Healthcare Professionals Involved in Medical Errors and Support Systems for Them: A Literature Review (오류를 경험한 의료인에 대한 연구 동향 및 지원 시스템)

  • Purpose: The purpose of this study was to identify the current state of research on healthcare professionals who make medical errors, who are known as "second victims", and support systems for them. Methods: An extensive search was conducted in electronic databases, Google, and websites related to patient safety using search terms such as "second victims", "medical errors", "adverse events", and "sentinel events". Results: Research to date in Korea has rarely focused on healthcare professionals' experiences after making medical errors. Abroad, there are comprehensive and systematic reviews of the impact of medical errors on healthcare professionals, their coping responses, and support systems for these second victims. Additionally, several institutes related to patient safety provide official guidelines and accessible support systems to support second victims in the aftermath of medical errors, especially serious adverse events. Conclusion: The impact of medical errors on healthcare professionals is profound and complex. Although systematic support systems for second victims are imperative, this has been overlooked in Korea. Thus, more research about the experiences of healthcare professionals after medical errors needs to be conducted prior to developing support systems or programs. Additionally, further efforts are required to raise awareness of the necessity of supporting healthcare professionals in healthcare systems.

COVID-19 vaccine hesitancy among medical students, health professionals, and health care workers: an umbrella review

  • Vaccination provides great protection against several infections, including coronavirus disease 2019 (COVID-19). However, the endeavor faces multiple context-specific problems that affect its uptake, leading to vaccine hesitancy. Vaccine hesitancy is a focal barrier to the success of COVID-19 vaccination programs. This umbrella review aimed to present a summary of global data regarding vaccine hesitancy and acceptance rates among medical students, health professionals, and health care workers. In this regard, two databases, PubMed and Scopus, were selected for data retrieval and analysis. A search term and an inclusion/exclusion criterion were applied to summarize the findings of existing systematic reviews. A pooled prevalence of vaccine acceptance and hesitancy with 95% confidence interval (CI) was taken as a prerequisite for this review. The results found a high percentage of COVID-19 vaccination ranging from 13.1% (95% CI, 6.9%-20.9%) to 46% (95% CI, 0.38%-0.54%), while the percentage of acceptance varied from 46% (95% CI, 37%-54%) to 83.0% (95% CI, 71%-96%) among medical students, health professionals, and health care workers. This umbrella review found a high percentage of COVID-19 vaccine hesitancy among medical students, health professionals, and health care workers. Further studies analyzing the determinants of vaccine hesitancy are important predictors for successful vaccination programs at the global level.

The Analysis of South Korea Medical Tourism Services for the Differences Perceptions by Medical Tourists and Hospital Staffs (의료관광객과 병원 종사자간의 한국 의료관광 서비스에 대한 인식차이 분석)

  • Recently, medical market has been changing not for the sake of treatment of the disease but for high quality of health. Therefore, South Korea medical tourism service needs to understand and improve for the differences perceptions of medical tourists and health professionals for customer satisfaction. The purpose of this study is to understand the structure of medical tourism market through theoretical consideration and to suggest the factors which need to be taken precedence for improvement of South Korea medical tourism service through analysis differences between consumers and suppliers. To analyze the difference of awareness, we reconstructed the questionnaires focussing on evaluation factors and articles about medical tourists selection attributes and health professionals way to invigoration through previous research and precedence study. After that, we also collected the data based on reconstructed questionnaires. After we compared each collected responses from medical tourists and health professionals, also analyzed the differences of awareness by applying Johari's Window to those differences. In conclusion, there is no relation between medical tourist and health professionals about th differences of awareness in functional quality and cost that is a direct effect to transaction in current medical tourism of South Korea. In contrast, there are differences of awareness between a process which support medical tourism and technical quality. Therefor, we expect to invigorate medical tourism by innovation of supportive process and technical quality.

Development of Educational Model for Health Professionals Regarding Smoking Cessation (보건의료인을 대상으로 한 금연교육모형 개발)

  • Objecive and Method: Smoking among health professionals has been shown to influence smoking related knowledge, attitude and educational practices in medical setting. And lack of health professionals' efficacy for smoking cessation intervention has been a major barrier to education on smoking too. In this regard, the present study was carried out to introduce and discuss the advanced cases of smoking cessation education for health professionals, and to develop theory-based educational models of smoking cessation for health professionals in order to improve the effectiveness and efficiency of intervention on smoking in a medical framework. Results: First, major issues of health professionals' smoking cessation intervention were discussed. Discussed issues were smoking prevalence among health professionals, importance of health professionals' roles both as health educators and examples, and health professionals' cognitive dissonance. As advanced cases of smoking cessation education for health professionals, ATOD(Alcohol, Tabacco, and Other Drug problem prevention) developed by US Department of Health & Human Services and the Rx for Change curriculum in California State were discussed. Finally, smoking cessation educational models for health professionals were developed on the basis of social cognitive model and TPB/TTM. Conclusions: For the effective and efficient smoking cessation intervention in medical setting, systematic efforts would be necessary to provide opportunities for ensuring the qualification of health professionals on smoking cessation through an analysis of major issues concerning smoking cessation education for health professional and the development of comprehensive curriculum for smoking cessation.

Cognitive Discrepancies of the Mutual Development Strategies of Western and Oriental Medicine in Korea - A Comparison between Western and Oriental Medical Practitioners and Health Professionals - (양.한방 상호보완 방안에 관한 인식도 연구 - 의사, 한의사 및 보건의료전문가간 비교를 중심으로 -)

  • This study is conducted to compare how the Western.Oriental medical practitioners and health professionals perceive reciprocal development strategies respectively of Western.Oriental Medicine in Korea. A total of 3,273 persons were questioned by direct distribution, e-mail, and mail with a self-developed questionnaire. Of those questioned, 362 responded (the response rate of 11.1%), and of them n persons (Western medical practitioners 206, Oriental medical practitioners 90, and health professionals 64) were analyzed with SPSS for Windows. The results were as follows: The need for legislation on the mutual development system for Western.Oriental Medicine was recognized by 66.9% (218 persons) of respondents. Among them, largest group who agreed with this need was the health professionals. Western medical practitioners chose the "the difference of approach methods on the diseases between Western.Oriental Medicine" as their response, and Oriental medical practitioners & health professionals selected "the indifference or bias of Western medical practitioners"as the reasons for the inactivity in developing a mutual system of Western.Oriental Medicine. Therefore, Western medical practitioners and health professionals selected the category of "the reformation of educational system", while Oriental medical practitioners selected the category of "the activation of joint research on Western.Oriental medical care" as the most important condition for setting a precedent. Also, Western medical practitioners preferred "cooperative health care system for Western medical care supported by Oriental medical care", but Oriental medical practitioners and health professionals preferred “cooperative health care system of Western.Oriental medicine on equal terms" In conclusion, Western '||'&'||' Oriental medical practitioners have to make every effort to close the gap between differing views through mutual understanding and respect if joint research of Western.Oriental medical care is to become a reality. Dental hygiene curriculum proposals to improve the ability of dental hygienists as medical professionals (의료인 치과위생사의 전문능력 향상을 위한 치위생학과 교육과정 제안)

  • Objectives: The purpose of this study was to propose contents of a curriculum and training program for dental hygienists as medical professionals by surveying the opinions of clinical dental hygienists and dental hygiene professors. Methods: The subjects were 192 clinical dental hygienists and 193 dental hygiene professors. They answered questionnaires that consisted of grading each task based on its importance: a) for dental hygiene students to learn, b) to perform autonomously in clinical practice, and c) the expectancy of the task to change when dental hygienists become medical professionals. Data analysis was performed using an independent sample T test to capture differences between clinical dental hygienists and dental hygiene professors. The terms in the answers of open-ended questionnaires were extracted. We used R 3.5.0, R Recommender, and Wordcloud software packages. Results: Calculus removal had the highest scores for dental hygiene students to learn and expectancy to change when dental hygienists become medical professionals. Physiotherapy of temporomandibular disorders (TMD), planning, performing, and assessment of community oral health programs had the lowest scores in autonomy in clinical practice. The dental hygiene professors gave higher scores in most of the tasks for dental hygiene students to learn, autonomy in clinical practice, and expectancy to change, than did clinical dental hygienists. Cardiopulmonary resuscitation (CPR), job ethics, and communication were the most frequently mentioned terms in the training as medical professionals program contents. Conclusions: In the future, it will be necessary to study the curriculum to improve the proficiency of dental hygienists as medical professionals.

  • The purpose of the present study is to analyze curriculums in colleges operating a department related to medical information in Korea and to propose the roles of medical information professionals, medical information professionals qualifying examination and a plan to raise medical information specialists demanded in the field. In addition, this study purposed to suggest establishing programs for job training and supplementary education of medical information professionals through sharing contents among colleges. For these purposes, this study proposed a plan to construct the infrastructure of Internet-based domestic medical information and education network for the service of integrated medical information system standardized centering on the Healthcare Information Center.

Fundamental Idea and Actuality of the Medical Dispute Mediation Act (의료분쟁조정법의 기본이념과 현실)

  • Medical treatment has great potential for conflict. Even the best-trained doctors can commit medical malpractice that result in continuing physical or mental disabilities or even death. Medical conflicts have been increasing over years. The medical conflicts between patient and medical professionals that result from medical professionals' mistakes are often fueled by a violation on the pretext of the injuries form medical malpractice and can lead to litigation. The litigation usually cost a lot of money and time. The extension of the litigation period as well as expensive cost and lack of medical knowledge placing a great burden on patients. Alternative Dispute Resolution(ADR) is more efficient than litigation. In 1988, the medical dispute mediation system has been introduces as the Act on Remedy for Damage from Medical Accident and Medical Dispute Mediation by Korean Medical Association came into effect after 23 years of enactment efforts. Medical Dispute Mediation Act(hereinafter referred to as the "MDMA") has finally entered into force from 8 April 2012. The purpose of the MDMA is to promptly and fairly redress injuries caused by medical malpractice and create a stable environment for medical services of public health or medical professionals by providing for matters regarding the mediation and arbitration of medical disputes(MDMA ${\S}1$). In an effort to secure the fair, speedy and inexpensive resolution of every malpractice case, the Korea Medical Dispute Mediation and Arbitration Agency(hereinafter referred to as the "K-MEDI") was established. Following the MDMA, the K-MEDI shall endeavor to ensure the medical dispute mediation or arbitration proceedings are conducted in a prompt, fair, and efficient manner, and patients and medical professionals shall attend proceedings in good faith with mutual trust and understanding when they participate in medical dispute mediation or arbitration proceedings.

Frequency, Expected Effects, Obstacles, and Facilitators of Disclosure of Patient Safety Incidents: A Systematic Review

  • Objectives: We performed a systematic review to assess and aggregate the available evidence on the frequency, expected effects, obstacles, and facilitators of disclosure of patient safety incidents (DPSI). Methods: We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines for this systematic review and searched PubMed, Scopus, and the Cochrane Library for English articles published between 1990 and 2014. Two authors independently conducted the title screening and abstract review. Ninety-nine articles were selected for full-text reviews. One author extracted the data and another verified them. Results: There was considerable variation in the reported frequency of DPSI among medical professionals. The main expected effects of DPSI were decreased intention of the general public to file medical lawsuits and punish medical professionals, increased credibility of medical professionals, increased intention of patients to revisit and recommend physicians or hospitals, higher ratings of quality of care, and alleviation of feelings of guilt among medical professionals. The obstacles to DPSI were fear of medical lawsuits and punishment, fear of a damaged professional reputation among colleagues and patients, diminished patient trust, the complexity of the situation, and the absence of a patient safety culture. However, the factors facilitating DPSI included the creation of a safe environment for reporting patient safety incidents, as well as guidelines and education for DPSI. Conclusions: The reported frequency of the experience of the general public with DPSI was somewhat lower than the reported frequency of DPSI among medical professionals. Although we identified various expected effects of DPSI, more empirical evidence from real cases is required.

  • The purpose of this study was to assess medical professionals' knowledge and attitude about AIDS. The subjects of study were medical doctors and dentists, belonging to Korea Association of Public Health Doctors. We sent a e-mailed questionnaire to 3,059 members and received 407 replies. Questionnaire was made on the basis of former studies' results and interviews with infected people. Major findings of this study were as follow. A lot of medical professionals had incorrect and biased knowledge about AIDS. More than half of them overestimated infection probability in case of being prickled with a contaminated needle. And many has negative attitude. The more they had correct informations about AIDS, the more they had possibilities to have friendly attitude to infected people. Incorrect knowledge cause excessive fears about AIDS, which amplify the stigma and discrimination. They contribute to people's vulnerability not only to HIV infection but also to other threats to health and well-being. It is in need of medical professionals' effort to increase their knowledge and improve attitude about AIDS.
