Does minimally invasive canal preparation provide higher fracture resistance of endodontically treated teeth? A systematic review of in vitro studies

  • This systematic review aimed to investigate whether minimally invasive root canal preparation ensures higher fracture resistance compared to conventional root canal preparation in endodontically treated teeth (ETT). A comprehensive search strategy was conducted on the "PubMed, Web of Science, and Scopus" databases, alongside reference and hand searches, with language restrictions applied. Two independent reviews selected pertinent laboratory studies that explored the effect of minimally invasive root canal preparation on fracture resistance, in comparison to larger preparation counterparts. The quality of the studies was assessed, and the risk of bias was categorized as low, moderate, or high. The electronic search yielded a total of 1,767 articles. After applying eligibility criteria, 8 studies were included. Given the low methodological quality of these studies and the large variability of fracture resistance values, the impact of reduced apical size and/or taper on the fracture resistance of the ETT can be considered uncertain. This systematic review could not reveal sufficient evidence regarding the effect of minimally invasive preparation on increasing fracture resistance of ETT, primarily due to the inherent limitations of the studies and the moderate risk of bias.

최소침습수복의 전통과 전망 (Perspectives on minimally invasive restoration)

  • In the filed of Dentistry, minimally invasive treatment modalities are new trend for conservation of natural teeth. Of them, laminate veneer and resin bonded fixed partial denture belong to restoration procedures. In this review, survival rates of each modalities and cause of unserviceability are analyzed, and criteria for success are presented. To get successful results of minimally invasive restoration, plenty of enamel layer, thin and strong materials, and high bonding strength are essential under cyclically loaded wet oral condition. Newly tried design of minimally invasive technique nowadays have to be evaluated only on the basis of requirement for long-term success.

소아환자의 점액종 치료 시 최소침습적 방법: 증례 보고 (Minimally Invasive Techniques for the Treatment of Mucoceles in Young Patients: A Case Series)

  • 구강 점액종은 구강의 소타액선의 병적인 변화로 인해 타액의 축적을 야기하며 발생하는 흔한 외향성 병소이다. 구강 점액종은 대개 증상이 없으며 무통성이며 반투과성의 구상의 파동성 결절모양인 것이 특징이다. 일반적인 구강 점액종의 치료방법으로는 외과적 제거술, 조대술, 냉동절제술, 그리고 스테로이드 주사 등이 있다. 본 증례는 치료 시 협조도를 얻기에 충분하지 못한 영아나 소아환자에서 외과적인 치료 대신에 최소침습적 방법의 일환으로 미세조대술과 봉합법을 사용하여 구강에 발생한 점액종을 성공적으로 치료한 증례를 보고한다. 본 증례를 바탕으로 영유아와 행동조절에 문제가 있는 아동를 대상으로 치료하는 소아치과분야에서 구강 점액종 치료 시 미세조대술과 봉합법은 하나의 대안적 방법이 될 수 있음을 시사한다.

소아의 구개부에 발생한 plasmacytoid myoepithelioma의 최소 침습적 제거술 (Minimally Invasive Surgery in a Pediatric Palatal Plasmacytoid Myoepithelioma)

  • Myoepithelioma는 타액선에 발생하는 드문 질환이다. Myoepithelioma는 소아 및 청소년보다 성인에서 호발한다. 구개부의 종창을 주소로 8세 여환이 본원으로 내원하였다. 환아의 나이와 상대적으로 큰 종양의 크기를 고려하여, 전신 마취 하에 구개부 점막 조직을 보존하는 보존적인 외과적 절제술이 시행되었다. 수술 부위의 치유가 적절히 일어났으며, 40개월의 관찰기간 동안 재발되지 않았다. 주변의 구개부 조직을 보존하는 최소 침습적 제거술은 소아에서 발생되는 myoepithelioma의 치료시 유용할 것으로 사료되었다.

Assessment of alveolar bone changes in response to minimally invasive periodontal surgery: A cone-beam computed tomographic evaluation

  • Purpose: The aim of this study was to evaluate 3-dimensional cone-beam computed tomography (CBCT) images of alveolar bone changes in patients who underwent minimally invasive periodontal surgery-namely, the pinhole surgical technique (PST). Materials and Methods: Alveolar bone height was measured and compared on CBCT images of 254 teeth from 23 consecutive patients with Miller class I, II, or III recession who had undergone PST. No patient with active periodontal disease was selected for surgery. Two different methods were used to assess the alveolar bone changes postoperatively. In both methods, the distance between the apex of the tooth and the mid-buccal alveolar crestal bone on pre- and post-surgical CBCT studies was measured. Results: An average alveolar bone gain >0.5 mm following PST was identified using CBCT(P=0.05). None of the demographic variables, including sex, age, and time since surgery, had any significant effect on bone gain during follow-up, which ranged from 8 months to 3 years. Conclusion: PST appears to be a promising treatment modality for recession that results in stable clinical outcomes and may lead to some level of resolution on the bone level. More long-term studies must be done to evaluate the impact of this novel technique on bone remodeling and to assess sustained bone levels within a larger study population.

Ten years of minimally invasive access cavities in Endodontics: a bibliometric analysis of the 25 most-cited studies

  • Objectives: This study aimed to analyze the main features of the 25 most-cited articles in minimally invasive access cavities. Materials and Methods: An electronic search was conducted on the Clarivate Analytics' Web of Science 'All Databases' to identify the most-cited articles related to this topic. Citation counts were cross-matched with data from Elsevier's Scopus and Google Scholar. Information about authors, contributing institutions and countries, year and journal of publication, study design and topic, access cavity, and keywords were analyzed. Results: The top 25 most-cited articles received a total of 572 (Web of Science), 1,160 (Google Scholar) and 631 (Scopus) citations. It was observed a positive significant association between the number of citations and age of publication (r = 0.6907, p < 0.0001); however, there was no significant association regarding citation density and age of publication (r = -0.2631, p = 0.2038). The Journal of Endodontics made the highest contribution (n = 15, 60%). The United States had the largest number of publications (n = 7) followed by Brazil (n = 4), with the most contributions from the University of Tennessee and Grande Rio University (n = 3), respectively. The highest number of most-cited articles were ex vivo studies (n = 16), and 'fracture resistance' was the major topic studied (n = 10). Conclusions: This study revealed a growing interest for researchers in the field of minimally invasive access cavities. Future trends are focused on the expansion of collaborative networks and the conduction of laboratory studies on under-investigated parameters.

접착치의학 개념에 따른 최소 침습적 치아 삭제법을 적용한 심미적인 치은 연상 변연 도재 부분피개관 수복 증례 (Clinical performance of esthetic ceramic partial-coverage restorations with supra-gingival margin using minimally invasive tooth preparation method according to the concept of adhesive dentistry)

  • 심미적인 도재 재료와 레진 시멘트의 비약적인 발전으로 임상에서 금속도재관에서 전부도재관으로의 전환이 활발히 이루어지고 있으며, 도재 수복물 제작 시 그에 적합한 형태와 기술을 이용하여야 그 장점을 최대화할 수 있다. 최근 법랑질을 최대한 보존하고 접착 효율을 높이는 최소 침습적 치아 삭제법을 적용하여 치은 연상 변연을 가진 심미적인 도재 부분피 개관 수복방법이 소개되었는데, 이를 통해 치질 삭제의 최소화, 시멘트 제거의 용이성 등 여러 장점들을 얻을 수 있다고 보고된다. 또한 이 제작법은 치은 압배사의 삽입이 불필요하여 치은 압배사 삽입에 소요되는 시간을 절약할 수 있으며 환자의 불편감도 감소시킬 수 있다. 더 나아가 치은 열구액이나 혈액의 개입 없이 명확하게 변연을 채득할 수 있어 더 정확한 수복물 제작이 가능하다고 보고된다. 본 증례에서는 각각 해당 치아 부위에 부분적인 결함이 있는 환자에서 통상적인 전부피개관이 아닌 접착치의학 개념에 따른 최소 침습적 치아 삭제법을 적용한 치은 연상 변연 도재 부분피개관으로 다양한 제작 방법을 활용하여 수복함으로써 불필요한 치아 삭제를 방지하고 잔존 치질을 최대한 보존할 수 있었다. 치료 후 환자와 술자 모두 심미적, 기능적으로 만족할 만한 결과를 얻었기에 이를 보고하는 바이다.

Traditional and minimally invasive access cavities in endodontics: a literature review

  • The aim of this review was to evaluate the effects of different access cavity designs on endodontic treatment and tooth prognosis. Two independent reviewers conducted an unrestricted search of the relevant literature contained in the following electronic databases: PubMed, Science Direct, Scopus, Web of Science, and OpenGrey. The electronic search was supplemented by a manual search during the same time period. The reference lists of the articles that advanced to second-round screening were hand-searched to identify additional potential articles. Experts were also contacted in an effort to learn about possible unpublished or ongoing studies. The benefits of minimally invasive access (MIA) cavities are not yet fully supported by research data. There is no evidence that this approach can replace the traditional approach of straight-line access cavities. Guided endodontics is a new method for teeth with pulp canal calcification and apical infection, but there have been no cost-benefit investigations or time studies to verify these personal opinions. Although the purpose of MIA cavities is to reflect clinicians' interest in retaining a greater amount of the dental substance, traditional cavities are the safer method for effective instrument operation and the prevention of iatrogenic complications.

Cerebral Oxygenation Monitoring during a Variation of Isoflurane Concentration in a Minimally Invasive Rat Model

  • Our previous study on monitoring cerebral oxygenation with a variation of isoflurane concentration in a rat model showed that near-infrared spectroscopy (NIRS) signals have potential as a new depth of anesthesia (DOA) index. However, that study obtained results from the brain in a completely invasive way, which is inappropriate for clinical application. Therefore, in this follow-up study, it was investigated whether the NIRS signals measured in a minimally invasive model including the skull and cerebrospinal fluid layer (CSFL) are similar to the previous study used as a gold standard. The experimental method was the same as the previous study, and only the subject model was different. We continuously collected NIRS signals before, during, and after isoflurane anesthesia. The isoflurane concentration started at 2.5% (v/v) and decreased to 1.0% by 0.5% every 5 min. The results showed a positive linear correlation between isoflurane concentration and ratio of reflectance intensity (RRI) increase, which is based on NIRS signals. This indicates that the quality of NIRS signals passed through the skull and CSFL in the minimally invasive model is as good as the signal obtained directly from the brain. Therefore, we believe that the results of this study can be easily applied to clinics as a potential indicator to monitor DOA.
