Heart Failure With Preserved Ejection Fraction and Frailty: From Young to Superaged Coexisting HFpEF and Frailty

  • Being commonly diagnosed in elderly women and associated with comorbidities as well as ageing-related cardio-vascular changes, heart failure with preserved ejection fraction (HFpEF) has been recently considered as a distinct cardiogeriatric syndrome. Frailty is another frequent geriatric syndrome. HFpEF and frailty share common underlying mechanisms, often co-exist, and represent each other's risk factors. A threshold of 65 years old is usually used to screen patients for both frailty and HFpEF in research and clinical settings. However, both HFpEF and frailty are very heterogenous conditions that may develop at younger ages. In this review we aim to provide a broader overview on the coexistence of HFpEF and frailty throughout the lifetime. We hypothesize that HFpEF and frailty patients' profiles (young, elderly, superaged) represent a continuum of the common ageing process modified by cumulative exposure to risk factors resulting to a presentation of HFpEF and frailty at different ages. We believe, that suggested approach might stimulate assessment of frailty in HFpEF assessment and vice versa regardless of age and early implementation of targeted interventions. Future studies of pathophysiology, clinical features, and outcomes of frailty in HFpEF by age are needed.

The Age of the Okcheon Metamorphic Belt-How Much Do We Know? (옥천 변성대의 시기-우리는 얼마만큼 알고 있나?)

  • The geologic age of the Okcheon metamorphic belt, used to be a longstanding puzzle, has been settled down to Neoproterozoic to Paleozoic with discovery of fossils and isotopic age dating of metavolcanic rocks. As isotopic ages become accumulated, there appeared a controversy over the age of peak metamorphism in the Okcheon metamorphic belt, i.e., a single late Permian-early Triassic metamorphism (CHIME allanite age and U-Pb age of metamorphic zircon), or earlier independent presence of early Permian metamorphism (U-Pb age of allanite within garnet porphyroblast). If we compare the isotopic ages that can represent metamorphism, the data for the latter have much larger error than those of the former with some overlap considering the error limits. It means that, the former, supported by two independent ages, is considered a better representation for the age of metamorphism of the Okcheon metamorphic belt. Therefore, I propose the idea of early Permian metamorphism should better be reserved until conclusive evidence appears. The late Permian-early Triassic metamorphic age suggest that the effect of continental collision influenced much of the middle part of Korean Peninsula, namely, the Imjingang belt, the Gyeonggi massif and the Okcheon belt.

  • The geology of the Kualakulun area in Middle Kalimantan, Indonesia comprises Permian to Carboniferous Pinoh Metamorphic Rocks and Cretaceous Sepauk Plutonics of the Sunda Shield, late Eocene Tanjung Formation, Oligocene Malasan Volcanics, Oligocene to early Miocene Sintang Intrusives and Quaternary alluvium. Sepauk Plutonic rocks are classified as the calc-alkaline series and the S-type granite. Sintang Intrusive rocks are basic-intermediate and intermediate rocks, and consists of basalt, basaltic andesite, basaltic trachyandesite and trachyandesite. The Malasan Volcanics are characterized by intermediate dacitic pyroclasticl and minor lavas and belong to the subalkaline (calc-alkaline and tholeiitic) series. The whole-rock K-Ar ages of the fine-grained biotite granites and medium-grained granitoids were determined to be 100.5-106.5 Ma and 91.9-102.6 Ma, respectively. The whole-rock K-Ar age of the diorite is 89.1 Ma. K-Ar ages of the Malasan Volcanics and Shintang intrusives show 31.5-36.8 Ma and 24.6-34.5 Ma, respectively, and correspond to the Tertiary time.

A Preliminary Study on Granite Suite and Supersuite for the Jurassic Granites in South Korea (우리나라 쥬라기 화강암의 스위트/슈퍼스위트 분류에 대한 예비적 연구)

  • Intruding ages for the Jurassic(${\sim}Triassic$) granites in South Korea can be reestablished as $210{\sim}170\;Ma$ and $180{\sim}160\;Ma$ according to the tectonic provinces of magma emplacement. Most Jurassic granites in the Gyeonggi massif have the intrusion ages of $180{\sim}160\;Ma$, indicative of middle Jurassic igneous activity. On the other hand the intrusion ages ($210{\sim}170\;Ma$) for the Jurassic granites in the Yeongnam massif represent late Triassic to middle Jurassic igneous activity. Using the concept of granite suite/supersuite, the Jurassic granites in South Korea can be hierarchically divided into two supersuites and two suites. Huge batholith of NE-SW direction in the Gyeonggi massif could be designated to be 'Gyeonggi Supersuite', which was originated from the mixture of igneous protolith and more evoloved crustal materials and formed in the post-orogenic environment after collision of the north China and south China blocks. There are one supersuite and two suites in the Yeongnam massif 'Yeongnam Supersuite' could be designated from the NE-SW trend batholith in the massif. This supersuite was originated from the mixture of igneous protolith and evolved crustal materials. Granitic rocks between Andong and Girncheon areas could be defined as 'Andong Suite'. This suite was originated from the mixture of depleted mantle and igneous protolith. The Daegang and Hamchang granties could be designated as 'Daegang Suite'. This suite was formed in the anorogenic environment which was different from the orogenic environment of the other supersuite/suite in the Yeongnam massif.

Middle-Old Age's Retirement Transition, Old Age Income Security and the Support of Gradual Retirement (중고령자의 퇴직전환 및 노후소득보장과 점진적 퇴직지원)

  • This study reviewed pension reform's overall characteristic and(anticipated) positive negative effect in OECD countries's and then analysed middle-old age's retirement transition and determinants of full/gradual retirement through the $3{\sim}7th$ Korea Labor and Income Panel considering that Korea has been aging society quickly and it is necessary to suggest not only solution of early retirement and working age reduction but also pension reform. As a result of this study, about 1/4 of 50 years and older have been continuing to work through various pathways after retirement and 98% among fully retired older who passed by re-employment step of occupational status including retirement are still searching for jobs. This showed that it is also inappropriate to typical retirement concept itself on the lines of labour market participation in Korea and part-time/temporary work or self-employment have been used by means of alternatives of maintaining works for middle-old ages. However, the duration of changed occupational status of gradual retirees is mostly only $1{\sim}2$ years. Therefore it is necessary to support the gradual retirement to minimize a term of income insecurity and promote the work of the old ages who have will and capacity of work. Most of all, partial pension system which is main program of gradual retirement, should make the rules that beneficiaries are those who age less than pensionable age and benefit levels should be actuarial fairness together with pension system and provide substantial help. But, the introduction of partial pension system is not the only way to solve and needs overall social economic approach. Especially guarantee the increase of quantitative qualitative employment for middle-old ages linking labor market policy and supporting gradual retirement not ought to be abused to force the part time works and early retirement route against their own will.

A Study on the Middle and High School Students′Clothing Gift Purchasing Behavior according to the Consumption Orientation (중.고등학생들의 소비성향에 따른 의류선물구매행동)

  • The purpose was to identify the middle and high school students'clothing gift purchasing behavior according to the consumption orientation. Self-administered questionnaires were distributed to 585 middle and high school students who had an experience of purchasing a clothing gift more than one time during recent one year in Chonbuk province from Mar. 6 to Mar. 15 2000. Frequency. percentage. mean. standard deviation. factor analysis. $\varkappa$$_2$-test. t-test. one way ANOVA were used for data analysis. Duncan's Multiple Range test was followed. The results of this study were as follows ; 1. The student's consumption orientation factors were consisted of plan, practicality. and pleasure. and they were divided into the practicality pursuit. the convenience pursuit. and the feeling pursuit. 2. The practicality pursuit considered practical aspects more. but considered external aspects of gifts less. gift purchasing frequencies was less. and selected cheaper gifts. The convenience pursuit considered practical aspects less. purchased gift more frequently. and selected ore expensive ones. The feeling pursuit considered practical aspects and external aspects of gift important. 3. Girls considered the external aspects of clothing gift more important than boys. and patronized the specialty store or discount outlet. The students in high schools or with more pocket money considered more criteria when selecting gifts. and selected more expensive ones. 4. Middle and high school students game and took gifts 1-3 times a year. and the common price rage was 10,000-30,000 won. Girls with more pocket money and in older ages selected more expensive gifts. On birthday or parents day. they used to select clothing as a gift more frequently. and the rank order of the favorite items were socks or stockings. handkerchiefs. and upper outer wears. They patronized specially store or opposite sexes.

Influencing Factors on Successful Aging of Middle and Old Aged Men (중·노년층 남성의 성공적 노화 영향요인)

  • Purpose: This study is to verify and analyze the influencing factors of successful aging with social support, self-efficacy, community environment and subjective health status on middle and old aged men. Methods: The survey was performed to the men between the ages of 50 and 79 in Seoul and Kyunggi province, Korea. 144 questionnaires were collected and analyzed by SPSS 18.0 program. Stepwise multiple regression was used to identify significant predictors of successful aging. Results: The most influencing factor for successful aging was social support, the second was self-efficacy, the third and the fourth was community environment and subjective health status. This regression model explained 56.7% of the variance in successful aging. Conclusion: The result shows that these variables should be considered in intervention and program of successful aging for middle and old aged men. It can be an important consideration in enhancing promoting care and welfare for the successful aging.

A Study on Eating Disorder, Depression and Body Dissatisfaction of Middle Aged Women (중년여성의 신체불만족, 우울과 섭식장애에 관한 연구)

  • The purpose of this study was to investigate the relationship between eating disorders, depression, and body dissatisfaction of middle aged women. Data was collected from June 1 to June 30, 1999 by means of a structured questionnaire. The subjects for this study were 99 middle aged women between the ages of 35 and 59, recruited from Seoul, Kyungido. The instruments were the BDI(Beck Depression Inventory) developed by Beck(1973), Eating Disorder Inventory(1997) developed by Garner & Garfinkel, Halm, Falk& Schwartz, Shin Mi Young. The instruments were the Body Dissatisfaction(1997) developed by Garner, Olstead & Polivy, Han osoo, yoo hee jung, shin Mi Young. The data was analyzed by the SPSS/PC program using frequency, percentage, mean, standard deviation, t-test, ANOVA and Pearson correlation coefficient. The results of this study are as follows 1. The mean body weight and height of the subjects were 57.49kg, 158.62cm. 2. Depending upon BMI, the obesity subjects were 18.2% and the rest(81.8%) were under weight or normal. More than 70% of the normal weight subjects(71 subjects) perceived their weight as overweight. 3. The total mean scores of anorexia nervosa were higher than bulimia nervosa. The total mean scores of eating disorders was 2.13. 4. The total mean scores of depression was 1.50. 5. The total mean scores of body dissatisfaction was 3.30. 6. There was a positive correlation between depression and eating disorders(r=.267, p<0.01)and, between body dissatisfaction and eating disorders(r=.273, p<0.01).There was not a significant correlation between depression and body dissatisfaction. According to the present study eating disorders in middle aged women developed because of the tendency to have negative perceptions of their bodies.

Our investigation of this study may provide the basic data for the knowledge system of aesthetic theory including the arena of physical, psychological and social health.

Low dietary calcium is associated with self-rated depression in middle-aged Korean women

  • It is thought that calcium (Ca) and magnesium (Mg) may be related to mental disorders such as depression; however, there have been few studies investigating the association between Ca and Mg nutrition status with depression in middle-aged female adults. Study subjects in this study included 105 women between the ages of 41 and 57 years. The subjects were divided into three groups according to the Zung Self-rating Depression Scale (SDS) score: Group I (SDS score < 33 percentile; n = 32), Group II (33 percentile ${\leq}$ SDS score < 67 percentile; n = 37), and Group III (67 percentile ${\leq}$ SDS score; n = 36). Anthropometric measurements, dietary intake survey using 3-day dietary records, SDS questionnaire and measurement of serum Ca and Mg were obtained and analyzed. No differences were observed in Ca, plant Ca, and Mg intake among the three groups. However, animal Ca intake by Group III was 141.3 mg, which was significantly lower than 207.6 mg by Group I and 198.3 mg by Group II (P = 0.0345). There were no significant differences in serum levels of Ca, and Mg among the three groups. Correlation analysis indicated that the SDS score had negative correlations with Ca intake (r = -0.2927, P<0.01) and animal Ca (r = -0.3411, P<0.001) after adjusting for age, menopause and energy intake. In conclusion, dietary Ca and animal Ca had negative associations with SDS score among middle-aged Korean female adults. Additional analysis of factors related to the association of calcium and magnesium nutritional status and depression is necessary.
