A Research on The Pulse & Disease-patterns and Diagnostic Theory of Exogenous Febrile Disease in the "Sanghanjeonsaengjip(傷寒全生集)" ("상한전생집(傷寒全生集).변상한발열례(辨傷寒發熱例)" 등에 대한 연구(硏究))

  • "Sanghanjeonsaengjip(傷寒全生集)" is a classic medical work, written by the outstanding physician Dohwa(陶華) in the Ming Dynasty. The characteristic of "SangHanJeonSaengJip" is that this book succeeded to the spirit of pattern identification and treatment of Treatise on Cold Damage Diseases, newly changed a table of contents by symptoms, and together with this indicated the prescriptions in accordance with diswase-pattern at "YujeunghwalInseo(類證活人書)", "Hwajegookbang(和劑局方)" etc. Also because this kept the existing ephedra decoction, cinnamom twig decoction, minor decoction of bupleurum, decoction for reinforcing middle-energizer and replenishing qi etc.'s name on and unlikely indicated the medicine composition, it caused confusion, but at the later ages "Euhakipmun(醫學入門)" the so-called 'Doci(陶氏)' was added to the prescription name, so we are able to distinguish. Together with this, this book dose not indicate the dosage of medicine and indicates the first, the second, and the third classes[上中下] below medicine. As this dose not mean the three grades of quality"good, fair, and poor[上中下] of "Shennong's Classic of Materia Medica" but expresses the sovereign medicinal as the first class[上], minister medicinal as the second class[中] and assistant and courier medicinal as the third class[下], doctors can voluntarily decide the dosage of medicine in accordance with the degree of disease. At this thesis, I single out ten chapters in contents of 2nd volume named Hyeong(亨) corresponding to the details, among "Sanghanjeonsaengjip(傷寒全生集)". I discussed superficial fever types of exogenous febrile disease in chapter 1, aversion to cold types of exogenous febrile disease in chapter 2, syndrome caused un-sufficient sweating in chapter 3, organic fever types of exogenous febrile disease in chapter 4, aversion to wind types of exogenous febrile disease in chapter 5, Tidal fever types of exogenous febrile disease in chapter 6, Alternative attacts of chills and fever in chapter 7, Dysphoria with smothery sensation in chapter 8, Fidgetiness of exogenous febrile disease in chapter 9, and Headache of exogenous febrile disease in chapter 10, and together with this I discussed, in detail, which influence the prescriptions which are listed on each chapter have caused on future generations In accordance with this, I think that the above-mentioned symptoms and prescriptions are important when I research cold damage and warm disease study. So I orderly research revision, annotation, rendering and an investigation.

Clinical Characteristics of Bronchopulmonary Dysplasia by Type and Severity (기관지폐이형성증의 유형 및 중증도에 따른 임상적 특징)

  • Purpose : The increased survival of preterm infants in the neonatal intensive care unit recently has resulted in an increased frequency of bronchopulmonary dysplasia (BPD), especially with atypical forms. However, there have been few studies compairing classic and atypical BPD. The aim of this study was to investigate the differences between these two types of BPD. Methods : Infants with a gestational age less than 32 weeks born at the Seoul National University Hospital and Bundang Seoul National University Hospital from May 2004 to April 2007 were included. The data were categorized in 2 groups, classic and atypical BPD. We determined the incidence of BPD, and compared perinatal factors and postnatal managements between two groups. Results : Among 260 study subjects, 141 (54.2%) infants had BPD. Classic BPD infants were 64 and atypical BPD infants were 77. Comparison of differences between 2 groups, classic BPD infants were associated with respiratory distress syndrome, patent ductus arteriosus, intrauterine growth restriction, more high-frequency ventilator (HFV) use, low 1 and 5 minute Apgar scores. Atypical BPD infants were associated with antenatal steroid use, maternal premature rupture of membrane and chorioamnionitis (CAM). In multivariate analysis, more HFV use was associated with classic BPD. Antenatal steroid use, clinical CAM and histological CAM were associated with atypical BPD. Conclusion : The results of this study showed that antenatal factors (antenatal steroid use, clinical CAM, histological CAM) were associated with atypical BPD and postnatal factors (HFV used more) were associated with classic BPD. Further studies are needed for prevention and treatment of BPD.

  • Objective : To find the main symptoms associated with atopic dermatitis and to get the utmost out of information about cause, treatment are contribute in the treatment of atopic dermatitis rather than looking for ways through how to connect modern atopic dermatitis and a listed symptom of classic literature. Method : In this research, we recorded about the main symptoms of atopic dermatitis such as itch, erythema, edema, pus and furuncle. Also we interpreted original text by extracting a sentence containing the terms like red[赤], pus[膿], flare[發赤], dermatosis[溞], swelling[洋], light red[紅], itch[瀁], worm running[蟲行], juice[汁], rash[癮疹], variegation[疹斑] and summarized cause of symptoms, disease sign, prescription. Results : There are the cause of many symptoms accompanied by atopic dermatitis in the body that recorded unpreparedness[虛], Wind[風], Fever[熱], unpreparedness & interruption of vitality hole[氣穴不通], unpreparedness & Wind, Wind Fever[風熱], Wind Coldness[風寒], Wind Humidity[風濕], Humidity Fever[濕熱], Wind Humidity Fever, Wind of Lungs[肺風], Wind of Lungs and Spleen[脾肺風], the Kidney Wind poison[腎臟風毒], Wind of Lungs Fever, Worm[蟲], miscellaneous vitality[雜氣], unpreparedness and Fever of Heart[心虛熱], old sore of kidney poison[久瘡腎臟風毒].

Rectal Injury Associated with Pelvic Fracture

  • Rectal injury is seen in 1-2% of all pelvic fractures, and lower urinary tract injury occurs in up to 7%. These injuries are rare, but if missed, can lead to a severe septic response. Rectal injury may be suspected by the presence of gross blood on digital rectal examination. However, this classic sign is not always present on physical examination. If an Antero-Posterior Compression type pelvic fracture is seen, we should consider the possibility of rectal and lower urinary tract injury. It is important to define the anatomic location of the rectal injury as it relates to the peritoneal reflection. Trauma to the intraperitoneal rectum should be managed as a colonic injury. Extraperitoneal rectal injury should be managed with fecal diversion regardless of primary repair. We present the case of a 46-year-old man who was referred to our hospital following a major trauma to the pelvis in a pedestrian accident.

  • Objectives : This study focused on how the modem blood vessel is defined in Oriental medicine through literary investigation. Methods : Contents from 35 classical texts representing each era were analyzed for the expression of Hyeolmaek and affiliate information, as well as terms used for the current concept of blood vessel. Results : Following results were obtained through investigating literary information on Hyeolmaek. 1. In the Yellow Emperor's Classic, Hyeolmaek is associated with heart and widely used for physiological, pathological, diagnosis, and treatment purposes. 2. During the Song Dynasty, a term Cheonggeun (靑節). is used to describe the vein in the abdomen. 3. During the Ming Dynasty, terms such as Cheonggeun(靑節) and Cheongmaek(靑脈) were used. 4. The term Hyeolmaek was used inclusively for blood, blood vessels, arteries, veins, and as well as meridians.

  • The purpose of this study is that translates ${\ulcorner}$Sun Gi Il Il Bun Wi Sa Si${\lrcorner}$ in the ${\ulcorner}$Young Chu(靈樞)${\lrcorner}$ as a modern words because it is hard to understand which was written by classical words. We revised the original text with the 7 other classic books and classified annotations of the 6 annotated books according to the similar contents. We classified this volume by 3 chapters, and added Hangul suffixes to the original text. The Five types of changes(五變) in the second chapter is meaning to the mutual relationships among the Five viscera and Color, Time, Day, Note, Taste. The word order of contents in the second chapter must be unified follow the Color, Time, Day, Note, Tastes. The Five types of changes in the third chapter must be revise the Five types of diseases(五病) on the bases of the ${\ulcorner}$You Kyoung(類經)${\lrcorner}$.

  • Objective : The region, observation period, figuration of pulse, pulse frequency, process of diagnosis and related diagnostic methods in traditional pediatric pulse diagnosis were studied in this article. Method : The articles related to traditional pediatric pulse diagnosis in Traditional medical classics, and analysis to the characteristics of it was done, then lastly, the principle of pulse manifestation and the meaning of application to modern times were considered. Result & Conclusion : It is difficult to catch the figuration of pediatric pulse because of dynamic change of body conditions, fast heartbeat and narrow region of pulse. Therefore, the pulse diagnosis appropriated for children have been developed and combined with the special diagnosis methods, for example, palpation of forehead and observation of index finger. The traditional pediatric pulse diagnosis is done with just one finger palpation in the period of 3 to 5 years old to identify the speed, length, size and height of pulse. The standard average of pulse frequency is 7~8 par breath in the period of 3 to 5 years old according to traditional medical classic.

  • Objectives : The purpose of this study was to investigate the interpretation of the Gwibi-tang(歸脾湯, GBT) prescription in order to obtain the traditional significance. Methods : We have analyzed the interpretation on the GBT prescription through translations and comparisons based on classic books about the oriental medical prescriptions. Results : 1. The source of GBT in Dongeuibogam(東醫寶鑑) is Guchiryuyo(口齒類要), Jeongcheryuyo(正體類要) and Gyojubuinyangbang(校註婦人良方) etc. 2. Meaning of "GwiBi(歸脾)" in the GBT name is returning to the Bi(脾) which is origin of generating Gi(氣) and Blood(血). 3. GBT is composed of Sagunja-tang(四君子湯), Hwanggi-tang(黃芪湯) and Danggwibohyeol-tang(當歸補血湯) etc. 4. According to wide range of symptom, GBT can be varied by adding or subtracting herbs. Conclusions : In this study, we have demonstrated various methodologies. This paper will be useful to the future researchers and clinicians to conduct a study on herbal medicines such as the GBT.

A Bibliographic Study on Wood-block Edition 『Donguibogam』 in Jeseon Dynasty during the 17th-18th Centuries (17-18세기 조선 간행 『동의보감』 목판본에 대한 서지학적 연구)

  • "Donguibogam" has been published several times in Japan and China since the first was published and established itself as representing the East Asian medicine in 17th Century. Also, modern far has attracted attention as a major classic book of Korea medicine. For these "Donguibogam" its contents, as well as several medical historians and bibliographers were early comment on versions. However, these prior studies did not confirm about the various editions of the publication time, especially for the initial wood-block edition estimate only. Through this new study found the following facts. First, Wood-block edition of two Jeollagamyeong published until the mid-18th century to be confirmed by the "Seungjeongwonilgi". Second, the timing of the editions published later modified in Naeuiwon is confirmed by the "Chakpan Catalog" and the "Seungjeongwonilgi". Third, the person who keeps wood-block edition of Chonnamgamyeong is the first empirical examples of the same edition. Fourth, the edition is not published editions group that bypasses the Naeuiwon modified.

A Study of Meridian and Collateral Diagrams of Chosun Dynasty (조선시대 경락도(經絡圖)에 관한 고찰)

  • The Meridian and Collateral Diagram is one of the most important 圖像s of Traditional Korean Medicine. A 圖像 is a picture made on a two-dimensional surface using lines and colors to portray an object or an image. Meridian and Collateral diagram is a 圖像 of the human body with indications of acupoints and meridian passageways and have different names such as 經穴圖, 輸穴圖, 鍼灸圖, 明堂圖, 銅人圖, in accordance with its classification. The documental basis of the Meridian and Collateral Diagram is the Internal Classic and the very first Meridian and Collateral Diagram confirmed through textual evidence can be found in 葛洪's "抱朴子 雜應". The Korean 동인도 in existence today, called '銅人明堂之圖', exists in two versions; a hand-copied version and a wooden engraving block version. It displays all the locations of the acupoints located on the anterior side of the human body, labels the names of the acupoints, and specifically distinguishes 起始穴 and 終止穴 of the eight meridian vessels.
