시각적 판단에 의한 얼굴유형 분류와 계측 특성 연구 (A Study on Women′s Face Types Classification by Visual Distinction and Difference from the Measurement)

  • The purpose of this study was to classify women's face types by visual distinction and to analyze the measurement of face types. A survey was conducted by subjects of 167 women's college students in Kwangju City and Chonnam area. Data were analyzed by Frequencies, Mean, one way ANOVA and Ducan's Multiple Range Test. The major results were as followed ; ·Women's face types were classified by 7 types and there were oblong shape(28.3%), egg shape(25.7%), round shape(23.9%), square shape(12.4%), inverted triangle shape(5.3%), diamond shape(3.5%), triangle shape(0.8%) in the subjects. ·From the measurements of the women's face, index of face length to face breadth was 1.38, it means that the index was different from the other refferences. And the lower face length was longer than the upper and the middle face lengths. ·Differences From those measurements like forehead breadth, face length/bizigion breath(p〈.001), bizigion breadth, bignathion slopper, stature(p〈.01) and trichion breadth, tragion-menton length(p〈.05) were significant in the classified face types.

  • This study was performed to establish the Cephalometric standards and growths change of Korean Teen aged group by McNamara's Analysis. The data from Cephalometric films of 161 male and 205 female subject aged from 10 to 18 years of Normal Korean were used for this study. The following results were obtained: 1. Means, standard deviation from measured values in each age group and sexual values were obtained. 2. Greater measurement stowed in male than female in Condylion to point A, Condylion to Gnathion, MaxilloMandible differential and ANS to mouton in linear measurement of 18 age group. 3. Condylion to point A, Condylion to gnathion, MaxilloMandible differential and ANS to Menton are increased as the age increased. 4. Facial axis angle and Mandible plane angle of 15 and 18 aged group showed less value than that of 10,11,12 age group. 5. There was no significant value changes of Lower incisor to A-Po, SNA angle and Lower pharynx by the increasing of age.

안면비대칭에 대한 주관적 인지도와 정모두부방사선사진 분석치의 연관성 (Relationship between Perception of Facial Asymmetry and Posteroanterior Cephalometric Measurements)

  • 본 연구는 한자의 비대칭 인지도에 영향을 미치는 정모두부방사선사진 계측치를 알아보기 위하여 시행하였다. 전남대학교병윈 교정과에 내원한 환자 중 정모두부방사선사진 촬영이 필요한 12세 이상의 환자 100명을 대상으로 "본인의 얼굴이 좌우 비대칭이라고 생각하고 있는가" 를 물어보아 비대칭 인지 정도에 따라 5개 군(아니다. 아니라고 생각한다. 모르겠다. 비대칭이라고 생각한다. 분명히 비대칭이다)으로 분류하였다. 각 대상자의 정모두부방사선사진 투사도에서 계관(crista galli)과 전비극첨 (anterior nasal spine)을 연결한 선을 기준으로 하여 비대칭 정도를 나타내는 9개의 선 계측치와 4개의 각도 계측치를 비교분석하여 다음과 같은 결과를 얻었다. 1. Menton의 변위와 상$\cdot$하악 정중선차이가 클수록 안면비대칭에 대한 본인의 인지도가 높음을 알 수 있었다. 2. "모르겠다" 라고 응답한 군의 모든 계측치는 "아니다", "아니라고 생각한다" 군과 통계적 유의차를 보이지 않았다. 3. "비대칭이라고 생각한다" 라고 응답한 군의 모든 계측치는 "분명히 비대칭이다" 군과 통계적 유의차를 보이지 않았다. 이상의 결과는 안면비대칭에 대한 환자 본인의 인지 정도를 물어보는 것이 교정진단에 도움이 됨을 시사하였다.

후진 이부성형술의 경조직과 연조직 측모 변화 예측에 관한 연구 (A Study on the Prediction of Hard and Soft Tissue Changes after Setback Genioplasty)

  • Purpose: The purpose of this setback genioplasty study is to develop a prediction method for the calculated osteotomy angle using horizontal and vertical changes as well as to evaluate the proportion of hard and soft tissue changes. Methods: Twelve patients who had received setback genioplasty with other maxillofacial surgery were examined. Three lateral cephalograms were taken just before surgery, immediately after surgery, and 3 months later surgery. A reference line was established to the reference point of the inner most point of the lingual symphysis cortex, incisor tip, and 2nd molar cusp tip. Measuring was conducted from pogonion (Pg), menton (Me), labrale inferius (Li), Mentolabial fold, soft tissue pogonion (Pg'), and soft tissue menton (Me') to the reference lines. Results: In setback genioplasty, the skeletal Pg moved posteriorly 5.07 mm. The ratios of soft tissue to hard tissue movement were 36% posteriorly and 62% inferiorly at Pg', 67% posteriorly and 104% inferiorly at Me', and 34% anteriorly and 164% posteriorly at Li. In reduction & setback genioplasty, skeletal Pg moved posteriorly 4.63 mm and skeletal Me moved superiorly 3.63 mm. The ratios of soft tissue to hard tissue movement were 76% posteriorly and 18% superiorly at Pg', 68% posteriorly and 42% superiorly at Me', and 44% anteriorly, 124% posteriorly at Li. The calculated mean slope angle, based on ${\Delta}H/{\Delta}V$ ratio, was 61.25 and the measured mean slope angle was 60.17. Thus, the calculated and measured slope angles have a similarity. Conclusion: In setback genioplasty, soft tissue moves posteriorly and inferiorly. In particular, at the Me' and Pg', the inferior movement of the soft tissue is greater than the posterior movement. Also, the predictable results (measured slope angle) after operation can be achieved by the calculated slope angle. Thus, the relationship of soft and hard tissue changes must be considered as the results are predictable.

방사선상 enhancement 정도에 따른 측모두부방사선규격사진 계측점 설정의 재현도 (Reproducibility of Lateral Cephalometric Landmarks According to Radiographic Image Enhancement)

  • 본 연구는 측모두부방사선규격사진의 디지털 영상을 enhancement하였을 때 계측점 설정의 재현도를 비교 평가하여 두부방사선사진 계측점 설정에 도움이 되고자 시행되었다. 10명의 측모두부방사선규격사진을 촬영하여 컴퓨터에 입력하고 Quick Ceph Image Pro$^{TM}$에서 gray-scale equalization number를 50으로, detail enhancement number를 50으로 설정하여 방사선상을 4단계까지 enhancement하였다. 5명의 조사자가 모니터 상에서 32개의 계측점을 설정하고 방사선상의 각 enhancement 단계에서 각 계측점에 대해 5명의 조사자가 설정한 점과 이 점들의 중심점간의 거리인 편위량으로 조사자간 계측점 설정의 재현도를 비교 평가하여 다음과 같은 결과를 얻었다. 1. Enhancement를 하지 않은 방사선상에서 조사자간 재현도는 계측점에 따라 다양하게 나타났다. 2. 방사선상 enhancement 따른 계측점의 편위량을 비교한 결과, 5개의 계측점에서 방사선상의 enhancement 단계간에 조사자간 계측점 설정의 재현도가 통계적으로 유의한 차이를 보였다. 3. 계측점 pterygomaxillary fissure는 enhancement 단계 1과 2의 방사선상에서, 계측점 Posterior nasal spine은 enhancement 단계 1의 방사선상에서, 계측점 menton은 enhancement 단계 2, 3과 4의 방사선상에서 조사자간 계측점 설정의 재현도가 enhancement하지 않은 영상에 비하여 높게 나타났다. 이상의 결과로 모니터 상에서 측모두부방사선규격사진의 계측점 설정시 디지털 방사선상의 enhancement를 통하여 일부 계측점에서 재현도를 증가시킬 수 있음을 알 수 있었다.

안면비대칭자의 하악골 악교정수술 후 정면 연조직 변화 예측을 위한 연구 (Prediction of frontal soft tissue changes after mandibular surgery in facial asymmetry individuals)

  • 본 연구는 안면비대칭 환자의 악교정수술 시 경조직 이동에 따른 연조직 변화를 정면에서 평가함으로써 정면 얼굴 이미지 시뮬레이션 프로그램 개발에 도움이 되고자 시행되었다. 안면비대칭이 동반된 하악골 수술 예정 환자 45명을 대상으로 정모 두부방사선규격사진과 얼굴사진(photo)을 술전 및 술후에 각각 같은 각도로 촬영한 후, 술전 및 술후의 방사선사진을 이용하여 경조직 계측점의 변화를, 얼굴사진을 이용하여 연조직 계측점의 변화를 수평 및 수직으로 구분하여 각각 측정한 후 경조직 변화에 따른 연조직 변화를 비교 분석하였다. 연조직 변화와 경조직 변화의 상관성을 살펴본 결과 수평 방향, 수직 방향 모두에서 전반적으로 낮은 상관성을 보였으며, 1 : 1 mean ratio 산출을 위하여 서로 상관성이 가장 높은 경조직 계측점을 연조직 계측점별로 선택한 결과 직하방에 있는 경조직보다는 다소 멀리 떨어져 있는 경조직 계측점이 선택되는 경우가 많이 나타났다. 경조직 변화를 이용하여 연조직 변화를 예측할 수 있는 회귀방정식을 연조직 계측점별로 산출한 결과 연조직 수평변화 예측에 경조직 수직변화도 사용되고 연조직 수직변화 예측에 경조직 수평변화도 사용되었으며, 수평과 수직변화 모두에서 가장 설명력이 높은 방정식은 연조직 menton에서 나타났다. 이상의 연구결과 하부 경조직 계측점과 상부 연조직 계측점의 비율을 이용하는 1 : 1 mean ratio 방법은 불가능한 것으로 나타난 반면 회귀분석을 이용한 연조직 변화 예측은 임상에 도움이 될 수 있는 것으로 나타나 정면 이미지의 경우 컴퓨터를 이용한 시뮬레이션 프로그램이 반드시 필요함을 시사하였다.

A comparative study of the deviation of the menton on posteroanterior cephalograms and three-dimensional computed tomography

  • Purpose: Facial asymmetry has been measured by the severity of deviation of the menton (Me) on posteroanterior (PA) cephalograms and three-dimensional (3D) computed tomography (CT). This study aimed to compare PA cephalograms and 3D CT regarding the severity of Me deviation and the direction of the Me. Materials and Methods: PA cephalograms and 3D CT images of 35 patients who underwent orthognathic surgery (19 males and 16 females, with an average age of $22.1{\pm}3.3years$) were retrospectively reviewed in this study. By measuring the distance and direction of the Me from the midfacial reference line and the midsagittal plane in the cephalograms and 3D CT, respectively, the x-coordinates ($x_1$ and $x_2$) of the Me were obtained in each image. The difference between the x-coordinates was calculated and statistical analysis was performed to compare the severity of Me deviation and the direction of the Me in the two imaging modalities. Results: A statistically significant difference in the severity of Me deviation was found between the two imaging modalities (${\Delta}x=2.45{\pm}2.03mm$, p<0.05) using the one-sample t-test. Statistically significant agreement was observed in the presence of deviation (k=0.64, p<0.05) and in the severity of Me deviation (k=0.27, p<0.05). A difference in the direction of the Me was detected in three patients (8.6%). The severity of the Me deviation was found to vary according to the imaging modality in 16 patients (45.7%). Conclusion: The measurement of Me deviation may be different between PA cephalograms and 3D CT in some patients.

Comparative analysis of craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders: a cross-sectional study

  • Objectives: The aim of the study was to quantify and compare craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders (TMDs). Materials and Methods: A total of 126 adult subjects were categorized into two groups (63 with a TMDs and 63 without a TMDs), based on detection of symptoms using the Temporomandibular Joint Disorder-Diagnostic Index (TMD-DI) questionnaire. Posteroanterior cephalograms of each subject were traced manually and 17 linear and angular measurements were analyzed. Craniofacial asymmetry was quantified by calculating the asymmetry index (AI) of bilateral parameters for both groups. Results: Intra- and intergroup comparisons were analyzed using independent t-test and Mann-Whitney U test, respectively, with a P<0.05 considered statistically significant. An AI for each linear and angular bilateral parameter was calculated; higher asymmetry was found in TMD-positive patients compared with TMD-negative patients. An intergroup comparison of AIs found highly significant differences for the parameters of antegonial notch to horizontal plane distance, jugular point to horizontal plane distance, antegonial notch to menton distance, antegonial notch to vertical plane distance, condylion to vertical plane distance, and angle formed by vertical plane, O point and antegonial notch. Significant deviation of the menton distance from the facial midline was also evident. Conclusion: Greater facial asymmetry was seen in the TMD-positive group compared with the TMD-negative group. The mandibular region was characterized by asymmetries of greater magnitude compared with the maxilla. Patients with facial asymmetry often require management of temporomandibular joint (TMJ) pathology to achieve a stable, functional, and esthetic result. Ignoring the TMJ during treatment or failing to provide proper management of the TMJ and performing only orthognathic surgery may result in worsening of TMJ-associated symptoms (jaw dysfunction and pain) and re-occurrence of asymmetry and malocclusion. Assessments of facial asymmetry should take into account TMJ disorders to improve diagnostic accuracy and treatment outcomes.

QSCC II 체질과 두면부 계측항목의 연관성 분석 (Analysis of the Relationship between Sasang Constitution and Measuring Items of the Head and Face)

  • The objectives of study are to investigate the relationship between Sasang Constitution(四象醫學) and measuring items of the head and face and to develop useful diagnosis standard of Sasang Constitution. Subjects took QSCCII(Questionnaire for the Sasang Constitution Classification II) tests repeatedly at an interval of one year and were diagnosed as same constitutions. The 9 Items were measured by Martin-type anthropometric method in 112 subjects. Compared with measurements of 3 constitutions, the mean values in Taeeum-in(太陰人) showed highest and there were significant differences in 7 items between Taeeum-in(太陰人) and other constitutions. Especially, Measurements of Bitragus to Submandibular Arc Length(BSAL) item and Bitragus to Menton Arc Length(BMAL) item showed significant differences between Taeeum-in(太陰人) male and other constitutions male. It seems that BSAL and BMA items can differentiate Taeeum-in(太陰人) from other constitutions and these items have possibility to be a diagnosis standard of Sasang Constitution.

Evaluation of buccolingual molar inclinations among different vertical facial types

  • Objective: The aim of this study was to compare the buccolingual inclination of maxillary and mandibular molars in adults with different vertical facial types. Methods: Cone-beam computed tomography images of 135 adult patients (age, 20-45 years) with skeletal Class I maxillomandibular relationships were assigned to normodivergent (n = 46), hypodivergent (n = 49), and hyperdivergent groups (n = 40) according to linear and angular sella-nasion/gonion-menton measurements. The normodivergent group consisted of 24 females and 22 males, hypodivergent group of 26 females and 23 males, and hyperdivergent group of 24 females and 16 males. Buccolingual inclination of the maxillary and mandibular first and second molars was measured relative to the occlusal plane. One-way analysis of variance was used for intergroup comparison. Gender differences were evaluated using independent t-tests. Results: Buccolingual molar inclinations did not differ significantly between females and males (p > 0.05). There were no statistically significant differences among the buccolingual inclinations of the first and second maxillary and mandibular molars of the groups (p > 0.05). Conclusions: Buccolingual inclinations of maxillary and mandibular molars are similar in normodivergent, hyperdivergent, and hypodivergent adults with Class I sagittal relationships.
