심층면담을 통한 난임 여성의 스트레스 양상에 관한 연구 (Stress Patterns in Women with Infertility: an In-depth Interview Study)

Objectives: To evaluate stress patterns and coping abilities in women with infertility by conducting in-depth interviews. Methods: Ten women with infertility who visited the Korean Medicine Hospital and provided consent after being informed of the purpose and method of the study were selected after meeting the inclusion/exclusion criteria. They were requested to complete a preliminary questionnaire developed by the researchers, the Infertility Stress Scale, and the Korean version of the Fertility Problem Inventory (FPI). Subsequently, each participant was interviewed individually. Results: The preliminary questionnaire was used to evaluate sensitivity to each type of infertility-related stress and ability to express and resolve it. Among all infertility stress types, the largest proportion, accounting for an average of 47.5±26.95%, was that felt by the patient herself. Considering stress awareness intensity, the stress felt by the patient was the highest, with an average score of 4.30±0.64. Relative stress sensitivity due to infertility was the highest, with an average score of 3.90±0.94. Compared with general work stress, the average ability to relieve stress related to problems with spouses was the highest, with a score of 2.50±1.20. The average Infertility Stress Scale score was 2.88±1.35 and FPI score was 2.87±2.52. Conclusions: The highest stress scores were observed for the following items: meaning of children, need for parenthood, and stress due to the diagnosis of infertility. The lowest stress scores were allocated to the item concerning relationships with friends and co-workers. Based on the in-depth interviews conducted after the survey, stress in women with infertility may be classified as emotional, physical, and economic. Thus, the requirement for providing appropriate psychological and emotional support depending on the stress type in addition to general medical treatment for infertility treatment was confirmed.
Objectives: The purpose of this study was to suggest the clinical characteristics and risk factors of facial palsy from the perspective of Korean Medicine. Methods: Medical records of 856 patients, who visited the Korean Medicine hospital with facial palsy from 2004 to 2019, were retrospectively analyzed. The clinical characteristics of facial palsy were suggested by figuring out the distribution of age, sex, occupation, onset season, obesity, modes, signs, symptoms, past and family history. Results: By gender, there were more males (54.6%), and the most common age groups were in their 50s (27.4%) and 40s (23.6%). As for the occupation, unemployed (27.7%) and service and sales workers (14.6%) were the most common, and the onset season was the most common in winter (28.9%). The most common clinical characteristics were overwork (33.0%), mental stress (24.8%), and exposure to cold (19.3%) in modes, 'none' (64.3%), postauricular pain (33.2%) in signs, postauricular pain (26.9%), parageusia (14.8%) in symptoms. Hypertension and diabetes were the most common in both past and family history. In particular, the obesity rate of the subjects (59.8%) was much higher than that of Koreans (32.8%). Conclusions: Based on the historical Korean Medicine literature and the results of this study, it is suggested that overwork, stress, exposure to cold, and obesity are identified as risk factors for facial palsy. Furthermore, dysfunction of the stomach meridian is thought to contribute to the cause of facial palsy.
The aim of this study is the development of a fee - based model day care center for the elderly by inquiring into the current condition of facilities in America and in Korea, and in surveying the opinion of domestic elderly about day care facilities. A field trip to U.S. day care services was held between July 5 and July 15 in 1997, and an on-the-spot study for domestic facilities took place during March in 1998. Our research reveals that the overall supply of day care facilities can not meet future demand in terms of quality and quantity. Therefore a model must be created for day care centers of a that consists of a director from a professional group. an adequate environment, and a standardized in order to offer a qualified public health service linked to the home and community in Korea. The director of a day care center is a critical variable in determining the quality of service. Professional skills related to the needs of the elderly and the person's quality of service should be considered in appointing director for the center. This study belleves that a professional nurse should be the director of a day care center. The operating environment of a day care facility should be made up of considerable space comparable to the number of residents, should be in a comfortable and safe location, and should have equipment that provides a qualified, safe service to the elderly. Our model is designed for 20 persons and allocates 4 Peng per person. This model is comprised of a reading room. a craft room, a health room, a room for physical therapy, a dining room, a staff office, and a multi -purpose room connected to other rooms. Day care service should be a comprehensive service program meeting the multidimensional needs of the elderly. A comprehensive service program needs a team of various professionals made up of the elderly family, participants, nurses, social workers, physical therapists, nutritionists, and medical doctors. The program will also include health care service, physical therapy, speech therapy. diet, occupational therapy, transportation service, health and an education program, etc. In conclusion, a model of a day care center is developed with the following components: a professional director and an environment and program, that considers the physical, mental, and social characteristics of the elderly. A model should also motivate self-reliance self-fulfillment in the elderly in order to fulfill their health needs and to prevent isolation from society and mental depression. Furthermore, This facility will be a beneficial factor in reducing a family's burden on caring for the elderly that includes unnecessary hospital expenses. The following is a suggestion based on results this study: A service program should be developed to fit the conditions of the elderly in Korea by specifically analyzing the needs of the elderly.
목 적:학대아동보호팀은 아동학대 의심 사례를 평가/판정하고, 이를 아동보호전문기관이나 경찰에 신고하는 역할을 하는 병원 내 기관이다. 연구자들은 1개 병원 학대아동보호팀에서 20년간(1987-2007) 경험한 76건의 사례를 분석하여 학대 아동의 현황과 후유증을 알아보고자 하였다. 방 법:학대아동보호팀의 사례 기록과 의무기록, 아동보호전문기관의 기록을 함께 분석하였으며, 별도 제작한 양식에 맞추어 현재 보호자들과 사회복지사의 인터뷰를 통해 후유증을 조사하였다. 결 과:총 76건(남아 28명, 여아 48명)의 사례 중, 연령별로는 1세 미만이 6명(7.9%), 1-3세 10명(13.2%), 3-10세 41명(53.9%), 10세 이상 19명(25.0%)이었다. 학대 유형별로는 방임이 9건(11.8%), 성학대 27건(35.5%), 신체학대 44건(57.9%)이었다. 가해자는 친부모가 절반 이상이었으며, 학대 장소는 가정 내가 41건(53.9%)으로 가장 많았다. 총 24명(31.6%)의 아동들에 대하여 후유증에 대한 답변을 받았으며, 평균 추적 기간은 58.5개월이었고 평균 연령은
VDT는 Video Display Terminals의 약어로 시각 표시 단말기를 뜻한다. 최근 핵의학과에 VDT가 대량 보급되면서 점차적으로 사용자들이 늘어남과 동시에 사용 기간이 급증함에 따라 VDT 증후군에 관심이 높아지고 있다. 본 연구는 서울대학교병원 핵의학과를 대상으로 VDT 유해 요인 평가 도구를 적용하여 그 실태를 조사하고 문제점을 발견하여 향후 개선에 관한 방안을 도출함으로써 VDT 증후군에 관한 의식을 정립하고 궁극적으로 예방하는 데 목적이 있다. 서울대학교병원 핵의학과를 본원 핵의학과, 소아 핵의학과, PET 센터의 3개 파트로 분류하고, 2009년 4월 한 달 동안 현재 근무 중인 방사선사 23명을 대상으로 근무 부서를 직접 방문하여 유해 요인을 평가하였다. 평가에 사용한 도구는 원진 노동환경건강연구소의 "VDT작업에 관한 체크리스트"를 적용하였고 VDT 작업 조건, 업무 책상의 조건, 의자의 조건, 키보드의 조건, 모니터의 조건, 작업 자세, 보건 관리 특성, 기타 작업 환경의 8개 부문, 총 55개 항목을 통하여 현장 조사 및 평가하였으며 이에 대한 분석 결과는 한림대학교 성심병원 산업의학과에서 검증하였다. 서울대학교병원 핵의학과의 VDT 작업 조건은 비교적 양호한 수준이었다. 책상의 경우 최근 도입한 경우에는 인체공학적 설계로 사용자에게 적합하였으나 기존 책상의 15%는 기준치에 미달하였다. 의자의 경우는 기준에 적합하였지만 노후화로 인하여 적정 기능을 상실한 것이 5%였다. 키보드는 98%가 기준에 적합하였고 모니터는 화면의 각도 조절은 모두 가능하였지만 위치 조절이 불가능한 것이 38%로 조사되었다. 작업 자세의 경우 부적절한 자세로 장시간 노출되는 경우가 10%였으며 일부 항목에서 기준에 적합하지 않은 것으로 조사되었다. 보건 관리 측면에서도 개선의 여지가 있는 것으로 나타났다. 또한 조명과 온도, 소음, 환기 등 기타 작업 환경은 일부 항목에서 문제점이 발견되었지만 권고치를 충족하였다. 핵의학과 업무는 과거에 비하여 육체적인 업무는 줄었으나, 장시간 정적인 자세로 고도의 정밀도와 집중력을 요구하게 되었으며 이는 필연적으로 VDT 증후군을 유발하는 계기가 되었다. VDT 증후군은 앞으로도 지속적으로 발생할 가능성이 높으며 개선에 대한 경제적 비용이 상당하다는 점과 다양한 발병 요인을 내재하며 본인도 모르는 사이에 발생한다는 점에서 보다 구체적이며 효율적인 관리 체계가 필요할 것으로 판단된다. 핵의학과의 VDT 증후군은 인체공학적 사무 환경의 개선, 업무 절차(procedure)의 개선과 업무의 효율화, 직원간의 협동작업(teamwork) 그리고 규칙적인 운동, 꾸준한 스트레칭 체조와 같은 사용자 스스로의 노력, 지속적인 관심과 작은 노력으로 충분히 개선될 수 있으며 적극적으로 예방할 수 있을 것으로 사료된다. 이것은 쾌적한 업무 환경속에서 사용자의 육체적, 정신적 상태를 최상으로 이끌고 업무 효율성의 증대와 함께 내부 고객 만족도 상승으로 이어질수 있을 것으로 판단된다.
According as the automation of clerical work(OA ; Office Automation) develops, the use of VDT(Visual or Video Display Terminal) is increasing suddenly. But, in proportion to the spread of office automation(OA tendency), the self-conciousness syptom attendant upon the work is appearing also (Kim, Jung Tae, Lee, Young Ook, 1990). The apparatuses of office enable the clerical workers to be convenient and perform mass businesses. But, they are increasing the opportunity to be exposed to VDT syndrom, techno stress, computer terminal disease, pain by muscle strain(RSI), bradycausia of noise nature, and electromagnetic waves, etc. which are referred to as the new type of occupational diseases to the workers. It is the real situation that the workers to use VDT is complaining of the physical inconvenience sense in the recent newspaper and literature, it is the point of time that the sydrome to come from VDT use and computer terminal disease, etc. must be classified into the occupational disease(Lee, Kwang Young 1990, Lee, Kyoo Hak 1990, Lee, Won Ho 1991, Lee, Si Young 1991, Lee, Joon 1991, Choi, Young Tae 1991, Heo, Seung Ho 1989). In addition, it is the real situation that the scientifitic study result about the scope that electromagnetic waves has influence on the human body has not been suggested yet, and criticism on the stable exposure permission standard about electromagnetic waves to be emitted from VDT and on the problem in the health about electromagnetic waves is continuing. (IEEE Spectrum, 1990). In addition according to the experience of nursery business of industry field, it is the real situation that the patients who consult complaining of physical and mental inconvenience sence, among the users of apparatus of office automation, are reaching 10% of the patients coming to doctor's room. Therefore, it is necessary to confirm the self-consciousness symptom that the clerical workers complain of multilaterally with the actual state examination about the use of the apparatuses of offices automaton. Thus, this study was tried as th basic data for the cosultation and education for the maintenance and furtherance of the health of workers as the nurse of industry field, by confirming the contents of self-consciousness symptom attendant upon the use of the apparatus for office outomation making the financial institution in which the spparatus for office automation in most frequently used as the subject, and by examining whether there is the difference according to the subject of study, the data were collected, by using the questionnaire method, making 200 workers who consented to the study participation as the subject, among the persons who have spent over 3 months since they used the apparatuses for office automation and didn't receive the treatment in hospital due to the clerical disease for recent 3 years. The period of data collection was from Oct. 9, 1991 to Oct. 12. As for the measurement instrument about the complaint if self-consciousness symptom attendant upon the use of apparatuses fo office automation, the question item on the complaint symptom of health problem attendant upon the treatment of VDT that Kim(1991) developed and on CMI health problem and the question items on the fatigue degree due to industry were used by previous examination to 25 persons. Collected data were analyzed with the statistical method such as percentage, arithmetic mean, Person correlation coeffient, Kai square verfication, t-test, ANOVA, etc. by using SPSS/PC+ program, and the result is as follows : 1. The self-consciousness symptom that the clerical workers complained of most frequetly appeared high in 'My eyes are tired'(99.4%), 'I feel fatigue and weariness'(99.4%), 'I feel that my head is heavy5(90.0%), 'eyesight fell'(88.8%), 'I have a stiff neck'(88.8%), 'I fell pain in the shoulder'(85.0%), 'I feel cold and painful in the eyes'(76.9%), 'I feel the dry sense of eyeball'(76.2%), 'My nerves are edgy, and I an fretful, (75.6%), 'I feel pain in the waist'(73.2%) and 'I fell pain in the back'(72.8%). It emerged that the subject use the apparatuses for office automation complained of self-consciousness symptoms related to visual symptoms and musculoskeletal symptoms. 2. As for the general feature of examination subjects, the result to see the distribution by classifying into sex, age, school career, use career of apparatuses for office automation, skillfulness degree of the use of apparatus for office automation, use hours of the apparatuses for office automation per 1 day, type of business of the apparatus for office automation, rest hours during the use of apparatus for office automation, satifaction degree of business of office automation, and work circumstance, etc. emerged as follows : As for the sex of subjects, the distribution showed that men were 58.8% and women were 41.3%, Age was average 26.9. As the distribution of school career, the distribution showed that4below the graduation of high school' was 58.8%, 'graduation from junior college-university' was 35.0%, and 'over graduate school' was 6.3%. In the question to ask the existence or non-existence of experience of health consultation in connection with the work of office automation, the response that I had the consultation exprience and I feel the necessity emergerd as 90.1% And, the case that the subject who didn't wear the glasses or lens before using the OA apparatus wear glasses or lens after using OA apparatus emerged as 28.3% of whole. As for the existence or non-existence of use career of OA apparatus, the case under 3 years was highest as 52. 7%. As for the skillfulnness degree about the use of apparatus for office automation, most of them are skillful with the fact that 'common' was 44.4%, 'skill' was 42.5%, and 'unskillful' was 13.1% As for the use average hours of the apparatus for office automation per 1 day, the distribution showed that the case under 3-6 hours was 33.1%, the case under 6-9 hours was 28.1%, the case under 3 hours was 30.6%, and the case over 9 hours was 8.1% Main OA business and the use hours for 1 day showed in the order of keeping and retrieval, business of information transmission(162min), business of information transmission(79.3 min), business of document framing(55.5 min), and business of duplication and printing(25.4min). as for the rest during the use of apparatus for affice automation, that I take rest occasion demands the major portion, but that I take after completing the work emerged as 33.8%. Though the subiness gets to be convenient by the use of the apparatus for of office automation, respondents who showed the dissatisfaction about the present OA business emergd high as 78.1%. The work circumstances of each office was good with the fact that the temperature of office was 21.8, noise was average 42.7db, and the illumination was average 364.4 lx, in the light of ANSi/HFS 100 Standard. 3. Sight syptom, musculoskeletal symptom, skin and other symptoms showed the significant difference according to the extent of skillfulness of the apparatus for office automation. All the symptoms exept skin symptom showed the difference according to the use hours of the apparatus for office automation. All the question items exept the sytoms of digestive organs and the rest hours during the apparatus for office automation showed the signicant difference. The question item which showed the signicant difference from the satisfaction degree of present OA business showed the significant difference from all the question item classified into 6 groups. But, age and school career didn't significant difference from the complaint of any self-consciousness symptoms.
