A Hidden Key to COVID-19 Management in Korea: Public Health Doctors

  • Public health doctors of Korea contributed significantly to massive coronavirus disease 2019 (COVID-19) testing. They were immediately dispatched to epicenters of the COVID-19 pandemic, and have run tests at screening centers, airport quarantines and hospitals. However, their expertise from in-field experience has been often neglected. It is time to reorganize public health doctor system to better prepare for future epidemics. Transforming and strengthening their roles as public health experts through systematic training is crucial.

Clinically Correlated Anatomical Basis of Cricothyrotomy and Tracheostomy

  • Objective : Cricothyrotomy and tracheostomy are performed by physicians in various disciplines. It is important to know the comprehensive anatomy of the laryngotracheal region. Hemorrhage, esophageal injury, recurrent laryngeal nerve injury, pneumothorax, hemothorax, false passage of the tube and tracheal stenosis after decannulation are well known complications of the cricothyrotomy and tracheostomy. Cricothyrotomy and tracheostomy should be performed without complications and as quickly as possible with regards the patients' clinical condition. Methods : A total of 40 cadaver necks were dissected in this study. The trachea and larynx and the relationship between the trachea and larynx and the surrounding structures was investigated. The tracheal cartilages and annular ligaments were counted and the relationship between tracheal cartilages and the thyroid gland and vascular structures was investigated. We performed cricothyrotomy and tracheostomy in eleven cadavers while simulating intensive care unit conditions to determine the duration of those procedures. Results : There were 11 tracheal cartilages and 10 annular ligaments between the cricoid cartilage and sternal notch. The average length of trachea between the cricoid cartilage and the suprasternal notch was 6.9 to 8.2 cm. The cricothyroid muscle and cricothyroid ligament were observed and dissected and no vital anatomic structure detected. The average length and width of the cricothyroid ligament was 8 to 12 mm and 8 to 10 mm, respectively. There was a statistically significant difference between the surgical time required for cricothyrotomy and tracheostomy (p < 0.0001). Conclusion : Tracheostomy and cricothyrotomy have a low complication rate if the person performing the procedure has thorough knowledge of the neck anatomy. The choice of tracheostomy or cricothyrotomy to establish an airway depends on the patients' clinical condition, for instance; cricothyrotomy should be preferred in patients with cervicothoracal injury or dislocation who suffer from respiratory dysfunction. Furthermore; if a patient is under risk of hypoxia or anoxia due to a difficult airway, cricothyrotomy should be preferred rather than tracheostomy.

성폭력행위자의 수강명령프로그램 효과 (Effect of the course order program of sexual violence offenders)

  • 본 연구는 성폭력행위자가 수강명령프로그램에 참여한 교육의 효과성을 분석하는데 목적이 있다. 이를 위해 연구대상은 법무부에서 시행하는 성폭력수강명령프로그램 40시간을 경험한 남성으로서, D시에 거주하는 성폭력행위자를 중심으로 연구하였다. 자료 수집은 본 연구자가 성폭력 수강명령프로그램을 직접 운영하는 강사 자격으로, 교육 전 사전 검사를 실시하였고 교육 종료 후 사후검사를 실시하였다. 성폭력 행위자를 위한 수강명령프로그램이 성폭력행위자의 공격성, 충동성, 강간통념, 고독감, 분노에 미치는 영향을 살펴본 결과 공격성, 충동성, 강간통념, 분노에서 유의한 차이를 보였으며, 고독감에는 유의한 차이를 보이지 않았다. 본 연구의 의의는 성폭력행위자들을 대상으로 법무부 성폭력 프로그램의 사전, 사후 검사를 통해 교육을 효과를 분석하였으며 그에 따른 행동변화를 위한 방법들을 모색하게 되었다고 보고하였다. 따라서 본 연구는 성폭력행위자 수강명령프로그램이 성폭력 재발방지에 효과적이라는 결론이 도출되었으며, 차후 수강명령프로그램 개발에 활용할 수 있는 자료로 기여할 수 있을 것이다.

A Study on the Reform of the Police in Korea: Focused on Institutional Approach

  • As part of the police reform, the current government is trying to establish a police bureau under the Ministry of Public Administration and Security and improve the personnel management of the police. Like the prosecution bureau under the Ministry of Justice, the main goal of the police is to establish a police support bureau in the Ministry of Public Administration and Security to have personnel, budget, disciplinary action, and inspection. However, the government's control of police rights was opposed mainly by the "Police Workplace Council," which is like a police union, and even police officials agreed with it, and the opposition was severe. It is becoming a form of struggle between the government and police organizations. As a police major, I don't want to side with anyone and support the police. However, I thought about what democratic control of the police should be with, considered what the police reform plan is, and analyzed the legal and institutional aspects of the current police, and expressed the following issues. First, this paper is as follows: First, we considered what the democratic control of the police was. Second, We focused on reform measures such as the personnel system and institutional approach. Third, I presented my argument on what the police reform plan is.

교정시설에 수용된 소년범의 교정 처우에서 '용서하기'의 가능성에 대한 고찰 (The Possibility of Forgiving Among Serious Juvenile Offenders in Correctional Facilities)

  • 본 연구는 현행 교정 처우의 보완적 방안으로서 소년 교정시설 내에서의 '용서하기'의 가능성을 고찰하는 데 그 목적이 있다. 문헌 고찰을 통해 지난 30여 년간 심리학자들을 중심으로 논의되어 온 용서의 개념과 용서 프로그램이 기반하고 있는 주요 용서 모델을 개관하였다. 그리고 최근 형사사법에서 주목받고 있는 회복적 사법에서 통용되는 용서와의 차이점을 살펴보았다. 또한, 교정시설에 수용된 소수의 고위험군 소년범은 상대적으로 심각성과 위험도가 낮은 소년범보다 아동기 부정적 생애경험을 비롯한 피해 사건을 더 많이 겪는다고 보고한 선행연구와 다양한 배경에서 시행된 용서 프로그램 결과를 토대로 소년 교정 현장에서의 '용서하기'의 가능성을 논증하였다. 이러한 논의 결과를 바탕으로 가해자인 소년범들도 이전에 경험한 피해 사실의 범주 내에서 제한적으로 '용서하기'가 가능하며, '용서하기'를 통한 심리적 어려움의 경감과 타인에 대한 부정적 태도의 개선이 교정 처우의 효과 제고에 기여할 수 있다는 점이 결론으로 제시되었다. 마지막에서는 본 연구의 제한점과 후속 연구에 대해 논의하였다.

효과적 검역체계를 위한 검역법 개선방안 (A Study on the Improvement of Quarantine Act for Effective Quarantine System)

  • The development of transport is being easily shared with people all over the world. It is necessary to appropriately and effectively revise the domestic quarantine law because the fatal infectious diseases are at risk of being easily shared. Today, Korea has an advanced quarantine system approved by World Health Organization, but it maintains partnerships with related ministries (Ministry of Foreign Affairs, Ministry of Justice, local medical institutions) and to introduce new medical technology (electronic quarantine) is important. And since the prevention of quarantine infectious diseases and prevention of the spread, in order to maintain international cooperation with the International Health Regulations, the quarantine law and the system should be amended and improved effectively and it is also a way to prepare for the outbreak of new quarantine infectious diseases. In the past, Korea has experienced great confusion during the past outbreak of swine flu and Middle East respiratory syndrome coronavirus. To prevent similar cases from recurring in the past, the revision of the quarantine law and the improvement of the system should be done to cope with the changing environment (new infections, increased number of overseas travelers, etc.).

Control of International Cyber Crime

  • The followings are required to establish uniform principle of criminal jurisdiction for international cyber crime into customary international law; (1) clear guideline of UN for promoting national practice (2) formation of general practices based on these guidelines (3) these general practices should obtain legal confidence. International society is in close cooperation for investigating and controlling cyber threat. The US FBI has closed down the largest online crime space called 'Darkcode' and prosecuted related hackers based on joint investigation with 19 countries including England, Australia, Canada, Bosnia, Croatia, Israel, and Rumania. More and more people in Korea are raising their voices for joining cyber crime treaty, 'Budapest Treaty.' Budapest Treaty is the first international treaty prosecuting cyber crime by setting out detailed regulations on internet criminal act. Member countries have installed hotline for cyber crime and they act together. Except European countries, America, Canada, and Japan have joined the treaty. In case of Korea, from few years before, it is reviewing joining with Ministry of Foreign affairs, Ministry of Justice and the National Police but haven't made any conclusion. Different from offline crime, cyber crime is planned in advance and happens regardless of border. Therefore, international cooperation based on position of punishing criminals and international standards. Joining of Budapest international cyber crime treaty shall be done as soon as possible for enhancing national competence.

일본 정치권의 독도인식과 우리의 대응전략 (Recognition of Japan politics about Dokdo and our strategy)

  • Last December, the Abe government came back, and it is critical core of Northeast Asia. They visited to Yasukuni Shrine, denied to Korea's invasion and they are denying all of their invasion history. I'm afraid they want to take Dokdo. Dokdo is Korean territory, but Japan politics assert it belongs to them. To make matters worse, they are waiting an opportunity to invade. Ministry of Foreign Affair blue paper and Ministry of Defense white paper have claimed Dokdo as Japanese territory, and many right wing politicians are taking part in the Cabinet. Liberal Democratic Party of Japan is becoming more right wing politicians than before by Japan Restoration Party, and the others also have more right wing ideologies. It can't control Japan right wing political parties. They finally aim to take Dokdo. In this situation, we have to defend Dokdo. Japan must be very important partner for our nation's development. But it is necessary to trust between two countries. Dokdo is effective controlled by Korea. It is the best way how to keep Dokdo. During Dokdo is effective controlled by Korea, the Japanese Government has limited Dokdo's ownership. Now we don't have any way to keep Dokdo except more effective control. We have strategies about Japanese claim of Dokdo's ownership as follows. First, we can overpower Japan right wing politics as Japan conscientious force's ideology. Second, Japan politics say to Dokdo's ownership is based on The San Francisco Treaty. But it is not right. Third, we have to exchange a lot of local government and civic society in Japan. Finally, we must prepare thoroughly to bring the matter to the International Court of Justice.

성도착증 혹은 성적 가해자의 인지행동치료 (Cognitive Behavioral Therapy of Paraphilia or Sexual Offenders)

  • Non-pharmacological treatments have long been used for paraphilia or sexual offenders, but few clinical studies were conducted. However, recently there were issued cognitive behavioral therapy of paraphilia or sexual offenders based on the research findings obtained so far. In addition, there were the changes of Diagnostic and Statistical Manual of Mental Disorder by American Psychiatric Association at 2014. The methods of cognitive behavioral therapy for paraphilia or sexual offenders were electric aversion therapy, olfactory aversion therapy, ammonia aversion therapy, covert sensitization, masturbatory reconditioning, directed masturbation, verbal satiation, social skill training, assertiveness training etc. By synthesizing the newly reported foreign guidelines for treatment and review articles, the aim of this study is to investigate the non-pharmacologic therapies used for treatment of paraphilia or sexual offenders.

자폐증 청소년에서 체중감소와 연관된 반추적 구토의 정신과적 치료 (Psychiatric Treatment of Ruminative Vomiting and the Associated Weight Loss in an Adolescent Boy with Autism : A Case Report)

  • We treated the persistent ruminative vomiting of a 13-year-old autistic boy by using a behavioral and psychopharmacological approach in a closed ward. Before the interventions, the patient had lost about 10kg of body weight due to very high-frequency ruminative vomiting. Together with psychopharmacological trials, the psychiatric treatment included a combination of a behavioral approach and food regulation that emphasized food restrictions, and we provided regular light meals. There was a considerable reduction of the ruminative vomiting and 2kg weight gain was achieved during the admission. These therapeutic gains were maintained at the 4-month follow-up assessment and the vomiting symptom reached a near-zero level and the weight loss were recovered by about 8kg. The issues related to the treatment approaches for ruminative vomiting with autism are discussed.
