Effects of Elastic Band Exercise Combined with Swiss Ball Exercise on Lower Extremity Muscle Strength, Balance, and Pain in Middle-Aged Women with Osteoarthritis

  • Objective: The aim of this study is to test the effects of elastic band exercise accompanied by Swiss ball exercise on lower limb muscle strength, balance and pain in middle-aged women with osteoarthritis. Design: A randomized controlled trial. Methods: Thirty-five participants were randomly assigned to the experimental group (n=18), which performed elastic band exercise combined with Swiss ball exercise, and the control group (n=17), which performed elastic band exercise only. Both groups did a 30-minute session of exercise three times a week for eight weeks and were assessed for lower extremity muscle strength, static and dynamic balance, and pain levels before the first therapy session. All participating patients underwent outcome assessment after eight weeks of therapy without any additional treatment. Results: The experimental group made a significant increase in muscle strength of the lower extremities, static and dynamic balance ability, and pain level (p<0.05). The control group made a significant improvement in lower limb muscle strength, dynamic balance ability and pain level (p<0.05) with no such improvement in static balance ability. The exercise group made a significant increase in static and dynamic balance ability and pain level compared to the control group (p<0.05). Conclusions: These results demonstrated that both Swiss ball exercise and elastic band exercise were effective for middle-aged women with osteoarthritis and found that elastic band exercise combined with Swiss ball exercise produced more significant effects on their balance and pain.

The role of oral physicians in predicting the risk of obstructive sleep apnea: A case-control study

  • Purpose: Obstructive sleep apnea (OSA) is a common medical disorder with serious complications if untreated. Dentists play a vital role in the early diagnosis of this condition, thereby improving patients' prognoses. The purpose of this study was to identify patients with a high risk of OSA using simple cephalometric measurements in patients receiving routine dental care. Materials and Methods: The present study was conducted on 206 patients divided into a high-risk group and a control group after answering the Berlin questionnaire. Cephalometric analysis of a digital cephalogram was performed to measure the upper airway diameter (UAD) and mandibular-to-hyoid bone distance (MP-H) by 2 observers at 2 different times. Results: Among 206 patients, 93 (45%) were included in the high-risk group and 113 (55%) were in the control group. No significant difference was present between the groups with regard to gender, and the patients ranged in age from 18 to 65 years. The UAD measurements in the high-risk group were significantly lower than in the control group, and the MP-H measurements were significantly higher in the high-risk group than in the control group. The UAD was lower in middle-aged patients in both groups. Conclusion: Our study found that the UAD was lower in individuals with a high risk of OSA. Also, we found that middle-aged individuals of both genders were more likely to develop OSA. Dentists play a vital role in diagnosing patients at a high risk for OSA via thorough clinical examinations, risk factor analyses, and simple cephalometric analyses.

원위 속목동맥과 중간대뇌동맥에 발생한 섬유근육형성이상 (Fibromuscular Dysplasia of the Distal Internal Carotid and Middle Cerebral Artery)

  • Fibromuscular dysplasiais an uncommon condition of idiopathic, non-inflammatory and non-atherosclerotic disease of the musculature of arterial walls. The disease is rare, but it commonly affects young and middle aged women. Isolated intracranial cerebral fibromuscular dysplasia is extremely rare because cerebral fibromuscular dysplasia usually affects extracranial vessels. A 20-year-old woman was admitted with light hemiplegia and global aphasia. Brain MRI and MRA demonstrated acute left middle cerebral artery territory infarction with a multifocal stenosis and dilatation of the left middle cerebra artery and left internal carotid. The characteristic conventional cerebral angiographic findings demonstrated a typical string-of-beads appearance in the left distal internal carotid artery and proxiaml portion of the left middle cerebral artery, which suggested a medial type fibromuscular dysplasia. We report a case of isolated intracranial fibromuscular dysplasia with left middle cerebral artery territory infarction. Fibromuscular dysplasia should he considered as a stroke risk factors in children and young adults, especially in patients with no known cardiovascular risk factors.

중년여성의 피로인지여부에 따른 피로 정도 (The Degree of Fatigue According to Subjective Recognition in Middle - Aged Women -)

  • This study was conducted to investigate the subjective degree of fatigue, the degree of fatigue depending on analysis of MMPI and SDNN in middle-aged women. Data were collected from 51 patients, registered at Oriental Medicine Hospital in J-city and K-city respectively. The instrument used in this study were the Multidimensional Assessment of Fatigue developed by Belza(1995) and revised by Jung & Kim(1999) and the Minnesota Multiphasic Personality Inventory(MMPI) and Standard Deviation of Normal to Normal(SDNN). The collected data were analyzed by SPSS/WIN 9.0 program using frequencies, percentage, mean, standard deviation, t-test. The degree of fatigue in middle-aged women: the average score 30.61 out of the total score 40. Subdivision of fatigue scores: common fatigue degree 7.71, distress due to fatigue 7.67, degree of daily activity fatigue 7.43, and fatigue frequency in the previous week 7.84. The degree of fatigue according to subjective recognition was shown significant difference(t=6.707, p=.000).: group of recognition (33.58), group of ignorance (23.47). The degree of fatigue according to MMPI was shown significant difference. : In the group of recognition, scales of Hs, D, Hy, Pt were higher. The degree of fatigue according to SDNN was shown significant difference (T=-4.062, P=.000). : group of recognition (33.47ms), group of ignorance (45.70ms). The total fatigue score and scales of Hs, D, Hy in MMPI was correlated positively (r=.576; r=.466; r=.518), and total fatigue score and SDNN was correlated negatively (r=-.576). Conclusively fatigue is one of difficult problems in middle-aged women. Therefore it is necessary to develope some nursing intervention for reducing fatigue in its early stages.

중년여성의 보완대체요법이용에 관한 연구 (A Study on Use of Complementary-Alternative Therapy in Middle-aged Women)

  • Purpose: The purpose of this study was to describe the use pattern of alternative therapies in middle aged women. Method: The subjects of this study were 530 women from 40 to 64 years of age. The data of this study was patients' records. The data was gathered from April 1st, 2004 to June 30th, 2004. Data was analyzed statistically by using the SPSS/WIN 10.0 program. Result: It was shown that 63.5% of the subjects have utilized one or more types of alternative therapy. The most common type of alternative therapy was dietary and nutritional therapy 38.3%, and the place of use was the home 64.1%. The most common reason was a recommendation by friends. The degree of satisfaction after the use of alternative therapy was 95.2%. Among users of alternative therapies, the most frequent responses to each question were as follows; Motive of Use- "for health maintenance and promotion." (62.7%) In the relation ship between general characteristics and utilization of alternative therapy, religion(x2=12.33, p=.02), exercise(x2=8.21, p=.004), and health status(x2=14.95, p=.005) showed a significant statistical difference. Conclusion: We found that middle aged women used alternative therapies more frequently than other populations. Therefore, it is suggested that medical doctors or nurses verify the true effects or side-effects from the most therapies or nurses verify the true effects or side-effects from the most common complementary or alternative therapies through experiments.

Arthroscopic-assisted Latissimus Dorsi Tendon Transfer for the Management of Irreparable Rotator Cuff Tears in Middle-aged Physically Active Patients

  • Background: Latissimus dorsi (LD) tendon transfer is used as a treatment option for massive irreparable posterosuperior rotator cuff tears, and recently, an arthroscopic-assisted technique was introduced. This study was undertaken to evaluate the clinical and radiological outcomes of arthroscopic-assisted LD tendon transfer for the management of irreparable rotator cuff tears in active middle-aged patients. Methods: The records of five patients (two males) with irreparable tears involving the supraspinatus and infraspinatus tendons managed by arthroscopic-assisted LD tendon transfer were retrospectively reviewed. Clinical outcomes were assessed using the visual analogue scale (VAS) pain scale, American Shoulder and Elbow Surgeon's (ASES) scores, the University of California Los Angeles (UCLA) scale, and ranges of motion. Postoperative integrities of transferred tendon were evaluated by magnetic resonance imaging in 4 patients and by ultrasound in one. Results: Mean patient age was 55 years (range, 48-61 years), and mean follow-up period was 20 months (range, 12.0-27.2 months). Mean VAS score significantly improved from $6.6{\pm}2.6$ preoperatively to $1.8{\pm}2.5$ postoperatively (p=0.009), mean ASES score increased from $67.6{\pm}9.2$ to $84.6{\pm}15.1$, and mean UCLA score from $18.0{\pm}1.4$ to $28.8{\pm}8.5$ (all p<0.001). Postoperative imaging of the transferred LD tendon showed intact repair in 4 patients. The remaining patient experienced LD transfer rupture and a poor outcome. Conclusions: Arthroscopic-assisted LD tendon transfer improved shoulder pain and function in patients with massive, irreparable rotator cuff tears, and may be an option for this condition, especially in physically active patients.

여성 노인입원 환자의 연령에 따른 수면장애 요인과 수면 양상 (Factors of Sleep Disturbance and Sleep Patterns According to Age in Older Hospitalized Women Patients)

  • Purpose: The purpose of this study was to investigate factors of sleep disturbance and sleep patterns of elderly hospitalized women patients between young-old (65-74 years), old (76-84 years) and old-old (85 years and above). Methods: The questionnaire included the environmental disturbance factors (Paik, 2000), degree of pain (Wang & Kim,1995), disease symptoms (Paik, 2000), depression (Kee, 1996) and sleep patterns (Oh, Song, & Kim, 1998). Data were analyzed using frequencies, means, ${\chi}^2$-test, t-test, ANOVA, Scheffe, Pearson correlation coefficients and multiple regression (SPSS 14.0). Results: The middle, and oldest group's environmental disturbance factors of degree of pain, disease symptoms, depression and sleep patterns were higher than those of the younger group. The younger, middle, and oldest group's sleep pattern had a significant negative correlation with environmental disturbance factors, degree of pain, disease symptoms and depression. The model including variables related to environmental, physical and psychological disturbance factors, explained the following variances in sleep pattern: 26.8% for the youngest group, 27.6% for the middle group and 40.7% for the oldest group. Conclusion: The result of this study offer basic data for the development of nursing intervention programs to improve sleep patterns for hospitalized women patients according to age differences.

Comprehensive Orthodontic Treatment in a Middle-Aged Patient with Missing Maxillary Left First Premolar: A Case Report

  • As the adult population continues to increase, orthodontic treatment for adult patients is becoming more common. This case report describes comprehensive orthodontic treatment of a middle-aged patient with closure of the extraction space without prosthetic restoration. A 55-year-old woman with her maxillary left first premolar extracted because of a periodontal problem, wanted to close the space with orthodontic treatment. Since she had generalized crowding and mild skeletal discrepancy, we planned comprehensive orthodontic treatment, including closure of the extraction space by protraction of the left maxillary molars using miniscrews and aesthetic alignment of anterior teeth. The total treatment period was 28 months. As a result of these treatments, the extraction space was successfully closed, good tooth alignment and satisfactory occlusion were achieved.

Necessity of Oral Health Care for Patients with Hypertension and Diabetes Related to Gwangju Community Care Project

  • In this study, 293 middle-aged and elderly people in Gwangju·Jeonnam were analyzed for prevalence of hypertension and diabetes and health behavior index using composite logistic regression analysis using SPSS ver 21.0. The prevalence of hypertension was significant in age, education level, and household type, and the prevalence of diabetes was significant in education level. The prevalence of hypertension according to health behavior related indicators was significant in subjective health awareness, prosthesis repair, and discomfort chewing, and the prevalence of diabetes was significant in subjective health awareness, oral examination, discomfort chewing. The variables affecting the prevalence of hypertension were age and prosthetic repair, and the variables affecting the prevalence of diabetes were age, prosthetic repair, discomfort chewing, and annual drinking frequency. The purpose of this study was to investigate the relationship between hypertension, diabetes mellitus prevalence and health behavior indicators of middle-aged and elderly people, and to help the importance of Community Care and oral health care improvement program.
