Estimation of remodeling of small-scale houses to improve the residential environment for the underprivileged - case study on the TV program 'Love House' for free residential remodeling - (소외계층의 주거환경개선을 위한 소규모 주택 리모델링에 관한 평가연구 - TV 방송 무료주택 리모델링 프로그램 '러브하우스' 사례조사를 중심으로-)

  • The objective of this research is not only making the planning guides regarding a small-scale residential remodeling but also examining closely the social roles of the interior design for the poor and the underprivileged class by analyzing the residential remodeling cases. The scope of research includes 40 remodeling houses free of charge which are broadcasted in the TV program 'Love House'. Taking in consideration the characteristics of users like parentless or single-parent families and families with disabled persons, this study uses the spatial approach, user's convenience approach and psychological problem solving approach, in terms of the methodology. In result of the analysis of their interior design, first of all, it is essential solving the problem of limited house space by comparison with the family number. Second, the universal design concept for the family with disabled persons(72.5%), old persons or children is strengthened guaranteeing privacy in small and old interior space. Third, the interior design is focused in increasing communication among family members, because mental and physical obstacle would obstruct accessibility among them. This research shows the importance of the interior design which makes wide social roles contributing to create "welfare society".

DENTAL TREATMENT USING TRIAZOLAM FOR THE MENTAL RETARDATION PATIENT : A CASE REPORT (Triazolam을 이용한 지적 장애인 환자의 치과 치료 : 증례보고)

  • Mental retardation is one of the major disorders covered in special care dentistry. Patients show poor oral health and are a challenge for the dentist due to lack of cooperation. Triazolam is a short-acting benzodiazepine and has been used in the dental office for many years because of its favorable properties. In this study, five patients with mental retardation were sedated with triazolam on a dental out-patient setting. Patients were administrated with 0.25 mg or 0.5 mg of triazolam on the first dose according to the patient's cooperation. Twenty minutes after the initial dose, patients received 0.25 mg of triazolam. Triazolam was administrated sublingually to one patient and orally to four patients. Hydroxyzine was also administrated to three patients in order to prevent nausea and vomiting. All patients were inhaled with 50% nitrous oxide during the sedation. Treatment time ranged from 20 minutes to 55 minutes. Two patients showed paradoxical reactions and were administrated with intramuscular injections of flumazenil. In result, depending on the range of dental treatment and level of intellectual disability, we can assume that triazolam may be beneficial for sedation in disabled patients.

A SURVEY OF GENERAL ANESTHESIA IN PEDIATRIC DENTAL CLINIC AT PUSAN NATIONAL UNIVERSITY (부산대학교병원 소아치과에서 시행한 전신마취에 대한 연구)

  • The use of general anesthesia as a special method of behavior management is necessary if certain handicapped or disabled child patient to receive dental treatment. This study was designed to report the results of 53 cases of complete oral rehabilitation under general anesthesia. The data were obtained from patients who were provided with dental treatment under general anesthesia for last 3years managed at the Dept. of pediatric dentistry in PNU Hospital. The distribution of age, gender, primary reason for general anesthesia, duration of dental procedure, number of treated tooth and periodic recall check-up were surveyed. In distribution of age, most(78%) were younger than 10 years and mean was 13.0 years. The reasons for providing general anesthesia were lack of cooperation due to various mental and physical handicapped situation(74%), congenital heart disease(13%), combined with medically compromised and behavior problem and others. The average duration of the treatments was 2 hours and 41 minutes and average duration of the anesthesia was 3 hours and 6minutes. The mean number of treated with restoration a children were 16.7 teeth. From the results, total dental rehabilitation under general anesthesia is a favorable modality to improve for disabled children's oral condition.

Prognosis after treatment with multiple dental implants under general anesthesia and sedation in a cerebral palsy patient with mental retardation: A case report

  • Cerebral palsy is a non-progressive disorder resulting from central nervous system damage caused by multiple factors. Almost all cerebral palsy patients have a movement disorder that makes dental treatment difficult. Oral hygiene management is difficult and the risks for periodontitis, dental caries and loss of multiple teeth are high. Placement of dental implants for multiple missing teeth in cerebral palsy patients needs multiple rounds of general anesthesia, and the prognosis is poor despite the expense. Therefore, making the decision to perform multiple dental implant treatments on cerebral palsy patients is difficult. A 33-year-old female patient with cerebral palsy and mental retardation was scheduled for multiple implant treatments. She underwent computed tomography (CT) under sedation and the operation of nine dental implants under general anesthesia. Implant-supported fixed prosthesis treatment was completed. During follow-up, she had the anterior incisors extracted and underwent the surgery of 3 additional dental implants, completing the prosthetic treatment. Although oral parafunctions existed due to cerebral palsy, no implant failure was observed 9 years after the first implant surgery.

  • The patient who has congenital hypothyroidism and pseudohypoparathyroidism could have mental retardation even though adequate hormone treatment and cannot endure conventional dental treatment. In this case, general anesthesia is selected to administer effective dental treatment. But, there could be symptoms such as anemia, neuropathy, associated pituitary or adrenal hypofunction, cardiac failure even in euthyroid state. And, bradycardia, mental dullness, hypothermia, slow reflexes can appear in case of inadequate thyroid hormone replacement. Especially, macroglosssia, slow drug metabolism, exaggerated responses to anesthetic agents and decreased ventilatory responses could be problem during general anesthesia. The presentation of hypoparathyroidism also varies depending on the chronicity of the result of hypocalcemia. Muscle spasms/tetany, paresthesias, and seizures may occur in an acute onset. Chronic hypocalcaemia causes fatigue, muscle cramps, lethargy, personality changes, and cerebration defects.

Mediating Effect of Therapeutic Relationship with Mental Health Professionals in the Relation between Empowerment and Recovery of Community Dwelling Patients with a Mental Illnesst (지역사회 정신장애인의 임파워먼트와 회복 : 치료적 관계의 매개효과)

  • This study was conducted to identify the mediating effects of the therapeutic relationship with mental health professionals between empowerment and recovery in mental illness. The participants were 168 adults in two community mental health centers and three rehabilitation centers in Jeonbuk. The data were collected by structured questionnaires from April 15 to April 30, 2015 and analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis with the SPSS 19.0 statistical program. The mean score of empowerment for mental illness, recovery and therapeutic relationship were $60.4{\pm}10.85$, $101.8{\pm}20.70$ and $74.5{\pm}14.30$, respectively. There were significantly positive correlations between recovery in empowerment and therapeutic relationship (r=0.33, p<0.001), as well as empowerment and recovery (r=0.51, p<0.001), while there were significantly positive correlations between therapeutic relationship and recovery (r=0.55, p<0.001). Therapeutic relationship had a partial mediated effect between empowerment and recovery, which was found to be significant by the Sobel test (z=3.706, p<0.001). The results of this study can be used as basic data for development of a practical recovery-enhancing program to promote therapeutic relationships and improve empowerment in relation to therapist of the mentally disabled.

THE RELATIONSHIP BETWEEN ORAL STATUS AND DENTAL CARIES ACTIVITY ON MENTAL RETARDATION STUDENTS (정신지체 학생의 구강실태 및 우식활성도에 관한 연구)

  • The purpose of this study is to evaluate oral health status, caries-inducing bacterial activity of the mental retardation students, and developing motivation method for improving oral hygiene management by clarifying the relationship between caries-inducing bacterial activity and oral health status of mental retardation students. Caries experience indices, caries susceptibility test, gingival health evaluation, and oral hygiene management and oral health survey were performed. Results were as follows : 1. The DMFT index of disabled students(12 years) was 2.07. 2. The gingival inflammation was occurred more frequently in older ages. 3. There also was a high positive correlation between caries incidences and the results of caries-inducing bacterial activity test especially in deciduous teeth. This result suggested that the Dentocult SM mutans test as a caries activity test is a reliable method for measuring the status of dental caries in mental retardation students. Because it would motivate the mental retardation students to care more actively for their oral hygiene if they learn how much dental caries-inducing bacteria are living in their mouth by observing the bacterial colony on the cultured test strip, it can become a possibly efficient educational tool for the mental retardation students.

Experiences of Institutions Utilization among Homeless Young Men with Mental Health Problems: Inevitable Repeated Usages between Two Facility Systems behind the Deinstitutionalization (정신장애를 가진 노숙 청년의 시설 이용 경험 : 탈시설화의 이면(裏面), 생존을 위한 두 체계의 반복 이용을 중심으로)

  • The purpose of this study is to raise the issue of the existing deinstitutionalization debate, considering only the shipment process to these two spaces, and still leaving the inter-facility circulation of vulnerable mental handicapped who are still hanging around the facility. In particular, we sought to supplement the discussion of deinstitutionalization by illuminating the problems of those who have to rely on facilities, as well as the mental health facility system and the homeless facility system. In the case of homeless persons with mental disabilities who repeatedly use the mental health facility system and the homeless facility system, they confirmed the pattern of using the facilities by constantly changing the place to manage the overlapping risk of mental illness and homelessness. Also it is triggered by the voluntary nature of the parties with no resources and resources. This situation appeared to be inevitable in the absence of resources and supportive systems that would actually enable a life in the community. Nevertheless, I asserted that the life of the mentally handicapped, which is circling the facility, is extremely unstable and is not desirable from a welfare point of view.

The Influence of Disability Discrimination on Job Satisfaction of Wage Earners with Disabilities - Focused on the Multiple Mediating Effect of Mental Health - (임금근로장애인의 차별경험이 직무만족도에 미치는 영향 -정신건강의 다중매개효과를 중심으로-)

  • The purpose of this study was to ascertain the multiple mediating effect of mental health, which is composed of depression, disability acceptance, social-economic position, life satisfaction, on the relationship between disability discrimination and job satisfaction against wage earners with disabilities. A data of 992 wage earners with disabilities which derived from the fourth wave panel data provided at Korea Employment Agency for the Disabled were utilized. SPSS 20.0 and SPSS Indirect macro was conducted, and the results were verified by SPSS 20.0 and SPSS Indirect macro. The results were as follows : (1) Depression influenced between disability discrimination and job satisfaction directly or indirectly. (2) Disability acceptance mediated between disability discrimination and job satisfaction. (3) Social-economic position mediated between disability discrimination and job satisfaction. (4) Life satisfaction mediated between disability discrimination and job satisfaction. Based on the results, Based on the results, it suggested some methods of ascent of job satisfaction for wage earners with disabilities.

A Convergence Study on Violence, Discrimination and Suicidal Ideation among Person with Disabilities (장애인의 폭력 및 차별경험과 자살생각에 관한 융합연구)

  • The purpose of this study is to investigate the relevance of discrimination and violence experienced by persons with disabilities to suicidal ideation of persons with disabilities. The study data used the 2014 national survey of the disabled persons and 6,332 people were included in the analysis. Suicidal ideation was defined as whether or not suicide was actually attempted in the last year. The reference group was a group without suicidal ideation and logistic regression analysis of violence discrimination and experience was performed. The study found that 18.5%(n=1.171) of people with disabilities had suicidal ideation. The risk of suicidal ideation was higher when there was language violence and mental violence because of being disabled. When adolescents experienced sexual harassment, sexual abuse, and sexual violence, the risk of suicide was 16.7(CI=4.22-66.18) times higher. The risk of suicidal ideation was 5.8(CI=2.34-14.6) times higher for those who did not know the violent offenders and 4.08(CI=2.05-8.12) times higher for coping strategies that ignored or tolerated violence. The risk of suicidal ideation was 1.60(CI=1.24-2.08) times higher in the case of discrimination experienced at the time of marriage and it was 2.73(CI=4.22-66.18) times higher when they had always felt the discrimination due to disability. The suicidal ideation that comes from experiences of violent experience and discrimination can appear as actual suicide. Therefore, it is necessary to support the suicide prevention program at the community level, along with the care and consideration of the family and society of the disabled for the well-being of the disabled.
