극소체중아 어머니의 경험 (Experience of mothers with very low birth weight infant)

  • A phenomenological study is conducted to investigate the experience of mothers with very low birth weight. Sample of 8 mothers participate. Seven theme clusters emerge when the formulated meanings were organized into categories. The participants are 8 mothers who gave birth in the general hospital located in Kyunggi-Province and still live there. The data are collected from the intensive interviews on the experience of mothers whose baby is very low birth weight infant. Data collection is through in depth personal interviews two or three times with each mother. On the basis of the meanings of the experience of mothers with very low birth weight infant, the seven essential themes have been obtained Theme 1:Being surprised and perplexed due to the unexpected sudden delivery Theme 2:Experiencing emotional pain such as anxiety, sorrow, fear, confusion and self-blaming caused by a small and feeble baby. Theme 3: Pouring all their time and efforts into the baby under the acute tension Theme 4:Undergoing intolerable difficulties in confronting reality `Theme 5:Raising the special infant into an normal one by themselves Theme 6: Being on the vigilance for their baby even though they can be relieved Theme 7:Finding out a reward for the difficulties they have experienced The results obtained in this study will enable us to comprehensively understand the experience by mothers whose baby is very low birth weight infant and furthermore to provide a new insight which helps baby.

저체중출생아를 위한 가정간호형 모성역할중재 프로그램 개발과 그 효과에 대한 연구 (Development of a Home-based Nursing Intervention, Mothering Program for Low-Birth-Weight Infants)

  • The purpose of this study was to develop a parenting intervention program and determine the efficacy of the program with low-birth weight infants and their mothers. Nine dyads for the experimental group and twelve dyads for the control group discharged from the Neonatal Intensive Care Unit of a University Hospital in Seoul were recruited for the study. For the intervention group, programmed education and support which focused on the maternal sensitivity of the infant's behavior. rearing environment. motherinfant interaction and infant care were given to each subject. Individual counseling and home visits were provided at discharge, one week after discharge. and one and three months of corrected age in every infant. Structured questionaires were administered and feeding interactions were videotaped and coded by a blinded certified observer. A Quasi-experimental design was conducted for this study. Postpartum depression, maternal self esteem. infant care burden, HOME. mother-infant interaction, and infant development were measured. Results were in favor of the intervention versus the control group. On the Beck depression inventory, intervention mothers showed decreasing trends in depressive symptom vs control mothers although, there were statistically no significant differences between the two groups at each time. The mean score of experimental group was 11.55(mild depression state) at discharge and became 8,6(normal state) at 1 month of corrected age. On the other hand, the mean score of the control group was 13.92(mild depression state) at discharge and became 14.0. Maternal self esteem in both groups improved over time. Infant care burden in both groups was also shown to increase over time. There was a significant difference between the two groups in HOME(p=.0340) at 3 months of corrected age. HOME scores of the experimental group and the control's were 31.10 and 25.58, respectively. Mothers' emotional and language responses were significantly high in the intervention group compared with the control group(p=.0155). Intervention group (53.33) showed a significantly high quality of motherinfant interaction compared with the in control group (42.80)(p =.0340). Intervention group mothers appeared have a better quality of mother-infant interaction behaviors. On the other hand, there was no statistical difference in the infant part between groups. Intervention group infants had higher trends in a general developmental quotient: although, there was no statistical difference between groups. The general developmental quotient of intervention infants was 102.56 and control's was 91.28. However, the developmental quotient of the domain of 'individuality-sociality' was higher in the intervention group infants compared with the control's(p=.0155). The concerns identified by parents revealed two domains of an infants' health management -knowledge and skills in caregiving of lowbirthweight-infants, characteristics of lowbirthweight infants, identifying a developmental milestone, coping with emergency situations and relaxation strategies of mothers from the infant care burden. Interview data with the mothers of low-birth weight infants can be used to develop intervention program contents. Limited intervention time and frequency due to time and cost limitations of this study should be modified. The intervention should be continuously implemented when low-birth weight infants become three years old. An NNNS demonstration appeared to be a very effective intervention for the mothers to improve the quality of mother-infant interactions. Therefore intervening in the mothers of low-birth weight infants as early after delivery as possible is desirable. This study has shown that home visit interventions are worthwhile for mothers only beyond the approach as an essential factor in ability of facilitating a growth fostering environment. In conclusion. the intervention program of this study was very effective in enhancing the parenting for the mothers of low-birth weight infants, resulting in health promotion of low-birth weight infants. The home-visit outreach intervention program of this study will contribute to the health delivery system in this country where there is a lack of continuous follow-up programs for low-birth weight infants after discharge from NICU, if it is activated as part of the home visit programs in community health systems.

The Effect of Maternal Pre-pregnancy Body Mass Index on Very Low Birth Weight Infants

  • Purpose: The pre-pregnancy body mass index (BMI) is associated with adverse neonatal outcomes. However, studies on very low birth weight (VLBW) infants are rare. This study aimed to investigate the effect of maternal pre-pregnancy BMI on VLBW infants. Methods: This retrospective study evaluated singleton VLBW infants born at the CHA Gangnam Medical Center from 2006 to 2016. The neonates were classified into three groups according to the maternal pre-pregnancy BMI: underweight (<$18.5kg/m^2$), normal weight (${\geq}18.5$ to <$23kg/m^2$), and overweight or obese (${\geq}23kg/m^2$). Clinical characteristics and morbidities of mothers and infants were analyzed. Results: A total of 181 infants belonging to underweight (16.6%), normal weight (58.6%), and overweight or obese (24.8%) groups were enrolled. The pre-pregnancy BMI had a significant negative correlation with gestational age (r=-0.198, P=0.001) and a significant positive correlation with the z-score of the birth weight (r=0.078, P=0.001) and body length (r=0.067, P=0.008). The number of extremely preterm infants was significantly higher in the overweight or obese group. The proportion of risk of small for gestational age infants was higher in the underweight group (adjusted odds ratio [OR], 2.958; 95% confidence interval [CI], 1.113 to 7.864), whereas that of infants with severe retinopathy of prematurity was higher in the overweight or obese group (adjusted OR, 9.546; 95% CI, 1.230 to 74.109). Conclusion: In our population of VLBW infants, the pre-pregnancy BMI was associated with gestational age, intrauterine growth, and adverse neonatal outcomes. Therefore, proper weight control before pregnancy is important.

  • Objectives A stable follow-up participation rate is a very important factor for validity in a cohort study. This study analyzed the factors that affect the participation rate at one hospital-based birth cohort in South Korean. Methods The participants were recruited from the Mothers' and Children's Environmental Health study between 2006 and 2010. The analysis targeted 1751 mothers who participated in a birth cohort. We conducted analyses of general characteristics during pregnancy and those of infants at birth that affect the participation rate of the 6-month follow-up survey. Results The participation rate for the 6-month follow-up survey was 60.4%. The participation rate in the follow-up of the subsequent period decreased within a 5% to 10% range compared to the number of subjects. The participation rate of premature infants was 16.9% lower than that of a full-term infant (52.6% vs. 69.5%). Analysis showed a 16.7% difference between the participation rate of low-birthweight infants in follow-ups (53.7%) and the participation rate of infants with normal weight (70.4%). The participation rate of mothers who were employed during pregnancy was significantly lower for the 6-month follow-up compared to the participation rate of mothers who were unemployed during pregnancy. Conclusions In this study, factors such as premature birth, low-birthweight, and the employment status of the mother during pregnancy affected the participation rate of the follow-up survey for the birth cohort at six months. A specific strategy is needed to encourage survey participation for the high risk groups in the follow-ups.

Differences in perceived parental stress between parents with very low birth weight infants and nurses in neonatal intensive care units, South Korea

  • Purpose: This descriptive study compared the perceived parental stress levels between parents with very low birth weight infants (VLBWIs) and nurses in the neonatal intensive care unit (NICU). Methods: In total, 83 parents of VLBWIs and 78 NICU nurses were enrolled. Data were collected with the Parental Stress Scale (PSS) and analyzed using the t-test and analysis of variance in SAS version 9.4. Results: The average PSS score was 3.31 among parents and 3.45 among nurses. The stress score was significantly higher among nurses with children (t=2.46, p=.016) and senior nurses (t=2.12, p=.037). There was a significant difference in the stress score according to parents' education (t=3.29, p=.002) and occupation (F=3.14, p=.049) in the sights and sounds subscale. Mothers had significantly higher stress scores than fathers in the parental role alterations subscale (t=2.32, p=.023). Parental stress scores were higher than those perceived by nurses in the infant's appearance and behaviors subscale for breathing patterns (t=2.95, p=.004), followed by jerky/ restless behavior (t=2.70, p=.008). Conclusion: Nurses should provide explanations to parents of VLBWIs in order to reduce parental stress about the appearances and behavior of VLBWIs. This is more important than aspect of the NICU environment and education about parental roles.

산모의 모유를 통하여 감염된 극소 저체중 출생아에서의 거대세포바이러스 감염 (Postnatal cytomegalovirus infection in an extremely premature infant transmitted via breast milk: A case report)

  • 거대세포바이러스(CMV)는 신생아에서 가장 흔한 선천성 감염의 원인 중 하나이며, 모든 정상 출산아 중 약 0.3-2.4%에서 감염되어 있다. 혈청에서 거대세포바이러스 양성인 모체 중 40-96%에서 모유를 통한 바이러스의 배출이 증명되었으며, 거대세포바이러스에 감염된 모유를 통한 감염은 전체 영아 거대세포바이러스 감염의 약 1/3을 차지한다. 본 증례에서는 극소 저체중 출생아에서 모유를 통하여 발생한 거대세포바이러스 감염을 기술하였다. 환아는 생후 7일 경부터 모유 수유를 하였으며, 심한 혈소판감소증, 빈혈, 그리고 패혈증과 유사한 임상 양상에 대하여 반복되는 혈소판 수혈, 적혈구 수혈, 면역 글로불린 요법을 시행받았다. 거대세포바이러스 감염은 생후 2개월 경 혈청 CMV IgM 양성 및 소변 CMV 배양 검사 양성 소견을 통하여 진단하였다. 출생 당시 혈청 CMV IgM 및 소변 CMV 배양 검사가 음성이었으므로 선천성 감염은 배제할 수 있었다. 환아의 혈청과 모유의 핵산 배열 순서 분석을 통하여 동일한 바이러스에 의한 감염임을 증명하였다. 저자들은 국내에서 최초로 nucleotide sequencing 방법을 이용, 모유 수유를 통한 거대세포바이러스 감염이 발생한 극소 저체중 출생아의 예를 보고하는 바이다.

극소 저출생체중아의 영아기 B형 간염 항체 생성률 조사 (Immune responses of hepatitis B vaccination among very low birth weight infant)

  • 목 적 : 의료기술과 신생아학의 발전으로 생존된 극소 저출생 체중아의 질병에 대한 관리가 더욱 필요한 시점이다. 이에 본 연구는 극소 저출생 체중아에서 영아기 B형 간염 항체 생성률과 항체 생성 실패에 미치는 요인에 대하여 알아보고자 하였다. 방 법 : 1997년 1월부터 2004년 12월까지 서울아산병원과 강릉아산병원 신생아 중환자실에 입원하였던 1,500 g 미만의 극소저출생 체중아 중에서 영아기에 B형 간염 항체에 대해 검사받은 243명을 대상으로 하였다. 영아들은 역 연령 약 40주경에 첫 간염 예방접종시행 받았으며 1개월 후, 6개월 후에 간염 예방접종을 시행 받았다. 그러나 모체의 B형 간염 항원이 양성인 13명의 영아들은 출생 시에 임신주수와 출생체중과 관계없이 B형 간염 면역글로불린과 B형 간염 예방접종을 1회 더 시행 받았다. 항체검사는 마지막 간염 예방접종 3-4개월 후에 시행하였고 B형 감염 예방접종후의 항체 양전은 항체의 역가가 ${\geq_-}10mIU/mL$로 정의하였다. 결 과 : 총 243명의 극소 저출생 체중아들의 B형 간염 예방접종 후 항체 양전율은 84.4%(205/243명)였다. 항체가 음성인 38명의 극소 저출생 체중아 중에서 재접종이 가능하였던 28명 중 17명(60.7%)에서 항체가 양성으로 전환되었다. 총 243명 중 34.6%(84/243명)을 차지한 초극소 저출생체중아의 항체 양전율은 84.5%(71/84명)이었다. 13명의 항체 음성인 초극소 저출생체중아 중 재접종이 가능하였던 10명의 항체 양전율은 80%(8/10명)로 재접종 후의 극소 저출생 체중아와 초극소 저출생 체중아의 항체 양전율은 각각 95.3%, 97.5%으로 향상되었다. 출생시 임신주수별 항체 양전율은 $28^{+0}$주 미만과 $28^{+0}-36^{+6}$주인 영아들에서 각각 83.5%(66/79), 84.8%(139/164명)이었다. 이들 중 항체가 생기지 않았던 영아에서 재접종 후 각 군에서의 항체 양전율은 각각 96%, 94.9%로 향상되었다. B형 간염 보유자에게 태어난 극소 저출생 체중아의 항체 양전율은 76.9%(10/13명)였고 모두에게서 B형 간염의 항원은 음성이였다. 항체 양전율에 영향을 미치는 요소로는 첫 B형 간염 예방접종시의 체중이 작을수록 항체 양전율이 저하됨을 알 수 있었다. 결 론 : 세 번의 B형 간염 예방접종 후 항체 생성이 되지 않은 극소 저출생 체중아들에서의 재접종은 항체 양전율을 매우 향상시킬 수 있음으로 저자들은 극소 저출생 체중아들에게 아직 국내에서 보편적으로 실시되고 있지 않는 예방접종 후 항체검사를 적극적으로 권장하는 바이다.

Effect of Prenatal Antibiotic Exposure on Neonatal Outcomes of Preterm Infants

  • 목적: 산모의 산전 항생제 사용은 여러가지 면에서 태아 및 신생아에 영향을 미칠 수 있다. 본 연구에서는 극소 미숙아 또는 극소 저체중출생아에서 산모의 산전 항생제 투여 기간이 신생아 예후에 미치는 영향에 대해 조사하였다. 방법: 2015년 9월부터 2020년 12월까지 고려대학교 안암병원에서 출생한 재태주수 32주 미만 또는 출생 체중 1,500 gram 미만인 신생아 및 산모를 대상으로 의무기록을 후향적으로 분석하였다. 산모의 산전 항생제 투여 기간에 따라 미투여 군, 7일 이하 군, 7일 초과 군의 세 군으로 나누어 산모의 특성, 환아의 특성 및 합병증 등을 비교 분석하였다. 결과: 총 145명 중 93명의 환아가 산전 항생제에 노출되었으며, 그 중 35명(37.6%)는 7일 이하 군, 58명(62.4%)는 7일 초과 군이었다. 7일 초과 군은 미투여 군에 비해 재태 연령에 따른 출생체중의 Z-score가 교란변수 보정 후에도 유의미하게 높았다(beta, 0.258; standard error, 0.149; P<0.001). 다변량 로지스틱 회귀 분석에서 7일 초과 군은 사망(adjusted odds ratio [aOR], 8.926; 95% confidence interval [CI], 1.482-53.775), 그리고 사망, 괴사성 장염, 후기 패혈증의 복합 평가 결과와 연관이 있었다(aOR, 2.375; 95% CI, 1.027-5.492). 결론: 산모의 장기간 산전 항생제 투여는, 극소 미숙아 또는 극소 저체중출생아에서 재태 연령에 따른 출생체중의 Z score을 증가시키며, 사망 뿐 아니라 사망, 괴사성 장염, 후기 패혈증의 복합 평가 결과의 위험도를 증가시킬 수 있다.
