Inferiorly Migrated Disc Fragment at T1 Body Treated by T1 Transcorporeal Approach

  • Upper thoracic vertebral bodies are difficult to access using standard anterior approaches. It may require sternotomy and claviculectomy, which carries significant possibility of morbidities. We report a case of inferiorly migrated cervicothoracic junction disc treated successfully by anterior upper-vertebral transcorporeal approach. This specific technique obviated the need of sternotomy, created favorable working space and saved the motion segment at cervicothoracic junction. This report is the first transcorporeal approach to a disc fragment at T1-2 space without fusion.

  • To manage and query moving objects efficiently in MMDBMS, a memory index structure should be used. The most popular index structure for storing trajectories of moving objects is 3DR-tree. The 3DR-tree also can be used for MMDBMS. However, the volume of data can exceed the capacity of physical memory since moving objects report their locations continuously. To accommodate new location reports, old trajectories should be migrated to disk or purged from memory. This paper focuses on migration policies of a main memory index structure. Migration policies consist of two steps: (i) node selection, (ii) node placement. The first step (node selection) selects nodes that should be migrated to disk. The criteria of selection are the performance of insertion or query. The second step (node placement) determines the order of nodes written to disk. This step can be thought as dynamic declustering policies.

Dorsal Extradural Lumbar Disc Herniation Causing Cauda Equina Syndrome : A Case Report and Review of Literature

  • A 73-year-old male presented with a rare dorsally sequestrated lumbar disc herniation manifesting as severe radiating pain in both leg, progressively worsening weakness in both lower extremities, and urinary incontinence, suggesting cauda equina syndrome. Magnetic resonance imaging suggested the sequestrated disc fragment located in the extradural space at the L4-L5 level had surrounded and compressed the dural sac from the lateral to dorsal sides. A bilateral decompressive laminectomy was performed under an operating microscope. A large extruded disc was found to have migrated from the ventral aspect, around the thecal sac, and into the dorsal aspect, which compressed the sac to the right. After removal of the disc fragment, his sciatica was relieved and the patient felt strength of lower extremity improved.

Disappearance of a dental implant after migration into the maxillary sinus: an unusual case

  • Migration of dental implants into the maxillary sinus is uncommon. However, poor bone quality and quantity in the posterior maxilla can increase the potential for this complication to arise during implant placement procedures. The aim of this report is to present a dental implant that migrated into the maxillary sinus and disappeared. A 53-year-old male patient was referred to us by his dentist after a dental implant migrated into his maxillary sinus. The displaced implant was discovered on a panoramic radiograph taken five days before his referral. Using computed tomography, we determined that the displaced dental implant was not in the antrum. There was also no sign of oroantral fistula. Because of the small size of the displaced implant, we think that the implant may have left the maxillary sinus via the ostium.

Diel Vertical Distribution of Phytoflagellates in a Small Artificial Pond

  • Diel vertical distribution of phytoflagellates and interactions between the phytoplankton components and environmental and biological factors were studied in a small artificial pond for three days on the December 18, 1998 and April 9 to 10, 1999. The phytoplankton population was dominated by Mallomonas akrokomos of chrysophytes and Cryptomonas marssonii and Chroomonas sp. of cryptophytes. The vertical distribution of these phytoflagellates taxa exhibited clear diel migration pattern. Moreover their migration patterns are showed differential fluctuation between M. akrokomos, C. marssonii and Chroomonas sp. The later two species upward migrated in the evening as well as night, whereas the former species migrated downward. Their distinctive migration pattern was found during the night but was not observed in the morning. During daytime C. marssonii and Chroomonas sp. showed maximum density above 2 m depth but M. akrokomos below 2 m depth. The diel vertical distribution of the dominant phytoflagellates did not show significant correlation between physical, chemical and biotic factors.

Removal of Kirschner Wire That Migrated from the Pelvic Bone into the Right Ventricle of the Heart

  • A sixty-year-old man was admitted due to chest pain. He had a history of pelvic bone fracture fixation with Kirschner wire about 20 years earlier. On examination, we detected a Kirschner wire that had migrated into the right ventricle. Without cardiopulmonary bypass, we removed the migrating Kirschner wire via median sternotomy. The patient recovered without complications and was discharged on the 5th postoperative day.

  • A parallel nonlinear analysis program of prestressed concrete frame is migrated on a PC cluster system and a massively parallel processing system, CRAY T3E system, using MPI. The PC cluster system is configured with Pentium Ⅲ class PCs and fast ethernet. The CRAY T3E system is composed of a set of nodes each containing one Processing Element (PE), a memory subsystem and its distributed memory interconnect network. Parallel computing algorithms are implemented on element-wise processing parts including the calculation of stiffness matrix, element stresses and determination of material states, check of material failure and calculation of unbalanced loads. Parallel performance of the migrated program is evaluated through typical numerical examples.

  • In this paper, we propose the algorithm which solve synchronization problem happened migrating service objects. Load imbalance is occurred due to allocate different amount of load to be proposed to each node in distribute system. To solve this problem, the service objects have to be migrated from heavily loaded node to lightly loaded node. And in such a process, the synchronization problem may occur when client request a service object migrated incompletely. Therefore, we describe the environment of executing service objects, the importer/exporter/trader model compatible with migration of service objects, and appropriate migration algorithm, Finally, we analyze the conditions of problems, and propose the solution of each situations. Also, the performance advantages of using proposed algorithms are quantified through a simulation study.

  • 견관절의 골절 고정에 주로 이용되는 K-강선(Kirschner wire)은 드물게 흉곽 내, 또는 다른 장기로 원격 이동되는 것이 보고되었다. 흉곽 내로 이동된 K-강선은 대부분 개흉술을 통하여 제거되어 왔으나, 최근에는 종격동의 중요 장기를 침범하지 않은 K-강선에 한하여 흉강경을 이용한 제거가 가능한 것으로 보고되었다. 본원에서는 쇄골 고정에 이용되었던 K-강선이 흉곽 내 폐의 우상엽을 침범한 환자에게서 흉강경 수술로 성공적으로 제거하였기에 보고하는 바이다.
