사랑니 발치환자에서 불안에 관한 연구 (THE ANXIETY IN PATIENTS UNDERGOING THIRD MOLAR EXTRACTION)

  • The purpose of this study is to understand anxiety in patients undergoing third molar extraction and care patients of third molar extraction. The subjects in the study were Sixty-three patients who visited the department of dentistry at Pusan Paik Hospital from May 2006 to September 2006 and who underwent third molar extraction. Patients' anxiety(state anxiety and trait anxiety) to use Spielberger's state-trait anxiety inventory(STAI) were measured. They were asked to fill out STAI question paper immediately before third molar extraction(pre-extraction) and on the day after the extraction(post-extraction). And visual analog scale(VAS) to measure patient's postoperative pain on the day after extraction(postextraction day) was used. Then Differences in anxiety before and after extraction, between men and women, between the first and second extraction, with impacted versus non impacted third molar extraction, between extraction time and anxiety, and between postoperative pain and anxiety were evaluated. The anxiety status of patients undergoing third molar extraction could be quantitatively evaluated using the STAI-KYZ. There were significant difference before and after third molar extraction. Especially women and patients of more severe postoperative pain was more anxious. We need to alleviate patient's anxiety(especially women) and to control postoperative pain throughout the tooth removal process.

Radiographic evaluation of the course and visibility of the mandibular canal

  • Purpose: This study was performed to investigate the course of the mandibular canal on panoramic radiography and the visibility of this canal on both panoramic radiography and cone-beam computed tomography (CBCT). Materials and Methods: The study consisted of panoramic radiographs and CBCT images from 262 patients. The course of the mandibular canal, as seen in panoramic radiographs, was classified into four types: linear, elliptical, spoon-shaped, and turning curves. The visibility of this canal from the first to the third molar region was evaluated by visually determining whether the mandibular canal was clearly visible, probably visible, or invisible. The visibihlity of the canal on panoramic radiographs was compared with that on CBCT images. Results: Elliptical curves were most frequently observed along the course of the mandibular canal. The percentage of clearly visible mandibular canals was the highest among the spoon-shaped curves and the lowest among the linear curves. On panoramic radiographs, invisible mandibular canals were found in 22.7% of the examined sites in the first molar region, 11.8% in the second molar region, and 1.3% in the third molar region. On CBCT cross-sectional images, the mandibular canal was invisible in 8.2% of the examined sites in the first molar region, 5.7% in the second molar region, and 0.2% in the third molar region. Conclusion: The visibility of this canal was lower in the first molar region than in the third molar region. The mandibular canal presented better visibility on CBCT images than on panoramic radiographs.

  • In clinical prosthetic dentistry the occlusal planes were represented to the dental arch form sagittally and composed of the occlusion morphologically. From now on the determinations of the occlusal planes were done through the facebow transfer and the cephalometric radiography, but they were not exact method for the diagnosis of the stomagnathic function. The purpose of this study is to examine the sagittal characteristics of occlusal planes using panoramic view between the normal subjects and the subjects with stomatognathic dysfunction. 60 normal subjects and 16 subjects with stomatognathic dysfunction were slectec for this investigation. We measured the inclination of occlusal planes to Frankfurt Horizontal plane. The occlusal planes were divided into 4 groups : 1) The plane between the midpoint of upper central incisor and the mesiobuccal cusp of upper 1st molar, 2) The plane between the midpoint of upper central incisor and the mesiobuccal cusp of upper 2nd molar, 3) The plane between the midpoint of upper central incisor and the distobuccal cusp of upper 2nd molar, 4) The plane between the mesiobuccal cusp of 1st molar and the distobuccal cusp of 2nd molar. The results were as follows : 1) The occlusal planes could be determined very easy by using panoramic view, 2) Among the angles between the occlusal planes and the FH plane, the angles related with the 2nd molar were more steep than those of the 1st molar. The angle between the 1st molar and the 2nd molar is the most steep in the other angles. 3) In the angles between occlusal planes and FH plane, the subjects with stomatognathic dysfunction were more significant steep(p<0.05) than the normal subjects.

기성금관수복 유구치의 변연 적합도에 관한 연구 (A STUDY ON MARGINAL ADAPTATION OF READY-MADE STAINLESS STEEL CROWN TO THE PRIMARY MOLAR)

  • In Class II amalgam restoration in deciduous molar, failure rate and incidence of recurrent caries are high as children become older. In order to preserve deciduous molars till the physiologic exfoliation time, stainless steel crown is a choice of the treatment. As a result of a careless treatment, such as overhanging margin, poor marginal adaptation, poor proximal contour and inadequate mesiodistal width give rise to interfering eruption of the adjacent teeth, recurrent caries and chronic gingival irritation and insufficient arch length respectively. In this study, 252 s.s. crowned teeth extracted due to physiologic exfoliation or periapical lesion. The purpose of this study is to analyze the marginal adaptation of stainless steel crown to the deciduous molar in order to obtain better clinical result. The results were as follows : 1. Between the length of s.s. crown and the marginal gap of crown, positive correlations were shown. 2. Largest amount of marginal gap was shown at buccal side in upper deciduous molars and lower first deciduous molar, lingual side in lower second deciduous molar. But no significant diffrence were found statistically compared to second most largest one. 3. Incidence of exposed restoration and recurrent caries were higher in proximal surface than buccal/lingual surface. And extension of restoration below the margin of s.s. crown gives rise to higher rate of recurrent caries. 4. Defect of contour was found in 34%, frequently found in lower 1st deciduous molar and upper 1st deciduous molar. 5. Marginal polishing defects were found in 23%. 6. Ledge was formed in 10% especially in lower 1st deciduous molar and lower 2nd deciduous molar. 7. 16% of the teeth had wear facet due to traumatic occlusion, 7% of them had occlusal perforation.

제1대구치의 맹출 연령에 관한 분석 연구 (Study on the Eruption Age of the First Molar)

  • This study was undertaken to obtain the data age determination following the eruption of individual cusps of the first molars in the point of forensic odontology. 532 children (294 male and 238 female, born between April, 1989 and March, 1986) from a kindergarten and a primary school in a reesidential district in Seoul were studied. The eruption state of the first molar was divides into 6 stages according to the degree of exposure of individual cusps, and correlation between the stage and age was stastically analysed. The results of the study lead to following conclusions : 1. The most frequently seen eruption sequence of cusps for the upper first molar was mesibuccal cusp, mesiopalatal cusp, distobuccal cusp, mesiodistal cusp, distopalatal cusp. As for the lower first molar : mesiobuccal cusp, mcsiolingual cusp, distobuccal cusp, distolingual cusp. 2. The time of eruption was earlier in the female ethan in the male, but it was stastically remarkable only in upper and lower parts of the right and the left teeth of subjects age between 6.0 and 6.5 and upper part of the right and the left teeth of subjects age over 7.5 3. The eruption of the lower first molar was comparatively earlier than that of the upper first molar and there was no significant stastical difference between the right and the left first molars in the time of eruption. 4. There was no noticeable difference in the eruption sequence of cusps, irrespective of sex and side. 5. The eruption of the upper first molar is started at the age of about 6.4 and complete the age of about 7.1 and as for the lower first molar, it is from 6.3 to 7.0

Relationship between mandibular condyle and angle fractures and the presence of mandibular third molars

  • Objectives: We retrospectively evaluated the impact of mandibular third molars on the occurrence of angle and condyle fractures. Materials and Methods: This was a retrospective investigation using patient records and radiographs. The sample set consisted of 440 patients with mandibular fractures. Eruption space, depth and angulation of the third molar were measured. Results: Of the 144 angle fracture patients, 130 patients had third molars and 14 patients did not. The ratio of angle fractures when a third molar was present (1.26 : 1) was greater than when no third molar was present (0.19 : 1; odds ratio, 6.58; P<0.001). Of the 141 condyle fractures patients, the third molar was present in 84 patients and absent in 57 patients. The ratio of condyle fractures when a third molar was present (0.56 : 1) was lower than when no third molar was present (1.90 : 1; odds ratio, 0.30; P<0.001). Conclusion: The increased ratio of angle fractures with third molars and the ratio of condyle fractures without a third molar were statistically significant. The occurrence of angle and condyle fractures was more affected by the continuity of the cortical bone at the angle than by the depth of a third molar. These results demonstrate that a third molar can be a determining factor in angle and condyle fractures.

Cone Beam형 전산화단층영상을 이용한 상악대구치 발치 후 상악동 함기화 평가 (Maxillary sinus pneumatization after maxillary molar extraction assessed with cone beam computed tomography)

  • Purpose : The purpose of this study was to examine the inferior expansion of the maxillary sinus floor following maxillary molar extraction. Materials and Methods : Cone beam computed tomographic images of 59 subjects were used to evaluate the height difference of the maxillary sinus floor between extraction sites and contralateral dentate sites. The height of the maxillary sinus floor was defined as the vertical distance to the Frankfort plane from the level of the anterior nasal spine to the most inferior point of the sinus floor. We examined the difference in sinus pneumatization according to the number of missing teeth and the vertical relationship of the molar roots to the sinus floor. Results : The inferior expansion of the maxillary sinus floor was $1.20{\pm}1.86\;mm$ on the maxillary first molar and $1.90{\pm}2.42\;mm$ on the maxillary second molar. Increased expansion was observed in cases where two proximate molars were extracted. There was no significant difference in sinus pneumatization following extraction according to the vertical relationship of the molar roots to the sinus floor. Conclusion : The results of this study confirm that sinus pneumatization occurs following maxillary molar extraction. In situations where pneumatization can affect treatment after molar extraction, three-dimensional radiography should be considered.

  • Molar-incisor malformation (MIM)은 치관은 정상이나 치근의 비정상적인 형태를 보이는 것이 특징적이다. 제1대구치에 가장 많이 발생하며, 제2유구치나 상악 중절치도 때때로 이환된다. 이 증례 보고에서는 제1대구치를 포함해, 제2유구치에 MIM이 나타난 환자의 증례를 보고하고, 치근이 건전하게 발거된 제2유구치의 micro computed tomography (CT)를 이용한 형태학적 분석을 시행하였다. 발거된 치아에서는 MIM의 외형적 특징인 치관 - 치근 이행부의 cervical constriction을 육안상으로 확인할 수 있었으며, micro CT 분석에서 백악법랑경계부의 광화된 부위 (Cervical Minelarized Diaphragm), 많은 부근관과 막힌 근관 등의 복잡한 근관 형태, 근관 내 석회화 물질, 영구치에 비해 상대적으로 협착된 치관부 치수강이 관찰되었다.

Immobilization of sodium-salt wastes containing simulated 137Cs by volcanic ash-based ceramics with different Si/Al molar ratios

  • In this study, volcanic ash was used as raw material to prepare waste forms with different silicon/aluminum (Si/Al) molar ratios to immobilize sodium-salt waste (SSW) containing simulated 137Cs. Effects of Si/Al molar ratios (3:1 and 2:1) and sodium salts on sintering behavior of waste forms and immobilization mechanism of Cs+ were investigated. Results indicated that the main mineral phase of sintered waste-form matrixes was albite, and the formation of major phases was found to depend on Si/Al molar ratios. Si/Al molar ratio of 2 was favorable for the formation of pollucite, and the formation and crystallization of mineral phases were also decided based on physicochemical characteristics of sodium salts. Furthermore, product consistency test results indicated that the immobilization of Cs+ was related to Si/Al molar ratio, types of sodium salts, and glassy phase. Waste forms with Si/Al molar ratio of 2 exhibited better ability to immobilize Cs+, whereas the influence of sodium salts and glassy phases on the immobilization of SSW showed more complicated relationship. In waste forms with Si/Al molar ratio of 2, Cs+ leaching concentrations of samples containing Na2B4O7·10H2O and NaOH were low. Na2B4O7·10H2O easily transformed into liquid phase during sintering to consequently achieve low temperature liquid-phase sintering, which is beneficial to avoid the volatilization of Cs+ at high temperature. Results clearly reveal that waste forms with Si/Al molar ratio of 2 and containing Na2B4O7·10H2O show excellent immobilization of Cs+.

A cone-beam computed tomography study of the prevalence and location of the second mesiobuccal root canal in maxillary molars

  • Objectives: This study aimed to investigate the incidence and location of the second mesiobuccal root (MB2) canal in maxillary molars with the aid of various measuring points and lines using cone-beam computed tomography (CT). Materials and Methods: A total of 205 images of patients who underwent cone-beam CT examinations between 2011 and 2015 as part of their dental diagnosis and treatment were included. There were 76 images of the maxillary first molar and 135 images of the maxillary second molar. Canal orifices were detected at -1 mm from the top of the pulpal floor on cone-beam CT images. Image assessment was performed by 2 observers in reformatted image planes using software. Assessments included measurement of the distance between the MB1 and MB2 canals, and the angles between the lines connecting the MB1-MB2 and distobuccal (DB)-palatal (P) canals. The data were analyzed using the student's t-test. Results: The prevalence of the MB2 canal was 86.8% in the first molar and 28.9% in the second molar. The angle between the lines connecting the MB1-MB2 and DB-P canals was 2.3° ± 5.7° in the first molar and -3.95° ± 7.73° in the second molar. The distance between the MB1 and MB2 canals was 2.1 ± 0.44 mm in the first molar and 1.98 ± 0.42 mm in the second molar. Conclusions: The angles between the lines connecting the MB1-MB2 and DB-P canals was almost parallel. These findings may aid in the prediction of the location of the MB2 canal orifice.
