Comparison of Kinesthesia Test of SIPT for Preschool Children (전 학령기 아동의 SIPT 운동감각(kinesthesia) 검사에 대한 비교 연구)

  • Objective : This study is to provide the norms of normal children when comparing the performance ability of preschool children while using the kinesthesia test of Sensory Integration and Praxis Tests(SIPT). Methods : Participants consisted of 90 normal children ranging in age from four to six years. The kinesthesia test of SIPT was utilized to investigate the performance ability. Results : 1. Regarding the kinesthesia ability according to age, the average value of kinesthesia performance error decreased as age getting older and that value showed the statistically significant differences between four and five, six age(p<0.05). 2. The kinesthesia performance ability according to gender, the accuracy of both hands and the dominant hand did not show the statistically significant differences. 3. Regarding the kinesthesia performance ability of test items, 1R item and 6R item(26.2cm), 5R item and 2L item(20.2cm) passing through the midline of body and having the large movement in distance and angle showed the difficulty to perform in all the children between 4 and 6 age. Conclusion : By providing the norms of the kinesthesia performance ability in normal children of the above results to the occupational therapists treating children, the helpful data to the hand skill development of children, exercise plan and implementation, and the performance therapy of ADL through the proper evaluation and training of kinesthesia is considered for the occupational therapists to be provided.

ANALYSIS OF SKELETAL FACIAL ASYMMETRY WITH SUBMENTOVERTEX CEPHALOMETRIC RADIOGRAPH (이하 두정 X-선 계측사진을 이용한 골격형 안면 비대칭분석)

  • This study was performed to investigate the midline having the least difference between the right and left structure among the lines that had been used in the submentovertex analysis and secondarily to know the distribution of asymmetry and it's degree existed normal persons and asymmetric patients. The subjects consisted of 40 normal adult patients and 40 asymmetric adult patients. The computerized analyses from submentovertex cepholometric radiograph were carried out. The results were as follows : 1. The right and left difference of the perpendicular bisecting line between right and left foramen spinosum was larger than the other midlines in the anterior area and it was decreased gradually as it progressed posteriorly. Specially the difference of this line was the smallest in the area where there was foramen spinosum. 2. The right and left difference of the perpendicular line through crista gali to the line between right and left foramen spinosum was smaller than the other midlines in the anterior area and it was increased gradually as it progressed posteriorly. 3. The right and left difference of the line between crista gali and anterior process of atlas was constant and smaller than the other midlines. 4. Asymmetry was a common finding in both normal and asymmetry group and left or right dominance of asymmetry was not statistically singnificant. 5. When the analyses were undertaken after submentovertex radiogram was divided into cranial base, upper face and lower face, the more inferior part showed relative asymmetry than the more superior part.

Giant Brain Abscess in a Neonate: Good Outcome with Single Transfontanelle Aspiration and Antibiotic Therapy

  • Brain abscesses in neonates are extremely rare and usually occur in patients with certain risk factors. A 1-month-old boy presented at the hospital with fever and irritability. As a result of preterm delivery and low birth weight, he had a history of admission to the neonatal intensive care unit. Neuroimaging revealed a large, space-occupying lesion in both frontal lobes, which was suspected to be an abscess with the midline shifting to the right. With a single aspiration and abscess drainage along with concurrent prolonged parenteral antibiotic therapy, the patient showed an excellent treatment outcome with normal development. The focus will be placed on minimally invasive surgical management as well as positive outcomes.

A Clinical Study on 76 Cases of Thyroglossal Duct Cyst and Fistula (갑상설관낭종 76례에 대한 임상적 고찰)

  • Clincal observation was made on 76 cases of thyroglossal duct cyst and fistula with brief review of literature. All of the cases were diagnosed and treated surgically in Kyung Hee University Hospital from 1971 to 1993. The following results were obtained. 1) Most cases were under 20 years old(72.3%). and 49(64.5%) cases were male and 27(35.5%) cases were female. 2) Morphologically cystic type were found in 60(78.9%) cases and fistula type were 16(21.1%) cases. 3) Fluctuating cystic mass and pus discharge from fistula on just below the hyoid bone at midline of anterior neck were most common symptoms and signs. 4) 9(11.8%) cases were recurrent type and 4(5.3%) cases were recurred twice, at the time of first visit. 5) There were 2(2.6%) cases of having connection with the thyroid gland which were identified grossly on the operation field. 6) The entire cases were operated according to the Sistrunk procedure under the general anesthesia. 7) There was no recurrence and other complication except 4(5.3%) cases of wound infection.

Anatomical Variation of the Lesser Saphenous Vein

  • Purpose: The lessor saphenous vein is an anatomical index in various surgical methods involving an approach to the popliteal fossa. However, occasionally, there have been some cases where the surgical process was difficult because the lessor saphenous vein was not found in the popliteal fossa during the operation process. The aim of this study is to determine the frequency of the anatomical variation of lessor saphenous vein not found in the popliteal fossa with a review of the literature. Materials and Methods: This study was conducted on 83 cases of selective neurectomy for hypertrophic calf performed in our clinic from March 1997 to June 2013. There were 42 patients, with a mean age of 32.8 years old. We confirmed the existence of the lessor saphenous vein in the popliteal fossa during the operation process. Results: Among 83 cases during this study period, the lesser saphenous vein was not found in four cases. In one patient, no lesser saphenous vein was found on either side of the popliteal fossa, and in two patients, no lesser saphenous vein was found on the left side of the popliteal fossa. As a result, the frequency of variation was found to be 4.8%. Conclusion: Due to the anatomical variation of the lessor saphenous vein, it may not be found in the midline of the popliteal fossa. Based on the literature review, several possibilities for failure to observe the lesser saphenous vein could be suggested. If surgeons are well aware of these possibilities, the steadier operation could be performed.

Crumbled or mashed feed had no significant effect on the performance of lactating sows or their offspring

  • Background: Physical and chemical properties of feedstuffs can be changed by feed processing. Moreover, through various mechanisms, feed processing can affect growth performance and feed efficiency of swine, nutrition value of the feed. Weaning-to service-intervals (WSI), subsequent farrowing rates, and total-born litter sizes were determined by feed intake and metabolic state during lactation. Methods: A total of 20 sows (Landrace ${\times}$ Yorkshire) with an average body weight (BW) of 266.1 kg 4 d before farrowing were used to determine the effect of feed processing on the performance of lactating sows and their offspring. The following two dietary treatments were used: 1) Crumble diet (C); and 2) Mash diet (M). Ten replications were used for each treatment. Back fat thickness of sows was measured 6 cm off the midline at the 10th rib using a real-time ultrasound instrument at 4 d before farrowing, 1 d after farrowing, and during weaning. Sow BW were also checked at 4 d before farrowing, 1 d after farrowing, and during weaning. Fecal score of sows were assessed on d 14. Fecal score of piglets were observed on d 7, 15, and 24. Data were analyzed using t-test procedure of SAS (2014) with sow as experimental unit. Results: No significant (p > 0.05) difference was observed in the reproduction performance of sows between the two treatments. In addition, there was no significant (p > 0.05) difference in the growth performance of piglets between the two treatments. Fecal score of sows or piglets showed no significant (p > 0.05) difference either. Conclusions: In conclusion, different feed processing (mash or crumble) did not make any significant difference on the performance of lactation sow or their piglets.

Abdominal Subcutaneous Fat Thickness Measured by Ultrasonography Correlates with Hyperlipidemia and Steatohepatitis in Obese Children

  • Purpose: The aim of this study is to evaluate the relationship between abdominal subcutaneous fat thickness measured by ultrasonography (US) and serum lipid profile and liver transaminases in obese children. Methods: One hundred and sixty-six children diagnosed with obesity from May 2001 to December 2013 were included in this study. Data on serum lipid profile and liver transaminases were collected from clinical records. Abdominal subcutaneous fat thickness and grade of hepatic steatosis were evaluated by US. Results: Of the 166 children, 107 were diagnosed with hepatic steatosis by US, 46 with grade I, 56 with grade II, and five children with grade III. According to the grade of hepatic steasosis, the average values of midline abdominal subcutaneous fat thickness and right flank abdominal subcutaneous fat thickness measured $2.9{\pm}0.8cm$ and $1.9{\pm}0.7cm$ in the normal group, $3.3{\pm}0.8cm$ and $2.0{\pm}0.7cm$ in grade I, $3.8{\pm}0.8cm$ and $2.3{\pm}0.8cm$ in grade II, and $4.1{\pm}0.8cm$ and $2.8{\pm}1.4cm$ in grade III, respectively. Abdominal subcutaneous fat thickness correlated with grade of hepatic steatosis (p<0.01). In addition, abdominal subcutaneous fat thickness correlated with concentration of serum lipids and liver transaminases in the age group of 12-14 years (p<0.01). Conclusion: Abdominal subcutaneous fat thickness measured by US can be used as a reliable predictor of possible hyperlipidemia and steatohepatitis in children, especially during the adolescent stage.

Comparison of Effects of Transcutaneous Electrical Nerve Stimulation (TENS) and San-Yin-Jiao (SP6) Acupressure on Primary Dysmenorrhea (경피신경전기자극(TENS)과 삼음교 지압이 원발성 월경통에 미치는 효과 비교)

  • PURPOSE: This study examined the effect of high-frequency transcutaneous electrical nerve stimulation (TENS) and San-Yin-Jiao (SP6) acupressure on primary dysmenorrhea. Furthermore, the difference in effectiveness between the two methods were compared. METHODS: Twenty-six students participated in this study, and were randomly assigned to a high-frequency TENS group(experimental 1, n=9), a San-Yin-Jiao (SP6) acupressure group(experimental 2, n=9), and no treatment group(control, n=8). They were assessed and treated on the first day of their menstruation. The TENS protocol included applying 20 minutes of stimulation with a frequency of 100Hz, $100{\mu}sec$ pulse width. Four electrodes were placed on the skin 3 cm from midline at T12-L1 and S2-3 paravertebral muscles. San-Yin-Jiao (SP6) acupressure should be applied with the thumb for 10 minutes(8 seconds pressure and 2 seconds rest) on the SP6 acupoint. This procedure should be repeated for the other foot. Dysmenorrheal pain measured two pain assessment tools (VAS, DPT) pre-treatment; immediate post-treatment; 30minutes, 1, 2hours; and 3, 4, 5, 6, 24 hours after the VAS test were added. RESULTS: The results showed significant differences in pain assessments (VAS, DPT) after treatment for subjects of experimental group1 (p<.05) and experimental group2 (p<.05), whereas the between-group comparison found no statistically significant differences. CONCLUSION: This result supports the idea that using two methods could be effective in pain reduction among students who suffered from primary dysmenorrhea.

Ethmoidal Meningoencephalocele Associated with Seizure in a Juvenile Alaskan Malamute

  • Signalment: An 8-month-old female Alaskan malamute was presented for progressive cluster seizure disorder. Results: There were no abnormalities on neurological examination, survey radiographs, or blood analysis. Magnetic resonance (MR) imaging and computed tomography revealed extension of the olfactory bulb and frontal lobe into the nasal cavity. They also confirmed abnormal anatomy of the nasal turbinates within the rostral part of the nasal cavity and the absence of a cribriform plate. On T2-weighted and fluid-attenuated inversion recovery images, the herniated brain showed heterogeneous and hyperintense signals consistent with intraparenchymal edema. Transverse MR images showed brain herniation into the right frontal cavity and an asymmetrical lateral ventricle because of a left midline shift. On contrast-enhanced MR images, the protruding brain parenchyma was mildly enhanced. Ethmoidal encephalocele was suspected as the final diagnosis. Despite symptomatic treatment, the dog continued to exhibit seizures and was euthanized. Clinical relevance: Ethmoidal encephalocele is a rare disease in dogs. However, it could be considered as a cause of seizure in young dogs.

Tailored Surgical Approaches for Benign Craniovertebral Junction Tumors

  • Objective : We report our surgical experience in the treatment of 16 consecutive patients with benign craniovertebral junction (CVJ) tumor, observed from 2003 to 2008 at our department. Methods : We had treated 6 foramen magnum meningiomas, 6 cervicomedullary hemangioblastomas, 1 accessory nerve schwannoma, 1 hypoglossal nerve schwannoma, 1 C2 root schwannoma, and 1 cavernous hemangioma. Clinical results were evaluated by Karnofsky Performance Scale (KPS) and all patients underwent preoperative neuroradiological evaluation with computed tomography (CT) and magnetic resonance image (MRI). Angiography was performed in 15 patients and preoperative embolization was done in 2 patients. Results : Five far-lateral, 1 supracondylar and 10 midline suboccipital approaches were performed. Gross total removal was achieved in 15 cases (94%) and subtotal removal in 1 patient (6%). None of the patients required occipitocervical fusion. Radiological follow-up showed no recurrence in cases totally removed. Postoperative decrease of KPS scores was recorded in only 1 patient. The treatment of cervicomedullary solid hemangioblastoma presented particular issues : by preoperative embolization, we removed tumor totally without an excessive bleeding or brainstem injury. In one of foramen magnum meningioma, we carried out subtotal removal due to hard tumor consistency and encasement of neurovascular structures. Conclusion : The choice of surgical approaches and the extent of bone resection should be defined according to the location and size of individual tumors. Moreover, we emphasize that preoperative neuroradiological evaluations on presumptive tumor type could be helpful to the surgeon in tailoring the technique and providing the required exposure for different lesions, without unnecessary surgical steps.
