Purpose: The purpose of this study is to identify the prevalence of the metabolic syndrome (MS) and factors associated with it among elders in a rural community. Methods: Data were collected from 683 subjects with a questionnaire, physical measurement, and blood test. The prevalence of the MS was determined by AHA/NHLBI and waist circumference cutoff points for Koreans. Results: The prevalence of the MS was 50.5% in total (41.6% in men, 56.3% in women) while the prevalence of 5 metabolic risk factors was 67.7% for elevated blood pressure, 51.0% for low HDL-cholesterol, and 50.2% for abdominal obesity. Risk factors associated with the prevalence of the MS included family history, BMI, and physical activity; significant factors associated with that of metabolic components included family history, BMI, smoking, drinking, and physical activity. Especially, a higher BMI was a strong risk factor of the prevalence of abdominal obesity as well as the MS and its components. Conclusion: The findings indicate that nurses should care for elders based on each metabolic component regarding its prevalence level and concentrate primarily on reducing elevated blood pressure, low HDL-cholesterol by controlling the main risk factor, abdominal obesity through lifestyle modification.
Background: The aim of this study was to explore the prognostic role of metabolic response to chemotherapy, determined by FDG-PET, in patients with metastatic non-small-cell lung cancer (NSCLC). Materials and Methods: Thirty patients with metastatic NSCLC were analyzed for prognostic factors related to overall survival (OS) and progression free survival (PFS). Disease evaluation was conducted with FDG-PET/CT and contrast-enhanced CT prior to and at the end of first-line chemotherapy. Response evaluation of 19 of 30 patients was also performed after 2-3 cycles of chemotherapy. Morphological and metabolic responses were assessed according to RECIST and PERCIST, respectively. Results: The median OS and PFS were 11 months and 6.2 months, respectively. At the end of first-line chemotherapy, 10 patients achieved metabolic and anatomic responses. Of the 19 patients who had an interim response analysis after 2-3 cycles of chemotherapy, 3 achieved an anatomic response, while 9 achieved a metabolic response. In univariate analyses, favorable prognostic factors for OS were number of cycles of first-line chemotherapy, and achieving a response to chemotherapy at completion of therapy according to the PERCIST and RECIST. The OS of patients with a metabolic response after 2-3 cycles of chemotherapy was also significantly extended. Anatomic response at interim analysis did not predict OS, probably due to few patients with anatomic response. In multivariate analyses, metabolic response after completion of therapy was an independent prognostic factor for OS. Conclusions: Metabolic response is at least as effective as anatomic response in predicting survival. Metabolic response may be an earlier predictive factor for treatment response and OS in NSCLC patients.
Objectives This study is to investigate the related factors to contribute the metabolic syndrome according to Sasang Constitution. Methods Nine hundred twenty six persons out of 1774 persons, over 40 years old, participated in community-based cohort in Wonju City of South Korea from June 2006 to August 2009. The diagnosis of metabolic syndrome was carried out by NCEP-ATP III(National Cholesterol Education Program in Adult Treatment Panel III) and Asian Pacific Criteria for abdominal obesity. The related factors were checked using questionnaire and blood samples. Sasang Constitution was verified by a Sasang Constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial pictures and simplified Sasang Constitutional questionnaires. Metabolic syndrome incidence rate according to Sasang Constitution and binary logistic regression analysis were performed with SPSS 19.0. Results Metabolic syndrome incidence rate was 30.3% and the majority of newly categorized as metabolic syndrome was Taeeumin(40.7%). There were significant risk factors like systolic blood pressure, fasting blood sugar, triglyceride and female and a significant defense factor like HDL-cholesterol. In terms of constitutional view, there were significant risk factors like waist circumference, systolic blood pressure, triglyceride in Soyangin, female, waist circumference, systolic blood pressure, fasting blood sugar, triglyceride in Taeeumin, female, waist circumference, systolic blood pressure, triglyceride in Soeumin. And there was a significant defense factor like HDL-cholesterol in only Taeeumin. Conclusions Regimens on metabolic syndrome were considered to be changed according to Sasang Constitution. Taeeumin female and Soeumin female should be cautious of body weight and metabolic syndrome when elderly. There are more cautious risk factors in each constitution; systolic blood pressure and triglyceride in Soyangin and fasting blood sugar and serum lipids levels in Soeumin and Taeeumin.
Background: The efficacy of ginseng, the representative product of Korea, and its chemical effects have been well investigated. The ginsenoside RG3 has been reported to exhibit apoptotic, anticancer, and antidepressant-like effects. Methods: In this report, the putative effect of RG3 on several cellular function including cell survival, differentiation, development and aging process were evaluated by monitoring each specific marker. Also, mitochondrial morphology and function were investigated in ultraviolet (UV)-irradiated normal human dermal fibroblast cells. Results: RG3 treatment increased the expression of extracellular matrix proteins, growth-associated immediate-early genes, and cell proliferation genes in UV-irradiated normal human dermal fibroblast cells. And, RG3 also resulted in enhanced expression of antioxidant proteins such as nuclear factor erythroid 2-related factor-2 and heme oxygenase-1. In addition, RG3 affects the morphology of UV-induced mitochondria and plays a role in protecting mitochondrial dysfunction. Conclusioin: RG3 restores mitochondrial adenosine triphosphate (ATP) and membrane potential via its antioxidant effects in skin cells damaged by UV irradiation, leading to an increase in proteins linked with the extracellular matrix, cell proliferation, and antioxidant activity.
Objectives In clinical studies, the visceral fat obesity has been emphasized because of its correlation with the metabolic syndrome. But the subcutaneous adipose tissue also would correlate with the risk factor of metabolic syndrome. Especially deep tissue, which is a subdivision of the subcutaneous adipose tissue would be more related. This study is to investigate the relationship between subcutaneous adipose tissue and various diseases. Methods We searched for papers which had subcutaneous adipose tissue, deep subcutaneous adipose tissue and obesity for subjects in the Pubmed site. Results : 24 papers were found. Subcutaneous adipose tissue, deep subcutaneous adipose tissue especially, was related with the insulin resistance, metabolic syndrome, sex hormones and other diseases. Conclusions Subcutaneous adipose tissue is a risk factor of insulin resistance but not lipoprotein. But deep subcutaneous adipose tissue was related with lipoprotein. So deep tissue, which is a subdivision of the subcutaneous adipose tissue is a more important risk factor of the metabolic syndrome.
We investigated the contribution of genetic variations of KLF5 to basal metabolic rate (BMR) and resting metabolic rate (RMR) and the inhibition of obesity in Korean children. A variation of KLF5 (rs3782933) was genotyped in 62 Korean children. Using multiple linear regression analysis, we developed a model to predict BMR in children. We divided them into several groups; normal versus overweight by body mass index (BMI) and low BMR versus high BMR by BMR. There were no differences in the distributions of alleles and genotypes between each group. The genetic variation of KLF5 gene showed a significant correlation with several clinical factors, such as BMR, muscle, low-density lipoprotein cholesterol, and insulin. Children with the TT had significantly higher BMR than those with CC (p=0.030). The highest muscle was observed in the children with TT compared with CC (p=0.032). The insulin and C-peptide values were higher in children with TT than those with CC (p=0.029 vs. p=0.004, respectively). In linear regression analysis, BMI and muscle mass were correlated with BMR, whereas insulin and C-peptide were not associated with BMR. In the high-BMR group, we observed that higher muscle, fat mass, and C-peptide affect the increase of BMR in children with TT (p < 0.001, p < 0.001, and p=0.018, respectively), while Rohrer's index could explain the usual decrease in BMR (adjust $r^2$=1.000, p < 0.001, respectively). We identified a novel association between TT of KLF5 rs3782933 and BMR in Korean children. We could make better use of the variation within KLF5 in a future clinical intervention study of obesity.
The prevalence of obesity in Korean children is estimated to be around 10%, and has increased significantly over the past 20 years. Metabolic syndrome, which includes central obesity, glucose intolerance, insulin resistance, dyslipidemia, and hypertension, is a well documented risk factor for cardiovascular diseases, but there is no general consensus concerning its definition for children. In this study, ablut 30-40% of overweight or obese children had metabolic syndrome compared to less than 10% in the general population. Dyslipidemia was the most common component and hyperglycemia was the least common component of metabolic syndrome. If the prevalence of childhood obesity continues to increase, it is highly likely that the prevalence of the metabolic syndrome will also increase. These findings emphasize the importance of preventing obesity in addition to the diagnosis and management of metabolic syndrome in Korean adolescents.
Objectives : The aim of this study was to investigate the prevalence of metabolic syndrome and the association between lifestyle factors and the risk of metabolic syndrome in Korean adolescents. Methods : The metabolic syndrome was defined with ${\geq}$ 3 metabolic abnormalities using the age-modified criteria of the Adult Treatment panel III specified by National Cholesterol Education Program in adolescents aged 12 to 18 years who have participated in the Kroean National Health and Nutrition Examination Survey in 1998(n=936) and 2008(n=620). Physical activity and dietary intakes were assessed. Results : The overall prevalence of metabolic syndrome in Korean adolescents declined over 10 years (P < 0.01). Adolescents engaging regular exercise significantly increased over 10 years (P < 0.001). Physically activity was associated with a lower risk of metabolic syndrome (P < 0.001). Dietary intake patterns were not associated with a risk of metabolic syndrome. Conclusions : The prevalence of metabolic syndrome among Korean adolescents declined over 10 years which may be in part due to the increased regular physical activity. The risk of metabolic syndrome is lower in individuals who are physically active.

Objectives : The purpose of this study was to find the prevalences and risk factors of the metabolic syndrome according to Sasang constitution. Methods : The medical records of 364 subjects who had taken health examinations and diagnosis of Sasang constitution from January to June of 2003 at a health examination center of a hospital in Seoul were reviewed. The prevalences and the risk factors of the metabolic syndrome according to Sasang constitution were compared and analyzed. Results : 1) Among the 364 subjects, 88 (24.2%) were Soeumin, 101 (27.7%) were Soyangin, and 175 (48.1%) were Taeumin. 2) The prevalences of high WC, high TG, low HDL-c, high BP and high FBS of Taeumin were significantly higher than those of the other constitutions (p-value < 0.05). 3) The prevalence of the metabolic syndrome of Taeumin, Soyangin, and Soeumin were 46.3%, 16.8% and 9.1% respectively. The rates were significantly different according to Sasang constitution (p-value < 0.001). 4) Taeumin had higher risk of high WC than Soeumin (adjusted OR : 3.83, 95% CI : 1.19-12.29) and higher risk of high FBS than Soeumin (adjusted OR: 5.93,95% CI : 1.11-31.77). 5) Taeumin had higher risk of the metabolic syndrome than Soeumin (adjusted OR : 3.40, 95% CI : 1.25-9.23). Conclusions : There were significant differences in the prevalence of metabolic syndrome according to Sasang constitution. Sasang constitution was identified as an independent risk factor of metabolic syndrome.
Purpose: The purpose of this study was to examine the effects of a Combined Exercise Program on Obesity and Metabolic Syndrome Factors for Chronic Psychiatric Inpatients. Methods: Thirty two subjects participated in this program who were admitted into a national mental hospital of C city. All measurements were done at baseline, 4 week, 8 week, and 12 week during the program. The effectiveness of the program was evaluated according to the change of body mass index, body fat, waist circumference, total cholesterol, high density lipoprotein cholesterol, triglyceride, and fasting blood sugar. Collected data was analyzed by frequency, percentage, and repeated measures ANOVA with SPSS WIN 19.0. Results: There were statistically significant differences in the body mass index (p=.002), body fat (p<.001), waist circumference (p<.001), triglyceride (p=.020), and fasting blood sugar (p=.008). However, there were no statistically significant differences in the total cholesterol, or high density lipoprotein cholesterol. Conclusion: The results of this study showed that a combined exercise program has positive effect on obesity and metabolic syndrome factor. Thus, a Combined Exercise program is recommended as an intervention to improve obesity and metabolic syndrome factors for chronic psychiatric inpatients.
