Submental intubation in maxillofacial fracture: a case report

  • It can be challenging to create a safe airway in maxilla facial fracture and some skull surgeries. In this case study, the patient experienced jaw fractures that disturbed the dental occlusion and associated fracture of the base of the skull. Neither nasal nor oral intubation was possible based on the side effects of tracheotomy; therefore, submental intubation was applied successfully. The procedure and results are presented in the text.

  • This paper describes design techniques of general purpose thermal array printer, especially easily applicable to medical measurement equipment. The'general'means that the proposed thermal array printer can be applied to not only medical field but other industrial field by establishing the eligible protocol. The thermal array printer is composed of i)MPU and its peripherals ii)printer head control, iii)diverse protection circuits, and iv)serial communication. In this paper the performance of the proposed thermal array printer was evaluated by applying to an 3 channel electrocardiogram recorder. The propped system can support'Hangul'text as well as alphanumeric characters and any bitmap image.

A Critical Review of Researches on the Wenyilun in South Korea -Focusing on the Selection and Analysis of Medical Theories- (국내 『온역론』 연구에 대한 비판적 검토 -의론 선정과 분석을 중심으로-)

  • Objectives : To examine texts dealing with the Wenyilun in South Korea and to re-evaluate its medical theories that have been underrated in previous texts. Methods : The contents and organization of the Gejiaxueshuo were analyzed. In addition, a research paper on the overall contents of the Wenyilun was studied. Results : Common theories of the miscellaneous qi that were mentioned in the two documents such as 'specificity', 'nine-part transition treatment theory', and 'one disease one formula' are either irrelevant or resulting from erroneous interpretation. While both texts evaluated the merits and harms of the Wenyilun, erroneously deducted contents were used as evidence for negative assessments in both. Conclusions : Should the contents of the Wenyilun be evaluated with a focus on the critical points that clinicians with vast experiences with epidemic disease patients raised, we would be judging the text differently.

  • Objective & Methods: In order to ascertain the names. locations. the way of getting acu-point and indications for moxibustion about Ko-Hwang(膏盲) and Go-Hwang-Su(膏盲兪). I have translated the text for Ko-Hwang(膏盲) and Go-Hwang-Su(膏盲兪) collected in several relative classical books. and performed literal investigation about it. Through the study, the following results are obtained. Results & Conclusions: 1. The two words Go(膏) and Hwang(盲) have different concepts each other originally. and Go-Hwang(膏盲) is the abbreviation of the words "Upon the Hwang(盲), Under the Go(膏)", which means "A gap of Heart and diaphragm" that is under the heart and on the diaphragm 2. Go-Hwang-Su(膏盲兪) is the acu-point which can treat the diseases located in the gap of Go(膏) and Hwang(盲), so they must not be named as Go-Hwang(膏盲) or Go-Hwang-Hyui(膏盲穴). 3. The origination of Go-Hwang-Su(膏盲兪) is speculated as the interval of Hwang-Bo-Mil(皇甫謐) and Son-Sa-Mak(孫思邈). And I think it had been created by Son-Sa-Mak(孫思邈) up to now. 4. The words "Four ribs and Three spaces(四肋三間)" those are founded in the text of The Entrance of Medicine(醫學入門) and The Great Success on Acupuncture and Moxibustion(針灸大成) are either the words those had been quoted from The Bible of Helping Life by Acupucture and Moxibustion(針灸資生經) in wrong way. or the misunderstanding of the original mind of Son-Sa-Mak(孫思邈) as being quoted in the text. So you should know it exactly as Son-Sa-Mak(孫思邈) had said. 5. When you search the acu-point Go-Hwang-Su(膏盲兪), you must pull the arms of patients forwardly to reveal the acu -point. because it is covered by the scapula in normal posture. 6. The indications for the use of Go-Hwang-Su(膏盲兪) are same as mentioned in this study. And it can grow the Yang-Gi(陽氣) more prosperous.

  • The Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) text of "Sang-hanron(傷寒論)" is adopted in the chapter of So-eumin Interior Disease Pattern in "Dong-uisusebowon(東醫壽世保元)". In this study, I have attempted to analyze the Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) texts for the purpose of understanding the So-eumin Interior Disease Pattern thoroughly. In specific, Ijema(李濟馬) stated that the Simhabigyeong(心下痞硬) symptom of the Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) text is identical to the symptoms of other conditions that apply Sasimtang(瀉心湯). Which means that the disease pattern of Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) is a intensified condition of that of Sasimtang(瀉心湯). Next, unlike Jang Jung-gyeong(張仲景) and other annotators of "Sang-hanron(傷寒論)", Ijema(李濟馬) did not perceive Ijungtang(理中湯) to manage the central cho(中焦). Instead, he perceived it appropriate for Tae-eumbyeong(太陰病_disease of Tae-eum), bringing the meaning of Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) into the category of Sameumbyeong(三陰病_disease of the three eum). With this, he made it possible to compare and analyze the texts based on the Sameum/Samyang(三陰三陽) theory. Lastly, Ijema(李濟馬) perceived the symptoms of Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) as So-eum pattern, and categorized it into Tae-eum pattern. Following this, a comprehensive review of Ijema(李濟馬)'s unique understanding of the Jeokseokji-uyeoryang-tang(赤石脂禹餘糧湯) text and its implications were discussed. This part is not included in this article, however, and will be exclusively dealt with in a subsequent article.

A Structural Analysis of Acupuncture & Moxibustion Points in the NaeGyeong Chapter of DongUiBoGam Using Text Mining (텍스트마이닝을 이용한 동의보감의 질병인식방식과 내경편 침구법 경혈 특성 분석)

  • Objectives : DongUiBoGam is a representative medical literature in Korea. This research intends to structurally grasp how DongUiBoGam understands the human body and review the methods of acupuncture and moxibustion in the NaeGyeong chapter of it using text mining. Methods : The structure of DongUiBoGam was analyzed with specific parts of the book that described contents, major premises of understanding the human body, and processes of treatment. We analyzed characteristics of each acupoints in a relationship with causes of diseases & symptoms in the NaeGyeong chapter using a Term Frequency - Inverse Document Frequency(TFIDF). Results : Three different categories of pattern identification(PI) were formed after structural analysis of DongUiBoGam. Every causes of diseases & symptoms were transformed according to the three categories of PI. After analyzing the relationship between acupoints and causes of diseases & symptoms, 114 acupoints were visualized with TFIDF values of three PI categories. Conclusions : The selection of acupoints in NaeGyeong chapter of DongUiBoGam were linked to causes of diseases & symptoms based on the three PI categories. Through visualization of bipartite relationships between acupoints and causes of diseases & symptoms, we could easily understand characteristics of each acupoint.

Characterization of Five Shu Acupoint Pattern in Saam Acupuncture Using Text Mininig (텍스트마이닝을 통한 사암침법 오수혈 사용 패턴 분석)

  • Background : Saam acupuncture were composed by applying the elemental concepts from the Five Phase theory - the relationships between the cycles such as Saeng(Sheng, 'nourishing' or 'creating') and Geuk(Ke, 'suppressing' or 'controlling') - onto the Five Phase points and 12 channels to compensate for the imbalance in each of the 12 main energy traits. Objective : The present study is aimed to find out the characteristics of Five Phase points pattern in Saam acupuncture. Methods : We analysed the characteristics of five elements of the Five Phase points in Korean medical texts such as Saamdoinchimguyogyeol, Dongeuibogam and Chimgugyeongheombang in mid Chosun Dynasty. Using non-negative factorization(NNMF) methods, we extracted the feature matrix of five elements of Five Phase points in each classic medical text. Results : In Saam acupuncture, two characteristics were most prominent: (1) "Self" component of Five elements, (2) "Mother" and "Grandmother" component of Five elements. Conclusions : Saam acupuncture used the combination of Five-Shu acupoint based on ZangFu pattern identification. Our findings suggest that grasping the characteristics of Five Phase points combinations can improve the understanding the selection of the relevant acupoints based on the ZangFu pattern identifications.

Individual experience reports and statements on general rules in medical classics (의학 고전의 개별 경험 진술과 일반 원칙 서술)

  • Objectives : In studying East Asia's medical classics, what must first be done before evaluating the validities of clinical grounds is the discernment of the techniques that are based on universal principles from the statements that arise from individual experience. The purpose of this paper is to discover a method of discerning the statements of individual experiences and the descriptions of universal principles. Ingredients and Methods : The paper investigates how each individual clinical experience was introduced in ancient medical classics such as Huangdineijing, Nanjing, Shanghanlun, Jinkuiyaolue, and Maijing, in the books that belong to Yian such as Canggongliechuan, Linzhengzhinanyian, Gujinyianan, and Xumingyileian, and in Korean ancient medical texts such as Eonhae-gugeubbang and Gugeubganibang-eonhae. Results : Books of precriptions and scriptures of medicine were found to include individual experiences. Furthermore, this paper's effort has revealed that the evaluation of the quality of a testimony in an ancient book must be preceded by discerning whether the said testimony is a mere personal testimony or a testimony based on the universal law of causality. Conclusions : Before conducting a clinical research of a precription contained in an East Asian medical classic, the text should first be evaluated to determine whether the prescription in question derives from clinical tests or not.

A Study on the Life, Works and Distinctive Features of Gogobong(高鼓峰)'s medicine (고고봉(高鼓峰)의 생애(生涯), 저서(著書) 및 의학내용(醫學內容)에 관한 고찰)

  • The actual name of Gogobong is Du-goe, also known as Dan-jung, his pseudonym. He is the author of "Uigasimbeop(醫家心法)", an abstract of his clinical pathology throughout his life and "Chwimopyeon(吹毛編)", a medical chart based on his researches. The chapter 25 Bangrons[二十五方論] is included within the text, which shows attempts on explaining diseases and treatments of the internal organs based on theories on the relations of generation and restriction of the five elements. Influenced by Joheonga(趙獻可), one of the well known Onbo(溫補) scholars-concerning the importance of warming and invigorating the body-of the Myeong-dynasty, his main treatments were forms of warming and invigorating the yang and physical debility, hence strengthen the primordial energy. "Uigasimbeop(醫家心法)" was later on renamed to "Samyeongsimbeop(四明心法)" by Yangseungryuk(楊乘六). The following paper deals on his life, publishment, distinct features of his medical science based on his epitaph and works.
