Metatarsalgia means the pain under the lesser metatarsal heads. The many causes of metatarsalgia can be categorized into three groups: local disease in the region, altered forefoot biomechanics, and systemic disease affecting the region. Surgical options need to be considered if nonsurgical treatment fails. The metatarsal osteotomies are designed primarily to reduce the weightbearing forces on the metatarsal head by elevating or shortening the metatarsal. Many lesser metatarsal osteotomies have been described, and their success depends on many factors. Regardless of the method employed, it is important to maintain or restore the metatarsal cascade to maintain an even pressure under the lesser metatarsal heads and prevent transfer lesions. The surgeon must understand the effects of the metatarsal osteotomy on the forefoot patho-biomechanics and decide, using a combination of clinical examinations and imaging, whether the desired effect of the osteotomy is to shorten or elevate the metatarsal head or both.
Among congenital brachymetatarsias fourth metatarsal bone shortening is most common form and unusually involves multiple metatarsal bones. We report a case of the patient with bilateral 1st and 4th multiple rays brachymetatarsia treated with callostasis. Twenty eight months after procedure, right side showed 1st metatarsal lengthening about 11.5 mm (29.23%) and 4th metatarsal 17.86 mm (35.87%). Twenty one months later, left side showed 1st metatarsal lengthening about 14.58 mm (36.8%) and 4th metatarsal 20.52 mm (43.01%). In healing index right side showed 1st metatarsal 3.4 month/cm, 4th metatarsal 2.5 month/cm and left side showed 1st metatarsal 4.2 month/cm, 4th metatarsal 2.3 month/cm. Final follow-up results presented excellent outcome in cosmetic problem, easy wear of shoes, and measurement of patient's self satisfaction.
Brachymetatarsia is a rare congenital deformity which occurs most commonly in the fourth metatarsal. Most common purpose of surgery is to make a normal metatarsal parabola for better external appearance as well as physiological load bearing under the metatarsal heads. Common surgical treatment for brachymetatarsia is lengthening of short metatarsal either by one-stage lengthening with intercalary bone graft or gradual lengthening by distraction osteogenesis. Sometimes shortening of long metatarsal is combined with lengthening of the short metatarsal. The function of the foot is diminished due to diverse complication accompanying lengthening of the metatarsal, therefore preoperative consultation about the functional result is essential.
Purpose: The purpose of this report is to investigate the clinical and radiological results of corrective osteotomy of the 3rd metatarsal bone for shortening and dorsal displacement without exposure around neuroma. Materials and Methods: Twelve cases of patients who underwent corrective osteotomy of metatarsal bone for a Morton's neuroma from November 2013 to September 2014 were retrospectively reviewed. Corrective osteotomy was performed through a dorsal approach at the 3rd metatarsal bone base and distal metatarsal bone was displaced dorsally and proximally. Preoperative and postoperative pain assessed using American Orthopaedic Foot and Ankle Society (AOFAS) score and radiographs were evaluated. Results: The mean age of patients was 41.4 years, and the mean follow-up period was 10.7 months. AOFAS score improved from 52 preoperatively to 90 postoperatively. The 3rd metatarsal bone was shortened by an average of 3.39 mm and elevated by 2.38 mm. Conclusion: Corrective osteotomy of metatarsal bone can be regarded as a new surgical option for Morton's neuroma without exposure around neuroma.
Fractures located at the metaphyseal/diaphyseal junction at the base of the fifth metatarsal were first described by Sir Robert Jones in 1902. However, ever since, there has been disagreement and debate regarding the diagnosis, classification, pathomechanics, the incidences, and potential causes of delayed unions and nonunions, and the optimal method of treatment. It appears to be widely agreed that proximal fractures of the metaphyseal/diaphyseal region of the fifth metatarsal are prone to delayed union or even nonunion. Several classifications of proximal fifth metatarsal stress fractures have been devised. Torg et al. classified fractures involving the proximal part of the diaphysis of the fifth metatarsal into three types. The Torg classification is a good grading system that can be used to determine the type of surgery needed as well as for the prediction of prognosis. The ''plantar gap'' might add to the decision-making process for surgery and improve the prediction of patient prognosis. In addition, the new classification using 'plantar gap' might be used for classification of fifth metatarsal stress fracture. Fifth metatarsal stress fractures can be treated conservatively or surgically, and excellent results have been reported for surgery with rapid recovery in athletes. Intramedullary screw fixation has become a popular form of fixation for fifth metatarsal stress fractures. Bone grafting presents the problems of a longer recovery time and additional skin incision for harvesting. The modified tension band wiring is an useful and simple option for surgical treatment of challenging fifth metatarsal stress fractures.
The purpose of this study was to evaluate the effects of forefoot rocker shoes equipped with a metatarsal bar on lower extremity muscle activity and plantar pressure distribution. Ten healthy women in the age of twenties were participated in this study as the subjects. All subjects walked on a treadmill(Gait Trainer, BIODEX, USA) wearing normal shoes and metatarsal bar shoes, during which the plantar pressure distribution and muscle activity were measured. Using Pedar-X system(Novel Gmbh, Germany), the plantar pressure was measured for six regions of the foot: forefoot, midfoot, rearfoot, 1st metatarsal, 2-3th metatarsal, and 4-5th metatarsal, and for each sub-region, 4 features such as maximum force, contact area, peak pressure, and mean pressure were analyzed based on the plantar pressure. EMG(Electromyography) activity was measured by attaching surface electrodes to the rectus femoris, biceps femoris, tibialis anterior, and gastrocnemius medial head, and magnitude of muscle contraction was analyzed in IEMG(Integrated EMG) value. The results show that the maximum force, contact area, peak pressure, and mean pressure in the midfoot all increased while maximum force, peak pressure, contact area, mean pressure in the 1st metatarsal and 2-3th metatarsal all decreased when wearing functional shoes. Also, muscle activities in the four muscles were all decreased when wearing the functional shoes. This paper suggests that forfoot rocker shoes equipped with a metatarsal bar can help disperse the high pressure and absorb the shock to the foot as well as give positive influence on gait pattern and postural stability by reducing muscle fatigue during walking.
Avascular necrosis of the first metatarsal head is uncommon. It is most often seen following a distal metatarsal osteotomy for hallux valgus. Although many cases may be subclinical, it is a powerful cause of failure of bunion surgery. Avascular necrosis of the first metatarsal head results in a series of events that begins with phases of avascularity; revascularization, with or without collapse; and reossification. Collapse may result in degenerative change of the metatarsophalangeal joint. We have recently experienced a case of avascular necrosis of the first metatarsal head.
Purpose: To evaluate the clinical results of Mitchell osteotomy and proximal metatarsal dome osteotomy in hallux valgus deformity. Materials and Methods: From January 1993 to June 2000, 28 cases (17 patients) with hallux valgus deformity who underwent Mitchell osteotomy were categorized as group I, 26 cases (16 patients) who underwent proximal metatarsal dome osteotomy were categorized as group II. We analyzed clinical results according to preoperative and postoperative clinical functional analysis and objective comparison of correction angle between two groups. Results: The average hallux valgus correction in the Mitchell osteotomy group went from $36^{\circ}$ to $11^{\circ}$, and in the proximal metatarsal dome osteotomy group, the hallux valgus angle was reduced from $32^{\circ}$ to $6^{\circ}$. The intermetatarsal angle in the Mitchell osteotomy group was corrected from $13^{\circ}$ to $9^{\circ}$, and in the proximal metatarsal dome osteotomy group the intermetatarsal angle was reduced from $14^{\circ}$ to $7^{\circ}$. Although, proximal metatarsal dome osteotomy group have shown better correction angle and radiographic results in the correction of hallux valgus angle and intermetatarsal angle but, all patients in the Mitchell osteotomy and proximal metatarsal dome osteotomy groups had no statistically significant differences of clinical functional results between two groups. Conclusion: In our studies, the proximal metatarsal dome osteotomy applied to. hallux valgus deformity was found as a good radiographic results than Mitchell osteotomy, but there were no differences between the two operations in terms of functional satisfaction.
Purpose: The aim of this study was to evaluate the result of combined first metatarsal and calcaneal osteotomy for static cavovarus deformity of the foot. Materials and Methods: We performed a dorsal closing wedge $1^{st}$ metatarsal osteotomy and a lateral and upward displacement calcaneal osteotomy for 9 patients, 12 feet (6 male and 3 female). The mean age at the time of operation was 37 years and the mean followup period was 27 months. The causes of deformity were 2 poliomyelitis, 1 cerebral palsy, 1 Charcot-Marie-Tooth disease and 5 idiopathic type. Five lateral ligament reconstructions of the ankle and six percutaneous Achilles tendon lengthenings were added. The surgical results in terms of pain, function and alignment of the foot were evaluated by means of AOFAS ankle-hindfoot score and talo-$1^{st}$ metatarsal, calcaneus-$1^{st}$ metatarsal and calcaneal pitch angles were checked with weight bearing radiographs in lateral projection. Results: Talo-$1^{st}$ metatarsal and calcaneal pitch angles were reduced from the mean preoperative values of $21^{\circ}$ and $25^{\circ}$ to $12^{\circ}$ and $19^{\circ}$, respectively, at last followup. Also, calcaneus-$1^{st}$ metatarsal angle was increased from the mean $114^{\circ}$ to $114^{\circ}$. The mean AOFAS score was improved from 44.5 points preoperatively to 89.2 points at followup. There were 1 metatarso-cueiform joint nonunion, 1 sural nerve injury and 3 remaining symptomatic claw toes. Conclusion: Combined first metatarsal and calcaneal osteotomy appears to be an effective procedure for the treatment of adult static cavovarus foot.
Purpose: To evaluate the radiographic characteristics of the tarsometatarsal osteoarthritis with hallux valgus deformity and report the clinical results of the operative treatment. Materials and Methods: This is a retrospective study of 20 patients, 22 feet who had been operated for non-traumatic tarsometatarsal osteoarthritis with hallux valgus (TMT group) and control group of hallux valgus patients without tarsometatarsal osteoarthritis (26 patients, 28 feet) from April 2004 to July 2011. Radiographic characteristics were compared between the groups, using hallux valgus angle, $1^{st}-2^{nd}$ intermetatarsal angle, metatarsal length ratio, metatarsus adductus angle, talonavicular coverage angle, talus-$1^{st}$ metatarsal angle, calcaneal pitch angle and medial cuneiform height. Pre- and postoperative difference of $1^{st}-2^{nd}$ metatarsal declination angle and distance between the $1^{st}-2^{nd}$ metatarsal head were evaluated. The clinical results were evaluated by American Orthopaedics Foot and Ankle Society (AOFAS) midfoot scale and visual analogue scale (VAS). Results: Metatarsal length ratio was significantly larger in TMT group (p<0.001). Metatarsus adductus angle, talonavicular coverage angle, talus-$1^{st}$ metatarsal angle on lateral radiograph, calcaneal pitch angle and medial cuneiform height were different from control group (p<0.001, p<0.001, p=0.001, p=0.010, p=0.006). Postoperative declination of the $2^{nd}$ metatarsal and distance between the $1^{st}-2^{nd}$ metatarsal head were increased (p=0.009, p=0.001). The AOFAS and VAS score were improved (p<0.001, p<0.001). Conclusion: Non-traumatic osteoarthritis of the tarsometatarsal joints seems to be associated with long 2nd metatarsal length, metatarsus adductus and flatfoot deformity. Spur excision may be successful to relieve symptoms when the arthritis was diagnosed in early stage.
