실험적 뇌허혈로 인한 편마비 흰쥐에서 운동 적응 훈련이 기능 회복에 미치는 영향 (The Effect of Exercise Adaptive Training on Motor Function after Experimental brain ischemia in Rats)

  • The purpose of this study was to test that the exercise adaptive training enhance behavioral outcome significantly after focal brain ischemia in rats. After occlusion of middle cerebral artery in rats, they were housed in individual standard cages fur 24 hours. The control group was sacrificed 24 hours after ischemic event. The experimental group I was housed in standard cages for 7days. The experimental group ll was housed in enriched environment and had got exercise adaptive training fur 7days. The rats were examined five motor behavioral tests. In motor behavioral tests :postural reflex test, limb placement test, beam-walking test, rotarod test, horizontal wire test. The outcomes of control group and group I were significantly lower than the group II. The conclusion was that exercise adaptive training induced functional repair.

  • 본 논문은 3D프린터와 모터를 활용해 무게와 부피를 줄인 재활용 웨어러블 로봇 손의 설계 및 구현에 관한 연구이다. 재활용 웨어러블 로봇은 재활의 효과도 중요하지만, 사용의 간편성 또한 중요하다. 하지만 현재 연구 및 개발된 재활용 외골격 로봇은 부피와 무게가 무겁거나 제자리에서 사용해야 하는 것이 대부분이다. 따라서 착용에 용이하고 사용자에게 중량이 부담되지 않는 웨어러블 로봇이 필요하므로 경량화된 전동식 재활용 웨어러블 로봇 손을 제안하였다. 3D프린터를 사용하여 무게와 부피를 줄이고 착용에 용이하도록 설계를 진행하였다. 또한, 휴대성을 높이기 위해 공압 방식이 아닌 모터기반의 전동방식을 채택해 구조를 간단하게 구성하였다. 최종적으로 경량화된 전동식 재활용 웨어러블 로봇 손의 실험을 통해 제안한 방식의 효용성을 검토하였다.

Integrated Rehabilitation for Children with Cerebral palsy: A Prospective Observational Study

  • Objectives: This study aimed to analyse: 1) the clinical effectiveness and safety of traditional Korean Medicine (TKM) and frequency of TKM therapies used and their relationship with conventional treatments. Methods: This prospective observational study enrolled children with cerebral palsy (CP) aged 6-78 months (n=126). The children who used herbal medication for >30 days or acupuncture treatment >12 sessions within 6 months were defined as the integrated rehabilitation (IR) group; the remaining participants were included in the conventional rehabilitation (CR) group. Results: Changes in the Gross Motor Function Measure-66 (GMFM-66) were greater in the IR group (6.4±6.1) than in the CR group (4.6±5.8). The reduction in the number of other health problems was greater in the IR group than in the CR group. The GMFM-66 improvement was greatest in gross motor function classification system level 1. There was no between-group difference in the frequency of rehabilitation therapy (10.9±6.6 and 12.0±9.9 in the IR and CR groups, respectively). Conclusion: TKM may offer additional benefits in terms of the GMFM-66 score and other health problems. However, there is a need for further randomized controlled trials involving a restricted CP type and a controlled treatment type and intensity to confirm these findings.

전침이 중풍재활에 미치는 영향에 대한 문헌적 고찰 (Literature Review of Electroacupuncture for Stroke Rehabilitation)

  • Objectives : Electroacupuncture(EA) has been suggested as a treatment for stroke rehabilitation. But whether, how much, by what mechanism and when it is effective has not been answered satisfactorily. Therefore it is important to critically review clinical trials and laboratory researches about EA for stroke rehabilitation. Subjectives : We researched various recent sources of EA for stroke rehabilitation such as medical journals and especially tried to review methodologically best randomized controlled trials(RCTs). Results and Conclusions : 1) EA increases brain plasticity, activity, blood flow and secretion of neuropeptides in CNS. 2) EA is significantly effective at the case that more than half of the neural motor pathway is reserved. 3) The acupoints, frequncy and intensity of EA should be determined by patient-specific symptoms of stroke. 4) More studies is needed for merdian functions for stroke rehabilitation.

Transcallosal Ipsilateral Motor Pathway from the Unaffected Hemisphere in a Patient with Traumatic Brain Injury

  • Purpose: We presented with a patient with traumatic brain injury who showed a transcallosal ipsilateral motor pathway from unaffected motor cortex to affected extremities, as evaluated by diffusion tensor tractography (DTT). Methods: One patient and six age-matched normal subjects were enrolled. A 42-year-old left hemiparetic male who suffered from brain injury by motor vehicle accident 9 years ago showed a leukomalactic lesion in the right corona radiata and parieto-temporal lobe. His left extremities were completely paralyzed initially, but recovered slowly over 2 years. At the time of the evaluation, he was able to grasp and release an object, and to walk with spastic gait pattern. DTT was performed using 1.5 T with a Synergy-L Sensitivity Encoding head coil. DTT was obtained with termination criteria of FA <0.2 and an angle change > $45^{\circ}c$. Results: The motor tracts of the unaffected (left) hemisphere of the patient and control subjects originated from the motor cortex and descended along the known corticospinal tract without any transcallosal tract. By contrast, the tract of the affected (right) hemisphere originated from the left premotor cortex, descended through the left corona radiata, and then crossed the mid-portion of the corpus callosum. The tract then descended through the known corticospinal tract pathway to the right medulla. Conclusion: We conclude that the transcallosal ipsilateral motor pathway from the unaffected hemisphere appeared to contribute to the motor recovery in this patient.

A Literature Study on Usage of and Satisfaction Levels with Combined Treatment Including Oriental and Western Medicine

  • Objective: This study aimed to summarize and analyze the usage of and the satisfaction levels with combined treatment including Oriental and Western medicine. Methods: We searched studies on the usage of and the satisfaction levels with combined treatment including Oriental and Western medicine over the past 10 yrs (2001-2011) from 3 Korean databases (National Assembly Library, Research Information Service System, and National Discovery for Science Leaders). The reviewers also conducted a summarizing analysis by sampling the literature according to the type of study, study period, region, study subjects, sample size, type of sampling, research method, data analysis, study instruments, main results, etc. Results: When the main results of six studies on combined treatment usage and satisfaction levels were considered together, the most important decisive factor in determining the usage of combined treatment was the illness of the patient, followed by the patient's occupation, sex, age, education, marital status, religion, treatment cost, and treatment results. In addition, the most important factor that determined satisfaction levels with combined treatment was age, followed by education, religion, income, health status, treatment procedures, staff attitude, and cleanliness. Conclusions: Elderly patients with musculoskeletal, cerebro-vascular, and circulatory system illnesses are more likely to prefer combined treatment over independent Oriental or Western treatment and are more likely to request specialized, adjusted medical care.

  • This study was performed to report the effectiveness of oriental medical treatment on a patient with Guillain-Barre syndrome complaining both lower extremity weakness. The patient was diagnosed as Guillain-Barre syndrome and treated with acupuncture, electroacupuncture, physical therapy and herbal medicine. We have evaluated the efficacy of oriental medical treatment by measuring changes of motor grade and ambulatory condition according to period of hospitalization. After treatment, motor grade and ambulatory condition were improved. These results suggest that oriental medical treatment was effective on the patient with Guillain-Barre syndrome.

보행 재활 로봇 개발을 위한 1자유도 무릎 관절 설계 (Design of an 1 DOF Assistive Knee Joint for a Gait Rehabilitation Robot)

  • One of the important issues for structural and electrical specifications in developing a robot is to determine lengths of links and motor specifications, which need to be appropriate to the purpose of robot. These issues become more critical for a gait rehabilitation robot, since a patient wears the robot. Prior to developing an entire gait rehabilitation robot, designing of a 1DOF assistive knee joint of the robot is considered in this paper. Human gait motions were used to determine an allowable range of knee joint that was rotated with a linear type actuator (ball-screw type) and links. The lengths of each link were determined by using an optimization process, minimizing the stroke of actuator and the total energy (kinetic and potential energy). Kinetic analysis was performed in order to determine maximum rotational speed and maximum torque of the motor for tracking gait trajectory properly. The prototype of 1 DOF assistive knee joint was built and examined with a impedance controller.

우리나라 중소도시 뇌졸중 환자의 임상적 특성과 재활서비스 수혜 실태에 관한 연구 (The study of clinical pictures and received rehabilitation service patterns of stroke patients)

  • This study was performed to investigate the clinical pictures and rehabilitation service patterns of stroke patients in cities of kimchon(kumi), jinju. pohwang, jeonju and jeju from January 1,2001 to march. Authors developed structured questionnaire, and distributed it to each physical therapist. Total number of distributed questionnaire was 400, and 301 questionnaire were col looted and analysed finally.1. The occurrence rate of hemorrhagic stroke(60.5%) was higher than that of ischemic stroke(39.5%). The highest incidence of the stroke was noted in the group of 50 years and ratio of male to female 1.3 : 1. 2. The several warning sign is motor deficit(42.5%). headache. dizziness, vomitting(31.9%) and difficulty speaking or understanding(10.6%). 3. The most important contributing factor of stroke was hypertension both hemorrhagic(50.0%) and ischemic stroke(40.3%). 4. In the painful stroke patients(53.2%). the major problems were shoulder pain(51.3%) and shoulder-hand syndrom(31.3%). 5. In hemorrhagic stroke. the seasonal preference was autumn and summer. In ischemic stroke. the seasonal preference was autumn and winter. 6. In the surgery, hemorrhgic stroke(44.5%) was higher than that of ischemic stroke(20.2%). 7 The major impairment were motor deficit(95.3%), psychological deficits(24.4%), hearing and speech deficit(23.4%). vision deficit(14.7%) and perception deficit(12.7%). 8. The rehabilitation services for stroke patients were physical therapy(98.0%), occupational therapy(41.5%), oriental therapy(35.1%), speech therapy(17.5%) and psychological therapy(7.4%). 9. The start of rehabilitation service was different according to surgery. 36.5% of patients started physical therapy in 4 week and 32.0% of patients started in 8 week later in surgery and 53.1% of patients started physical therapy in 2 week in not surgery( P<0.05). 10. The highest frequency of rehabilitation service was six times per week.

햅틱 장치를 이용한 상지 운동기능 장애인의 재활치료에 관한 기초 실험 (Basic Experiment on Rehabilitation of Upper-Limb Motor Function Using Haptic-Device System)

  • 상지 운동기능 장애인의 재활치료에 대한 지속적인 관심과 훈련의 정량적인 평가가 요구되고 있다. 이러한 문제를 해결하기 위해서 햅틱 장치를 개발했다. 피험자가 햅틱 장치의 손잡이를 움직일 때, 가상의 힘을 제시하여 그들의 동작에 대해 어시스트력이나 항력을 제시할 수 있다. 개발한 상지 운동훈련 프로그램과 시스템의 유효성을 검토하기 위해, 5 명의 운동기능 장애인을 대상으로 직선동작 훈련 프로그램을 수행할 때의 손잡이 위치, 수행 속도, 손잡이의 오차 및 상지의 EMG 신호를 측정하였다. 5 명의 피험자의 손잡이의 궤적 및 EMG 활동이 유사함을 보였다. 이러한 결과는 가상 현실의 힘을 손잡이에 제시함으로써 EMG 활동이 증가하였다고 볼 수 있다. 본 실험의 결과, 객관적이며 정량적인 상지 재활치료 시스템의 개발 및 평가 방법이 시사되었다.
