Development and Feasibility of a Video Recording-Based Standardized Handoff Program for Ward Nurses: A pilot study (비디오 녹화 기반 병동 간호 인수인계 프로그램 개발 및 적용가능성 : 예비연구)

  • Purpose : This study develop a Video Recording-Based Standardized Handoff (VRSH) program at shift change for ward nurses. Methods : The study was conducted in five medical, three surgical, and one comprehensive nursing care service wards affiliated with a secondary general hospital. In this methodological study, the VRSH program was developed between April and December, 2017. It is noted that 65 nurses who were involved in the VRSH program participated in this study. Results : In line with the modified Situation, Background, Assessment, Recommendation tool, the VRSH program consisted of three phases. In the VRSH program, the average time for handoff duration was 3-5 minutes per patient. More than 90% of the ward nurses were satisfied with the VRSH program since it benefited them by reducing overtime work and improving the performance, as well as effective communication, of nurses. The content analysis of nurses' VRSH program experience, revealed three categories and eight sub-categories. Conclusions : This study provides evidence that the VRSH program improves effective nursing performance and, the handoff communication and relationships between nurses. Future studies on large sample sizes and multiple settings are required to substantially evaluate the impact of the VRSH program on clinical outcomes.

  • Since the output of retina for visual stimulus is carried by neurons of very diverse functional properties, it is not adequate to use conventional single electrode for recording the retinal action potential. For this purpose, we used newly developed multichannel recording system for monitoring the simultaneous electrical activities of many neurons in a functioning piece of retina. Retinal action potentials are recorded with an extra-cellular planar array of 60 microelectrodes. In studying the collective activity of the ganglion cell population it is essential to recognize basic functional distinctions between individual neurons. Therefore, it is necessary to detect and to classify the action potential of each ganglion cell out of mixed signal. We programmed M-files with MATLAB for this sorting process. This processing is mandatory for further analysis, e.g. poststimulus time histogram (PSTH), auto-correlogram, and cross-correlogram. We established MATLAB based protocol for waveform classification and verified that this approach was effective as an initial spike sorting method.

Role of Transesophageal Pacing in Evaluation of Palpitation in Infants and Children (심계항진을 호소하는 소아에서 경식도 심전도 검사의 유용성)

  • Purpose : We intended to find out the role of transesophageal atrial pacing in evaluation of infants and children with palpitation of unknown origin. Methods : We tried transesophageal atrial pacing study in 69 infants and children with palpitation, in whom tachycardia wasn't documented in electrocardiogram and reviewed retrospectively the records of transesophageal pacing and medical records of theses patients to find out the induction rate of tachycardia by transesophageal atrial pacing and the possible mechanism of tachycardia if induced. Results : In 70.1% infants and children with palpitation, tachycardia was induced, so we could conclude that tachycaridia was the cause of palpitation in these cases. In most cases, tachycardia was induced by rapid atrial pacing, and in 21% by using isoproterenol. Tachycardia induction rate was higher in <6-year-old children than ${\geq}6$-year-old children(P<0.05). In cases of the induced, we reviewed the mechanism of tachycardia. Of these induced tachycardias, 53.2% is atrioventricular reentry tachycardia, 34.0% were atrioventricular nodal reentry tachycardia, and 12.8% were idiopathic left ventricular tachycardia. Invasive electrophysiologic study was done to 10 patients of those induced. The results of electrophysiologic study and transesophageal pacing and recording were the same except for one patient. Conclusion : Transesophageal atrial pacing and recording is a less invasive, safe and useful method to find out the cause of palpitation and the mechanism of tachycardia in infants and children.

  • Esophageal cancer (EC) incidence rates in Kazakhstan were assessed by component analysis based on primary registered cases in 2001-2010. It was found that despite an apparent general decrease in the number of EC patients in Kazakhstan, a potential increase should be evaluated, due to changes in aging as well as the increase in population. Some problems of EC patients' registration were broached with an emphasis on the importance of the expected absolute number and reasons for undercounting in the country. Based on these, ways of improving the recording and registration of such patients in the country were suggested.

  • For the development of feasible retinal prosthesis, one of the important elements is acquiring proper judging tool if electrical stimulus leads to patient's visual perception. If evoked potential to electrical stimulus is recorded in primary visual (V1) cortex, it means that the stimulus effectively evokes visual perception. Therefore, in this study, we established VEP recording system on V1 cortex using BioPAC modules as the judging tool. And the measuring system was evaluated by recording VEP of mice. After anesthesia, normal mice (C57BL/6J strain; n = 6) were secured to stereotaxic apparatus (Harvard Apparatus, USA). For the recording of VEP, the stainless steel needle electrode (impedance: $2-5k{\Omega}$) was positioned on the surface of the cortex through the burr hole at 2.5 mm lateral and 4.6 mm caudal to bregma. DA 100C and EEG 100C BioPAC modules were used for the trigger signal and VEP recording, respectively. When left eye was blocked by black cover and right eye was stimulated by flash light using HMsERG (RetVet Corp, USA), VEP response at left V1 cortex was detected, but there was no response at right V1 cortex. Amplitudes and latencies of P2, N3 peaks of VEP recording varied according to the depths of the electrodes on V1 cortex. From the surface upto $600{\mu}m$ depth, amplitudes of P2 and N3 increased, while deeper than $600{\mu}m$, those amplitudes decreased. The deeper the insertion depth of the electrode, the latency of N1 peaks tends to be delayed. However, there was no statistically significant difference among the latencies of P2 and N3 peaks (P > 0.05, ANOVA). Our VEP recording data such as the insertion depth and the latency and amplitudes of peaks might be used as guidelines for electrically-evoked potential (EEP) recording experiment in near future.

Spontaneous Electrical Activity in Cerebellar Purkinje Neurons of Postnatal Rats

  • Although cerebellar Purkinje cells display spontaneous electrical activity in vivo and in slice experiments, the mechanism of the spontaneous activity generation has not been clearly understood. The aim of this study was to investigate whether cerebellar Purkinje cells of postnatal rats generate spontaneous electrical activity without synaptic inputs. Dissociated cerebellar Purkinje cells were used for reducing synaptic inputs in the present study. Cerebellar Purkinje cells with dendrites were dissociated from postnatal rats using enzymatic treatment followed by mechanical trituration. Spontaneous electrical activities were recorded from dissociated cells without any stimulus using whole-cell patch clamp configuration. Two types, spontaneously firing or quiescent, of dissociated Purkinje cells were observed in postnatal rats. Both types of cells were identified as Purkinje cells using immunocytochemical staining technique with anti-calbindin after recording. Spontaneously active cells displayed two patterns of firing, repetitive and burst firings. Two thirds of dissociated Purkinje cells displayed repetitive firing and the rest of them did burst firing under same recording condition. Repetitive firing activities were maintained even after further isolation using either physical or pharmacological techniques. Neither high magnessium solution nor excitatory synaptic blockers, AP-5 and DNQX, block the spontaneous activity. These results demonstrate that spontaneous electrical activity of isolated cerebellar Purkinje cells in postnatal rats is generated by intrinsic membrane properties rather than synaptic inputs.

A Survey of Utilizing Status and Demand for Medical Devices in Traditional Korean Medicine (한방의료기기 사용 현황 및 개발 수요에 대한 조사 연구)

  • Objectives: The aim of this survey was to investigate utilization, intended use, problems with and demand for medical devices by surveying members of the traditional Korean medical society. Methods: We distributed questionnaires to 13,957 traditional Korean medical doctors via e-mail, and received replies from 1,225. The questionnaire consisted of 4 multiple-choice questions for survey respondent information, 8 multiple-choice questions about the status of medical devices utilizing, and a short answer question about the demand for medical devices. Results: Use of medical devices in traditional Korean medical clinics is common. Diagnostic medical devices are mainly used to assess the patient's condition and to establish a close rapport with clients. In case of therapeutic medical devices, they are usually used for secondary treatment. Issues with traditional Korean medical devices currently in use were ineligibility for national health insurance, low reliability, uncertain validity, and high price. In development of traditional Korean medical equipment, the need for diagnostic medical devices was greater than for therapeutic, and the need for the recording and analysis of medical image data and visualization of medical information was great. Conclusions: There is growing demand for facilitating the development and commercialization of traditional Korean medical devices. To satisfy this demand, research on evaluation indicators that reflect functional and structural clinical information and how to clinically assess the indicators should proceed.

The Electrophysiological Identification of the Cone- and the Rod- HCs Dissociated from Goldfish Retina

  • Goldfish retina has been well studied to a great extent. In spite of that, electrical characteristics of dissociated horizontal cells(HCs) have not been identified in detail. Thus the cone-and the rod- HCs dissociated from goldfish retina were investigated electrophysiologically using whole-cell patch-clamping recording. To explore the basic electrical property, We examined voltage-dependent channels in all types of HCs. For the futher understanding of GABAergic pathway, the localization and distribution of GABA receptors was examined in cone- HCs including HC axon terminals(ATs).

  • For the sake of the Restoration(to the original state) of "Sanbeon-bang(刪繁方)", in this study observed carefully the bibliographical results of "Oedaebiyo-bang", "Cheongeumyo-bang", "Cheongeumik-bang" and "Uisim-bang", which directly quoted "Sanbeon-bang". "Sanbeon-bang" is mostly remained in "Oedaebiyo-bang", and the other provisions of "Sanbeon-bang" mostly lie scattered in "Cheongeumyo-bang" and "Cheongeumik-bang", established 100years before "Oedaebiyo-bang" and "Uisim-bang", established 200years after "Oedaebiyo-bang". And there are few other documents which quoted the "Sanbeon-bang". "Cheongeumyo-bang", a synthetic medical work was compiled in 652, and which refered large quantity of medical documents before Dang-dynasty(唐代) by Son Sa-mak(孫思邈). And he linked the technical opinion of himself. The comments of medicine, as well as the prescriptions are refleting the outcome before Dang-dynasty systematically. For the most part of the medical arguments in "Sanbeon-bang" lie scattered in "Cheongeumyo-bang". "Cheongeumik-bang", also a synthetic medical work which is for the sake of supplement of "Cheongeumyo-bang" was complied in 682 by Son Sa-mak. As well as "CheongeumYo-bang" it doesn't make clear the sources of quotation from "Sanbeon-bang". So it used as the data for confirmation. "Oedaebiyo-bang" was compiled in 752 of Dang-Dynasty by Wang Do(王燾) the a governor-general of Eop-gun. Wang Do was descended from a noble family, and usually visited the palace and dealed with large number of books in Hongmunguan(弘文館), the national library. He programmed the book on the base of wide scopic collection of medical documents. And he took charge of gverment post at the same time as Wang Bing(王氷) who compiled "Chaju-Hwangje-Naegyeong-Somun(次注黃帝內經素問)" in 762. So we can guess they might have seen the same documents and holded the medical knowledge in common. The 40 volume "Oedaebiyo-bang" was a great medical complete book in those days. In particular, it became the model of medical complete books of after ages. The description of the book is mostly the qutation of medical documents of before ages. The character is recording of quotation documents and the order of volume. On the base of the recording, we can reconstruct the table of contents of Sanbeon-bang. By way of the contens, we can understand the purpose of "Sanbeon-bang". Besides, I can see a lot of qouotations of "Sanbeon-bang" in "Cheongeum-bang" and "Uisim-bang" are repeatedly quotated in "Oedaebiyo-bang". For that reason, I observed the bibliographical results of "Oedaebiyo-bang" in detail. "Uisim-bang" was compiled in 982 by the Japanese Niwayasyori(丹波綱賴). It followed the example of the structure of "Oedaebiyo-bang", refered to the contents of "Cheongeumyo-bang", collected chinese medical classic books spreaded in Japan. It contains medical documents of before Su-Dang(隋唐)-era, indicated the source of quotation. The importance of the book is summarization and preservation of large quantity of chinese medicine before 10th century. In this study, a lot of the quotation in "Uisim-bang" from "Sanbeon-bang" were not in "Oedaebiyo-bang".

Influences of Inter-electrode Distance on Electrogastrography Measurements (위전도 측정을 위한 전극간 부착거리에 관한 연구)

  • Cutaneous electrogastrography is the measurement of electrical activity of the stomach on the abdominal surface. The validity of cutaneous electrogastrography is dependent upon the quality of the recording technique. The locations of electrodes are an important issue. We examined the influences of the inter-electrode distance of bipolar leads on electrogastrography measurements. The sensitivity distributions of EGG leads were calculated based on a 2D body fat model and evaluated according to the region of interest sensitivity ratio (ROISR). We simulated the ROISR of the inter-electrode distance in relation to various body fat thicknesses. The distance between the electrodes was proportional to the distance between the ROI and the surface of the abdomen. The results imply that inter-electrode distance can be applied in electrogastrography according to human body fat thickness.
