  • Purpose: This study was done to identify the parenting experience of mothers of premature infants in order to provide basic data for educational solutions and desirable directions. Methods: Q-methodology was used as it provides a method of analyzing the subjectivity of each item. The participants were 33 mothers of premature infants who sorted 34 selected Q-statements which were then classified into the shape of a normal distribution using a 9-point scale. Subjectivity on parenting experience among the mothers was analyzed using the pc-QUANAL program. Results: Four types of parenting experience were identified. Type I was named 'struggling', type II, 'self blame', type III, 'information collecting', and type IV, 'self-introspection'. Conclusion: The results of this study indicate that different approaches to educational programs are needed for mothers of premature infants based on the four types of parenting experience.

  • Premature rupture of membrane is the most frequent cause of low birth weight infant delivery which increase the maternal and fetal morbidity and perinatal mortality. A retrospective case-control study was performed on 315 mothers who delivered low birth weight infants($\leq$2.5kg) with premature rupture of membrane and as control group 546 mothers who delivered normal birth weight infants(2.9-3.7kg) without premature rupture of membrane were chosen. The results obtained from this study were as follows: 1. The proportion of low birth weight infants due to premature rupture of membrane among all low birth weight infant deliveries was 14.5%, and this is equivalent to 1.1% among all deliveries. 2. The most significant maternal risk factor of low birth weight infant deliveries with premature rupture of membrane was infections on vagina, cervix and uterus during pregnancy. Compared with control, adjusted odds ratio was 7.61(95% confidence interval(CI) 1.88-30.88, p=0.004). Other significant maternal risk factors were the history of induced abortion, spontaneous abortion, and the experience of premature delivery. The risk ratios were 1.82, 2.07, 4.42, respectively. 3. Breech presentation did increase the risk of low birth weight infant delivery with premature rupture of membrane compared with control(Adjusted Odds ratio=2.66, 95% CI 1.35-5.26, p=0.005). 4. Mothers who had not taken antenatal care were having higher risk of low birth weight infant delivery with premature rupture of membrane against control(Adjusted odds ratio=1.73, 95% CI 1.19-2.53, p=0.004). These study results show that maternal factors such as the infection of genital organs during pregnancy, the history of induced abortion and breech presentation are significantly associated with the premature rupture of membrane in the low birth weight deliveries, and that most of these risk factors are controllable ones through proper antenatal cares.

  • Purpose: This research was conducted to evaluate the effects of asystemic follow-up care program on health promotion and risk reduction in 64 high-risk infants(HRI) including premature infants and their mothers. Method: The intervention consisted of systemic NICU education, tele-counseling and 3 home visits in 6 months. The subjects were divided into either the intervention group or the control group receiving the conventional NICU education without the tele-counseling and home visiting. Infant health promotion was measured using physical assessment, types of health problems, reflexes, OPD visiting history, DDST, immunization, feeding assessment, Infant death rate, etc. Maternal self-esteem, postpartum depression and family function were measured using the maternal self-report inventory(MRI), EPDS, and family apgar score(Fapgar), retrospectively. Result: All premature infants in the intervention group were in the normal range of growth and development, and the regular vaccination schedule. The health problems in the intervention group were addressed early so not to develop into adverse effects. The follow-up program for 6 months showed beneficial effects on MRI, EPDS, and Fapgar. Conclusion: A systemic follow-up health care program is beneficial on health promotion and risk reduction in 64 HRI including premature infants and their mothers.

  • Purpose: The purpose of this study was to investigate the effects on weight of premature infants when Yakson therapy was conducted by mothers and also to measure role confidence and role satisfaction of the mothers. Method: A non-equivalent control group non-synchronized design was used. The data were collected from January to July, 2005. Thirty preform infants and mothers were assigned to either an experimental (15) or a control group (15). Yakson therapy consists of three phases: laying on of hands, caressing by hands, and laying on of hands again, each phase taking 5 minutes. Yakson therapy was provided to infants in the experimental group for 15-minutes 5 times a day far 15 days. Infant weight, mothers' role confidence and satisfaction with role were measured for both groups. Results: Compared to the control group, weight gain of infants in the experimental group (t=2.05, p=.049) was significantly increased after 15 days. The experimental group also had significantly higher scores in mothers'.ole confidence (t=1.98, p=.038). Conclusion: The results suggest that Yakson therapy can be used to increase weight gain in preform infants and improve the role confidence of mothers.

  • Purpose: The purpose of this research is to provide basic informations for the encouragements of premature infants' breast feeding. Method: From August 10 to October 9, 2002, we have carried out a statical research which surveyed 148 mothers of premature infants registered in NICU. The sample had generated cluster-randomly from 25 General Hospitals, all over the Korea peninsula and being surveyed with 74 questionnaires. Result: Mean hospitalized day of premature infants was 27.9 days. The mean total feeding period was 19.1 days and continuous breast feeding period 12.4days. They were interested in breast feeding education-they answered that they would join the breast feeding education if they were given the chance 87.8%. There was significant relation between babies fed only breast milk and the body weight of birth(p<.05). The reasons why mother gave the baby her breast milk include 'for her baby's health' and 'people said breast milk is good for babies' with a portion of 99%. The main reason why mothers could not execute breast feeding was 'the deficit of breast milk volume' 50.0% and other reason were 'because of start to support more nutritions '18.2%, 'difficulty to carry out the breast milk to hospital'13.6%. The reasen why mothers could not try breast feeding at first were 'deficit of breast milk volume'37.0%, 'not to be prepared for breast feeding because of unexpected delivery'32.6%. Conclusion: We need a program to inform importance and excellent of the breast feeding and a plan to increase the premature infants' breast feeding through the importation of fortifier.

  • Purpose: This quasi-experimental study was conducted to investigate the effects of supportive nursing management on postpartum depression in the mothers with premature infants. Methods: The subjects were 21 mothers who delivered premature babies in a university hospital. The experimental group of 10 mothers was provided with supportive nursing management program by nurses in neonatal ICU and the control group of 11 mothers was provided with usual management only. The designed programs were given 4 times to the experimental group while their babies were hospitalized, and telephone consultation was provided 3 times after discharge. The stress, anxiety, identity, support from their husbands & family members, and postpartum depression were measured 3 times using Edinburgh Postnatal Depression Scale (the 4th day of premature's hospitalization, the day of discharge and the day of 4 weeks after discharge). Results: There was no significant difference in general characteristics and the influential factors of postpartum depression between the two groups, so they were homogeneous. There was no significant difference in depression (F=0.01, p=.917). However there was significant difference over time (F=6.74, p=.003) and the interaction between measurement time and treatment (F=3.59, p=.037). Conclusion: The supportive nursing management on postpartum depression of mothers with premature infants is considered effective and useful in reducing postpartum depression. Further research is warranted to investigate paternal depression and the program's long-term effects.

  • Purpose: The research was conducted to investigate the experience of maternal role attainment of mothers of premature infants admitted in NICU and to conceptualize the phenomena. Method: The grounded theory method was utilized for data collection and analysis. 8 mothers of premature infants were selected and in-depth interview was performed. Paradigm model was utilized for data analysis and presentation. Result: The central category was 'unstable maternal identity'. The properties of the core phenomena was 'ambivalent feeling to baby' 'negative emotion' 'commitment to baby'. The loss of control due to premature delivery was the causal condition. contextual condition was the 'perceived threats' due to severity of the premature infant and uncerainty of the baby's life. The mother's health status, economic status, and familial and social support was recognized as intervening conditions during the process of maternal role attainment. The strategic action/interactions were emotion-focused coping, reappraisal of the situation, problem-focused coping, and information seeking. The consequence was the maternal role attainment with competence and expectation. Conclusion: The process of maternal role attainment was affected by threats due to severity of the baby and intervening factors and interaction strategy. Further research is recommended to develop adequate intervention method during the process of maternal role attainment.

  • Purpose: This study was done to investigate the effects of a massage program on the growth of premature infants and on the mothers' confidence and satisfaction in the mothering role. Methods: The participants were 29 mothers of premature infants hospitalized in a neonatal intensive care unit at CNU Hospital (14 in the massage program group and 15 in control group). Massage was done for 10-15 minutes, 2-3 times a day for 4 weeks. The data were using the SAS PC+ program. Result: Significant differences were found in the ratios of weight gain (Z=2.24, p=.013), grow in length (Z=2.50, p=.006) and increase in head circumference (Z=1.91, p=.028) between the massage program group and the control group. Confidence in the mothering role was significantly higher for mothers in the massage program group compared to those in the control group (Z=2.69, p=.004), but there was no significant difference in satisfaction with the mothering role between the two groups (Z=.88, p=.191). Conclusion: These results suggest that the massage program enhances growth of premature infants as well as enhancing the mother's confidence in her role as mother. Therefore, the massage program for premature infants can be use as an effective nursing intervention.
