당뇨병성다발신경병증의 전기생리학적 특징: 운동신경전도검사 (Electrophysiological Features of Diabetic Polyneuropathy: Motor Nerve Conduction Studies)

  • 신경전도검사는 당뇨병성다발신경병증 및 당뇨병의 불현성 신경병증환자를 진단하기위한 중요한 검사이며 신경의 기능을 정량적으로 측정할 수 있는 검사기법이다. 본 연구에서는 당뇨병성다발신경병증 환자에서 보이는 전기생리학적 특징을 평가하기위하여 당뇨병성다발신경병증으로 진단된 120명과 정상대조군 77명의 운동신경전도검사의 결과를 비교?분석하였다. 상지와 하지의 각 운동신경에서 정상기준치에 대해 비정상 값을 보인 비율의 특징을 신경전도속도, 말단잠복기, 복합근육활동전위의 진폭, No potential의 빈도, 전도차단의 항목으로 나누어 분석하였다. 당뇨병성다발신경병증은 상지에 비해 하지를 더욱 침범하는 전신성 탈수초성 말초다발신경병증의 전기생리학적 특징이 관찰되었고, 특히 비골신경에서 비정상의 정도가 심한 것으로 분석되었다. 하지만 전도차단의 특징은 대조군에 비하여 유의한 차이가 없는 것으로 나타났다. 운동신경전도검사의 결과를 분석하여 당뇨병성다발신경병증의 전기생리학적 특징을 분석한 본 연구의 결과 하지의 비골신경이 중요한 지표가 될 수 있음이 확인되었고, 이는 당뇨병성다발신경병증의 중요한 전기생리학적 소견으로 사료된다.

수근관 증후군 치료결정의 한 요인으로 연령의 중요성 (The Importance of Age as a Factor of Carpal Tunnel Syndrome management)

  • Background : Carpal tunnel syndrome(CTS) is the most common entrapment neuropathy that refers to a group of signs and symptoms resulting from compression of the median nerve at the wrist. The course of CTS in older patients is different from the younger patients. This difference may be the result of different underlying mechanisms. The different nerve conduction studies of CTS may signify different approaches in management. This study was done to assess the differences in nerve conduction study of CTS in younger and older patients. Methods : This study involved 224 patients who visited Gachon Medical School, Gil Medical Center and was diagnosed by nerve conduction study from October 1997 to October 1999. We compared the results of nerve conduction study to age, especially in between those under 60 years and those 60 years or over CTS patients. Nerve conduction study consists of motor studies of both median nerves(terminal latency, compound action potential) and sensory studies(nerve conduction velocity, nerve action potential). And we also evaluated the variables between younger and older patients group. Those variables include sex, symptom period, laterality, abnormal physical findings and radiculopathy. Results : We found that a significant increase of terminal latency(p<0.1), but a decrease in compound motor action potential(p<0.05) in older patient's group. There was no significant differences in sensory nerve conduction velocity and action potential between those under 60 years and those 60 years or even patients. And also there was no significant difference in sex, symptom period, laterality, abnormal physical findings, radiculopathy between older and younger patients. Conclusions : This study showed a significant increase in the terminal latency and a decrease in compound action potential in older patients. The different nerve conduction studies of CTS by age effect may need different approaches in management.

POEMS 증후군의 임상적, 전기생리학적 특성 (Clinical and Electrophysiological Features of the Patients with POEMS Syndrome)

  • Backgrounds and objectives: POEMS (polyneuropathy, organomegaly, endocrinopathy, M protein, and skin changes) syndrome is the rare cause of polyneuropathy. Although the polyneuropathy is essential for the diagnosis of the disease, the pattern of electrodiagnostic abnormalities has not been characterized in detail. The purpose of this study was to elucidate the features of nerve conduction abnormalities in POEMS syndrome. Methods: We reviewed the medical records and nerve conduction studies (NCS) of 12 consecutive patients with POEMS. Results: A total of 68 motor and 46 sensory nerves were examined. Compound muscle action potentials (CMAPs) and sensory nerve action potentials were abnormally attenuated or not elicited in majority of motor and sensory nerves (80.88% in motor, and 82.6% in sensory nerves). Frequency of the nerves with no potential was significantly higher in lower limbs than in upper limbs (p<0.01 in both motor and sensory nerves), and CMAP amplitude was more reduced in lower limbs than in upper limbs (p<0.01). Conduction slowing was very frequently observed with 95% and 76% of motor and sensory nerves, respectively, having the abnormally reduced values of conduction velocity. Distal motor latencies were abnormally prolonged in 75% of motor nerves, and terminal latency indices were significantly higher in patients than in normal controls (p < 0.05). Conduction block was observed only in 5% of motor nerves. Conclusions: NCS in POEMS syndrome showed characteristic patterns, in which conduction abnormalities were more frequently and severely affected in the lower limbs, and more predominantly in the intermediate nerve segments than in the distal portions. The recognition of these characteristic patterns may be helpful in early diagnosis of polyneuropathy in POEMS syndrome.

수근관증후군 진단을 위한 전기생리학적 척도들의 민감도 비교 (The Comparison of Sensitivities of Electrophysiological Parameters for the Diagnosis of Carpal Tunnel Syndrome)

  • Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy in clinical practice, with a 0.1% life time risk in the general population. Conventional neurophysiological studies have been useful in the diagnosis of this condition, as have a number of more specialized procedures. Therefore, we evaluated the diagnostic sensitivities of several parameters in nerve conduction technique for CTS patients. We analyzed 100 patients (159 hands) who were diagnosed with CTS clinically and electrophysiolosically. Median motor and sensory nerve conduction velocities (MCV and SCV) with wrist, palm, and finger stimulation were performed in traditional methods. Sensitivities of each test were calculated and compared to normal control data. The sensitivities of existing nerve conduction method were noted in terminal latency on median nerve, 2nd finger-wrist segment, 3rd finger-wrist segment, palm-wrist segment and distoproximal ratio, as 72.96%, 92.45%, 94.34%, 94.97%, and 97.48%, respectively. In the early course of CTS, sensory nerve conductions in the median nerve are more valuable than motor nerve conduction. Sensory nerve conductions are usually affected before motor nerve conductions in CTS. In this study, we detected that slowing of median SCV was the most frequent in the distoproximal ratio.

정상 장딴지 신경 반응을 보이는 다발 신경병증 환자에서의 등쪽 장딴지 신경 전도 검사의 임상적 유용성 (Clinical Utility of Dorsal Sural Nerve Conduction Studies in Patients with Polyneuropathy and Normal Sural Response)

  • Background: The most distal sensory fibers of the feet are often affected first in polyneuropathy. However, they are not evaluated in routine nerve conduction studies. Thus we evaluated the dorsal sural sensory nerve in patients with sensorimotor polyneuropathy with normal sural response, in order to assess the usefulness in electrodiagnostic practice. Methods: In this study, 53 healthy subjects and 27 patients with clinical evidence of sensorimotor polyneuropathy were included. In all subjects, peripheral motor and sensory nerve studies were performed on the upper and lower limbs including dorsal sural nerve conduction studies. On electrodiagnostic testing, all patients had normal sural responses. Results: The dorsal sural sensory nerve action potentials (SNAPs) mean amplitude was $13.12{\pm}5.68{\mu}V$, mean latency was $3.12{\pm}0.43msec$, and mean sensory conduction velocity (SCV) was $36.50{\pm}3.40m/s$ in healthy subjects. In 7 of 27 patients, the dorsal sural nerve SNAPs were absent bilaterally, and in 20 patients, the mean dorsal sural nerve distal latency was longer($3.40{\pm}0.48ms$, P=0.006), and mean SCV was slower than in healthy subjects($35.08{\pm}4.59$, P=0.043). However, dorsal sural nerve amplitude was not different between the groups (P=0.072). Conclusions: Our findings suggest that dorsal sural nerve conduction studies should be included in the routine electrodiagnostic evaluation of patients with suspected polyneuropathy and normal sural nerve responses.

신경전도검사의 정상치에 관한 연구 (Study on Normal Nerve Conduction Parameters)

  • Background and Aims : Nerve conduction study is invaluable in clinical neurology, especially for assessing peripheral neuropathies. Abnormal nerve conduction studies may result not only from peripheral nerve dysfunction itself, but also from other various mechanical, technical, and physiological factors such as age, sex, height and temperature. So we conducted this study to establish the our own normal values. Methods : In this study, from March. 1997 to July. 1998, 40 Korean adults among person came to Health Promotion Center over the age of 20 without any suspicion of neurological deficits were analysed to determine the effect of compound effects of several physiological factors. Results : The nerve conduction velocities of the upper extremity and proximal segments were faster than those of the lower extremity and distal segments. Physiological factors such as age, height and temperature affect the results of nerve conduction studies in multiple regression analysis. The sex difference is recognized over peroneal motor nerve. There are no sex differences in amplitude transformed into normal distribution. The significant physiological factor affecting the amplitude of nerve conduction is age, whereas height and temperature play no role. Conclusions : In multiple regression analysis, height is widespread variable for the nerve conduction velocities and temperature is important variable for lower extremities. The parametric statistical analysis cannot be applied to the amplitude of the compound muscle or nerve action potentials because of marked left shift in distribution. Sqareroot transformation of the CMAP and CNAP may be useful in normalizing the distribution. The most significant physiological factor affection the amplitude is age. Sex differences are not seen in nerve conduction study.

Associations of nerve conduction study variables with clinical symptom scores in patients with type 2 diabetes

  • Background: Diabetic peripheral polyneuropathy (DPN) is associated with a variety of symptoms. Nerve conduction studies (NCSs) are considered to be the gold standard of nerve damage assessments, but these studies are often dissociated from the subjective symptoms observed in DPN patients. Thus, the aim of the present study was to investigate the correlations between NCS parameters and neuropathic symptoms quantified using the Michigan Neuropathy Screening Instrument (MNSI). Methods: Patients with type 2 diabetes mellitus (T2DM) with or without symptoms of neuropathy were retrospectively enrolled. Demographic data, clinical laboratory data, MNSI score, and NCS results were collected for analysis; DPN was diagnosed based on the MNSI score (${\geq}3.0$) and abnormal NCS results. Pearson's correlation coefficients were used to evaluate the relationships between MNSI score and NCS variables. Results: The final analyses included 198 patients (115 men and 83 women) with a mean age of $62.6{\pm}12.7$ years and a mean duration of diabetes of $12.7{\pm}8.4$ years. The mean MNSI score was 2.8 (range, 0.0-9.0), and 69 patients (34.8%) were diagnosed with DPN. The MNSI score was positively correlated with the median motor nerve latency and negatively correlated with the median motor, ulnar sensory, peroneal, tibial, and sural nerve conduction velocities (NCVs). When the patients were categorized into quartiles according to MNSI score, peroneal nerve conduction velocity was significantly lower in the second MNSI quartile than in the first MNSI quartile (p = 0.001). A multivariate analysis revealed that the peroneal NCV was independently associated with MNSI score after adjusting for age, sex, and glycosylated hemoglobin A1c (HbA1c) levels. Conclusions: The present results indicate that a decrease in peroneal NCV was responsible for early sensory deficits in T2DM patients.

Nerve length measurement method in a radial motor nerve conduction study

  • Background: Previous studies of radial nerve conduction study (NCS) did not present how to measure the length of the radial nerve across the elbow, and did not even mention how to manage the spiral course of the nerve. This study aimed to applicate the most reliable method to measure the length of the radial nerve during NCS. Methods: Three points (A, B, and C) were determined along the relatively straight course of the radial nerve. The distance was measured using three different methods: L1) straight distance corresponding to the A-C distance, L2) sum of the distances corresponding to the A-B-C distance, L3) based on the L2, but the elbow is flexed at a $45^{\circ}$ angle. We compared the three methods of distance measurement and the calculated nerve conduction velocities (V1, V2, and V3) in normal healthy subjects. Results: 19 normal participants were enrolled. The mean value for method L1, L2 and L3 were $22.5{\pm}1.8cm$, $24.0{\pm}2.1cm$, and $23.2{\pm}2.1cm$ (p < 0.001). Calculated conduction velocities using those distance measurement methods as follows (p < 0.001): V1 ($60.9{\pm}2.7m/s$), V2 ($64.6{\pm}3.3m/s$), and V3 ($63.4{\pm}3.9m/s$). V2 was significantly greater than V1 and V3 (p < 0.001, p = 0.010, respectively). Conclusions: The distance measurement using a stopover point near the lateral epicondyle between two stimulus points in position of a fully extended elbow with forearm pronation is the most appropriate posture for radial motor NCS.

  • Nerve conduction study (NCS) is an electrophysiological tool to assess the overall function of cranial and peripheral nervous system, therefore NCS has been diagnostically helpful in the identification and characterization of disorders involving nerve roots, peripheral nerves, muscle and neuromuscular junction, and are frequently accompanied by a needle Electromyography. Furthermore, NCS could provide valuable quantitative and qualitative results into neuromuscular function. Usually, motor, sensory, or mixed nerve studies can be performed with using NCS, stimulating the nerves with the recording electrodes placed over a distal muscle, a cutaneous sensory nerve, or the entire mixed nerve, respectively. And these findings of motor, sensory, and mixed nerve studies often show different and distinct patterns of specific abnormalities indicating the neuromuscular disorders. The purpose of this special article is to review the neurophysiologic usefulness of NCS, to outline the technical factors associated with the performance of NCS, and to demonstrate characteristic NCS changes in the setting of various neuromuscular conditions.

한국인에서 신장과 다리길이에 따른 하지 신경전도검사속도의 상관관계조사 (Correlation of the Lower Limb Nerve Conduction Velocity with Height and Leg Length)

  • 신경전도검사는 말초신경의 기능을 객관적으로 정량화 할 수 있어 말초신경병의 진단 및 추적관찰에 필수적인 검사로 활용된다. 신경전도검사는 여러 생리학적 요인에 의해 영향을 받으며, 현재 미국 신경근 및 전기진단 의학 협회(American Association of Neuromuscular & Electrodiagnosis Medicine)에서는 신경전도 표준화 사업(Normative Data Task Force, NDTF)을 구성하여 정상치를 제시하고 있으나, 그 수가 불충분하다. 현재 한국인을 대상으로 한 신장과 다리길이에 따른 신경전도속도를 함께 비교 및 상관관계를 조사한 연구가 존재하지 않아 본 연구를 통해 신장과 다리길이에 따른 하지 신경전도속도를 비교하고자 한다. 총 49명의 대상자를 모집하였으며, 키와 다리길이에 따른 운동신경전도검사와 감각신경전도검사를 비교한 결과, 키에 따른 양측 종아리신경 운동신경전도속도 및 좌측 정강신경의 운동신경전도속도 모두 통계적으로 유의한음의 상관관계를 보였으며, 다리길이에 따른 양측 얕은종아리신경 감각신경전도속도 및 양측 장딴지신경의 감각신경전도속도 모두 통계적으로 유의한 음의 상관관계를 보였다. 하지만 NDTF에서는 연령을 나누어 제시되는 것에 반해 이번 연구에서는 모든 대상자가 20대의 성인으로, 다양한 연령의 한국인을 대상으로 추가적인 연구를 통해 키와 다리길이에 따른 신경전도속도를 보다 정확하게 관찰할 수 있을 것으로 예상된다.
