comprehension of Herpes Zoster and The Approach of Physical Therapy)

  • Purpose : Herpes zoster is a common dermatologic disorder and is caused by reactivation of varicella zoster virus lying dormant in the ganglion of the dorsal root Methods : The aim of this study is to elucidate the clinical characteristics of herpes zoster and it's nature of pain, and is to review the method of physical therapy for pain control. Results : Herpes zoster is characterized by segmental rash, pain, and sensory symptoms, For most patients skin healing and pain resolution occur within 3-4 weeks, However, pain can continue after the rash has healed. Pain and paresthesia often the eruption of herpes zoster and vary from itching to stabbing. The preeruptive pain may simulate other diseases and may lead to misdiagnosis and misdirected interventions. Motor symptomatology is less well known and is most often related to central nervous system disease, although true lower motor neuron application is also thought to exist Subclinical motor involvement is relatively more common than clinical motor weakness and is easily detected by using electromyography. Higher incidences of herpes zoster were observed in female and in the elderly. Conclusion : The nature of pain associated with herpes zoster varied from a superficial itching to server stabbing or bursting, and paresthesia occurred most frequently. Therefore, the study of herpes zoster will be more research and comprehend, and the approach of physical therapy should be need positively.

한방치료로 운동 기능장애와 실성증이 호전된 파킨슨증후군 환자 치험 1례 (A Case Report of Korean Medical Treatment on Parkinsonism Patient Complaining of Motor Disorder and Aphonia)

  • ■Objectives This case study is to report the effectiveness of Korean medicine in Parkinsonism patient's treatment. ■Methods We used the acupuncture, electro-acupuncture, moxibustion, cupping therapy, herbal medicine, especially Palmulgunja-tang to the Parkinsonism patient with motor disorder such as Postural Instability and Gait Difficulty(PIGD) and aphonia. Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait pattern, voice dB and self-evaluation of speed and volume were used to assess the change of symptoms. ■Results ‌After treatment, the UPDRS score decreased in overall category and the walking pattern has improved. In addition, the improvement was observed in voice volume and in self assessment of the patient. ■Conclusion This case suggests the effect of Korean medical treatment on motor disorder and aphonia in Parkinsonism.
