본 임상사례에서 보는 것처럼 눈을 위로 치켜뜨는 단일 운동 틱과 경항통 등의 겸증을 호소하는 상태에서 TBT의 턱관절균형장치와 복합한방치료로 증상의 대한 현저한 치료 효과가 나타남을 확인하였다. 하루 수차례 발생하던 위로 치켜 올려보는 증상은 치료 후 간혹 긴장하였을 때를 제외하고는 나타나지 않았으며, 목과 견정부의 뻐근한 통증은 치료 종결 후 환자가 소실되었다고 언급하였으며, 각각의 증상에 따른 scale에서도 볼 수 있듯 틱 증상은 YGTSS 17점, VAS 6점에서 YGTSS 3점, VAS 1점으로 감소, 경항통 및 견정부 통증의 NDI score는 7점에서 4점으로 감소됨을 확인하였다. 이에 추후 본 증례와 유사한 운동 틱 장애 TBT 치료에 관하여 임상상 참고가 될 수 있을 것이라 사료되어 보고하는 바이다.
PURPOSE: Low-intensity exercise with transient restriction of blood flow to muscle could be an alternative rehabilitation method which avoids the problems associated with conventional high-intensity exercise. However, the mechanism of low-intensity exercise with transient restriction of blood flow is not clearly known. Thus, the purpose of this study was to investigate the mechanism of improvement of muscular function after low-intensity exercise with transient restriction of blood flow using H-reflex analysis. METHODS: Twenty one healthy young adults with no medical history of neurological or musculoskeletal disorder voluntarily participated in this study. The ${\alpha}$-motor neuron excitability of the triceps surae was assessed using the H-reflex. The amplitude of the M-wave and H-reflex were measured across three conditions: rest, after low-intensity exercise without restriction of blood flow and after low-intensity exercise with restriction of blood flow. The subjects performed low-intensity ankle plantar flexion exercise at their own pace for one minute without or with transient restriction of blood flow achieved by a sphygmomanometer cuff on popliteal fossa at a pressure of 120mm of mercury(120 mmHg). RESULTS: No significant changes of the excitability of the ${\alpha}$-motor neuron were obtained across three different conditions. CONCLUSION: This study found that low-intensity exercise with transient restriction of blood flow did not influence to ${\alpha}$-motor neuron excitability of the triceps surae. From the results, I could come to the conclusion that further study will be required.
목적 : 체계적 고찰을 통해 시운동기술 평가를 위해 사용된 도구, 치료접근방법, 치료의 효과를 알아보고, 아동의 시운동 기술에 가장 적합한 치료방법과 평가도구를 제시하고자 한다. 연구 방법 : 체계적 고찰을 사용하였다. 2001년 1월부터 2010년 6월까지 학회지에 게재된 논문을 PubMed을 통하여 검색하였다. 주요 검색용어는"visual motor and occupational therapy", "visuomotor and occupational therapy"," perception and motor and occupational therapy"이었고, 161개의 논문 중 총 13개 논문을 대상으로 분석하였다. 결과 : 1. 시운동기술 치료가 필요한 대상 진단군은 발달장애아동이 20.00%로 가장 많았고, 글씨쓰기에 문제가 있는 아동이 13.33%이었다. 결과 : 2. 시운동기술을 측정하기 위해 사용된 평가도구는 Developmental Test of Visual Perception(DTVP)을 사용한 논문이 33.33%로 가장 많았고, Developmental Test of Visual Motor Integration(VMI)을 사용한 논문이 26.67%이었다. 결과 : 3. 시운동기술과 관련하여 가장 많이 사용된 치료접근방법은 발달적 기술기반 프로그램으로 58.82%를 차지 하였고, 감각통합과 감각기반 치료는 23.53%를 보였다. 결과 : 4. 시운동기술과 관련하여 작업치료 효과가 유의하게 나타난 경우는 72.73%이었고, 유의하지 않는 경우가 9.09%이었다. 부모 직접적 또는 간접적 접근과 집중행동 치료에서 효과가 유의하게 나타났다. 결론 : 아동 작업치료에서 시운동기술을 평가하기 위해 사용된 도구는 DTVP와 VMI가 60%를 차지하였고, 70%이 상에서 치료에 대한 효과가 있는 것으로 나타났다. 아동의 작업수행과 관련된 영역에 대한 작업치료의 효과를 분석하는 연구가 지속되어야 할 것으로 보인다.

Mammalian physiology and behavior are regulated by an internal time-keeping system, referred to as circadian rhythm. The circadian timing system has a hierarchical organization composed of the master clock in the suprachiasmatic nucleus (SCN) and local clocks in extra-SCN brain regions and peripheral organs. The circadian clock molecular mechanism involves a network of transcription-translation feedback loops. In addition to the clinical association between circadian rhythm disruption and mood disorders, recent studies have suggested a molecular link between mood regulation and circadian rhythm. Specifically, genetic deletion of the circadian nuclear receptor Rev-$erb{\alpha}$ induces mania-like behavior caused by increased midbrain dopaminergic (DAergic) tone at dusk. The association between circadian rhythm and emotion-related behaviors can be applied to pathological conditions, including neurodegenerative diseases. In Parkinson's disease (PD), DAergic neurons in the substantia nigra pars compacta progressively degenerate leading to motor dysfunction. Patients with PD also exhibit non-motor symptoms, including sleep disorder and neuropsychiatric disorders. Thus, it is important to understand the mechanisms that link the molecular circadian clock and brain machinery in the regulation of emotional behaviors and related midbrain DAergic neuronal circuits in healthy and pathological states. This review summarizes the current literature regarding the association between circadian rhythm and mood regulation from a chronobiological perspective, and may provide insight into therapeutic approaches to target psychiatric symptoms in neurodegenerative diseases involving circadian rhythm dysfunction.
Excess manganese (Mn) is neurotoxic. Increased manganese stores in the brain are associated with a number of behavioral problems, including motor dysfunction, memory loss and psychiatric disorders. We previously showed that the transport and neurotoxicity of manganese after intranasal instillation of the metal are altered in Hfe-deficient mice, a mouse model of the iron overload disorder hereditary hemochromatosis (HH). However, it is not fully understood whether loss of Hfe function modifies Mn neurotoxicity after ingestion. To investigate the role of Hfe in oral Mn toxicity, we exposed Hfe-knockout ($Hfe^{-/-}$) and their control wild-type ($Hfe^{+/+}$) mice to $MnCl_2$ in drinking water (5 mg/mL) for 5 weeks. Motor coordination and spatial memory capacity were determined by the rotarod test and the Barnes maze test, respectively. Brain and liver metal levels were analyzed by inductively coupled plasma mass spectrometry. Compared with the water-drinking group, mice drinking Mn significantly increased Mn concentrations in the liver and brain of both genotypes. Mn exposure decreased iron levels in the liver, but not in the brain. Neither Mn nor Hfe deficiency altered tissue concentrations of copper or zinc. The rotarod test showed that Mn exposure decreased motor skills in $Hfe^{+/+}$ mice, but not in $Hfe^{-/-}$ mice (p = 0.023). In the Barns maze test, latency to find the target hole was not altered in Mn-exposed $Hfe^{+/+}$ compared with water-drinking $Hfe^{+/+}$ mice. However, Mn-exposed $Hfe^{-/-}$ mice spent more time to find the target hole than Mn-drinking $Hfe^{+/+}$ mice (p = 0.028). These data indicate that loss of Hfe function impairs spatial memory upon Mn exposure in drinking water. Our results suggest that individuals with hemochromatosis could be more vulnerable to memory deficits induced by Mn ingestion from our environment. The pathophysiological role of HFE in manganese neurotoxicity should be carefully examined in patients with HFE-associated hemochromatosis and other iron overload disorders.
A patient with respiratory disorders such as a sleep apnea is increasing as the obese patient increase on the modern society. Positive Airway Pressure (PAP) devices are used in curing patient with respiratory disorders and turn out to be efficacious for patients of 75%. However, these devices are required for evaluating their performance to improve their performance by the mechanical breathing simulator. Recently, the mechanical breathing simulator was studied by the real time feedback control. However, the mechanical breathing simulator by an open loop control was specially required in order to analyze the effect of flow rate and pressure after operating the breathing auxiliary devices. Therefore the aims of this study were to make the mechanical breathing simulator by a piston motion and a valve function from the characteristic test of valve and motor, and to duplicate the flow rate and pressure profiles of some breathing patterns: normal and three disorder patterns. The mechanical simulator is composed cylinder, valve, ball screw and the motor. Also, the characteristic test of the motor and the valve were accomplished in order to define the relationship between the characteristics of simulator and the breathing profiles. Then, the flow rate and pressure profile of human breathing patterns were duplicated by the control of motor and valve. The result showed that the simulator reasonably duplicated the characteristics of human patterns: normal, obstructive sleep apnea (OSA), mild hypopnea with snore and mouth expiration patterns. However, we need to improve this simulator in detail and to validate this method for other patterns.
Purpose : Herpes zoster is a common dermatologic disorder and is caused by reactivation of varicella zoster virus lying dormant in the ganglion of the dorsal root Methods : The aim of this study is to elucidate the clinical characteristics of herpes zoster and it's nature of pain, and is to review the method of physical therapy for pain control. Results : Herpes zoster is characterized by segmental rash, pain, and sensory symptoms, For most patients skin healing and pain resolution occur within 3-4 weeks, However, pain can continue after the rash has healed. Pain and paresthesia often the eruption of herpes zoster and vary from itching to stabbing. The preeruptive pain may simulate other diseases and may lead to misdiagnosis and misdirected interventions. Motor symptomatology is less well known and is most often related to central nervous system disease, although true lower motor neuron application is also thought to exist Subclinical motor involvement is relatively more common than clinical motor weakness and is easily detected by using electromyography. Higher incidences of herpes zoster were observed in female and in the elderly. Conclusion : The nature of pain associated with herpes zoster varied from a superficial itching to server stabbing or bursting, and paresthesia occurred most frequently. Therefore, the study of herpes zoster will be more research and comprehend, and the approach of physical therapy should be need positively.
Objective This case study was about a long-term clinical effects after using Korean medicine treatment and western treatment in three Soyangin patients with Parkinson's disease. In this study, we reported significant improvement of non-motor and motor symptoms of these patients after Sasang constitutional medicine treatments for more than a year. Method The patients were identified as Soyangin and treated with Yangguksanhwa-tang, Hyeongbangdojeok-san and acupuncture. The Hoehn & Yahr scale, Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (UPDRS) were used to assess the overall function of the patients. And the Global Assessment Scale (GAS) was used to assess the change of chief complaints after the treatments. Result and Conclusion The non-motor and motor symptoms of Parkinson's disease improved after Sasang constitutional medicine and western medicine treatments for more than a year. Furthermore these patients' chief complaints improved without increasing of amount of western medicine (levodopa). In conclusion, this study showed that Sasang constitutional medicine with western medicine can be effective treatment for long-term clinical care of Parkinson's disease.
Purpose: This study examined the effects of sagittal spinopelvic alignment on the clinical parameters, motor symptoms, and respiratory function in patients with mild to moderate Parkinson's disease (PD). Methods: This study was a prospective assessment of treated patients (n=28, Hoehn and Yahr (H&Y) stage 2-3) in a PD center. Twenty-eight subjects ($68.5{\pm}5.7yrs$) participated in this study. The clinical and demographic parameters, including age, sex, symptoms duration, treatment duration, and H&Y stage, were collected. Kinematic analysis was conducted in the upright standing posture with a motion capture system. A pulmonary function test (PFT) was performed in the sitting position using a spirometer. The motor symptoms were assessed on part III of the movement disorder society sponsored version of the unified Parkinson's disease rating scale (MDS-UPDRS). SPSS 18.0 was used to analyze the collected data. Results: The exceeding 12 degrees group of the lower trunk showed significantly higher on the clinical parameters than the below 12 degrees group. In addition, the exceeding 12 degrees group of the lower trunk showed a significantly lower forced expiratory volume at one second (FEV1) / forced vital capacity (FVC) (%) and 25-75% forced mid-expiratory flow (FEF) (L/s) than in the below group. On the other hand, there was no difference in the upper trunk and the cervical pelvis between the groups. Conclusion: These findings suggest that the sagittal balance in the lower trunk is related to the clinical parameters and respiratory function, but not the motor symptoms in patients with mild to moderate PD.
본 연구는 섭식장애가 있는 프래더 윌리(PWS) 아동을 대상으로 구강운동촉진기술(OMFT) 적용 후 섭식발달의 변화를 확인하고 전반적인 발달과 비교·분석하여 OMFT가 섭식발달에 미치는 효과를 알아보고자 하였다. 대상 아동은 경관 식이로 섭식 중인 교정 연령 8개월 PWS 여자 1명으로 2020년 7월부터 2021년 6월까지 OMFT를 주 1회, 1년 동안 제공하였다. OMFT 적용 후 구강 섭식 전환 및 연령에 맞는 음식 섭취, 씹기 기능 증진, 빨대 사용, SSB 조절 향상 등의 섭식발달 단계 변화가 관찰되었다. 본 연구를 통해 OMFT가 PWS 아동의 섭식 발달 및 구강 섭취 전환에 효과적인 것으로 확인되었다. PWS 아동의 섭식 및 연하장애에 대한 조기 평가 및 치료 접근이 중요하며 근긴장도 및 근력 저하의 구강운동기능 문제 해결을 위한 OMFT 적용을 통한 섭식 발달 향상을 위한 접근이 필요할 것이다.
