Patients with acute respiratory diseases, such as Middle East Respiratory Syndrome (MERS) due to COVID-19, must wear masks, protective clothing, face shields, and gloves to prevent infection during treatment and performance. Even if it is applied to disabled people, families who protect them are severely mentally tired from severe physical fatigue and stress from exposure to high-risk infectious diseases. As such, the spread of infectious diseases such as respiratory diseases has not only caused difficulties in using existing welfare and medical services but also caused various problems throughout the daily life of disabled people due to the prolonged infectious disease, and its scope is gradually expanding. Therefore, it should not be overlooked that disabled people may experience various difficulties, from the spread of infectious diseases such as respiratory diseases to isolation, diagnosis, and treatment, and it is time to actively assess the life changes felt by families caring for disabled people and consider and research to provide adequate services. According to the survey of disabled people is being conducted in the context of the spread of infectious diseases such as respiratory diseases, while research on the spread of infectious diseases such as respiratory diseases is rare for parents with disabilities. There is a need for additional investigation into the characteristics in other areas of everyday life, including the health field, which is deteriorating through prior research. Therefore, through this survey, the purpose of this study is to investigate the life changes of parents with disabilities in the context of the spread of infectious diseases such as respiratory diseases and to compare and analyze them to find out how parents were affected by each type of disability. It will be used as evidence to identify more necessary needs and problems for parents with disabilities in the spread of infectious diseases such as respiratory diseases and to provide more appropriate health care and welfare services in the future.

질병 확산 모형은 질병의 확산 과정을 모형화 함으로써 질병이 발생하고 퍼지는 시간 내에서 통제하기 위하여 활용하고자 하는 모형이다. 본 연구에서는 질병 확산 모형의 가장 대표적인 SIR 모형에 기본적인 확장 접근을 하여 접촉군 (exposed)이라는 단계를 추가한 SEIR 모형을 이용하여 모형 구축을 하였다. 이 모형은 감염 대상군 (susceptible)의 사람들이 질병에 노출 된 잠복기를 거쳐 일정 시간이 경과한 후 감염되어 감염군 (infected)으로 이동한 후 다시 회복군 (removed)으로 이동하는 모형이다. 이와 같이 질병에 감염된 후 감염력이 생기는 잠복기가 있는 경우에 연구에 활용될 수 있다. 본 연구에서는 2015년 국내에서 발생한 메르스 코로나바이러스 (Middle East respiratory syndrome coronavirus; MERS CoV)에 의한 호흡기 감염증 자료를 수집하였다. 질병의 확산 과정이 결정적이 아닌 확률적인 흐름을 따른다고 가정하여 포아송 확률과정을 따른다고 보고 확률적 화학반응 모형을 이용하여 모형을 구축하였다. 모형을 구현하기 위해서 SEIR 모형의 세 모수인 질병에 노출된 정도를 나타내는 접촉률 (exposed rate), 질병의 감염 정도를 나타내는 감염률 (transmission rate), 질병의 회복정도를 나타내는 회복률 (recovery rate)를 추정함으로써, SEIR 모형에 적합하고 전염병 확산에 대한 예측을 수행하였다. 또한 접촉군이 정확하게 관찰되지 않을 부분을 보완하기 위하여 접촉군을 생성하는 과정을 전체 모형 구축 과정에 추가하였다.
Purpose: This study aims to examine how elementary, middle and high school health teachers in Gyeonggi province react to infectious disease like MERS. Methods: This is a descriptive study using a convenience sample of 1,267 school health teachers. Results: When MERS was an epidemic, school health teachers who took the 'leading' role were 92.4%. A school heath teacher's countermeasures against infectious diseases showed a high score during MERS outbreak (3.81) compared to regular days (3.59). Conclusion: Based on such results, it has been determined that schools need to keep two types of procedures manual, such as 'countermeasures on a regular basis' and 'countermeasures in the event of a pandemic disease outbreak' manual in the staff offices so that anyone can view them at any time. It is necessary to prepare regulations to strengthen their authority so that school health teachers can take the role of control tower, in a professional capacity, when communicable disease is spreading. In order to prevent any confusion and chaos, there should be a unitary reporting system at each school when dealing with an epidemic. School health teachers need to get continuous training to reinforce their abilities to react to communicable diseases.
This study examines how South Korean governments responded to the outbreak of Middle East Respiratory Syndrome Coronavirus (MERS) using the adaptive governance framework. As of November 24, 2015, the MERS outbreak in South Korea resulted in the quarantine of about 17,000 people, 186 cases confirmed, and a death of 38. Although the national government had overall responsibility for MERS response, there is no clear understanding of how the ministries, agencies, and subnational governments take an adaptive response to the public health crisis. The paper uses the adaptive governance framework to understand how South Korean governments respond to the unexpected event regarding the following aspects: responsiveness, public learning, scientific learning, and representativeness of the decision mechanisms. The framework helps understand how joint efforts of the national and subnational governments were coordinated to the unexpected conditions. The study highlights the importance of adaptive governance for an effective response to a public-health related extreme event.
The COVID-19 pandemic has increased demand for safe and effective vaccines. Research to develop vaccines against diseases including Middle East respiratory syndrome, Ebolavirus, human immunodeficiency virus, and various cancers would also contribute to global well-being. For successful vaccine development, the advancement of technologies such as antigen (Ag) screening, Ag delivery systems and adjuvants, and manufacturing processes is essential. Ag delivery systems are required not only to deliver a sufficient amount of Ag for vaccination, but also to enhance immune response. In addition, Ag types and their delivery systems determine the manufacturing processes of the vaccine product. Here, we analyze the characteristics of various Ag delivery systems: plasmids, viral vectors, bacterial vectors, nanoparticles, self-assembled particles, natural and artificial cells, and extracellular vesicles. This review provides insight into the current vaccine landscape and highlights promising avenues of research for the development and improvement of Ag delivery systems.
2015년 5월 중동 사우디아라비아와 아랍에미레이트를 다녀온 68세의 남자가 고열, 근육통, 기침 호흡곤란으로 자택부근 2개 병원)과 삼성서울병원(SMC) 응급실을 거치면서 시작된 메르스(MERS)라는 전염병은 거의 3개월에 걸쳐 최근까지 대한민국을 유행성 전염병의 공포로 몰아넣었다. 특히 이 전염병은 중동지역 아라비아 반도를 중심으로 2012년 9월부터 현재까지 총 26개국에서 1,392명에게 발생하여, 538명의 사망환자가 보고(유럽질병센터, 2015.7.21. 기준)되었고, 우리나라의 경우에도 총 확진자 186명, 사망자 36명(치사율 19.4%) 격리자 16,693명이라는 기록을 남기고 점차 우리의 뇌리에서 사라져가고 있다. 이 전염병의 파급으로 인하여 현 정부의 전염병 관리체계에 대한 많은 비판과 문제점을 제기하는 계기가 되었다. 특히 병원내 감염문제, 민간의료기관-지역 보건소-질병관리본부를 연결하는 감염병 관리전달체계의 미작동, 정부의 전염병과 관련된 정보 공개 거부, 냉난방 공조설비의 환기 등과 관련된 시설문제, 감염병 격리병상의 절대부족 문제, 지역보건소의 기능 및 권한의 제한문제, 격리자 및 의료기관에 대한 보상문제, 제도적으로는 감염병에 대한 건강보험 수가 개선문제, 법정감염병 재분류 문제, 질병관리본부의 역할 재정립 문제, 전염병 관련 전문인력 양성을 위한 제도적 보완문제, 병원 이용문화의 개선으로서 다인실 조정과 간병인 대체문제, 사회적 후유증을 치유하기 위한 정책적 배려문제 등이 있으며, 이에 대한 광범위한 논의와 해결방안이 필요한 때이다. 국민들은 2015년 5월 이전에는 아주 낯설었던 메르스라는 외부유입 전염병에 국가 방역체계가 흔들리는 모습을 보고 극도의 공포감을 느꼈으며, 외국에서는 중동지방을 제외하고는 전세계적으로 유례가 없는 메르스라는 전염병이 대유행하는 대한민국에 발길을 끊었다. 국민의 건강권이 심각하게 위협받았음은 물론이고 관광산업과 유통업계 등의 경제도 얼어붙었다. 정부는 메르스라는 전염병 확산의 초기대응 실패를 인정하고, 사태를 악화시킨 점에 대한 책임을 통감하여야 한다. 국민들에 대한 신뢰상실을 반성하고 향후 각종 필요한 정보를 공개함으로써, 메르스와 같은 전염병에 대한 정부정책을 투명하게 집행하고, 지방자치단체와 국민들이 지역사회 감염에 대처하고 확산방지에 나설 수 있도록 제도를 개선하며, 변경된 제도를 홍보하여 전염병 전파 예방에 노력해야 할 것이다.
Background: Pharmacy curriculum change was made from a 4-year program to a 2+4 year program in year 2009 in Korea. The change has resulted in more educational exposures on patient-centered practice environments for about 1,400 hours in the last year of the professional pharmacy program. When the Middle East Respiratory Syndrome (MERS) outbreak hit Seoul and suburban areas and propagated to other provinces in Korea, emergency response to avoid student infection in the pharmacy practice sites became an urgent issue. While other health professional programs such as medicine and nursing had activated emergency preparedness manuals, timely and clear guidelines were not disseminated to all pharmacy programs and protective measures largely relied on individual pharmacy program. Methods: A survey was developed by the Committee on Pharmacy Practice Experience Programs in the Korean College of Clinical Pharmacy to document the status of pharmacy programs during the Korea MERS outbreak in 2015. The 10-question survey was distributed to the pharmacy practice experience coordinators to 34 out of 35 pharmacy schools in Korea by emails. Results: Our findings showed that 82.4% of the program coordinators (28/34) responded to the survey, 96.4% of the programs did not have emergency preparedness manuals, administrative meetings were held in 89.3% of the pharmacy programs, the rotation schedules were modified or withheld in 53.6% of schools, and the changes were mostly observed from the programs classified as MERS outbreak regions. Conclusion: Further needs in establishing the emergency preparedness manual should be explored for pharmacy education stakeholders.
본 연구는 2015년 국내 메르스(중동호흡기증후군) 유행시 상급종합병원에 임상실습을 경험하는 학생의 메르스에 대한 지식, 태도 및 예방행위 수행 정도를 파악하고 메르스 예방행위 수행 예측요인을 확인하기 위해서 시도되었다. 본 연구 대상자는 3, 4학년 간호학과와 의과대학 학생 480명이었으며, 자료수집은 2015년 6월 9일부터 6월 30일까지 구조화된 설문지를 이용하였다. 자료분석은 SPSS/WIN 24.0을 이용하여 분석하였고, 메르스 예방행위 수행에 영향을 미치는 예측요인들은 위계적 회귀분석(hierachial regression)을 이용하였다. 임상실습 학생의 메르스에 대한 지식 수준은 9.56점(13점 기준)으로 보통 이상으로 높았으며, 메르스의 심각성 인지와 예방가능성에 대한 신념인 메르스에 대한 태도는 4.15점(5점 기준)으로 긍정적이었다. 메르스 예방행위 수행정도는 3.02점(5점 기준)으로 보통 수준이었다. 메르스 예방행위는 여학생인 경우, 메르스에 관한 교육 경험이 있는 경우, 지난 해 인플루엔자 접종을 한 경우, 올해 인플루엔자 접종 의도가 있는 경우, 메르스 감염에 대한 두려움이 높은 경우, 메르스에 대한 지식 수준이 높고, 태도가 긍정적일수록 수행 정도가 높게 나타났다. 이에 감염환자들을 치료하고 돌보게 될 간호학과와 의과대학 임상실습 학생의 메르스 예방행위 수행률을 높이기 위해서는 메르스 예방행위 수행에 영향을 미칠 수 있는 임상실습 학생의 특성을 고려해야 한다. 더 나아가 임상실습 학생들이 메르스를 예방하기 위하여 필수적으로 습득해야 하는 내용이 포함된 효과적인 감염예방 교육프로그램을 개발하고 실시하여 미래 보건의료인들의 메르스 관련 지식 수준을 높이고, 질환 전파를 막으려는 태도를 강화함으로써 전염성 감염질환 예방에 기여해야 한다.

Coronavirus causes an infectious disease in various species and crosses the species barriers leading to the outbreak of zoonotic diseases. Due to the respiratory diseases are mainly caused in humans and viruses are replicated and excreted through the respiratory tract, the nasal fluid and sputum are mainly used for diagnosis. Early diagnosis of coronavirus plays an important role in preventing its spread and is essential for quarantine policies. For rapid decision and prompt triage of infected host, the immunochromatographic assay (ICA) has been widely used for point of care testing. However, when the ICA is applied to an expectorated sputum in which antigens are present, the viscosity of sputum interferes with the migration of the antigens on the test strip. To overcome this limitation, it is necessary to use a mucolytic agent without affecting the antigens. In this study, we combined known mucolytic agents to lower the viscosity of sputum and applied that to alpha and beta coronavirus, porcine epidemic diarrhea virus (PEDV) and Middle East respiratory syndrome coronavirus (MERS-CoV), respectively, spiked in sputum to find optimal pretreatment conditions. The pretreatment method using tris(2-carboxyethyl)phosphine (TCEP) and BSA was suitable for ICA diagnosis of sputum samples spiked with PEDV and MERS-CoV. This sensitive assay for the detection of coronavirus in sputum provides an useful information for the diagnosis of pathogen in low respiratory tract.
