A survey for Korean Medicine doctors on the activation of Mibyeong management (한의사 설문을 통한 미병관리의 제도적 활성화 방향 조사)

  • Objectives : This study aims to investigate the recognition on Mibyeong among Korean Medicine doctors (KMDs) and to suggest institutional approaches for the active use of Mibyeong. Methods : On-line survey for KMDs who were registered in the Association of Korean Medicine was performed. The survey questionnaires included definitions and categories of Mibyeong, clinical use of Mibyeong, and the need for developing Mibyeong care packages, etc. Results : Of 18,344 KMDs who were received survey e-mail, 884 KMDs replied. As for the awareness of the Mibyeong, over 80% of the KMDs agreed the definitions based on patients' subjective symptoms and Korean Medicine theories. 36.1% of respondents used the term Mibyeong in the medical record, and 38.6% used when they explained the patients' condition to the patients. A majority of respondents agreed with the need for a developing Mibyeong care package. Developing the standard methods for Mibyeong assessment was the primary requisite on Mibyeong research. Conclusions : Currently the level of recognition and use of Mibyeong among Korean Medicine doctors was relatively low. To promote the use of Mibyeong in the clinical field, the standard methods for Mibyeong assessment and Mibyeong care package need to be developed.

Virucidal efficacy of a fumigant containing orth-phenylphenol against classical swine fever virus and porcine reproductive and respiratory syndrome virus (Ortho-phenylphenol을 주성분으로 하는 훈증소독제의 돼지열병바이러스와 돼지생식기호흡기증후군바이러스에 대한 살바이러스 효과)

  • In this study, the virucidal efficacy of a fumigant containing 20% ortho-phenylphenol against classical swine fever virus (CSFV) and porcine reproductive and respiratory syndrome virus (PRRSV) was examined. After each carrier deposited with CSFV and PRRSV suspensions was exposed to the fumigant in a $25-m^3$ test room for 15 h, all carriers were neutralized and diluted, and each diluted suspension was inoculated into each proper cell line. After incubation, CSFV and PRRSV viability in each cell line was examined and 50% tissue culture infectious dose $(TCID_{50})/mL$ was calculated. In the results, the concentration of viable virus in all of pathogen control-carriers was more than $2{\times}10^5TCID_{50}/mL$, and there were no cytotoxicity in all of toxicity control-carriers. In addition, the fumigant inactivated ${\geq}4.8{\log}_{10}(TCID_{50}/mL)$ of both CSFV and PRRSV. These findings will be useful for preventing the spread of CSFV and PRRSV infection.

Synergistic antibacterial effects of the combination of cephalexin with gentamicin against Staphylococci isolated from dairy cows with clinical mastitis in Gyeongnam province (경남지역에서 유방염 젖소로부터 분리한 포도상구균에 대한 세팔렉신과 겐타마이신 합제의 항균상승효과)

  • This study was investigated the synergistic effects from the combination of cefalexin and gentamicin (CCG) against Staphylococcus spp. isolated from bovine milk. Minimum inhibitory concentrations (MICs) and fractional inhibitory concentration (FIC) index were determined for 16 bacteria from 13 bovine farms in Gyeongsangnam province by the National Committee for Clinical Laboratory Standards broth microdilution procedure. Antimicrobial agents tested included cefalexin, gentamicin and CCG. Against Staphylococcus spp. tested, the MIC ranges of cefalexin, gentamicin and CCG were 0.25~1.0, 0.5~0.125 and $0.06{\sim}0.125{\mu}g/mL$, respectively. In addition, the FIC index ranges of CCG against most of the isolates was 0.28~0.43 and the FIC index of CCG against 2 strains of Staphylococcus haemolyticus (S. haemolyticus) was 0.59. In conclusion, CCG has a high antibacterial activity against Staphylococcus isolates, then the combination may be applied for the treatment of bovine mastitis caused by Staphylococcus spp.

  • Background : The system for collecting and managing the terminologies of Korean oriental medicine is essential to facilitate researches of developing intelligent information architecture based on ontology. Objectives : The purpose of this paper is to develop system for collecting and managing system for terminologies of Korean oriental medicine. Methods : First we defined roles of users used in this system, and define the system workflow based on them. Then we developed web-based collecting and managing system in which multiuser can work simultaneously, and it is compose of relational database system. Results : The terminologies of Korean Oriental Medicine can be efficiently collected, purified, inspected, and utilized by using proposed system. Conclusions : The system, proposed in this paper, can be used the whole field of oriental medicines for basic technology and has great contributions to make intelligent information architecture of oriental medicine based on ontology concept.

Effects of Injection Laryngoplasty with Hyaluronic Acid in Patients with Vocal Fold Paralysis

  • Objectives: The purpose of this study was to explore the effects of injection laryngoplasty (IL) with hyaluronic acid in patients with vocal fold paralysis (VFP). Methods: A total of 50 patients with VFP participated in this study. Pre- and post-IL assessments were performed, which included analyzing the sustained vowel /a/ phonation, and the patient reading 1 Korean sentence from the "Walk" passage that comprised 25 syllables in 10 words. To investigate the effect of IL on vocal fold function, acoustic analysis (acoustic voice quality index, cepstral peak prominence, maximum phonation time, speaking fundamental frequency) was conducted and auditory-perceptual (grade and overall severity), visual judgment (gap), and self-questionnaire (voice handicap index-10) assessments were performed. Results: The patients with VFP showed statistically significant differences between pre-and post-IL assessments for acoustic and auditory-perception, visual judgment, and self-questionnaire assessments. Conclusion: The patients with VFP showed positive change in vocal fold function between pre- and post-IL measurements. The findings showed that IL with hyaluronic acid is an effective method to improve vocal fold function in patients with VFP.

Lung Function Profiles among Individuals with Nonmalignant Asbestos-related Disorders

  • Background: Inhalation of asbestos fibers can lead to adverse health effects on the lungs. This study describes lung function profiles among individuals with nonmalignant asbestos-related disorders (ARDs). Methods: The study population was from the Workers' Compensation (Dust Diseases) Board of New South Wales, Sydney, Australia. Lung function measurements were conducted in males with asbestosis (n = 26), diffuse pleural thickening (DPT; n = 129), asbestosis and DPT (n = 14), pleural plaques only (n = 160) and also apparently healthy individuals with a history of asbestos exposure (n = 248). Standardized spirometric and single-breath diffusing capacity for carbon monoxide ($DL_{CO}$) measurements were used. Results: Mean age [standard deviation (SD)] was 66.7 (10.3) years for all participants. Current and ex-smokers among all participants comprised about 9.0% and 54.8%, respectively. Median pack-years (SD) of smoking for ex- and current-smokers were 22.7 (19.9). Overall 222 participants (38.6%) and 139 participants (24.2%) had forced expiratory volume in 1 second ($FEV_1$) and forced vital capacity (FVC) measurements < 80% predicted, and 217 participants (37.7%) had $FEV_1/FVC$ results < 70%. A total of 249 individuals (43.8%) had DLCO values < 80% predicted and only 75 (13.2%) had DLCO/VA results < 80% predicted. A total of 147 participants (25.6%) had peak expiratory flow (PEF) measurements < 80% predicted. The presence of ARDs lowered the lung function measurements compared to those of healthy individuals exposed to asbestos. Conclusion: Lung function measurement differs in individuals with different ARDs. Monitoring of lung function among asbestos-exposed populations is a simple means of facilitating earlier interventions.

Comparative Study on the National Health Screening Programs according to the Health Insurance System (국가별 건강보험체계에 따른 국가건강검진제도 비교 연구)

  • Background: Korea's health screening program has been faced the need for changes as the population and diseases structure are changing. In addition to Korea, many countries operate state-led health check-up programs to improve the health level of the people, and the operating methods of the program appear in various forms according to each country's health insurance system. This study examines other state-led health screening programs and proposes a direction for the development of Korea's health screening program. Methods: The study was conducted using the literature review method, and the "country" was set as a unit for the case analysis. The operating method of the health screening programs and the financial resources were compared according to the health insurance system. Five countries were selected as Korea, the United States, the United Kingdom, Japan, and Taiwan. Results: The analyzed countries mainly operate the health screening program as a management method for chronic diseases, but there were differences in the operating method, financing, and targeted subjects and examination items. In most countries, a risk assessment was performed prior to the examination (screening), and the subjects who needed the examination were first selected, and a follow-up management service was provided in accordance with the risk each individual exposed. Conclusion: Rather than applying the same screening method to populations with different risk levels, a health screening program will be constructed in consideration of the individual's health level and exposure risk, and the healthcare delivery system will be reorganized so that screening and treatment services can be linked.

Panel Session toward Improved Communication and Engagement with the Public after the Fukushima Daiichi Nuclear Power Plant Accident: Study Reports and Discussion with Specialists from Relevant Fields

  • Background: From 2018 to 2020, the Expert Study on Public Understanding after the Fukushima Daiichi Nuclear Power Plant Accident (the Expert Study Group) identified and analyzed activities designed to promote public understanding of science and radiation since the Fukushima accident, and held discussions on how to achieve public understanding in the situation where public confidence has been lost, and how experts should prepare for dealing with the public. This panel session was held at the 53rd meeting of the Japan Health Physics Society on June 30, 2020. Materials and Methods: First, three subgroup (SG) leaders reported their research methods and results. Then, two designated speakers, who participated as observers of the Expert Study Group, commented on the activities. Next, the five speakers held a panel discussion. Finally, the rapporteur summarized. Results and Discussion: SG leaders presented reports from researchers and practitioners in health physics and environmental risks who provided information after the Fukushima accident. During the discussion, experts in sociology and ethics discussed the issues, focusing on the overall goals of the three groups, local (personal) and mass communication, and ethical values. Many of the activities instituted by the experts after the accident were aimed at public understanding of science (that is, to provide knowledge to residents), but by taking into account interactions with residents and their ethical norms, the experts shifted to supporting the residents' decision-making through public engagement. The need to consider both content and channels is well known in the field of health communication, and overlaps with the above discussion. Conclusion: How to implement and promote the public engagement in society was discussed in both the floor and designated discussions. Cooperation between local communities and organizations that have already gained trust is also necessary in order to develop relationships with local residents in normal times, to establish an information transmission system, and to make it work effectively.

The Effects of Humanistic Knowledge and Empathy Ability on Cultural Competence of Nursing students (인문학적 소양과 공감능력이 간호대학생의 문화적 역량에 미치는 영향)

  • This study was conducted in order to examine the effects of humanistic knowledge and empathy ability on cultural competence of nursing students. A convenience sample of 250 nursing students were recruited. The average scores were 2.50 for humanistic knowledge, 3.40 for empathy ability, and 3.33 for cultural competence. Humanistic knowledge had positive correlation with cultural competence. Also, the empathy ability was positively correlated with humanistic knowledge and cultural competence. Finally, cultural competence, empathy ability and level of academic score have shown to influence cultural competence. The research founds that humanistic knowledge, empathy ability and level of academic score influenced cultural competence of nursing students. Therefore, this study provides the basic information for improving the curriculum and quality of nursing for nursing students' cultural competency enhancement.

Specialization and Present Status of Doctor of Osteopathy in the U.S.A (미국의 D.O.의 전문화과정과 현황)

  • Backgrounds : Doctor of Osteopathy (D.O.) in the United States have drawn attention as one of the future models of Korean Medicine doctors in Korea in that they have their own fields of care and therapies that distinguish them from medical doctor (M.D.), but are also able to carry out the treatment of general doctors. By analyzing D.O.'s specialization strategy, this study intends to preview points for establishing the future role of Korean Medicine doctors. Methods : We searched books, research papers, reports, conference presentations, and media articles, and chronologically classified and organized the collected data. In addition, the latest update information on related institutions' web pages and expert opinions released were also reviewed. Results : The D.O. emerged as a form of doctor in alternative medicine, however it rapidly turned to an M.D. substitute during the pandemic of the 1910s and World War II in the 1940s. Through the American Osteopathic Association (AOA)'s organizational activity, curriculum specialization, research development, and financial support, D.O. now has secured the status of M.D. in 50 states and federal law in the US. It has its own and exclusive full practice rights, capable of prescribing drugs and practicing surgery, as well as manual therapy. Beginning in July 2020, M.D.-D.O. achieved the full integration-unification of the professional training and residency program. Conclusions : In order to introduce the D.O. model to Korean Medicine system, it is necessary to strengthen biomedicine in the curriculum, and significantly expand the educational infrastructure and faculty manpower.
