슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향 (The Effects of Active Exercise Program using Sling on the Pain and Balance Following Total Knee Replacement)

  • 본 연구는 슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향을 알아보고자 실시하였다. 대상자는 무릎 관절 전치환술을 받고 입원 중인 환자 20명이며, 수술 후에 CPM (Continuous passive motion) 만을 적용하는 집단 (CPM group; CG)과 CPM과 슬링을 이용한 능동 운동프로그램을 병행하는 집단 (CPM with active exercise program using sling group; CSG)으로 10명씩 무작위로 배치되었다. CG는 CPM을 주 5일, CSG는 CPM을 주 2일, 슬링을 이용한 능동 운동프로그램을 주 3일 실시하였고, 각 중재는 하루 40분씩 총 4주간 실시하였다. 통증은 VAS(Visual analog scale)을 이용하여 평가하였고, 균형은 BT4 (Balance training 4)를 이용하여 C90 area, Trace length, Sway average velocity를 눈을 뜬 상태와 감은 상태로 측정하였다. 그 결과, 두 집단 모두 통증에서 집단 내에서 유의하게 감소하였고, 집단 간의 변화량에서도 유의한 차이가 있었다. 균형에서 CG의 C90을 제외한 모든 변인이 중재 후 유의한 변화가 있었으며, 집단 간 변화량에서는 눈을 감은 상태의 C90과 Vel에서 유의한 차이가 있었다. 따라서 CPM과 슬링을 이용한 능동 운동프로그램을 함께 시행하는 것은 무릎 관절 전치환술 환자의 통증 감소와 균형 향상에 효과적인 중재라고 생각한다.

슬관절 전치환술 후 한방병원에 입원한 환자 20명에 대한 후향적 분석 (Effects of Korean Medicine on 20 Post-TKR Inpatients: A Retrospective Observational Study)

  • This study aimed to evaluate the effectiveness of Korean rehabilitation treatement for patients with post-TKR. The medical records of inpatients undergoing Korean rehabilitation treatment after TKR from 2016.01.01. to 2021.12.31. during the admission, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale(NRS) and range of motion(ROM) of knee. Statistical analysis was done using the IBM SPSS statistics 26 program. If the period of hospitalization was within 7 days or change of NRS and ROM was not recorded properly, the case was excluded. All inpatients received acupuncture treatment, Electro-acupuncture. Soyeom pharmaco-acupuncture were used at a high rate. Blood letting cupping therapy, dry cupping therapy, Interferential Current Therapy(ICT) continuous passive motion(CPM), cryotherapy were used at a high rate. A significant improvement was noted when comparing the NRS and knee ROM results at the time of admission and discharge. Korean medical rehabilitation can be effectively used for patients who have undergone TKR, to relieve pain and enable returning to daily activities. It is expected to be helpful in future studies of post-TKR in Korean Medicine. Also It can be used for medical treatment of post-TKR in Korean Medicine Hospital and clinic. However, further research with a high level of evidence is necessary to support this finding.

무릎 폄근 강화 운동 동안 압력 생체되먹임 장비 적용이 무릎관절 전치환술 환자들의 근력과 균형에 미치는 영향 (Effects of Pressure Biofeedback Unit Application on Muscle Strength and Balance in Total Knee Arthroplasty Patients during Exercise for Strengthening the Knee Extensor Muscle)

  • Purpose : The purpose of this study is to verify the effect of selective muscle strengthening of the knee joint extensor muscles using a pressure biofeedback unit to improve knee extensor strength and the balance ability of total knee replacement patients. Through this, we tried to provide clinical information. Methods : In this study, 12 patients with total knee replacement were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n=6) and a control group (n=6). All patients received 30 minutes of continuous passive motion and leg-strengthening exercises for 15 minutes five times a week for two weeks. Subjects performed knee extension exercises with or without biofeedback units in the sitting position. The knee extensor strength and balance ability were measured before and after exercise. Knee extensor strength was measured by Biodex system 3 and balance ability was measured by Balancia software. Results : Both the experimental group and the control group showed a significant difference in the muscle strength of the knee joint extensor muscles after intervention (p<.05). In comparison, the experimental group showed a significant difference than the control group (p<.05). Both the experimental group and the control group showed a significant difference in the velocity average, path length, area 95 % center of pressure (COP), weight distribution, five times sit to stand test (FTSST) after intervention. In comparison, the experimental group showed a significant difference in velocity average, area 95 % COP, and FTSST than the control group (p<.05). Conclusion : In order to strengthen the knee extensor muscle and improve the balance ability in total knee replacement patients, it is necessary to consider providing pressure biofeedback unit during leg strengthening exercises.
