  • Currently, the nation's medical industry is changing due to the rapid development of technology. In addition, medical accidents occurring in the medical industry are gradually increasing amid the drastic changes. Therefore, a possible solution to medical accidents in the changing health care industry is needed. Accordingly, this study uses network-centrality analysis to examine the business ecosystem of smart surgical medical systems to find ways to increase the efficiency of surgery as well as the resolution of medical accident problems, and to suggest the direction of development of the medical system in the future from a systematic business ecosystem perspective.

Performance Evaluation of the QoS Guarantee Mechanisms for Medical Multimedia Network Using DiffServ (DiffServ를 이용한 의료용 멀티미디어 네트워크의 QoS 보장에 대한 성능평가)

  • The goal of Medical multimedia server is to develop computer hardware and software which would enable electronic access, storage, transmission, and display of patient data and images. Since the current network only provides so called "best-effort" services, it is impossible to satisfy QoS guarantee that is required for real time application services for emergency room, operating room etc. Accordingly, world-wide research is being made for a variety of services to provide QoS. he goal of DiffServ is to offer scalable differentiated service in Internet which are made possible by traffic classification and conditioning only performed at an edge(or a boundary) node. In case DiffServ was deployed in the Medical multimedia network, it is difficult to estimate how the QoS mechanism would affect totally the network performance. Therefore, we need to verify by simulation the design of algorithm which provide a variety of differentiated services. In QoS for Medical multimedia network, a simulator is designed and implemented using OPNET to investigate the performance of DiffServ QoS support mechanism. The developed DiffServ simulator may generate packets according to random, and bursty traffic models in order to incorporate diverse traffic conditions in the Medical multimedia network environment. Based on our simulation results, we confirmed that service differentiation is possible by using the EF(Expedited Forwarding) class in DiffServ networks.

  • Chiari's networks are present in 1.5% to 4% of the population. They are a congenital disease characterized by a remnant of the right valve of sinus venosus and rarely have clinical significance. Chiari's network, as the name implies, has network-like shape, but there are other forms of appearance. We have experienced a case of a 60-year-old woman who had a cystic mass on the right atrium. Surgical treatment was performed forthe mass removal and differential diagnosis of the mass. There was no evidence of other tumor, but Chiari's network. As cystic form of Chiari's network have not been reported before, it is the first report of cystic form of Chiari's network.

Vasorelaxing Activity of Ulmus davidiana Ethanol Extracts in Rats: Activation of Endothelial Nitric Oxide Synthase

  • Ulmus davidiana var. japonica Rehder (Urticales: Ulmaceae) (UD) is a tree widespread in northeast Asia. It is traditionally used for anticancer and anti-inflammatory therapy. The present study investigated the effect of an ethanol extract of UD on vascular tension and its underlying mechanism in rats. The dried root bark of UD was ground and extracted with 80% ethanol. The prepared UD extract was used in further analysis. The effect of UD on the cell viability, vasoreactivity and hemodynamics were investigated using propidium iodide staining in cultured cells, isometric tension recording and blood pressure analysis, respectively. Low dose of UD ($10{\sim}100{\mu}g/ml)$ did not affect endothelial cell viability, but high dose of UD reduced cell viability. UD induced vasorelaxation in the range of $0.1{\sim}10{\mu}g/ml$ with an $ED_{50}$ value of $2{\mu}g/ml$. UD-induced vasorelaxation was completely abolished by removal of the endothelium or by pre-treatment with L-NAME, an inhibitor of nitric oxide synthase. UD inhibited calcium influx induced by phenylephrine and high $K^+$ and also completely abolished the effect of L-NAME. Intravenous injection of UD extracts (10~100 mg/kg) decreased arterial and ventricular pressure in a dose-dependent manner. Moreover, UD extracts reduced the ventricular contractility (+dP/dt) in anesthetized rats. However, UD-induced hypotensive actions were minimized in L-NAME-treated rats. Taken together, out results showed that UD induced vasorelaxation and has antihypertensive properties, which may be due the activation of nitric oxide synthase in endothelium.

Analysis of common and characteristic actions of Panax ginseng and Panax notoginseng in wound healing based on network pharmacology and meta-analysis

  • In recent years, an increasing number of reports have explored the wound healing mechanism of these two traditional Chinese herbal medicines- Panax ginseng and Panax notoginseng, but there is no systematic research on the related core functions and different mechanisms in the treatment of wound healing up to now. Based on network pharmacology and meta-analysis, the present work aimed to comprehensively review the commonality and diversity of P. ginseng and P. notoginseng in wound healing. In this study, a wound healing-related "ingredients-targets" network of two herbs was constructed. Thereafter, meta-analysis of the multiple target lists by Metascape showed that these two medicines significantly regulated blood vessel development, responses to cytokines and growth factors and oxygen levels, cell death, cell proliferation and differentiation, and cell adhesion. To better understand the discrepancy between these two herbs, it was found that common signaling pathways including Rap1, PI3K/AKT, MAPK, HIF-1 and Focal adhesion regulated the functions listed above. In parallel, the different pathways including renin-angiotensin system, RNA transport and circadian rhythm, autophagy, and the different metabolic pathways may also explained the discrepancies in the regulation of the above-mentioned functions, consistent with the Traditional Chinese Medicine theory about the effects of P. ginseng and P. notoginseng.

Transcriptome Network Analysis Reveals Potential Candidate Genes for Esophageal Squamous Cell Carcinoma

  • The esophageal squamous cell carcinoma (ESCC) is an aggressive tumor with a poor prognosis. Understanding molecular changes in ESCC should improve identification of risk factors with different molecular subtypes and provide potential targets for early detection and therapy. Our study aimed to obtain a molecular signature of ESCC through the regulation network based on differentially expressed genes (DEGs). We used the GSE23400 series to identify potential genes related to ESCC. Based on bioinformatics we constructed a regulation network. From the results, we could establish that many transcription factors and pathways closely related with ESCC were linked by our method. STAT1 also arose as a hub node in our transcriptome network, along with some transcription factors like CCNB1, TAP1, RARG and IFITM1 proven to be related with ESCC by previous studies. In conclusion, our regulation network provided information on important genes which might be useful in investigating the complex interacting mechanisms underlying the disease.

  • Medical information system based on ad hoc network designed for general information systems and information networks have different security requirements. Malicious code infiltration and security features are performed on same medical information network architecture along the route. Security domain of medical information systems is the ground of penetration and defense performed over the network architecture and it is also the traffic handling areas separated by a concept of differentiated group. Ad hoc-based medical information systems in the network security domain, set some standards about what should be the methodology of this study. In this paper, medical information system network configuration, the determining factor based on the security domain, the structure selection criteria and blocking positionings are presented according to the traffic route configuration. If you apply this methodology designed to increase security, efficiency can be possible. Health information systems in accordance with the security domain areas requires differentiated protection needs of the security mechanism that is proposed by this study.

  • Medical diagnosis systems have been developed to make the knowledge and expertise of human experts more widely available, therefore achieving high-quality diagnosis. In this study, in order to support the diagnosis by the medical diagnosis system, we have preformed medical diagnosis inference three times: first by a neural network with the backpropagation algorithm, secondly by a discriminant analysis with all of the variables, and thirdly by a discriminant analysis with the selected variables. A discussion on comparison of these three methods has been provided.

An U-Healthcare Implementation for Diabetes Patient based on Context Awareness

  • With ubiquitous computing aid, it can improve human being's life quality if all people have more convenient medical service under pervasive computing environment. In this paper, for a pervasive health care application for diabetes patient, we've implemented a health care system, which is composed of three parts. Various sensors monitor both outer and inner environment of human such as temperature, blood pressure, pulse, and glycemic index, etc. These sensors form zigbee-based sensor network. And as a backend, medical information server accumulates sensing data and performs back-end processing. To simply transfer these sensing values to a medical team may be a low level's medical service. So, we've designed a model with context awareness for more improved medical service which is based on ART(adaptive resonance theory) neural network. Our experiments show that a proposed healthcare system can provide improved medical service because it can recognize current context of patient more concretely.

Medical Texts as the Health Care System in the Joseon Dynasty :An Anthropological View on the Meaning of Medical-Text Publication (의료체계로서의 조선 의서: 인류학적 시선으로 읽는 의서 발간의 의미)

  • This study examines the meaning of medical-text publication in the Joseon Dynasty by applying anthropological notions of "health care system" and "popular health care sector" to the social and political contexts of the pre-modern state. The present study focuses on the social network of senders and receivers in which medical knowledge is communicated and shared. Exploring the multi-layered structure of the network among the state, the author-practitioners, and populace, this study argues that the network of knowledge sharing system by publication of medical texts itself is a core structure in the health care system of the Joseon Dynasty. This pre-modern health care system aimed to vitalize and reinforce the "popular health care sector" by sharing medical knowledge with populace through the book-publication system. Foucault's notion of "biopolitics" provides a comparative window between the modern health care system and the health care system of the Joseon period, articulating the particularity of the pre-modern health care system.
