The purposes of this study were to find correlations among Fugl-Meyer Assessment scale, gait speed, and Timed Up & Go test (TUG) and to predict gait ability from subscales of Fugl-Meyer Assessment scale. The study population consisted of 30 stroke patients referred to the Department of Rehabilitation Medicine in the Bundang Jaesang General Hospital. All subjects were ambulatory with or without an assistive device. All participants were assessed on Fugl-Meyer Assessment scale and gait speed (m/s), TUG (s). The data were analyzed using independent t-test, Pearson product moment correlation analysis and stepwise multiple regression. The results revealed that all items of Fugl-Meyer Assessment scale, except passive joint range of motion were significantly correlated with gait speed and TUG. In particular, sensation score, lower extremity motor and coordination score have a significant correlation with gait speed and TUG (p<.05). The sensation score and lower extremity motor score were important factors in comfortable gait and maximal gait speed. Their power of explanation regarding comfortable gait and maximal gait speed were 63.0% and 65.0%, respectively. The sensation score and lower extremity coordination score were important factors in TUG. Their power of explanation regarding TUG was 55.0%. These results showed that Fugl-Meyer Assessment scale is significantly correlated with gait speed and TUG. Therefore Fugl-Meyer Assessment scale is an appropriate assessment tool to predict gait ability of patients with stroke. Further study about gait speed and TUG by change of Fugl-Meyer Assessment score is needed using a longitudinal study design.
Purpose : The purpose of this research is to find clinical effects of functional resistance training using weighted vest on gross motor and balance abilities of children with dyskinetic cerebral palsy. Methods : This study selects 3 subjects for 8~12 years old who were diagnosed with children with dyskinetic cerebral palsy. The Design is ABA design of single-subject research design. Baseline(A) and TypeII Baseline(A : 12weeks) phases were received with NDT treatment, Intervention(B : 12weeks) phase provided with 40 minute functional resistance training using weighted vest in a session twice a week. In order to analyze the measure results of gross motor function and performance, balance abilities in children dyskinetic cerebral palsy during baseline, intervention and typeII baseline phase. Result : A statistically significant differences in the total GMFM including walking/running/jumping during baseline, intervention, typeII baseline, but no significant differences in the lying/rolling, sitting, crawling/kneeling and standing. A statistically significant differences in the total GMPM including dissociated movement, coordination, weight shift, stability during baseline, intervention, typeII baseline, but no significant differences in the body alignment domains. A statistically significant differences in the length and surface area ellipse of center of pressure during baseline, intervention, typeII baseline. Conclusion : The intervention method to facilitate multi-joint and closed kinematic chain movement equipped weighted vest applied functional resistance training on children with dyskinetic cerebral palsy effectively improve on gross motor function and performance, balance abilities.
Background and Purpose : The hemiplegic upper extremity is affected in many stroke patients, and recovery is often poor. The purpose of this study was to assess the efficacy of electroacupuncture (EA) in enhancing the upper extremity motor and functional recovery of ischemic stroke patients. Subjects and Methods : Forty ischemic stroke patients (the upper extremity Fugl-Meyer motor scale (FM) score lower than 46, lesion location within middle cerebral artery territory) within 2 weeks of stroke onset were randomly allocated to either an EA group that received EA treatment or a control group that received only routine ward care. The EA was applied at Quchi-Shousanli (LI11-LI10), Waiguan-Hegu (TE5-LI4) points on the hemiparetic side six times per week for 4 weeks. The frequency of stimulation was 25-50Hz and the intensity was set at a level sufficient to induce muscle contraction. EA treatment was given for 30 minutes and all patients of both groups received standard rehabilitation program. Outcomes were assessed, in a blind manner, before treatment began and at 4 weeks after treatment, with the FM, the Motor Power score (MP) for shoulder/elbow, and the subsection of the Modified Barthel Index (MBI) for drinking/feeding/dressing upper body/grooming. Results : These 2 groups had comparable clinical characteristics, lesion location, lesion size, and pretreatment impairment scores. By the end of treatment, the EA group showed significantly more improvement than the control group in the subsection of the FM for shoulder/elbow/coordination (6.4 vs. 3.7; P=0.047) and the MP for shoulder/elbow (5.3 vs. 3.3; P=0.008). The subsection scores of the MBI for drinking/feeding/dressing upper body/grooming were not significantly different between two groups. No adverse effects due to treatment were found Conclusion : These results suggest that EA enhances the upper extremity motor recovery of acute stroke patients. However, this study failed to demonstrate any significant functional benefit related with upper extremity. Future study should be carried out in a larger sample size and use the functional outcome measure that is more specific and sensitive to the upper extremity.
본 연구에서는 다이아프램(diaphragm)을 설치한 하이브리드 로켓에서 발생하는 연소불안정의 발생 메커니즘을 알아보고, 고체연료의 연소율은 증가하면서 연소불안정은 줄일 수 있는 그레인 설계를 제안하였다. 고체연료의 연소율을 증진시키기 위해 다이아프램을 설치한 하이브리드 로켓 모터에서 관찰되는 큰 연소불안정의 가진 요인은 전방 연소실에서 생성된 와류가 후방의 다이아프램에 부딪치면서 나타나는 Hole-tone으로 판단된다. 또한 다이아프램의 고연소율 발생 메카니즘을 적용하면서 연소불안정은 줄일 수 있는 'Stepped Grain'을 제안하였다.
Aging is related to anatomical and physiological changes in respiratory and phonation organs. These changes influence articulation which leads to inaccurate speech and slow articulatory diadochokinesis(DDK). DDK indicates the range, rate, regularity, accuracy, and agility of articulation that reflect motor speech function. The purpose of this study is to investigate the rates and regularities of DDK in healthy Korean elderly through passive acoustic analysis (Praat). Thirty subjects between the ages of 65 and 94 participated in this study. Rate was observed for 5 seconds, while regularity was calculated based on the standard deviation on the following: 1) syllable duration of each task; 2) gap duration between syllables. Then, simple regression analysis was conducted in order to examine the effect of age on performance. The result showed that the slow rate was not a significant factor in terms of advancing age. Furthermore, regularity indicated a significant difference in the following: 1) /pʌ/, /kʌ/ and /pʌtʌkʌ/ in syllable duration; 2) /kʌ/ duration in the gap between syllables. In conclusion, articulatory coordination is reduced with the onset of aging. In particular, /kʌ/ would be a sensitive task for articulatory coordination.
Purpose: We aimed to investigate whether visuomotor function would be modulated, when healthy subjects performed tracking task after tDCS application over the primary sensorimotor cortex (SM1) in the non-dominant hemisphere. Methods: Thirty four right-handed healthy participants were enrolled, who randomly and evenly divided into two groups, real tDCS group and sham control group. Direct current with intensity of 1 mA was delivered over SM1 for 15 minutes. After tDCS, tracking task was measured, and their performance was calculated by an accuracy index (AI). Results: No significant difference in AI at the baseline between the two groups was observed. The AI of the real tDCS group was significantly increased after electrical stimulation, compared to the sham control group. Two way ANOVA with repeated measurement showed a significant finding in a large main effects of time and group-by-repeated test interaction. Conclusion: This study indicated that application of the anodal tDCS over the SM1 could facilitate higher visuomotor coordination, compared to sham tDCS group. These findings suggest possibility that tDCS can be used as adjuvant brain modulator for improvement of motor accuracy in healthy individuals as well as patients with brain injury.
정신분열증 환자에서 연성 신경학적 증상이 많이 나타나는 것은 많은 연구에서 확인된 바 있으나 이것이 항정신병 약물 복용 등에 의하여 발생하는 부수적인 현상일 가능성이 배제되지 않고 있다. 본 연구는 항정신병 약물 복용군과 비복용군간의 연성 신경학적 증상 발현의 차이를 알아보고자 시도되었다. 항정신병 약물을 전혀 복용하지 않은 28명과 현재 항정신병 약물로 치료 중에 있는 31명의 정신분열증 환자에서 연성 신경학적 증상을 구조화된 평가척도인 한국어판 신경학적 평가척도로 조사하였고 약물용량과 투여기간 및 부작용과의 관계도 알아보았다. 항정신병 약물 비복용군의 신경학적 평가척도의 총점은 $27.14{\pm}5.66$점으로 복용군의 $19.58{\pm}6.55$점보다 유의하게 높았다 (t=-4.76, P<0.001). 하위 기능영역 중에서도 운동조정(t=-5.52, P<0.001), 복합운동활동 순서화(t=-2.20, P<0.05), 기타 항목(t=-5.14, P<0.001)등의 점수는 비복용군에서 높았다. 감각통합 영역은 약물 복용군과 비복용군 사이에 유의한 차이가 없었다. 약물 용량, 연령, 성별 등을 통제한 후의 조사에서는 총점(F=11.13, P=0.00), 운동조정(F=28.46, P=0.00), 기타 항목(F=10.79, P=0.00)등은 약물 비복용군에서 높았으나 복합운동활동 순서화는 이들 공변량의 영향을 받아 약물 복용군과 비복용군사이에 유의한 차이가 없었다. 약물 복용군에서 현재의 항정신병 약물 복용량은 신경학적 평가척도 점수 중 운동 조정 영역과만 유의한 상관성을 보였으며 약물 복용기간, 부작용 등은 연성 신경학적 증상과 상관성이 없었다. 위와 같은 결과로 항정신병 약물은 정신분열증의 신경학적 증상 발현에 상대적인 영향만을 준다고 할 수 있었으며 연성 신경학적 증상이 정신분열증의 주요한 형질 표지자의 하나가 될 가능성이 있다고 하겠다. 또한 향후 보다 통제된 집단에서의 추적연구가 필요할 것으로 생각되었다.
Background: Posture balance control is the ability to maintain the body's center of gravity in the minimal postural sway state on a supportive surface. This ability is obtained through a complicated process of sensing the movements of the human body through sensory organs and then integrating the information into the central nervous system and reacting to the musculoskeletal system and the support action of the musculoskeletal system. Motor function, including coordination, motor, and vision, vestibular sense, and sensory function, including proprioception, should act in an integrated way. However, more than half of stroke patients have motor, sensory, cognitive, and emotional disorders for a long time. Motor and sensory disorders cause the greatest difficulty in postural control among stroke patients. Objects: The purpose of this study is to determine the effect of visual and somatosensory information on postural sway in stroke patients and carrying out a kinematic analysis using a tri-axial accelerometer and a quantitative assessment. Methods: Thirty-four subjects posed four stance condition was accepted various sensory information for counterbalance. This experiment referred to the computerized dynamic posturography assessments and was redesigned four condition blocking visual and somatosensory information. To measure the postural sway of the subjects' trunk, a wireless tri-axial accelerometer was used by signal vector magnitude value. Ony-way measure analysis of variance was performed among four condition. Results: There were significant differences when somatosensory information input blocked (p<.05). Conclusion: The sensory significantly affecting the balance ability of stroke patients is somatosensory, and the amount of actual movement of the trunk could be objectively compared and analyzed through quantitative figures using a tri-axial accelerometer for balance ability.
본 논문에서는 유도전동기 회전자 자속 기준제어를 위하여 비선형 관측기인 확장된 루엔버거 관측기 원리를 적용한 새로운 회전자 자속관측기를 제안하였다. 확장된 루엔버거 관측기는 확장된 칼만 필터와 유사하게 동특성 오차의 선형화 기법을 따트고 있으나 통계학적 속성의 노이즈 공분산을 고려하지 않는 결정론적 관측기로서 비선형 상태관측기 설계시 요구되는 좌표변환 및 선형화 파정에서 비선형 편미분 방정식의 직접적인 해를 펼요로 하지 않아 구현이 비교적 용이하다. 제안된 회전자 자속관측기는 직교좌표의 고정자 전류, 회전자 자속, 속도 및 부하 토크로 구성된 6차 미분방정식으로부터 유도되었으며 축약된 형태의 이득행렬을 갖는다. 시뮬레이션 및 실험은 파라 미터중 회전자 저항 값이 변동된 상황을 가정하여 수행하였으며, 시뮬레이션 결과 제안된 관측기를 이용한 자속 추정시 극점 재배치를 통하여 동특성 오차의 수렴성을 제어할 수 있으며, 부하 설험결과 제안된 관측기를 적용하는 경우에는 슬립적분형 간접벡터제어에 비해 보다 정확한 벡터제어가 가능함이 확인되었다.
목적 : Interactive Metronome(IM) 훈련프로그램이 ADHD 아동의 타이밍과 주의력 그리고 운동기능에 미치는 영향을 사례보고 하고자 한다. 연구방법 : ADHD진단을 받은 일반초등학교 1학년에 재학 중인 2008년생 남아(사례1)와 일반중학교 2학년에 재학 중인 2001년생 남아(사례2)를 대상으로 2015년 1월 12일부터 2월 6일까지 4주간 IM 훈련프로그램을 적용하였다. 본 연구에서는 IM Pro 9.0을 이용하여 타이밍 향상을 위한 중재로 사용하였다. 총 3주간, 주 5회, 총 15회기, 회기 당 50-60분간 적용하였다. 또한 IM 훈련프로그램의 중재 효과를 알아보기 위해 중재 전후에 타이밍, 주의력, 운동기능을 평가하였는데 평가 도구로는 타이밍 향상을 평가하기 위해 IM의 Long Form Assessment(LFA)를 사용하였다. 주의력을 평가하기 위해 Rhehacom 판별 검사(RehaCom screening module)를 사용하였고, 운동 기능을 평가하기 위해 Bruininks-Oseretsky Test of Motor Proficiency 2판(BOT-2)를 사용하였다. 결과 : LFA의 타이밍 능력의 평가 결과는 사례 아동 모두에서 전체 운동과제의 평균 반응 속도가 감소되어 타이밍 기능이 향상되었음을 알 수 있었다. 주의력은 사례1 아동에서 시각적 분리 주의력(visual attention division), 편측무시(neglect) 그리고 충동 조절(response inhibition) 기능이 향상되었고. 사례2 아동에서는 지속적 주의력(sustained attention) 기능이 향상되었다. 운동기능은 BOT-2 단축폼(short form)의 백분위(%ile Rank)가 사례1 아동은 42%ile에서 96%ile로 향상되었고, 사례2 아동은 21%ile에서 66%ile로 향상되었다. 두 사례 아동 모두 양측 협응(bilateral coordination)항목과 상지 협응(upper-limb coordination)항목의 등가연령이 향상되었다. 결론 : 본 사례보고는 IM 훈련프로그램이 ADHD 아동의 타이밍, 주의력, 운동기능에 긍정적 효과가 있음을 시사한다.
