Nurse Educators' Experience of Developing and Implementing a High-fidelity Simulation-based Interprofessional Education Module for Medical and Nursing Students

  • Objective: Despite the recommendation of the use of high-fidelity simulation (HFS) in interprofessional education (IPE), there is little known about its work for nursing students. Thus, this study aimed to explore nurse educators' perceptions and experiences in developing and implementing the HFS-based IPE for nursing and medical students. Methods: This study used a case study, using reflective filed notes. Results: Nursing educators perceived HFS as an effective educational approach to engaging nursing and medical students actively in interprofessional collaborative practice (ICP) experiences and in evaluating their actual performance on it. In terms of their perspectives on the elements necessary for effective HFS-based IPE, four themes were identified: collaborative learning, co-facilitating debriefing, leadership commitment and active faculty involvement.

응급외상 환자 시뮬레이션 적용 효과 (Implementation Effects of Emergency Trauma Patient Simulation)

  • Purpose: The purpose of this study was to explore EMT-paramedic students' experience of simulation education and analyze the confidence before and after education, learning attitude and course evaluation. Method: Research survey was conducted on 38 EMT-paramedic students during November, 2011 and EMT-paramedic students' experience of simulation education was analyzed after applying head, spinal, and chest injury scenario. The confidence before and after education, learning attitude and course evaluation in gender were analyzed by Mann-Whitny U test and the difference of confidence before and after education was analyzed by Wilcoxon signed rank test and learning attitude & course evaluation were analyzed by evaluating frequency, percentage, mean, standard deviation by using SPSS WIN 17.0 program. Results: 1. Students experienced various advantages such as increasing interest and self-reflection on learning, critical thinking ability, and EMT-paramedic-role experience and recognition of importance of teamwork. Students also pointed out disadvantages such as gap between real situation and simulation, limit of time and equipments, and burden of demonstration. 2. The confidence between before and after education, learning attitude and course evaluation in gender were not significant different statistically. 3. Confidence mean score elevated from 5.53(before education) to 5.87(after education), but the difference in their confidence did not show significant difference statistically. 4. Total mean score in learning attitude after simulation education was 3.70 out of 5.00, which is considerably very high. 5. Total mean score in course evaluation was 3.89 with score of 3.83 in evaluation in learning environment and 3.99 in evaluation of debriefing. Conclusion: The finding of this study demonstrate that the simulation education can provide a safe and repetitive practice environment, improve problem-solving ability and critical thinking, and increase the confidence in prehospital emergency care; therefore, simulation may be the new effective EMT-paramedic education strategy.

시뮬레이션 기법을 이용한 검진센터의 환자관리방안 (Patient Management Through Simulation Modeling in the Medical Center)

  • 본 연구는 시뮬레이션 기법을 이용하여 검진센터의 환자 흐름을 효과적으로 관리할 수 있는 방안을 마련하고자 수행되었다. 이를 위해 실제 병원의 검진자료를 활용하여 15개의 검진환자군을 분류하고, 최대 검사시간, 최소 검사시간, X-ray 추가도입에 근거한 3개의 시나리오를 개발하였으며, 시뮬레이션 프로그램은 Flexsim HC 2.7을 이용하였다. 분석결과 최대 검사 소요시간에 근거한 제 1시나리오에서 평균 검사 소요시간과 최대 검사 소요시간의 차이가 가장 크게 나타났으며, 시간대별 환자 수의 증가에 따른 평균 검사 소요시간이 가장 큰 폭으로 증가하였다. X-ray 장비 추가에 따른 평균 검사 소요시간의 차이는 미미한 것으로 나타나 도입의 필요성은 없는 것으로 분석되었다. 시뮬레이션 기법을 이용한 분석을 통하여 환자대기시간 관리, 장비의 효율적인 도입을 위한 의사결정 지원이 가능함을 확인할 수 있었으며, 구체화된 시나리오를 통해 가장 적정한 검사흐름과 시간대별 환자 수를 파악한다면 병원 경영의 효율화에 크게 이바지할 수 있을 것이다.

자동차 사고를 재현한 시뮬레이션 교육 프로그램 개발을 위한 요구도 조사 (Survey of the Need for the Development of a Simulated Training Program that Reproduces Automobile Accidents)

  • 본 연구는 119 구급대원 60명과 응급구조학과 교수 9명을 대상으로 조사한 자동차 사고 시뮬레이션 교육 프로그램에 대한 요구도 조사이다. 연구결과 응급처치 항목에서 경추보호대 착용에 대한 시뮬레이션 교육 요구도가 가장 높았고(76.8%), 시나리오 주제 항목에서 머리 손상에 대한 시뮬레이션 교육 요구도가 가장 높았다(75.4%). 구급대원의 근무경력 43개월을 기준으로 두 그룹별 요구도 차이를 확인한 결과 응급처치 항목은 항쇼크바지 사용법과 KED 사용 및 신속 구출에 대한 요구도가 두 그룹에서 유의한 차이를 보였고(p = 0.01, p = 0.05), 시나리오 주제는 얼굴, 복부, 기타 손상에 대한 요구도가 두 그룹에서 통계적으로 유의한 차이가 있는 것으로 나타났다(p = 0.05, p = 0.04, p = 0.03). 자동차 사고 시뮬레이션 교육 프로그램에 대한 119 구급대원과 응급구조학과 교수의 요구도도 높게 나타났으므로 이를 기반으로 추후 시뮬레이션 프로그램 개발이 필요하다고 평가되었다.

Simulation and assessment of 99mTc absorbed dose into internal organs from cardiac perfusion scan

  • Directly, it is not possible to measure the absorbed dose of radiopharmaceuticals in the organs of the human body. Therefore, simulation methods are utilized to estimate the dose in distinct organs. In this study, individual organs were separately considered as the source organ or target organ to calculate the mean absorption dose, which SAF and S factors were then calculated according to the target uptake via MIRD method. Here, 99mTc activity distribution within the target was analyzed using the definition and simulation of ideal organs by summing the fraction of cumulative activities of the heart as source organ. Thus, GATE code was utilized to simulate the Zubal humanoid phantom. To validate the outcomes in comparison to the similar results reported, the accumulation of activity in the main organs of the body was calculated at the moment of injection and cardiac rest condition after 60 min of injection. The results showed the highest dose absorbed into pancreas was about 21%, then gallbladder 18%, kidney 16%, spleen 15%, heart 8%, liver 8%, thyroid 7%, lungs 5% and brain 2%, respectively, after 1 h of injection. This distinct simulation model may also be used for different periods after injection and modifying the prescribed dose.

Effectiveness of Simulation-Based Education for Caring Patients with COVID-19

  • Purpose: The role of medical staff gained immense significance in the context of the prolonged coronavirus disease (COVID-19) pandemic. However, few studies had explored the impact of simulation-based education on the ability of nursing students to care for the patients of COVID-19. This study provided nursing students with simulation-based education in caring for the patients of COVID-19 and confirmed its effectiveness. Methods: This study used a non-equivalent control group pretest-posttest design. The participants were recruited from the nursing departments of two universities in Korea through convenience sampling. A total of 79 participants were included: 37 in the intervention group and 42 in the control group. The intervention group received four sessions of simulation training based on the National League for Nursing Jeffries simulation theory. Results: The intervention group showed an improvement compared to the control group in terms of knowledge related to coronavirus, confidence in performing infection control skills, and perception of preparedness for caring for the patients of COVID-19, with a high-level of satisfaction and self-confidence in learning. There was no significant difference between the two groups in terms of anxiety. Conclusion: This simulation is expected to be a significant strategy for alleviating the global burden in terms of staff safety and patient outcomes by improving the competencies of prospective medical staff in responding to pandemics.

  • In plastic and reconstructive craniomaxillofacial surgery, careful preoperative planning is essential to get a successful outcome. Many craniomaxillofacial surgeons have used imaging modalities like conventional radiographs, computed tomography(CT) and magnetic resonance imaging(MRI) for supporting the planning process. But, there are a lot of limitations in the comprehension of the surgical anatomy with these modalities. Medical models made with rapid prototyping (RP) technique represent a new approach for preoperative planning and simulation surgery. With rapid prototyping models, surgical procedures can be simulated and performed interactively so that surgeon can get a realistic impression of complex structures before surgical intervention. The great advantage of rapid prototyping technique is the precise reproduction of objects from a 3-dimensional reconstruction image as a physical model. Craniomaxillofacial surgeon can establish treatment strategy through preoperative simulation surgery and predict the postoperative result.

응급의료센터를 위한 위험기반 운영계획 모델 (Risk-based Operational Planning and Scheduling Model for an Emergency Medical Center)

  • In order to deal with high uncertainty and variability in emergency medical centers, many researchers have developed various models for their operational planning and scheduling. However, most of the models just provide static plans without any risk measures as their results, and thus the users often lose the opportunity to analyze how much risk the patients have, whether the plan is still implementable or how the plan should be changed when an unexpected event happens. In this study, we construct a simulation model combined with a risk-based planning and scheduling module designed by Simio LLC. In addition to static schedules, it provides possibility of treatment delay for each patient as a risk measure, and updates the schedule to avoid the risk when it is needed. By using the simulation model, the users can experiment various scenarios in operations quickly, and also can make a decision not based on their past experience or intuition but based on scientific estimation of risks even in urgent situations. An example of such an operational decision making process is demonstrated for a real mid-size emergency medical center located in Seoul, Republic of Korea. The model is designed for temporal short-term planning especially, but it can be expanded for long-term planning also with some appropriate adjustments.

Effects of collimator on imaging performance of Yttrium-90 Bremsstrahlung photons: Monte Carlo simulation

  • Yttrium-90 is a useful therapeutic radioisotope for tumor treatment because of its high-energy-emitting beta rays. However, it has been difficult to select appropriate collimators and main energy windows for Y-90 Bremsstrahlung imaging using gamma cameras because of the broad energy spectra of Y-90. We used a Monte Carlo simulation to investigate the effects of collimator selection and energy windows on Y-90 Bremsstrahlung imaging. We considered both MELP and HE collimators. Various phantoms were employed in the simulation to determine the main energy window using primary-to-scatter ratios (PSRs). Imaging performance was evaluated using spatial resolution indices, imaging counts, scatter fractions, and contrast-to-noise ratios. Collimator choice slightly affected energy spectrum shapes and improved PSRs. The HE collimator performed better than the MELP collimator on all imaging performance indices (except for imaging count). We observed minor differences in SR and SF values for the HE collimator among the five simulated energy windows. The combination of an HE collimator and improved-PSR energy window produced the best CNR value. In conclusion, appropriate collimator selection is an important component of Bremsstrahlung Y-90 photon imaging and main energy window determination. We found HE collimators to be more appropriate for improving the imaging performance of Bremsstrahlung Y-90 photons.
