Gamma Knife Radiosurgery for Brainstem Metastasis

  • Objective : Brainstem metastases are rarely operable and generally unresponsive to conventional radiation therapy or chemotherapy. Recently, Gamma Knife Radiosurgery (GKRS) was used as feasible treatment option for brainstem metastasis. The present study evaluated our experience of brainstem metastasis which was treated with GKRS. Methods : Between November 1992 and June 2010, 32 patients (23 men and 9 women, mean age 56.1 years, range 39-73) were treated with GKRS for brainstem metastases. There were metastatic lesions in pons in 23, the midbrain in 6, and the medulla oblongata in 3 patients, respectively. The primary tumor site was lung in 21, breast in 3, kidney in 2 and other locations in 6 patients. The mean tumor volume was $1,517mm^3$ (range, 9-6,000), and the mean marginal dose was 15.9 Gy (range, 6-23). Magnetic Resonance Imaging (MRI) was obtained every 2-3 months following GKRS. Follow-up MRI was possible in 24 patients at a mean follow-up duration of 12.0 months (range, 1-45). Kaplan-Meier survival analysis was used to evaluate the prognostic factors. Results : Follow-up MRI showed tumor disappearance in 6, tumor shrinkage in 14, no change in tumor size in 1, and tumor growth in 3 patients, which translated into a local tumor control rate of 87.5% (21 of 24 tumors). The mean progression free survival was 12.2 months (range, 2-45) after GKRS. Nine patients were alive at the completion of the study, and the overall mean survival time after GKRS was 7.7 months (range, 1-22). One patient with metastatic melanoma experienced intratumoral hemorrhage during the follow-up period. Survival was found to be associated with score of more than 70 on Karnofsky performance status and low recursive partitioning analysis class (class 1 or 2), in terms of favorable prognostic factors. Conclusion : GKRS was found to be safe and effective for management of brainstem metastasis. The integral clinical status of patient seems to be important in determining the overall survival time.

건측(健測) 취혈(取穴) 다종(多種) 침자법(鍼刺法)이 백서(白鼠)의 신경병리성(神經病理性) 동통(疼痛)에 미치는 영향 (Effects of Acupuncture, Electro-acupuncture, Low Level He-Ne Laser Therapy at Oe-gwan($TE_5$) ${\cdot}$ Chogimup ($GB_{41}$) on L5 Spinal Nerve Ligation Model in Rats)

  • Objectives : To find effects of acupuncture, electro-acupuncture, low level He-Ne laser therapy(LLLT) at $TE_5$, $GB_{41}$ in the neuropathic pain. We made experiment on rats ligated L5 spinal nerve like general herniation of nucleus pulposus(HNP). Methods : A model of neuropathic pain was made by isolating Left 5th lumbar spinal nerve of rats. Three days after the neuropathic surgery, acupuncture and LLLT, electro-acupuncture was injected at $TE_5$, $GB_{41}$ one time a day for a week. Each group was divided two. one is opposite side performed the surgery which is right, another is left side performed the surgery. After that, the author examined the withdrawal response of neuropathic rats' legs by van Frey filament and acetone stimulation. And also the author examined c-Fos, Nociceptin and KOR-3 in the midbrain central gray of neuropathic rats. Results : As we have observed the effect of mechanical allodynia, LT-R group were diminished on 6th day compared with control group, EA-L group, EA-R group and LT-L group were diminished on 7th day compared with control group. As we have observed the effect of cold allodynia, EA-R group were diminished on 6th day, 7th day compared with control group. As we have observed the effect of activity of c-Fos in the central gray part, EA-R group and LT-R group were diminished compared with control group. As we have observed the effect of activity of Nociceptin in the central gray part, EA-R group were a little increased compared with control group but it is not reliability. As we have observed the effect of activity of KOR-3 in the central gray part, EA-R group were significantly increased compared with control group. Conclusions : We have noticed that effect of acupuncture at opposite side of sickness and powerful stimulation could be more effective, because of EA-R group have more controllable effect all test we have done on the other hand EA-L group have only effect on mechanical allodynia. This study can be used in clinical therapy for neuropathic pain. But it is not reliability that Nociceptin have effectively to control pain. Therefore We have to follow up about that.

과학 교수-학습 프로그램의 평가를 위한 두뇌기반 분석틀의 개발 (The Development of the Brain-based Analysis Framework for the Evaluation of Teaching-Learning Program in Science)

  • 이 연구의 목적은 과학 교수-학습프로그램을 평가하기 위한 두뇌기반 분석틀을 개발하는 것이다. 분석틀의 개발을 위해, 이 연구는 과학 교수-학습과 관련된 선행연구들로부터 교수-학습 프로그램의 분석항목을 3가지로 범주화 하였다: 인지, 동기, 감성. 첫 번째로, 각 항목에 관련된 두뇌활성 영역을 파악하기위하여 과학수업과 관련된 두뇌 기능에 대한 93편의 뇌과학 문헌들을 분석하였다. 두 번째로, 두뇌의 해부학적 영역별로 범주화된 연구결과를 바탕으로 과학 교수-학습프로그램 분석을 위한 분석틀을 제작하였다. 분석틀의 제작은 R & D 방법을 따랐다. 그 결과, 두뇌활성 결과들은 대뇌 피질, 보상계, 변연계의 세 영역으로 범주화되어 나타났다. 이를 바탕으로 개발된 두뇌기반 과학 교수-학습 프로그램 분석틀인 'CORE Brain Map'은 양측 배외측전전두피질, 양측 복외측 전전두피질, 양측 안와전두피질, 전대상이랑, 양측 두정피질, 양측 측두피질, 양측 후두피질, 양측 해마, 양측 편도체, 양측 측좌핵, 양측 선조체 그리고 중뇌영역으로 구성된다. 두뇌기반 과학 교수-학습프로그램 분석틀은 다양한 과학 교수-학습프로그램의 분석 및 진단에 활용 가능할 것으로 전망된다.

환도혈(GB30) 봉독 약침 자극이 백서의 신경병리성 동통 억제에 미치는 영향 (Effects of Bee Venom Acupuncture Injected at Hwando(GB30) on Neuropathic Pain in Rats)

  • Objectives : The purpose of this study is to examine if Bee Venom Acupuncture may be effective to the neuropathic pain(mechanical allodynia, cold allodynia) in a rat model of neuropathic pain. Methods : To produce the model of neuropathic pain, under isoflurane 2.5% anesthesia, tibial nerve and sural nerve was resected. After the neuropathic surgery, the author examined if the animals exhibited the behavioral signs of alloynia. The allodynia was assessed by stimulating the medial malleolus with von Frey filament and acetone. Three weeks after the neuropathic surgery, Bee Venom Acupuncture was injected at Hwando(GB30) one time a day for one week. After that, the author examined the withdrawl response of neuropathic rats' legs by yon Frey filament and acetone stimulation. And also the author examined c-Fos in the midbrain central gray of neuropathic rats and the change of WBC count in the blood of neuropathic rats. Results : The Bee Venom Acupuncture injected Hwando(GB30) decreased the withdrawl response of mechanical allodynia in BV-2, BV-3 group as compared with control group. The Bee Venom Acupuncture injected Hwando(GB30) decreased the withdrawl response of chemical allodynia(cold allodynia) in BV-2, BV-3 group as compared with control group. The Bee Venom Acupuncture injected Hwando(GB30) showed the significant difference between control group and BV-2 group, control group and BV-3 group in the c-Fos expression and U count. Conclusion : We have noticed that Bee Venom Acupuncture at Hwando(GB30) decreased mechanical allodynia and cold allodynia in the model of neuropathic pain compared with the control group. C-Fos expression in the central gray of that group was also decreased compared with the control group. Psin control using Bee Venom Acupuncture was accumulated as time goes by. This study can be used as a basic resource on a study and a treatment of pain.

서로 다른 뇌 부위 손상으로 인한 공격성에 대한 약물학적 연구 (Pharmacological Studies on Aggressive Behavior Induced by Three Different Regional Brain Lesions)

  • 본 연구는 측좌각, 봉선핵 및 후구 등 서로 다른 뇌 부위의 손상으로 야기된 공격성에 대한 약물의 효과를 비교 검토하여 다음과 같은 결과를 얻었다. 1. 측좌각, 봉선핵 및 후구 손상으로 야기된 muricide는 atropine으로 억제되었으나 hyperirritability는 억제되지 않았다. 2. 측좌각, 봉선핵 손상으로 인한 muricide는 L-DOPA, L-5-HTP에 의하여 현저히 억제되었으나 후구 손상으로 인한 muricide는 거의 억제되지 않았다. 3. 측좌각, 봉선핵 및 후구 손상으로 인한 muricide는 항우울제에 의하여 현저히 억제되었다. 특히 측좌각 및 봉선핵 손상으로 인한 muricide 억제에는 nomifensine과 clomipramine이 효과적이었으며 후구 손상으로 인한 muricide 억제에는 imipramine이 가장 효과적이었다. 그러나 hyperirritability는 항우울제에 의하여 거의 억제되지 않았다. 이상의 결과에서 측좌각 손상으로 인한 공격성의 약물학적 특징은 후구보다는 봉선핵 손상으로 인한 공격성과 유사하였으며 muricide 억제에 대한 항우울제의 효과가 서로 다른 것을 이용하여 항우울제의 작용평가에 도움이 될 것으로 사료된다.

위중(委中)(BL40)에 시술된 GaAlAs Laser와 침자가 신경병리성 동통에 미치는 영향 (Effects of GaAlAs Laser and Acupuncture Therapy at BL40 on Neuropathic Pain in Rats)

  • Objectives : We have studied the effects of GaAlAs (808 nm) low level laser therapy (LLLT) and acupuncture at BL40 on neuropathic pain in rats induced by lumbar spinal nerve 5 ligation. Methods : To produce the model of neuropathic pain, under isoflurane 2.5% anesthesia, the lumbar spinal nerve 5 was ligated by 6-0 silk thread. After neuropathic surgery, we examined if the animals exhibited the behavioral sign of allodynia. The allodynia was assessed by stimulating the medial malleolus with von Frey filament and acetone. Three weeks after the neuropathic surgery, GaAlAs (808 nm) low level laser and acupuncture was inserted at BL40 once a day for 6 days. We examined the withdrawal response of neuropathic rats' legs by von Frey filament and acetone stimulation. And also the author examined c-Fos, nociceptin and nociceptin receptor in the midbrain central gray of neuropathic rats. Results : The GaAlAs (808 nm) low level laser therapy and acupuncture at BL40 decreased the withdrawal response of mechanical allodynia that assessed with von Frey filament in LLLT group on 5 and 6 times and with acetone in AT group and LLLT on 6times. The LLLT and acupuncture at BL40 decreased the c-Fos protein expression in AT and LLLT groups. The 808 nm LLLT and acupuncture at BL40 decreased the nociceptin protein and nociceptin receptor protein in LLLT group. Conclusions : We have noticed that GaAlAs (808 nm) LLLT and acupuncture at BL40 decreased mechanical allodynia in the model of neuropathic pain. c-Fos, nociceptin and nociceptin receptor expression in the central gray of that group was also decreased. This study can be used as a basic resource on a study and a treatment of pain.

하행성 조절계 : 만성 통증에 대한 제어 작용 (Descending Controls: The Self-Regulation of Chronic Pain)

  • 하행성 억제계란 중뇌, 연수, 뇌교에 존재하는 해부학적 유해수용 조절성 기전을 일컫는 용어이다. 이들 부위를 전기적으로 자극을 하면 진통효과가 나타나며, 하행성 억제계의 실패시 지속적인 통증이 야기된다는 것을 알 수 있다. 또한 우울불안 같은 질환은 만성 신경병성 통증 상태로 쉽게 진행됨이 밝혀졌다. 이러한 요인들이 만성 신경병성 통증에 영향을 주는 경로는 아마도 하행성 억제계일 가능성이 있다. 흥미롭게도, 광범위하게 하행성 억제계가 작동하지 않을 경우 과민성 대장증상이 호발하는 것으로 보인다. 또한 이러한 환자들은 높은 불안, 우울 지수가 관찰되기도 한다. 다양한 연구에서, 하행성 억제계에 관여하는 ${\alpha}2$ 아드레날린성 약물, 아편유사약물들이 만성 통증에 사용될 수 있음을 동물에서 평가 중이다. 아직 신체내에서 얼마나 하행성 억제계가 일어나고 있는가에 대해서는 임상적으로 증명하기 힘든 감이 있지만, 여러 감각 신경기전의 수정에 중요한 구실을 하고 있는 것으로 믿어진다. 즉 중추신경계는 대상을 인식하기 위해 말초정보를 받아들이는 기능만 있는 것이 아니라 여러 방법으로 정보의 홍수를 조절하고 선택하는 기능을 동시에 갖추고 있는 것으로 생각된다.

대장(大腸)과 관련(關聯)된 경혈(經穴)들의 신경해부학적(神經解剖學的) 연구(硏究) (Neuroanatomical Studies on the Acupoints Related to the Large Intestine)

  • The purpose of this morphological studies was to investigate the relation between the meridian, acupoints and viscera using neuroanatomical tracers. The common locations of the spinal ganglia, sympathetic chain ganglia, spinal cord and brain projecting to the large intestine meridian were observed following injection of transganglionic tracer, WGA-HRP and transsynaptic neurotropic virus, pseudorabies virus(PRV), Bartha strain(Ba) and PRV-Ba-Gal (Galactosidase)) into the the large intestine(cecum, colon and rectum), ST37 and LI4. After survival times of 96 hours following injection into the thirty rats with WGA-HRP, PRV-Ba and PRV-Ba-Gal. They were perfused, and their spinal ganglia, sympathetic chain ganglia, spinal cord and brain were frozen sectioned($30{\mu}m$). These sections were stained by HRP and X-gal histochemical and PRV immunohistochemical staining method, and observed with a light microscope. The results were as follows : 1. WGA-HRP labeled neurons innervating the large intestine were observed bilaterally within the T13-L4 sympathetic chain ganglia, and T9-11 spinal ganglia. WGA-HRP labeled neurons innervating ST37 were observed within the L3-5 sympathetic chain ganglia, and L2-4 spinal ganglia. WGA-HRP labeled neurons innervating LI4 were observed in the middle cervical ganglion and stellate ganglion, and C5-8 spinal ganglia. 2. In spinal cord, PRV-Ba labeled neurons projecting to the large intestine, ST37 and LI4 were found in thoracic, lumbar and sacral spinal segments. Densely labeled areas of each spinal cord segment were founded in lamina N, V, VII(intermediolateral nucleus), Ⅸ, X and dorsal nucleus. 3. In medulla oblongata, PRV-Ba and PRV-Ba-Gal labeled neurons projecting to the large intestine, ST37 and LI4 were commonly found in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, dorsal motor nucleus of vagus nerve, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus and gigantocellular nucleus. 4. In pons, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in locus coeruleus, Kolliker-Fuse nucieus and A5 cell group. 5. In midbrain, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in central gray matter. 6. In diencephalon, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in paraventricular hypothalamic nucleus. These results suggest that PRV-Ba and PRV-Ba-Gal labeled common areas projecting to the large intestine may be correlated to that of the large intestine meridian, ST37 and LI4. Especially, These morphological results provide that interrelationship of meridian-acupoints -viscera may be related to the central autonomic pathways.

현종(懸鍾)($GB_{39}$).양릉천(陽陵泉)($GB_{34}$)의 전침(電鍼) 및 레이저침이 백서(白鼠)의 신경병리성(神經病理性) 동통(疼痛)에 미치는 영향 (Effects of Electro and Laser Acupuncture Treatment with $GB_{39}$ and $GB_{34}$ on Neuropathic Pain in Rats Induced by Tibial and Sural Nerve Ligation)

  • Objectives : We studied the effects of electro and laser acupuncture treatment with $GB_{39}$ and $GB_{34}$ on neuropathic pain in rats induced by tibial and sural nerve ligation. Methods : To produce the model of neuropathic pain, the tibial and sural nerves of rats were ligated by a 6-0 silk thread. Three days after the neuropathic surgery, only electro acupuncture(EA), electro acupuncture and 830 nm laser acupuncture(EA-LA-1), and electro acupuncture and 904 nm laser acupuncture(EA-LA-2) were treated with $GB_{39}$ and $GB_{34}$ twice a week for 8 weeks. We observed the withdrawal response of neuropathic rats' legs by von Frey filament and acetone stimulation. We also observed c-fos and nocieptin on the central gray area in the midbrain of neuropathic rats. Results : As we observed the effect of mechanical allodynia, the EA and EA-LA-1 groups in 5 and 6 weeks and the EA-LA-2 group in 6 weeks increased significantly compared with the control group. As for the effect of c-fos activity in the central gray region, the EA, EA-LA-1, and EA-LA-2 groups decreased significantly compared with the control group. The EA-LA-2 group increased significantly compared with the control group as regards the effect of nociceptin activity in the central gray region. Conclusions : We noticed the synergic effect of electro and laser acupuncture treatment because the EA-LA-1 and EA-LA-2 groups had more controllable effect compared with the control group. This study can be used in clinical therapy for neuropathic pain.

족소양담경(足少陽膽經)에서 투사(投射)되는 신경원(神經元)의 표지부위(標識部位)에 대한 연구(硏究) (Localization of the Neurons Projecting to the Gallbladder Meridian)

In medulla oblongata, The PRV labeled neurons projecting to the gallbladder, GB34 and common peroneal nerve were founded in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, rostroventrolateral reticular nucleus, medullary reticular nucleus, dorsal motor nucleus of vagus nerve, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus, gigantocellular nucleus, lateral paragigantocellular nucleus, principal sensory trigeminal nucleus and spinal trigeminal nucleus. 3. In Pons, PRV labeled neurons were parabrachial nucleus, Kolliker-Fuse nucleus and cochlear nucleus. 4. In midbrain, PRV labeled neurons were founded in central gray matter and substantia nigra. 5. In diencephalon, PRV labeled neurons were founded in lateral hypothalamic nucleus, suprachiasmatic nucleus and paraventricular hypothalamic nucleus. 6. In cerebral cortex, PRV labeled neuron were founded in hind limb area.This results suggest that PRV labeled common areas of the spinal cord projecting to the gallbladder, GB34 and common peroneal nerve may be first-order neurons related to the somatic sensory, viscero-somatic sensory and symapathetic preganglionic neurons, and PRV labeled common area of the brain may be first, second and third-order neurons response to the movement of smooth muscle in gallbladder and blood vessels.These PRV labeled neurons may be central autonomic center related to the integration and modulation of reflex control linked to the sensory system monitoring the internal environment, including both visceral sensation and various chemical and physical qualities of the bloodstream. The present morphological results provide that gallbladder meridian and acupoint may be related to the central autonomic pathways.
