본 연구는 부산광역시 소재 대학병원에 근무하는 간호사와 간호조무사 321명을 대상으로 방사선사의 인식도를 조사하여 방사선사의 위상과 전문적 이미지를 향상시키기 위한 기초 자료를 제공 하고자 연구를 실시한 결과 인식도는 $3.02{\pm}0.42$점이며 영역별로는 직업적 이미지가 높았고 업무적 이미지가 낮았다. 문항별로는 '전문직이다'가 높았고 '환자의 문제를 잘 파악하고 있다'가 가장 낮았다. 교육과정, 시험주관기관을 정확히 알고 있고 의료법상 의료인, 의료기사라고 응답한 대상자들의 인식도가 높았고 호칭을 기사로 사용하는 대상자의 인식도가 낮았다. 방사선사 직업 선택을 찬성하고 의료기관내 위치가 높다는 대상자의 인식도가 높았다. 인식도를 높이기 위한 방안으로 환자에 대한 적극적인 관심과 친절한 태도로 부정적인 이미지를 개선하고 호칭을 정립하기 위한 개인과 협회차원의 홍보와 대책이 필요하며 과거 전문대학의 이미지에서 탈피하기 위한 노력과 학술대회와 보수교육 활성화, 전문방사선사제도의 법령 제정으로 방사선사의 자질과 위상을 높이고 대중매체를 통한 홍보와 모니터링으로 전문직업인으로서의 이미지 향상을 위한 개인과 협회의 노력이 필요할 것으로 생각한다.
본 연구는 신임 119구급대원의 출동경험을 바탕으로 응급구조학 교육의 현장 적용 및 효용성을 파악한 후 대학교육의 한계점과 개선점 및 발전방향을 도출해내고자 한 질적 연구이다. 연구대상자는 응급구조학과를 졸업한 119 구급대원으로 구급활동을 시작한 지 3년 이내의 신임구급 대원 6명이다. 내러티브 인터뷰(narrative interview)방식으로 심층면담을 진행한 후 현상학적 방법 중 하나인 Colaizzi방법으로 분석하였다. 의미있는 진술에서 82개의 구성된 의미가 도출되었다. 구성된 의미로 부터 23개의 theme, 4개의 theme cluster, 2개의 category가 확인되었다. 4개의 주제모음은 '대학교육의 효용성', '대학교육의 한계점', '교육방법론에서의 개선방향', '교육내용에서의 개선방향'으로 신임구급대원들은 대학교육이 전반적으로는 도움이 되었으나 소방 현장보다는 병원 쪽에 치우친 이론교육이 주입식 강의로 이루어진 한계점을 지적하였다. 이에 신임대원들은 사례중심의 학습, 현장사례리뷰, 역할극방식 교육, 시뮬레이션 교육의 확대와 함께 실습위주로 장비를 활용한 술기교육을 강화할 것을 제안하였다. 또한 신임대원들은 대학교육과 현장사이에서 이론과 실제사이의 괴리감을 토로하며 소방현장에 맞는 교육의 도입을 제안하고 다양한 시각을 위한 다양한 지도를 언급하였다. 응급구조학 대학교육은 전반적으로 도움이 되었으나 제한점이 동시에 관찰되므로 교육방법론과 교육내용의 개선을 통해 발전해가야 한다.

본 연구는 2급 응급구조사가 호흡부전 및 호흡정지 환자에게 백-밸브 마스크(BVM) 인공호흡 시 호흡량 측정기를 사용하여 적정량의 호흡량과 및 성공률을 알아보고자 한다. 연구는 2023년 12월 11일부터 12일까지 D 대학교에 재학 중인 2급 응급구조사를 대상으로 20명을 선정하여 10명은 호흡량 측정기를 이용하여 BVM 인공호흡을 교육한 실험군으로, 다른 10명은 호흡량 측정기 없이 BVM 인공호흡을 교육한 대조군으로 선정하여 2분간 인공호흡을 제공하는 실험을 하였다. 연구 결과, 대조군에서는 정확한 호흡량을 제공하지 못하였고(p=.025), 호흡 성공률 역시 실험군에서는 2분간 호흡 성공률이 높았으나 대조군에서는 유의한 차이를 보였으며(p=.001), 주관적 호흡량과 측정된 객관적 호흡량이 대조군에서 유의한 차이를 보였다(p=.010). 따라서 호흡량 측정기를 사용한 교육은 2급 응급구조사가 느끼는 주관적 호흡량을 객관적 호흡량과 일치시키고 호흡 성공률을 높여 호흡부전 및 호흡정지와 심폐소생술 시 심장정지 환자의 생존율을 높이는 데 도움이 될 것이다.
Background: In Korea, laws for many medical technicians were revised in the Enforcement Decree of the Medical Technicians Act (MTA), which was announced on December 2018, whereas those related to dental hygienists remained unchanged. This study aimed to determine the awareness and opinions of dental hygienists regarding MTA. Methods: Dental hygienist were recruited as participants via convenient sampling in Seoul, Gyeonggi-do, and Chungcheong-do; data from 291 self-reported questionnaire responses were used for the final analysis. We investigated the participants' general characteristics, awareness, and request for the amendment of the MTA. The compliance with the work scope specified in the MTA and level of demand for revision of the MTA were analyzed by independent t-test and one-way analysis of variance. For all statistical analyses, the significance level was set at 0.05. Results: For the 2018 MTA revision, 99 (34.02%) knew that dental treatment assistance and surgery assistance were excluded, whereas 192 (65.98%) did not know. The item "The current medical technician law must be revised" was scored 4.13±0.80 out of 5 points, and significant differences were identified according to the education level, career, and position (p<0.05). The item "It is necessary to institutionalize the expanded work scope beyond the work scope of dental hygienists specified in the MTA" was scored 4.02±1.04 out of 5 points, and significant differences were identified according to age (p<0.05). Conclusion: The participants wanted the MTA to be revised to reflect the real-world work performed by dental hygienists in the dental clinical field. The legal system must ensure the legal protection of the work area of the dental hygienist as an oral health professional, and recognize the legal work scope of the dental hygienist.
Purpose: This study investigated the use of infection management and the frequency of emergency equipment sterilization by emergency rescue crews to provide basic guidelines and suggestions for infection control. Methods: A self-reported questionnaire was completed by 160 emergency medical technician rescue crews in J area from May 7 to May 22 in 2015. The questionnaire consisted of 95 items, including the general characteristics of the subjects (8 items), the use of emergency equipment (33 items), the frequency of facility sterilization (33 items), infection control (12 items), vaccination (4 items), and the use of personal protective equipment (5 items). Results: In all, 97.3% of respondents were aware of the "Guidelines of Infection management for Rescue Operators' and 90.9% received training on infection control. A total of 72.7% and 47.3% of rescue centers were equipped with disinfection facilities and laundry rooms, respectively. The average frequency of sterilization was $3.17{\pm}0.75$, which significantly differed for teams with more clinical experience (p=.050) and teams with three members (p=.030). The average score for individual protective equipment supplies was $1.95{\pm}0.66$. Conclusion: For proper infection management of emergency equipment, our results suggest that the number of crew members should be increased for each ambulance, protective equipment and rescue products should be supplied, and the number of facilities for sterilization and laundry at rescue centers should be doubled.
Purpose: This study examined the effect of wearing personal protective equipment (PPE) on cardiopulmonary resuscitation (CPR), positive airway pressure, and the posture of emergency medical technicians (EMTs) when conducting CPR. Methods: Twenty 119 EMTs performed 30:2 CPR on a manikin for 4 min. Imaging data were digitized with Kwon3D XP (version 4.0). Data were collected by analyzing the motion when starting in one cycle, such as pressing to the maximum and in the final position (relaxed), and were analyzed with SPSS 18.0. Results: The angle of the elbow joints was significantly reduced (p < .05). The trunk angle was statistically significantly (p < .01, p < .001) increased. The angular velocities of the shoulder joint and left elbow joint were reduced (p > .05). The angular velocity of the trunk was significantly reduced in the starting and maximum compression postures. The hand-escape time was increased. The average compression depth was increased but not significantly (p > .05). The positive airway pressure was reduced (p > .05). Conclusion: The angle of the elbow joints and the angular velocity of the trunk were reduced, and the angle of the trunk was increased. The success of CPR and positive airway pressure was reduced.
This study was conducted to determine whether level-1 emergency medical technicians (EMTs) can adequately recognize ST-segment elevation myocardial infarction (STEMI) in the emergency department (ED) and whether their ability to do so differs from that of emergency medicine physicians (EMP). From December 2022 to November 2023, patients aged 20 years or older visiting the ED with chief complaints suggesting acute coronary syndrome (ACS) were enrolled. As soon as the patient arrived at the ED, a level-1 EMT conducted a 12-lead electrocardiogram (ECG) to assess STEMI; an EMP subsequently assessed whether to activate the percutaneous coronary intervention team. Demographic characteristics, test results, and final diagnoses were collected from the medical records. Among the 723 patients with case report forms, 720 were included in the analysis. These were categorized as follows: 117 (16.3%) with STEMI, 159 (22.1%) with non-ST-segment elevation ACS, and 444 (61.7%) with other conditions. STEMI was correctly recognized in 100 patients (91.7%) by level-1 EMTs and in 104 patients (95.4%) by EMPs (kappa=0.646). EMTs with less than 1 year of ED work experience correctly recognized 60 out of 67 STEMI patients (89.6%), which was comparable with the EMPs who recognized 65 out of 67 STEMI patients (97.0%, kappa=0.614). EMTs with more than 1 year of ED work correctly recognized 40 out of 42 STEMI patients (95.2%), and therefore performed better than EMPs, who recognized 39 out of 42 STEMI patients (92.9%, kappa=0.727). The level-1 EMTs adequately recognized STEMI using a 12-lead ECG and were in substantial agreement with the evaluations of the EMPs.
성공적인 보철치료를 위해선 초진과정에서 부터 시작되는 진료실과 기공실의 협업이 중요하다. 다만 진료실과 기공실이 서로 모든 치료과정에서 협업이 용이하지 않고, 그중 보철물의 이상적인 형태를 부여하는 일은 기공파트가 책임 지어야 할 부분 중 하나이다. 제한된 환경에서 치과기공사가 어떤관점으로 case에 접근하는지 설명하고자 한다.
Purpose: The purpose of this study was to provide an appropriate direction for effective field practice program by analyzing the influencing factors on satisfaction in ambulance ride practice of paramedic students in Korea. Methods: This study analyzed of 180 paramedic students from 20th to 30th september, 2019. The collected data were analyzed with MedCalc 18.1.0 program. Results: The results of the satisfaction of ambulance riding practice according to the degree of procedure practice are as follows. Satisfaction was high in all groups except the practice time. The lowest satisfaction rate was seen in the category of'There were many cases of applying theories learned at college'within the variable of practice environment(2.82±0.82). The factors affecting the satisfaction of ambulance riding practice were the nurse (β=-0.1472, p=.024) and emergency medical technician(β=-0.3318, p=.002) according to the ambulance worker qualification paramedic, and the practice score (β=0.0048, p<.001). Conclusions: For effective ambulance riding practice, it is necessary to be able to select the practice place where many cases can be experienced, the appropriate selection of the practice leader, and the degree of procedure practice.
본 연구는 중소병원에 근무하는 의료기사의 병원감염관리에 대한 인식과 수행정도를 확인하기 위한 연구이다. 의료기사(임상병리사, 방사선사, 물리치료사) 235명을 대상으로 하였으며 통계적 처리는 SPSS Ver.23.0으로 평균과 표준편차, 분산분석, 회귀분석을 실시하였다. 연구결과, 감염관리가 제대로 수행되지 않는 주원인은 전문가 부재($3.17{\pm}1.18$)이며, 감염관리에 대한 인지도보다 수행도가 통계적으로 유의하게 낮았다. 감염사고 후 아무런 조치를 하지 않았다40.0%(94명)가 가장 높았으며, 소독 검사 예방접종 후 부서장에게 보고는 표준화회귀계수 베타가 -0.265, p=0.025로 유의한 음(-)의 영향을 미쳤다. 감염관리 수준은 교육여부 및 감염관리지침여부와 유의한 양의 상관관계를 보였다. 따라서 중소병원 의료기사들의 감염관리에 대한 전문인력, 교육, 감염관리지침 등의 필요성을 확인하였으며 이를 통해서 더욱더 효율적인 감염관리가 가능해질 것으로 사료된다.
